Temporary Employee Housing Permit — Well Pad 35L MTEH -6694 Dear Mike: This office is in receipt of your Application (received December 27, 2010) for a Minor Temporary Employee Housing Facility operated by Noble Energy Inc. located in the Rural Zone District southeast of Parachute (Section 35, T7S, R95W). The Administrative Permit requested is for property owned by Noble Energy Inc. associated with Well Pad Noble 35L. Based on our Staff review, the application this office received has been deemed technically incomplete. Please respond to the following application submittal requirements which were not provided or adequately addressed in the application. 1. No vicinity and site maps were included with the hard copy of the Application. Mapping was obtained from your building permit submittal, however, the vicinity and site maps need to be updated to comply with the Unified Land Use Resolution of 2008 as amended (ULUR) requirements for scale and topography. Submittal of a larger format copy should address some of these issues including legibility of the maps. The detailed site map needs to show and identify the boundary for the permitted site and its relationship to your existing and proposed improvements. 2. The electronic version (computer disk) of the Application provided could not be opened. Please provide a replacement especially if it contains upgraded mapping information. 3. Service commitment letters for water, sewer, and trash should be updated to include a current date no more than 6 months old and should include reference to providing service to the proposed site (Well Pad Noble 35L) or corresponding well field. 4. Please provide a copy of the Colorado Department of Public Health and Environment (CDPHE) registration number and certifications for your water hauling company (Stallion Oilfield Services). 5. The Application represents that waste water will be disposed of by hauling off site to a permitted disposal location; however, your transmittal cover letter indicates that you have previously obtained an ISDS permit. In regard to water storage for fire suppression your Application does not reflect the storage amount required by the Grand Valley Fire Protection district. Please clarify the amount of storage proposed and show the storage /tank location on the site plan map. Please include representations regarding any fire suppression sprinklers and compliance with other fire protection requirements including fire extinguishers. 7. Please provide copies of any existing or required county road access permits. 8. Public notice requirements contained in Section 4- 501(J)(2) of the ULUR require notice to adjacent property owners within 200 ft. of the "subject lot" when a site is located in the Rural Zone District. Please provide ownership information (Assessor's Office information is the preferred source) for property within 200 ft. of your Parcel (Tax Parcel ID #240735100058) and include with your submittals evidence of proper public notice. Please contact the Building and Planning Office if you have any specific question regarding the public notice requirements. 9. The power of attorney documentation and authorizations to represent do not appear to include Olsson Associates and also appear to have expired as of December 31, 2010. Please clarify who the County's contacts are for the processing of the Application and update the authorizations as needed. 10. The County Attorney has also noted minor inconsistencies with your representations as to ownership of mineral rights. Please review your Application and clarify whether mineral rights are owned by Noble Inc. or by the Federal Government (both representations are included in the submittals). Please note, as per Section 4 -103 (C) (1), of the ULUR, this letter is notification of deficiencies in your Application and this office will take no further action on the application until the deficiencies are remedied. If the Applicant fails to correct the deficiencies within sixty (60) calendar days, the Application shall be considered withdrawn and returned to the applicant. Please contact this office to discuss the deficiencies noted above and any questions you may have regarding clarification of any of the above items.
