To determine the proper amount for the application fee, multiply the length of the building (lineal feet) by the width of the building (lineal feet). Divide the total by 1000, round the result !!Q to the next whole number and multiply this number by 5$. This is the square footage fee. Add the square footage fee to the base fee of $50 and submit the total amount to the Division along with the demolition notice application form. Ex: 40ft. x 52ft. = 2080 square feet; 2080/1000 = 2.08 (round up to 3); 3 x $5 = $15 (square footage fee) $15 + $50 (base fee) = $65 total application fee. 2. In the event that only a load-bearing member is demolished, the square footage fee is calculated the same way as in "number 1". However, you only need to calculate the actual footprint of the load-bearing member. 3. All spaces must be filled in on the application. If the information is not applicable, please write N/A. Incomplete information may result in a delay in processing the application, which may delay your project. 4. We must have proposed start and end dates for the demolition. 5. There is a 10 working-day advance notification requirement for permit applications. Day 1 is the 1st business day following the postmark or hand-delivery date. (Working Day means Monday through Friday and including holidays that falls on any of the days Monday through Friday.) If a demolition follows a permitted or noticed asbestos abatement project within 10 business days of the completion of the abatement project, the 10 working-day advance notification requirement will be waived. 6. The Colorado-certified asbestos inspector must sign the form in blue ink. (Original signature must be submitted.) The building owner or the contractor must also sign the application certifying that all refrigerants and luminous signs have been properly removed from the site. 7. If the notice must be modified after the application has been submitted, notify the Asbestos Unit by fax at 303-782-0278 or e-mail at asbestos@state.co.us by the end of the next regular State business day following the modification. Project modifications include discovery of unidentified asbestos-containing materials, changes in scope of work or the scheduled work dates. Please use the PermiUNotice Modification Form. 8. Recycling of materials, such as concrete or wood, that are bonded or contaminated with asbestos-containing material (ACM), such as floor tile or mastic, is NOT permitted. 9. Demolition of a building that has non-friable asbestos-containing materials remaining must be completed without causing the asbestos-containing materials to become friable. Burning a building with any asbestos-containing materials is PROHIBITED. Concrete floors covered with floor tile shall be removed in as large sections as possible. Operations such as crushing, pneumatic jacking, etc. of materials containing asbestos are not permitted. 10. All provisions of laws and ordinances governing this type of work shall be complied with whether specified herein or not. Demolition permits or approval notices appearing to give authority to violate or override the provisions of any other laws or ordinances shall be invalid. The inspection was performed by Mr. Douglas Close, a Colorado State and EPA Asbestos Building Inspector/Management Planner. The scope of work included only the house structure on the property. The house is scheduled for demolition in the near future. The scope of this inspection was limited to sampling and analysis of materials which were suspect for asbestos content, readily accessible, and present in or on the building only. Suspect materials within the scope of work were sampled and analyzed as per Colorado State Regulation #8. Discussion & Recommendations The building possessed the following Asbestos Containing Materials (ACMs) which are regulated by the State of Colorado, the EPA and . OSHA during building demolition, and must be properlv removed from the unit by a licensed Colorado State asbestos abatement contractor prior to achieving an inspector sign-off on the CDPHE demolition permit application: • Wallboard Texture -present in the rear office room (walls & ceiling), the adjoining hallway ceiling, the master bedroom walls and ceiling and the living room walls & ceiling (the balance of the house did not possess asbestos in the wallboard texture ... this Includes the dining room, the bathrooms, the kitchen, the upper level and the rear entry area). Estimated 1500 square feet of this ACM present. • Sheet Vinyl Flooring (yellow in color) -present in the bedroom hallway bathroom beneath carpeting. There is an estimated 50 square feet of this ACM present. • Sheet Vinyl Flooring (yellow in color) -present In the east bathroom and rear entry hallway as the bottom layer of sheet vinyl flooring. There is an estimated 150 square feet of this ACM present. • Cement Asbestos Board Siding Shingles -these siding shingles are present beneath the metal siding. It is assumed that this ACM is present beneath all of the metal siding, but this cannot be verified without destructive removal of significant portions of the metal siding materials. If this ACM is present beneath all of the metal siding there Is an estimated 2000 square feet of this ACM present on the house. The State of Colorado requires that the client retain a licensed Colorado State asbestos abatement contractor (General Abatement Certificate, or GAC, holder) to properly remove and dispose of these ACMs. The homeowner is prohibited from removing these ACMs by themselves prior to demolition due to the quantity of ACM exceeding the State trigger level of 160 square feet. The homeowner cannot optout of these requirements. Following complete removal of these ACMs, Phase Con must return to the site to verify that these ACMs are completely removed from the house after which the certified inspector can sign the demolition permit application. Achievement of this signature necessitates removal of £ll of the ACMs. There cannot be small pieces, debris or dust present from the ACMs or the inspector cannot sign the demolition permit application. Removal of these ACMs by non-certified personnel and/or by a nonGAC holder could result in significant asbestos exposure to personnel, an accompanying health hazard, potential contamination of significant areas of the building, and failure to achieve the Inspector sign-off on the demolition permit. Further information on the asbestos regulation can be found on the CDPHE web site or by contacting the CDPHE at 303-692-3100. The joint compound present in association with the upper level wallboard wall and ceiling systems was found to contain 3% asbestos. The State of Colorado and the EPA consider the wall system as a whole in determining total asbestos content. To be considered an ACM by the State and the EPA a material must contain> 1 % asbestos. As the wallboard wall/ceiling system as a whole (consists of wallboard sheets which do not contain asbestos and asbestos containing joint compound present at the seams and screw holes only) contains < = 1 % asbestos this material is not considered an ACM and Is not required to be removed prior to demolition. If any previously unknown or undiscovered materials are found during demolition activities which are suspect for asbestos content then these materials should be tested for potential asbestos content prior to continuing any further demolition work. Disclaimer This inspection was performed using non-invasive sampling techniques. This report does not imply, or guarantee that every material on the property, or in the property building, which may potentially have asbestos as a component has been identified and/or sampled. A guarantee that all asbestos materials have been identified and/or sampled would require cost-prohibitive and destructive sampling protocols. Furthermore, identified asbestos containing materials may be in areas which are inaccessible or hidden due to their application during the construction process and their subsequent enclosure or covering with building and finish materials. Areas behind walls, inside chases, or other hidden, covered or enclosed areas were not included within the scope of this inspection, as destructive techniques would have been required to access these areas. These areas should be inspected whenever renovation or demolition activities are scheduled which may disturb the materials within or beneath these barriers. 'Overspray' or excess materials from the installation or application process of asbestos containing materials is common, should be expected in the vicinity of installed asbestos containing materials, and is often present either covered by other building materials or in hidden or unexpected locations. This inspection cannot determine the presence of these materials and their locations and therefore the determination of these locations and material quantities is outside of the scope of this inspection. Bulk material samples are obtained in accordance with applicable regulations, Industry standard techniques and analyzed by a NVLAP accredited laboratory; however, due to the asbestos content fluctuations which may occur in a building material due to the application and/or initial mixing process no guarantee can be made as to an 'exact' percentage asbestos content (this Includes 'no asbestos detected' and 'trace' contents) which represents the entirety of the material (asbestos content fluctuations can and will occur throughout a building material). Abatement cost estimates and material quantity estimates are approximate only (due to the hidden nature of many of the materials), and are provided only as a general guideline to the client. More than one licensed Colorado State asbestos abatement contractor should be consulted to determine actual abatement costs of the ACBMs described above. Point Count Analysis and Method Point counting was performed on a polarized light microscope with a calibrated reticle according to the revised NESHAP method of November 20, 1990 (Federal Register, V.55, N.224, 11120190). Origionai asbestos content of bulk materials was determined using procedures outlined in the interim method (40 CFR part 763, Appendix E to subpart E) and AHERA method (EPA-6001R-93/116). Samples were prepared using HCL acid washing for carbonate based samples, chemical reduction for organically bound components, oil immersion for identification of asbestos types by dispersion staining I becke line method. Qualifications CA Labs is accredited by the National Voluntary Accreditation Program (NVLAP) for selected test methods for airborne fiber analysis (TEM), and for bulk asbestos fiber analysis (pLM). All analysts have a college degree in a natural science (geology, biology, or envirorunental science) or are recognized by a state professional board in one of these disciplines. Extensive in-house training programs are used to augment education background of the analyst. The group leader of polarized light has received supplemental McCrone Research training for asbestos identification. This report is not covered by the scope ofNVLAP or AJHA accreditation. The State of Colorado requires that the client retain a licensed Colorado State asbestos abatement contractor (General Abatement Certificate, or GAC, holder) to properly remove and dispose of these ACMs. The homeowner is prohibited from removing these ACMs by themselves prior to demolition due to the quantity of ACM exceeding the State trigger level of 160 square feet. The homeowner cannot optout of these requirements. Following complete removal of these ACMs, Phase Con must return to the site to verify that these ACMs are completely removed from the house after which the certified inspector can sign the demolition permit application. Achievement of this Signature necessitates removal of gjl of the ACMs. There cannot be small pieces, debris or dust present from the ACMs or the inspector cannot sign the demolition permit application. The State of Colorado and the EPA consider the wall system as a whole in determining total asbestos content.
