The following items are required by Garfield County for a final Inspection: 1) A final Electrical Inspection from the Colorado State Electrical Inspector. 2) Permanent address assigned
by Garfield County Building Department and posted at the structure and where readily visible from access road. 3) A finished roof; a lockable building; completed exterior siding; exterior
doors and windows installed; a complete kitchen with cabinets, sink with hot & cold running water, non-absorbent kitchen floor covering, counter tops and finished walls, ready for stove
and refrigerator; all necessary plumbing. 4) All bathrooms must be complete, with washbowl, tub or shower, toilet, hot and cold running water, non-absorbent floors, walls finished, and
privacy door. 5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks over 30" high must be constructed to all mc and IRC requirements including
guardrails. 6) Outside grading completed so that water slopes away from the building; 7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of extenuating

BUILDINGS In order to understand the scope of the work intended under a pennit application and expedite the issuanoe of a pennit it is important that complete infonnation be provided.
When reviewing a plan and it's discovered that required infonnation has not been provided by the applicant, thiswill result in the dehiy of the pennit issuance and in proceeding with
building construction. The owner or contractor shall be required to provide this infonnation before the plan review can proceed. Other plans that are in line for review may be given
attention before the new infonnation may be reviewed after it has been provided to the Building Depmtment. Please review this document to determine if you have enough information to
design your project and provide adequate information to facilitate a plan review. Also, please consider using a design professional for assistance in your design and a construction professional
for construction of your project. Any project with more than ten (10) occupants requires the plans to be sealed by a Colorado Registered Design Professional. To provide for a more understandable
plan in order to detennine compliance with the building, plunlbing and mechanical codes, applicants are requested to review the following checklist prior to and during design. Applicants
are required to indicate appropriately aud to submit the completed checklist at time of application for a permit. Plans to be included for a Building Permit, must be on drafting paper
at least 18"x24" and drawn to scale. Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with 1
decks, balcony, steps, hand rails and guard rails, windows and doors, including the fInish grade line . and original grade. A section showing in detail, from the bottom of the footing
to the top of the roof, including re-bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation, sheeting, house-rap, (which is required), siding or
any approved building material. Engineered foundations may be required. A window schedule. A door schedule. A floor framing plan, a roof fi'aming plan, roof must be designed to withstand
a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind speed, wind exposure B or C, and a 36 inch frost depth. All sheets to be identifIed by number and indexed.
All of the above requirements must be met or your plans will be retumed. All plans submitted must be incompliance with the 2003 IRC. 1. Is a site plan included that identifIes the location
of the proposed structure or addition and distances to the property lines from each comer of the proposed structure(s) prepared by a licensed surveyor and has the surveyors signature

Subdivision, 0434 Lions Ridge Road, Garfield County, Colorado Dear Gary: As requested, a representative of Hepworth-Pawlak Geotechnical, Inc_ observed the existing crawlspace at the
subject site on AprilS, 2010 to evaluate the soils exposed for foundation support. The fiudings of our observations and recommendations for the foundation design are presented in this
report. The services were performed in accordance with our agreement for professional engineering services to you, dated April 5,2010. The existing residence was built in 1992 and is
a 1,250 square foot, one story, wood frame structure over a crawlspace. Existing wall loads were estinlated by Jim Romeo to be about 1,220 plf with footing widths estimated at 18 to
20 inches. The existing footing bearing pressure on the subgrade is 700 to 800 psf. The foundation reportedly shows no evidence 0 f cracking or settlement. The proposed addition will
be a second story over the eastern half of the house. Proposed wall loads in the addition area will be about 1,900 plf or about 50% higher than the current loading. For the existing
footings, the proposed bearing pressure on the sub grade would increase to 1,150 to 1,250 psf. . The soils exposed in the bottom of the crawlspace consisted of medium stiff sandy silt
and clay. Results of swell-consolidation testing performed on samples taken from the east half ofthe crawlspace, shown on Figures I and 2, indicate the soils have low to moderate compressibility
under existing low moisture conditions and light loading, and have a low collapse potential (settlement under constant load) when wetted. The silt and clay soils were moderately to highly
compressib Ie under increased loading after wetting. No free water was observed in the crawlspace and the soils were slightly moist. A small moist soil area was observed adjacent to
the exterior footing on the north side of the crawlspace in the area of an exterior hose bib near where the water line enters the house. Based on borings drilled on Lots 4 and 7, the

Gary Pax April 8, 2010 Page 2 Considering the conditions exposed in the crawlspace and the nature of the proposed construction, using the existing spread footings for support of the
second story addition should be feasible. The exposed soils tend to compress when wetted and there could be some post-construction settlement of the foundation if the bearing soils become
wet. Based on the consolidation test results, potential settlement of the foundation under the existing loading could be 2Yz to 3Yz inches. The difference in potential settlement after
wetting, between footings at 800 psf and the same footings at 1,200 pst; is estimated to be about Yz% of the wetted depth. Assuming a wetted depth oflO to 15 feet, the potential additional
settlement for the higher footmg load could be Yz to I inch. To date, it appears that the subsoils have not been wetted significantly. For the long term performance ofthe foundation,
it is important that wetting of the bearing soils be avoided. We understand that the lawn area on the east and north side of the house is planned to be replaced with xeriscape. The south
and west sides of the house are well drained and rock covered which is appropriate. The recommendations submitted in this letter are based on our observation of the soils exposed within
the crawlspace and do not include subsurface exploration to evaluate the subsurface conditions within the loaded depth of foundation influence. This study is based on the assumption
that soils beneath the footings have equal or better support than those exposed. The risk of foundation movement may be greater than indicated in this report because of possible variations
in the subsurface conditions. In order to reveal the nature and extent of variations in the subsurface conditions below the excavation, drilling would be required. It is possible the
data obtained by subsurface exploration could change the recommendations contained in this letter. Our services do not include determining the presence, prevention or possibility of
mold or other biological contaminants (MOBC) developing in the future. If the client is concerned about MOBC, then a professional in this special field of practice should be consulted.
