Description of Project: Chevron is seeking approval to install and operate two (2) 14-foot by 60-foot skid-mounted crew quarters to
accommodate up to 12 contract rig employees in support of drilling operations in the Chevron Skinner Ridge Field. The proposed
Temporary Employee Housing would be on site for approximately 4 weeks beginning on or about August 19, 2013 and would
be removed immediately following drilling completion. Please see attached Project description and attachments.

PAYMENT AGREEMENT FORM
GARFIELD COUNTY ("COUNTY") and Property Owner ("APPLICANT") Chevron U.S.A. Inc.

1. The Applicant has submitted to the County an application for the following Project: __ _

2. The Applicant understands and agrees that Garfield County Resolution No. 98-09, as
amended, establishes a fee schedule for each type application, and the guidelines for the
administration of the fee structure.
3. The Applicant and the County agree that because of the size, nature or scope of the
proposed project, it is not possible at this time to ascertain the full extent of the costs
involved in processing the application. The Applicant agrees to make payment of the Base
Fee, established for the Project, and to thereafter permit additional costs to be billed to the
Applicant. The Applicant agrees to make additional payments upon notification by the
County, when they are necessary, as costs are incurred.
4. The Base Fee shall be in addition to and exclusive of any cost for publication or cost of
consulting service determined necessary by the Board of County Commissioners for the
consideration of an application or additional County staff time or expense not covered by
the Base Fee. If actual recorded costs exceed the initial Base Fee, the Applicant shall pay
additional billings to the County to reimburse the County for the processing of the Project.
The Applicant acknowledges that all billing shall be paid prior to the final consideration by
the County of any Land Use Change or Division of Land.

Proposed Minor Temporary Employee Housing Facility -Chevron U.S.A. Inc.
1.0 Introduction
Chevron U.S.A. Inc. (Chevron) has obtained approval from the Colorado Oil and Gas
Conservation Commission (COGCC) for Applications for Permit to Drill (APDs) four ( 4) new
underground injection wells for the disposal of produced water. The site selected for drilling is
an existing location previously used for agricultural purposes and is of adequate size to
accommodate a drilling rig and associated equipment, but does not have large enough area to
accommodate Temporary Employee Housing for drilling crews without significant enlargement
and therefore Chevron is seeking approval to provide Temporary Employee Housing offsite at
another existing location that has been retained for that purpose. The subject location is an
abandoned well site and has previously been permitted and used to accommodate Temporary
Employee Housing. By utilizing two existing locations, Chevron would be able to accomplish
drilling operations without further surface disturbance.
1.1 Project Summary
As detailed in this application, Chevron is seeking approval to install two (2) 14-foot by 60-foot
skid-mounted trailers to accommodate up to 12 contract rig employees during the drilling of four
new underground disposal wells in the Chevron Skinner Ridge Field. The Minor Temporary
Housing Facility (TEH) would be occupied by Ensign Rig# 122 drilling rig crews during their off-shift
hours.
The proposed TEH would be located entirely within Chevron's Skinner Ridge Natural Gas
Development on property owned and operated by Chevron. Chevron owns all surface and
minerals at the subject location (See attached Property Deed). The TEH would be onsite for
approximately four (4) weeks beginning on or about August 26, 2013 and would be removed
immediately following drilling operations. Drilling Rig employees would occupy the TEH in
12-hour shifts such that six (6) individuals would be housed at the facility at any given time.

Minor Temporary Employee Housing Facility
Performance Standards and Enforcement Provisions Sign-Off
(Form to address Section 4-301 (A)(13) and (A)(14))
The Applicant shall adhere to the following Minor Temporary Employee Housing Facility
characteristics and standards required in Article 15 Definitions and Section 7-706 Temporary
Employee Housing Facility, Minor of the Land Use and Development Code (LUDC).
Minor Temporary Housing Facilities shall have all of the following basic characteristics:
A. The Minor Facility and any associated infrastructure must be completely contained within
a State or Federally-permitted parcel (i.e. COGCC-approved oil/gas well pad) in which
reclamation and revegetation are secured with the permitting agency (Permitted Site);
B. The Minor Facility is located at the Permitted Site for less than a cumulative of 1 year;
and
C. The Minor Facility shall have occupancy of 9 to 24 people who are employees,
contractors, or subcontractors of the operator and are supporting the proper execution
and safety of the related commercial, industrial, extraction, or highway operations.
Minor Temporary Housing Facilities shall meet all of the following standards:
A. Federal, State, and Local Laws and Regulations.
Minor Facilities must comply with all applicable Federal, State, and local laws and
regulations.
B. Notification of Facility Installation and Removal.
The Sheriff's Office and relevant fire protection district(s) must be notified at least 24
hours prior to installation and removal of each Minor Facility. The Community
Development Department shall be copied on all such notification, whether hard copy or
electronic.
C. Water Systems.
Water systems shall comply with standards set forth in Section 7-705.B.
D. Wastewater Systems.
Wastewater systems shall comply with standards set forth in Section 7-705.C.
E. Clean, Safe, and Sanitary Condition.
Minor Facilities shall be maintained in a clean, safe, and sanitary condition, free of
weeds and refuse. Any hazardous or noxious materials that must be stored at the Minor
Facility for operational or security reasons must be managed in accordance with all
applicable Federal, State, and local laws and regulations.

F. Trash and Food Storage.
Wildlife-proof refuse containers must be provided for trash. Outdoor food storage is
prohibited unless facilities are provided that prevent the attraction of animals to the Major
Facility site.
G. Fire Protection.
1. Provisions for giving alarm in case of fire and fire suppression must be installed per
fire codes and as required by the fire protection district.
2. Single-station carbon monoxide alarms must be placed in each Manufactured Home
or Recreational Vehicle unit.
H. No Domestic Animals Allowed.
Domestic animals are prohibited.
I. Removal of Facility.
Within 10 days following the expiration or other termination of the Land Use Change
Permit or represented date of removal identified within the Land Use Change Permit, all
housing structures, foundations, and associated infrastructure shall be completely
removed. The Operator shall provide the Director with photos, dated and signed by the
Operator's compliance officer, indicating that all housing structures, foundations, and
associated infrastructure have been removed within the specified timeframe.
I have read and understand the above referenced sections of the LUDC. In addition, I hereby
certify that the aforementioned facility(ies) will be installed in accordance with all applicable
Garfield County, relevant fire district, and federal regulations. Further, I understand that non-
compliance with any of the provisions as outlined in the LUDC shall be enforced in accordance
with Article 12 and any additional enforcement provisions within this code.

3.0 Location Description
The proposed TEH Facility would be situated on an existing Plugged and Abandoned well pad
located approximately 0.6 miles north of the intersection of County Road 204 and County Road
211 and approximately 10 miles south of the active drilling location.
The existing well pad location was constructed and permitted with the COGCC in early 1990 for
Wasatch Formation delineation (Well Name: Chevron Shale WI 1-34; API: 05-045-6682). The
well was subsequently plugged and abandoned in 1991 but the location was not reclaimed at the
time and surface reclamation surety is in effect under the Chevron Blanket Surface Bond: Surety
ID: 1995-0031 (See attached APD and Surety Bond information on file with COGCC). The
abandoned well was cut off at 4-feet below ground level and no above-ground well marker is on
location.
3.1 Legal Description
That portion of Tract 112 situated in the SWNW of Section 34, Township 6 South, Range 98
West, 61h Principal Meridian.
3.2 Site Description
The existing well pad encompasses I. 7 acres and the pad surface is cleared of all vegetation and
has been surfaced with gravel to allow all-weather access. Soil piles and slopes are vegetated
and stabilized. Stormwater management and weed control inspection and monitoring programs
are in place for this location. The site has been previously used for the purpose of Temporary
Employee Housing and the site is of adequate size to accommodate two 14 x 60' trailers,
associated water and septic tanks, and parking for up to 12 personal vehicles, if necessary. No
additional surface disturbance or infrastructure installation would be required to prepare the site
for use.
3.3 Access
The proposed location can be accessed from Interstate 70, exit 62, by traveling north on Roan
Creek Road/Garfield County Road (CR) 204 about 12.5 miles to the intersection of CR 211, then
by traveling northeast on CR 211approximately0.6 miles to the driveway access of the well pad
location (See attached Vicinity Map).

3.4 Site Security
The proposed TEH Facility is located entirely with the Chevron Skinner Ridge natural gas
development and will be occupied by contract employees at all times of the day and night. All
active field operations and locations, including the proposed location, are monitored by Chevron
field personnel around the clock. Access to the location is secured by a locked gate to prevent
public access.
Only Chevron personnel and contractor employees would be allowed on site. No visitors or
guests would be allowed on location.
4.0 Anticipated Dates oflnstallation/Removal
The proposed TEH would be expected to remain on location for approximately four weeks:
Anticipated Installation Date: August 26, 2013
Anticipated Removal Date: September 26, 2013
4.1 Hours of Operation
Typical drilling operations are scheduled for 24 hours a day, 7 days a week. The drilling rig
crews consist of 12 people who work in 12-hour shifts; Six on duty/Six off duty. Under normal
conditions, six ( 6) crew members would occupy the proposed TEH while the other six ( 6) were
working their shifts.
5.0 Housing Unit Description
Mountain West Oil Field Services and Supply (Mountain West), located at 756 Valley Court in
Grand Junction, CO, has been contracted by Chevron to supply and set up two (2) 14-foot by 60-
foot skid-mounted trailers. Each trailer would have two (2) bedrooms, each with three (3) beds,
two (2) baths, and common living and kitchen areas (no catering provided).
Mountain West would also supply and service the vault-n-haul water and wastewater systems.
See below for more detail on these systems.
6.0 Utilities
Utilities necessary for ensuring the safety and comfort of the rig crews include electric, potable
water and wastewater systems.

6.1 Electrical
Electrical service is already installed at the proposed location. The existing power pole and
electrical box are situated on the northeast comer of the well pad (See attached Site Plan).
An underground electric line was installed previously from the electrical box to the west side of
the pad where the proposed housing units would be installed. State Electrical permitting and
hookups would be managed by Mountain West.
6.2 Potable Water, Wastewater, and Trash
See Section 9.0, Standards, for applicable water and wastewater system and trash services
descriptions.
7.0 Emergency Response
See Section 9.0, Standards, for 24-hour Emergency Contact list. Also see attached Emergency
Action Plan.
8.0 Reclamation and Remediation
Chevron intends to retain the subject location for future use and site reclamation and remediation
would not occur until Chevron deemed the location no longer useful in the operation of its
Skinner Ridge Natural Development program.
This location is an abandoned well site and Chevron, as landowner, has elected to retain the site
for current and future use as needed. The use of the existing disturbed site eliminates the need
for new surface disturbance and minimizes the overall surface footprint associated with Chevron
natural gas operations. This location would not be reclaimed upon removal of the Minor
Facility.
Reclamation surety for this location is in effect under the Chevron Blanket Surface Bond: Surety
ID: 1995-0031, on file with COGCC.
9.0 Standards
Garfield County Land Use and Development Code, Section 7-706 Standards for Temporary
Employee Housing Facilities, Minor are included below
A. Federal, State, and Local Laws and Regulations
The proposed Minor Facility would comply with all applicable Federal, State and Local laws and
regulations.

B. Notification of Facility Installation and Removal.
Chevron would notify the Garfield County Sheriffs Office and De Beque Fire Department at
least 24 hours prior to installation and removal of the proposed Minor Facility. The Garfield
County Development Department would be copied on all such notification.
C. Water Systems.
Mountain West would install and service the potable water system as detailed below:
Water System: One (1) 3,440 gallon potable water tank inside metal seabox container. Mountain
West would maintain records of any water testing, both source and onsite, and would make those
records available upon request.
Water Usage: Average water usage is typically listed as 50 to 100 gallons per person per day.
For a 12-man crew, water usage per day would range from 600 gallons per day to a maximum of
l ,200 gallons per day. The water system would have capacity to provide 100 gallons per person
per day for about three (3) days. Actual usage would vary depending on individual crew
members. The system would be serviced every Monday, Wednesday and Friday, and filled as
needed, for the duration of occupancy.

Standard 7-705.B. Water Systems
I. Water systems are compliant with all applicable State and local laws and regulations and
would abide by all standards set forth in Section 7-705.B. of the Garfield County Land Use
and Development Code.
2. This Standard does not apply as the potable water system would be hauled to the site. See #3
below:
3. Mountain West would install and service the potable water systems proposed in this
application.
a. Mountain West would maintain all appropriate records demonstrating water is supplied
from an approved source and would provide those records to Garfield County upon
request.

f. Each of the two wastewater systems is designed for a m1mmum of 75 gallons of
wastewater per person per day or 450 gallons per person (6) per day for each unit. Each
wastewater tank has capacity to service six (6) occupants for about four (4) days. Actual
usage would vary depending on individual crew members. The system would be serviced
every Monday, Wednesday and Friday, and emptied as necessary, for the duration of
occupancy.
E. Clean, Safe, and Sanitary Condition.
The proposed Minor Facility would be maintained in a clean, safe, and sanitary condition at all
times. No hazardous or noxious materials would be used or stored at the Facility.
F. Trash and Food Storage.
One (1) 30-cubic yard wildlife-proof refuse container would be brought onsite to manage
miscellaneous solid waste, refuse and trash. Trash services would be provided by Redi Services,
LLC., 2143 Airport Road, Rifle CO. Trash would be disposed at the Garfield County Landfill.
No food would be stored outdoors and all food wastes would be disposed in the designated trash
receptacle.
G. Fire Protection.
1. Each Housing Unit is equipped with smoke alarms at each of the sleeping areas. ABC type
fire extinguishers are provided with each unit.
Smoking would only be permitted in designated smoking areas at least 35-feet from any
vegetated areas.
Note: Chevron does not permit any employee or contractor to attempt to fight fire beyond
the incipient stage. Fire emergencies beyond incipient stage would be reported to the
DeBeque Fire Department and all employees would be evacuated from the area. See
attached Emergency Action Plan.
2. Each housing unit is equipped with Carbon Monoxide monitors.

provided in the attached Emergency Action Plan.

Wastewater System: Two (2) 1,900 gallon wastewater tanks (one each housing unit) equipped
with beacon light alarms for overflow protection.
System Capacity: Each of the two systems is designed for a minimum of 75 gallons of
wastewater per person per day or 450 gallons per person (6) per day for each unit. Each
wastewater tank has capacity to service six (6) occupants for about four (4) days. Actual usage
would vary depending on individual crew members. The system would be serviced every
Monday, Wednesday and Friday, and emptied as necessary, for the duration of occupancy.
Standard 7-705.C. Wastewater Systems
I. Wastewater systems are compliant with all applicable State and local laws and regulations
and would abide by all standards set forth in Section 7-705.C. of the Garfield County Land
Use and Development Code.
2. Wastewater would be disposed by vault and haul services provided by Mountain West. The
disposal of domestic wastewater is governed by the municipality accepting the waste; no
CDPHE permit is required for disposal. Mountain West would discharge wastewater to the
Clifton Sanitation District Water Treatment System (Mountain West Permit to discharge is
attached):

a. All roads leading to the proposed location, including the pad access, are well maintained
year around by Mesa and Garfield Counties and/or Chevron and are easily accessible 24/7
and 365 days per year. Access to the well pad location is secured by locked gate to
prevent public access. Garfield County Driveway Permit is attached.
b. Mountain West agreement to provide and service the systems is attached
c. Mountain West Oil Field Services would maintain all appropriate records associated with
wastewater maintenance and disposal.
d. Mountain West would maintain all records and would provide those records to Garfield

e. The proposed TEH and associated wastewater system is not expected to be on location for

I. Removal of Facility.
Chevron would remove all housing structures, potable water and wastewater tanks, and trash
bins immediately following completion of drilling activities. Electrical infrastructure would
remain on location for future use.
Upon removal of the facility, Chevron would provide Garfield County Planning and
Development with photographs, signed and dated by Chevron's Compliance Office, Julie Justus,
as evidence that all housing structures have been removed with the specified I 0-day timeframe.

In accordance with the Garfield County Unified Land Use And Development Code,
Chevron U.S.A. Inc. (Chevron) has identified you as an adjacent surface and mineral
property owner and is hereby notifying you that Chevron has filed an application with
Garfield County Community Development to install and operate a Minor Temporary
Employee Housing Facility within 200 feet of public lands as shown on the attached
Garfield County Assessor Map.
The Minor Temporary Employee Housing Facility would consist of one (2) 14-foot by 60-

Hi Nick,
Chevron is applying to Garfield County for a Minor Temporary Employee Housing (TEH)
Facility and I am hereby requesting approval from the DeBeque Fire Department to install
and operate the crew quarters as proposed. I have included a location map and site plan for
your review. A brief project summary is provided below:
Chevron is seeking approval from Garfield County to install two (2) 14-foot by 60-foot skid-
mounted trailers to accommodate up to 12 contract rig employees during the drilling of four
new underground disposal wells in the Chevron Skinner Ridge Field. The Minor Temporary
Housing Facility (TEH) would be located on an existing site that has been used for this
purpose in the past. Each trailer, provided by Mountain West Oil Field Services, would have
two (2) bedrooms, each with three (3) beds, two (2) baths, and common living and kitchen
areas (no catering provided). The crew quarters would be occupied by Ensign Rig # 122
drilling rig crews during their off-shift hours. Smoke alarms, CO monitors, and ABC type
fire extinguishers are installed in each unit.
The TEH would be onsite for approximately four (4) weeks beginning on or about August 29,
2013 and would be removed immediately following drilling operations on or about
September 29, 2013. Drilling Rig employees would occupy the TEH in 12-hour shifts such

Temporary housing permit for Chevron
I have received communication from Julie Justus with Chevron concerning the special use permit for temporary housing
in the Debeque area. I have reviewed the information that she provided concerning the temporary housing permit. I do
not see an issue with this permit request at this time. However, I do have one request. I would request that the Garfield
Communication Center and the Debeque Fire Department be made aware by Chevron of the housing units, their
location, and the on site emergency contact information in case of an emergency.

Applicant submits herewith for the consideration and approval of the Board of County Commissioners, a sketch of the
proposed installation showing all the necessary specification detail including:

General Provisions
1) The applicant represents all parties in interest, and affirms that the driveway approach (es) is to be constructed by
him for the bona fide purpose of securing access to his property and not for the purpose of doing business or
servicing vehicles on the road right of way.
2) The applicant shall furnish all labor and materials, perform all work, and pay all costs in connection with the
construction of the driveway(s). All work shall be completed within thirty (30) days of the permit date.
3) The type of construction shall be as designated ancVor approved by the Board of County Commissioners or their
representative and all materials used shall be of satisfactory quality and subject to inspection and approval of the
Board of County Commissioners or their representative.
4) The traveling public shall be protected during the installation with proper warning signs and signals and the Board
of County Commissioners and their duly appointed agents and employee shall be held harmless against any action
for personal injury or property damage sustained by any reason of the exercise of the Permit.
5) The Applicant shall assume responsibility for the removal or clearance of snow, ice, or sleet upon any portion of
the driveway approach (es) even though deposited on the driveway(s) in the course. of the County snow removal
operations.
6) In the event it becomes necessary to remove any right-of-way fence, the posts on either side of the entrance shall
be surely braced before the fence is cut to prevent any slacking of the remaining fence and all posts and wire
removed shall be turned over to the District Road Supervisor of the Board of County Commissioners.
7) No revisions or additions shall be made to the driveway(s) or its appurtenances on the right-of-way without
written permission of the Board of County Commissioners.
8) Provisions and specifications outlined herein shall apply on all roads under the jurisdiction of the Board of County
Commissioners of Garfield County, Colorado, and the Specifications, set forth on the attached hereof and
incorporated herein as conditions hereof.
9) Final inspection of driveway will be required upon completion and must be approved by person issuing
permit or representative of person issuing permit.
The inspection and sign off must be done prior to any CO from the Building and Planning
Department being issued.

Specifications
I. A driveway approach is understood to be that portion of the county road right-of way between the pavement edge
and the property line that is designed and used for the interchange of traffic between the roadway and abutting
property.
2. At any intersection, a driveway shall be restricted for a sufficient distance from the intersection to preserve the
normal and safe movement of traffic. (It is recommended for rural residence entrances that a minimum
intersection clearance of 50 feet be provided and for rural commercial entrances a minimum of I 00 feet be
provided.)
3. All entrances and exits shall be so located and constructed that vehicles approaching or using them will be able to
obtain adequate sight distance in both directions along the county road in order to maneuver safely and without
interfering with county road traffic.
4. The Applicant shall not be permitted to erect any sign or display material, either fixed or movable, on or
extending over any portion of the county road right-of-way.
5. Generally, no more than one approach shall be allowed any parcel or property the frontage of which is less than
one hundred (100) feet. Additional entrances or exits for parcels having a frontage in excess of one hundred 100)
feet shall be permitted only after showing of actual convenience and necessity.
6. All driveways shall be so located that the flared portion adjacent to the traveled way will not encroach upon , · ..

7. No commercial driveway shall have a width greater than thirty (30) feet measured at right angles to the centerline ·

8. The axis of an approach to the road may be at a right angle to the centerline of the county road and of any angle
between ninety (90) degrees and sixty (60) degrees but shall not be less than sixty (60) degrees. Adjustment will
be made according to the type of traffic to be served and other physical conditions.
9. The construction of parking or servicing areas on the county road right-of-way is specifically prohibited.
Commercial establishments for customer vehicles should provide off-the-road parking facilities.
10. The grade of entrance and exit shall slope downward and away from the road surface at the same rate as the
normal shoulder slope and for a distance equal to the width of the shoulder but in no case less than twenty (20)
feet from the pavement edge. Approach grades are restricted to not more than ten percent (10%).
11. All driveways and approaches shall be so constructed that they shall not interfere with the drainage system of the
street or county road. The Applicant will be required to provide, at his own expense, drainage structures at
entrances and exits, which will become an integral part of the existing drainage system. The Board of County
Commissioners or their representative, prior to installation, must approve the dimensions and types of all drainage
structures.
Note: This permit shall be made available at the site where and when work is being done. A work sketch or
drawing of the proposed driveway(s) must accompany application. No permit will be issued without drawing,
blueprint, or sketch.

The Water Quality Control Division (the "Division") has been informed that the Mountain West Oil Field
Services Hauler obtains water from the Town of Rangely and delivers it throughout the year, for drinking water
purposes. Based on the information, Mountain West Oil Field Services Hauler is being classified as a transient
non-community water system. A transient non-community water system is defined by the Colorado Primary
Drinking Water Regulations (the "Regulations") as a water system" ... that does not regularly serve at least
twenty-five of the same persons over six months per year. The Public Water System ID Number (PWSID#)
assigned to Mountain West Oil Field Services Hauler is C00252516. Please use this number when submitting
sample results and correspondence to this office.
As a transient non-community water system, you are subject to the laws set forth by the above regulations and
must comply with the following:
• The waterworks of the public water system, including sources and treatment processes, require the
approval of the Division. If the design and construction of your existing waterworks have not previously
been approved, submit plans and specifications for your water system to the Engineering Section for
review and approval within 30 days of the date of this letter. A copy of the design criteria and
information regarding plan reviews, including all forms and guidance, can be obtained from the
Department's website at:

If you have questions about the approval process, please contact the Engineering Section at 303-692-
3568. Failure to contact the Engineering Section within 30 days of the date of this letter to develop a
compliance schedule for your system constitutes a violation of the Regulations. Failure to operate
Division approved treatment is subject to formal enforcement, which may include monetary penalties. If
the design and construction of your existing waterworks have previously been approved by the Division,
please provide copies of the approval letter(s).
• All systems that deliver chlorinated water to consumers must monitor disinfectant residuals (chlorine or
chloramines) in the distribution system at the same time and place that total coliform samples are
collected. Residual concentration must be reported with each total coliform sample result.
• Article 1.12 of the Regulations requires that all public water systems submit a "Monitoring Plan" to the
Division within I 0 days following the first quarter of any required monitoring. Systems are also required
to submit any changes related to the monitoring plan to the Division within thirty (30) calendar days
following the effective date of the change.
Also, please refer to the enclosed "DRINKING WATER POLICY (Policy# DWT-8) MONITORING
REOUIREMENTS FOR WATER HAULERS".
The water system must collect and have analyzed at least one total coliform sample each month, from each water
tank/truck used to haul water during that month.
Section I 00.21.1 of Regulation I 00 stipulates that no owner of a water or wastewater facility shall allow the
facility to be operated without the direct supervision of a certified operator in responsible charge. Please contact

Failure to obtain a certified operator within 30 days of the date of this feller constitutes a violation of the
Regulations.

PROGRAM CONTENTS
STATEMENT OF POLICY
Facility and personnel safety will always take precedence over maintaining operation.
Personnel will not attempt to continue working on or remain at a location if the facility
is in an unsafe condition. All personnel are authorized to stop work if an unsafe
condition posing an immediate hazard occurs. All emergency response actions,
processes, and procedures will follow and be conducted in accordance with the
Chevron Way and Chevron Tenets of Operation. No reprimands will be given if the
situation later proves less serious than thought. Hazards of a less serious nature
should be brought to the attention of the supervisor who will assess the situation and
initiate appropriate action.
Under emergency response conditions or drills, employees are expected to act only in
accordance with their training, the Chevron Way, and the Chevron Tenets of
Operation. At no time will any employee place themselves, or allow themselves to be
placed, into a life-threatening situation.
The following priorities will govern our response to any emergency situation:
1. The safety of employees takes precedence over all other considerations.
2. We will minimize impact on the environment once the safety of all personnel
has been secured.
3. We will endeavor to minimize damage to or loss of property and equipment,
keeping safety and environmental concerns at the top of our priority list.
EMERGENCY ACTION PLAN {EAP)
SCOPE
The following plan identifies the most probable risks for emergencies on MCBU Drilling
& Completions {D&C) locations. This plan outlines mitigation steps, including
emergency response (ER) actions to ensure the safety of personnel, and the public, as
well as, to protect the environment and assets.
The scope and application of the EAP covers all operations within MCBU D&C
operational control. This includes, but is not limited to, drilling, completions, workover
and well service operations.
This plan is intended to comply with 29 CFR 1910.38, 119, and Chevron Operational
Excellence (OE) Elements 1 (1.1, 1.3) and 11 (11.2 and 11.3)
PROCEDURES
The following section identifies the most probable risks within D&C operations. For
these risks; mitigation steps, including identified emergency responses actions, are
outlined in this section for all operations within MCBU D&C operational control.
Site Security and Access
D&C Field Operations: To access the field sites or pads, all employees,
contractors, and visitors must comply with orientation requirements. MCBU
employees must have completed MCBU HES Boot Camp; contractors must
have completed SafeLand training from an accredited provider; visitors will
receive site-specific orientation on location. Upon arrival, employees,
contractors, and visitors must comply with site signage and stop at the
entrance to location.

1. Drilling or completions sites: All personnel entering a drilling or
completions location must immediately report to the Field Safety
Representative's office (located at the entrance). All personnel must
receive site-specific orientation and sign in with the Field Safety
Representative prior to proceeding onto the location.
2. Workover or well service sites: All personnel entering a
workover/well service location must remain at the entrance until
greeted by the Well Site Manager (WSM) or Field Safety
Representative. All personnel must receive site-specific orientation
and sign in with either the WSM or the Field Safety Representative
prior to proceeding onto the location.

All personnel entering D&C locations must wear the proper Personal
Protective Equipment (PPE). The authorized PPE is:
1. Hard Hat
2. Fire Retardant Clothing (FRC)
3. Safety Glasses
4. Steel Toed Safety Footwear
5. Hearing Protection (in identified areas of location)
6. Personal H2S monitor (in known H2S areas/wells)
Site Safety Orientation
D&C Field Operations: The MCBU Drilling & Completions Location Brief will be
given to all new employees, contractors, and visitors upon arrival. This
orientation includes information on current/upcoming work in progress, PPE,
alarms and emergency procedures, motor vehicle safety, appropriate
certifications for tasks, E-Colors, and Tenets of Operation. Any Chevron
delegate on site can give the orientation, but normally the Field Safety
Representative will conduct the orientation. Completion of the orientation is
documented via the location sign-in sheet. Workers with less than six months
of service in the same job/position or with his/her present employer will be
considered a Short-Service Employee (SSE) unless he/she is granted
exception by the site manager. SSE Exception documentation must be
completed. Contractor SSEs must be assigned an experienced mentor.
Chevron SSEs will wear orange hardhats. The MCBU Contractor Handbook
details further requirements for the SSE program.

Notification of an emergency situation may be given using either of the

location sign-in sheet and rig crew roster. Emergency muster points (primary
and alternate) are established upon initial arrival on location and noted in
pre-job meetings and during site-specific orientations. In the event of an
emergency, personnel will evacuate to the designated muster point where a
headcount will be conducted to ensure all are accounted for.

All entrants to location are required to sign a logbook upon entering the field
office. They also note the time of their arrival and departure from the facility.
In the event of an emergency, Chevron personnel will use the logbook, verbal
communication and cell phones at the muster point to account for all
personnel. If necessary, available personnel may initiate a search for any
missing person as long as it is safe to do so.
The Drill/Well Site Manager (or designee) is designated to perform a final
location/building check to ensure all areas have been evacuated. This
individual will then remove the sign-in sheet and report to the muster
point.
All-Clear Signal: When employees have been evacuated from the building, it
may only be re-entered after it has been determined to be safe to do so by
the senior Chevron person onsite. This may be done in consultation with local
emergency responders or Senior Chevron Management as appropriate. The
senior Chevron person onsite shall communicate the all-clear signal verbally
to all affected employees.
Site evacuation drills will be conducted in accordance with local fire codes on
a weekly basis.
Shelter in Place
Chemical contaminants may be released into the environment in such
quantity and/or proximity to a building or facility that it is safer to remain
indoors rather than to evacuate employees. To "shelter-in-place" means
selecting an interior room or rooms within the facility, or ones with no or few
windows, and taking refuge there. Quickly close exterior doors, windows, air
vents, and dampers. Turn off all fans, heating and air conditioning systems,
and clothes dryers. Some systems automatically provide for exchange of
inside air with outside air. These systems, in particular, need to be turned off,
sealed, or disabled. It is ideal to have a hard-wired telephone in the room(s)
you select. Call emergency contacts and have the phone available if you need
to report a life-threatening condition. Cellular telephone equipment may be
overwhelmed or damaged during an emergency. Write down the names of
everyone in the room and pass along to the emergency contact to report who
is in the room with you, and their affiliation with the facility (employee,
visitor, contractor). Listen to the radio, watch television, or use the Internet
for further instructions until you are told all is safe or to evacuate. Local
officials may call for evacuation in specific areas at greatest risk in your
community.
Personnel on location shall follow the procedures noted below in the
subsequent sections for evacuation in emergency situations.

The following are utilities and/or shutdown procedures for equipment and
machinery within D&C locations. Contractor shutdown procedures will be
followed for contractor equipment.
To minimize the dangers associated with an emergency, any trained
employee may shut off the following energy sources while evacuating.

Well Control Emergencies
The plan involves three types of well control emergencies classified as Level
1, Level 2 and Level 3 depending on the seriousness of the incident. Refer to
the Chevron Well Control Contingency Plan for comprehensive direction on
well control detection, response, and return to normal operations.
A Level 1 incident involves an uncomplicated well control situation that
requires handling procedures by the personnel at the wellsite. These
situations are not all necessarily well control events. Situations that may lead
to a well control event are also included in this category. Standard well
control techniques are used to deal with kicks and wellbore pressure including
the Wait-and-Weight (or Engineer's) Method and the Driller's Method. The
Chevron On-Scene Supervisor at the wellsite and the Drilling Management
and Staff are trained in these methods and are familiar with these techniques
and procedures.

The criteria for a Level 1 event are as follows:
• Drilling kick less than 1.0 ppg intensity
• Drilling kick smaller than 25 bbls
• Swabbed in kick smaller than 25 bbls
• Loss of one well control barrier during normal operations mode (i.e. no
kick in the well). Examples of this situation include the following during
drilling or completion operations:
o BOP or wellhead component pressure test failure when open
hole or perforations are present
o Mild lost circulation
o Valve (BOP or wellhead) leak
• Unable to kill live well to start workover
• Hole in surface/intermediate/production casing strings
• Tripping with complication (plugged drilling, completion, or work string,
etc.) and no kick in the well
A Level 2 incident involves a complication(s) of some type that requires
unconventional or unfamiliar procedures by the on-scene personnel. The
Chevron engineering and in some cases the management staff will be
involved in the technical solution to the situation. Cudd Well Control should
be contacted for consultation and mobilization if necessary. Non-standard
well control methods are usually required to resolve the situation in a Level
2 event.
The criteria for a Level 2 event are as follows:
• Drilling kick greater than 1.0 ppg intensity
• Drilling kick greater than 25 bbls
• Swabbed kick greater than 25 bbls
• Severe lost circulation defined as losses greater than 60 bbl/hr
• Kicks while tripping, fishing, logging, or kicks with pipe out of the hole
• Secondary BOP or wellhead equipment leaks
• Lower pipe rams
• Wellhead outlet valve
• Mechanical failure of BOP, wellhead, or related equipment
• Leaking liner tops
• Kicks with plugged working string (drilling or completion operation)
• Sustained pressure on protective casing
• Kicks while running casing or cementing
• Any complication experienced during live well operations involving
snubbing, coiled tubing, or wireline equipment
Level 3 situations are those that present serious risks to personnel, the
environment, and assets. These situations require the immediate application
of specialized techniques and well-developed safety assessment and hazard
mitigation programs. Specific examples of Level 3 scenarios include:

• Surface blowout (regardless of severity, with or without fire)
• Underground blowout (UGBO)
• Pressure beyond the rating of the well control equipment
• Other situations that constitute a clear and present danger to
personnel, environment or equipment that cannot be resolved by
conventional means.
Fire/Explosion
Detection and Notification
Fires or other emergency situations should be verbally reported to the
Chevron person in charge. Where appropriate, 9-1-1 should also be called to
initiate public emergency response resources. The Chevron Drill or Well Site
Manager will follow the internal incident reporting procedures to notify MCBU
D&C Leadership and Emergency Management assistance, if necessary.
Response
If the fire is still in the incipient stage, personnel who are trained in the use of
a handheld fire extinguisher may attempt to extinguish the fire. Once the fire
has grown past the incipient stage, all personnel will retreat to a safe distance
to ensure their own safety and wait for the responders. Personnel will only
respond to fire if they are trained to do so and will not jeopardize their health
and safety in the process.
Return to Normal Operations
When the fire is extinguished and the site deemed safe to enter, the site will
be secured pending an incident investigation. Lessons learned from the
incident will be incorporated in existing process and will be shared with the
other MCBU and Chevron operations. Office activities will be moved to
another MCBU office.
If it is determined the site will return to service, the normal repair and
maintenance processes will be utilized to repair and/or replaced equipment on
site.
Gas Release
Detection and Notification
A significant gas release will be verbally reported to the Chevron person in
charge. Where appropriate, 9-1-1 should also be called to initiate public
emergency response resources. The Chevron Drill or Well Site Manager will
follow the internal incident reporting procedures to notify MCBU Leadership
and Emergency Management assistance, if necessary.

Response
Personnel will only respond to gas releases if they are trained to do so and
will not jeopardize their health and safety in the process. In D&C field
operations, the appropriate personnel may be the Drill or Well Site Manager,
Rig Manager or crew, or Field Safety Representative. Response actions can
include securing valves, either on-site or remotely, allowing the gas to safely
vent to the atmosphere, and/or evacuating the site and surrounding area.
Prior to entering, the area should always be evaluated for respiratory
protection requirements.
Return to Normal Operations
When the release is secured and the site deemed safe to enter, the site will
be secured pending an incident investigation. Lessons learned from the
incident will be incorporated in existing process and will be shared with the
other MCBU and Chevron operations.
H2S Release
Per the MCBU H2S Safety Program, D&C Field Operations will develop
operation-specific H2S Contingency Emergency Response Plans. These plans
will address any unique conditions or situations that are present during
operations. The Drill or Well Site Manager, HES, engineering or any other
persons knowledgeable of the wellsite should perform an onsite evaluation of
the location to determine if the potential for H,S exposure may be present
and encroach from nearby facilities or buried lines. This information should be
added to the H,S Contingency Plan as necessary. If the operations of nearby
operators' pipelines or buried cables could be impacted by the drilling project
or affect the location, those companies should be contacted to obtain
emergency response contact numbers and place those emergency contact
numbers in the Emergency Response Plan. Also, the location of any valves,
switches or shutoff devices that may reduce exposures to the site during an
emergency should be added to the H,S Contingency Plan.
The Chevron Drill or Well Site Manager will follow the internal incident
reporting procedures to notify MCBU Leadership and Emergency Management
assistance, if necessary. Personnel will only respond to H2S releases if they
are trained to do so and will not jeopardize their health and safety in the
process. In D&C field operations, the appropriate personnel may be the Drill
or Well Site Manager, Rig Manager, or Field Safety Representative. Refer to
the operation-specific H2S Contingency Emergency Response Plan for further
detail.

Severe Weather
Types of severe weather events that can occur on or near D&C locations
include, but are not limited to: tornado, high winds, low visibility due to
blowing dust or fog, hail storm, extreme cold weather, ice or winter storm,
and extreme warm weather or heat wave.
Response
Any response to severe weather is to protect the health and safety of
personnel. For most severe weather events requiring shelter-in-place, field
equipment may remain operating unless it will create a hazard to do so. If
weather warrants, calmly shelter in the personnel trailers, or if safe to do so,
evacuate to a nearby FMT office.
Return to Normal Operations
When the severe weather ceases and the site is deemed safe to enter, the
site will be secured pending damage assessment. Lessons learned from the
incident will be incorporated in existing process and will be shared with the
other MCBU and Chevron operations.
If it is determined the site will return to service, the normal repair and
maintenance processes will be utilized to repair and/or replace equipment on
site.
Medical Emergency / Man-Down Procedures
Types of medical emergencies that can occur on or near D&C locations
include, but are not limited to: illness, confined space rescue, entrapment,
and injured personnel at height.
Response
Any response past basic first aid will be conducted by local emergency
services or contractor approved physician/clinic. Man-down response will be
completed by contractors' man-down procedures. If the incident occurs while
in the derrick, contractors' rescue procedures will be followed.
Return to Normal Operations
Once the medical emergency has been mitigated and the site deemed safe to
enter, the site will be secured pending an incident investigation. Lessons
learned from the incident will be incorporated in existing process and will be
shared with the other MCBU and Chevron operations.
If needed, employee assistance will be requested to help with any emotional
issues that may arise after a medical emergency on-site.

Security Breach
There are various types of security breaches that can occur within the D&C
field operations. They include vandalism, bomb threat, sabotage, disgruntled
employee, unwanted visitors, and theft. For almost all cases, the local law
enforcement authorities will handle response efforts. Personnel may assist in
this effort if it does not place them in harm's way. Personnel will fully
cooperate with requests for information from law enforcement personnel
during a security breach response.
Detection and Notification
Any unauthorized site entry will be verbally reported to the Chevron person in
charge. When appropriate, 9-1-1 should also be called to initiate public
emergency response resources.
Response
Depending on the security breach situation, personnel safety is paramount.
Any response to the security breach will be handled by the local law
enforcement authorities.
Return to Normal Operations
Until local law enforcement authorities have determined the location safe for
re-entry, personnel will not return to operations. Only after an "all clear" is
issued by authorities, personnel may return to work.
Spills
Detection and Notification
All spills will be verbally reported to the Chevron person in charge. If
necessary, 9-1-1 should also be called to initiate public emergency response
resources. The Chevron Drill or Well Site Manager will follow the internal
incident reporting procedures to notify MCBU D&C Leadership and Emergency
Management assistance, if necessary.
Spill Response Plan (OSRP)
Upon recognition of a spill, first determine the material being released. If safe

close valve, etc.). Personnel trained to contain the spill/release will do so. The
site will then be secured to prevent unwanted entry and the spill/release will
be reported per the Chevron D&C notification procedures. The H2S
Contingency Plan will be activated if necessary. If emergency evacuation is
warranted, follow evacuation and mustering procedures listed within this
document.
Spills of significant volume may require contractor assistance for containment
and clean-up. Refer to the MCBU D&C Waste Management and SPCC Plans for
further guidance.

Return to Normal Operations
When the spill is secured/cleaned-up and the site is deemed safe to enter, the
site will be secured pending an incident investigation. Lessons learned from
the incident will be incorporated in exiting processes and will be shared with
the other MCBU and Chevron operations. Any damage at the location will be
repaired and the site will return to service, if it is deemed appropriate.
Average Most Probable Discharge (AMPD) Spill Scenario:
The most probable discharge scenarios within D&C Field Operations are
release of produced water or brine to land from a breach in the flowline or pit
or overflow while filling equipment.
Probable steps that lead to discharge:
1. Improper flowline connection
2. Worn flowline
3. High flow from discharge directed at liner seam
4. Improper equipment filling technique
Response chain of events:
1. Turn off equipment causing discharge
2. Contain spill
3. Arrange for contract clean-up crew (vacuum trucks) if necessary
4. Follow Chevron D&C notification procedures
D&C EMERGENCY MANAGEMENT ORGANIZATION
Chevron utilizes a standard, National Incident Management System (NIMS) compliant,
response team organization to respond to emergencies at their facilities and sites.
This team is led by an Incident Commander, who is supported by a Safety Officer,
Operations Section Chief, Planning Section Chief, and a Logistics and Finance Chief.
This team is employed for significant emergency events or incidents to provide
guidance and clear communication paths for all response team personnel involved.

D&C Emergency Response Team Roles & Responsibilities
Incident Commander -The Incident Commander's responsibility is the
overall management of the incident and the ensuring that operations are
carried out safely, effectively, and efficiently.
Safety Officer -The Safety Officer's function is to develop and recommend
measures for assuring personnel safety and to assess and/or anticipate
hazardous and unsafe situations.
Operations Section Chief -The Operations Section Chief is responsible for
the management of all operations directly applicable to the primary, response
mission.
Planning Section Chief -The Planning Section Chief is responsible for the
collection, evaluation, dissemination and use of incident information and
maintaining status of assigned resources. Information is needed to: 1)
understand the current situation; 2) predict the probable course of incident
events; 3) prepare alternative strategies for the incident; and 4) submit
required incident status reports.
Logistics and Finance Chief -The Logistics and Finance Section Chief is
responsible for providing facilities, services, and material in support of the
incident.

On-site Response Team (ORT) -The primary responsibilities of an ORT is
to ensure at-the-scene tactical response operations are carried out in a safe,
well-organized, effective fashion and identify, organize, and manage the work
that must be done to address or mitigate incident-related problems.
Muster/Shelter Area Managers -Responsible for managing a Muster Area
during mustering operations, including: taking a head count, determining
condition of mustered personnel, communicating with and receiving directions
from the Operations Section Chief.
Wardens -Responsible for assisting in the movement of personnel to a

D&C INCIDENT COMMAND CENTER LOCATIONS {ICCs)
In the event of an emergency or event requiring the activation of the D&C Field
Emergency Management Team (FEMT), members will meet in the follow locations to
manage the incident, receive information and direct Emergency Operations. The
Incident Commander(IC) and his/her EM staff will not direct activities from the field.
Incident conditions could dictate location of the ICC.
The following locations have been designated for ICCs:
Primary ICC: HOU140/43066 or MID/4305
Alternate ICC: Closest FMT Office
D&C EMERGENCY RESPONSE TRAINING & EXERCISES
Training
D&C FEMT personnel complete annual emergency response/management
training. The training topics for this annual training vary between numerous
emergency management and response topics. Topics may include, but are
not limited to:

Fire Extinguisher Training
Security
Confined Space Rescue
First Aid I CPR
Spill Response Training
The Chevron Learning Management System (LMS) is utilized to document and
track annual training requirements. A copy of the roster is submitted to the
MCBU Emergency Management/Response Specialist for tracking and
verification purposes. The MCBU Emergency Management/Response
Specialist will audit 100% of the submitted records yearly to ensure 100% of
all MCBU FEMT have completed their required yearly emergency
management/response training requirement. LMS is utilized to track these
reviews and participants.

Exercises/Drills
D&C personnel conduct an emergency response/management tabletop
exercise once a year in addition to any other regulatory required or mandated
exercises or drills. For the annual emergency response/management tabletop
exercise, local emergency response services (local police, fire, and EMS) are
invited to attend and participate. This is in addition to any other interaction
with local emergency services that might occur within the year.
Documentation of the annual tabletop exercise and participants is recorded in
LMS and a copy of the roster is submitted to the MCBU Emergency
Management/Response Specialist for tracking and verification purposes. The
MCBU Emergency Management/Response Specialist will audit 100% of the
submitted records yearly to ensure 100% of all MCBU FEMTs have completed
their required yearly emergency management/response tabletop exercise
requirement. LMS is utilized to track these reviews and participants.
CONTINUOUS IMPROVEMENT
The D&C HES Team will review this EAP of any changes, error and/or revisions at
least once every year. Documentation of this review, along with a new or current copy
of the EAP, will be submitted to the MCBU Emergency Management/Response
Specialist for tracking and verification purposes. The MCBU Emergency
Management/Response Specialist will audit 100% of the submitted records yearly to
ensure 100% of all MCBU FEMTs EAPs have completed their required yearly HES
review. When a current copy of the D&C FEMT EAP is received by the MCBU
Emergency Management/Response Specialist, he/she will save a copy of the plan to a
central folder on the Chevron "O" Drive and upload the document to the Chevron
MCBU Emergency Management website.
Reviews are conducted with employees covered by the plan yearly. Documentation of
each review is contained in LMS and a copy is submitted to the MCBU Emergency
Management/Response Specialist for tracking and verification purposes. The MCBU
Emergency Management/Response Specialist will audit 100% of the submitted
records yearly to ensure 100% of all MCBU FMT's have completed their required
yearly EAP review. LMS is utilized to track these reviews and participants.
Other reviews of this EAP may take place in addition to the yearly review. These
additional reviews include:
1. Upon employee's initial assignment to D&C, employees will receive training on
this plan.
2. Whenever the employee's responsibilities or designated actions under the plan
change.
3. Whenever the plan is changed.

l. The constituent business corporations participatins in the merger herein certified
are:
(i) Geuy Oil Exploration Company, which is incorporated under the laws of the
State of Delaware; and
(ii) Chevron U.S.A. Inc., which is incorporated under the laws of the State of
Pennsylvania.
2. An Agreement of Merger has been approved, adopted, certified, executed and
acknowledged by each of the aforesaid constituent corporations in accordance with the provisions
of subsection (c) of Section 252 of the General Corporation Law of the State of Delaware, to wit, by
Getty Oil Exploration Company in the same manner as is provided in Section 251 of the General
Corporation I.aw of the State of Delaware and by Chevron U.S.A. Inc. in accordanco with the Jaws
of the State of its incorporation.
3. The name of the surviving corporation in the merger herein certified is Chevron
U.S.A. Inc., which will continue its existence as said surviving corporation under the name
Chevron U.S.A. Inc. upon the effeclive dale of said merger pursuant to the provisions of the laws of
1he State of its incorporation.
4. The certificate of incorporation of Chevron U.S.A. Inc., as now in fo1te and
effect, shall continue to b!l the certificate of incorporation of said surviving corporation until
amended and changed pwsuant to the provisions of the Jaws of the State of i is incoij)Oration.
5. The executed Agreement of Merger between the aforesaid constituent
corporations is on file at an office of the aforesaid surviving corporation at: 575 Market Street, San
Francisco, CA 94105.

6. A copy of the nforesaid Agreement of Merger will be furnished by the aforesaid
surviving corporation, on requesl, and withcut cost, to any stockholder of each of 1he aforesaid
constituent corporations.
7. The aforesaid surviving coiporation does hereby agree that it may be served with
process in :he State of Delaware in any proceeding for enforcement of any obligation of Getty Oil
Exploration Company, as well as for enforcement of MY obligation of said surviving corporation
arising from the merger herein certified, including any suit or other proceeding to enforce the right,
if any, of any ~tockholder of Getty Oil Explomtion Company us tlelennined in appnlisal
proceeding.1 pursuant to the provisions of Section 262 of the General Corporation Law of the State
or Delaware; does hereby irrevocably appoint the Secretary of State of the State of Delaware as its
agent to accept service of process in any such suit or other proceedings; and does hereby specify the

That this deed is made effective as of the 31st day of Oecomber, 1984 by and
between Gettr, Oil Company, a Delaware corporation,. (hereinafter referred to
"' "Granter' l and Getty Oil Exploration Company, a Delaware corporation
(hereinafter referred to as 11Grantee 11}.
W I T N E S S E T H:
WHEREAS, Grartor is the owner and/or holder of those certain shale oil prop-
erties, and lands in Mesa ond Garfield Counties, Colorado, described in
Exhibits "A 11
and 11
811 attached hereto and by this reference made a part hereof
{hereinafter collectively referred to as the 11Properties 11
}; and
NOW THEREFORE, for and in consideration of one dollar and other good and
valuable considerations, the receipt of which is hereby acknowledged, Granter
does hereby quitclaim, bargain, sell and convey unto Grantee, all of
Grantor's right, title znd interest in and to the Properties, together with
all improvenents situated thereon ar.d all water rights, permits and
applications, and reservoir and ditch rights, appertaining or belonging
thereto or used in connection therewith, and other appurtenances thereunto
belonging.
Grantee does hereby accept thts deed subject to easements, rights of ways,
exceptions and any and all reservations appearing of record affecting any of
the Properties and/or rights granted.
To have and to hold the same together with all and singular the appurtenances
thereunto belonging or In anywise appertaining to Grantee, and all the
estate, right, title, claim and demlnd whatsoever, of the Grantor, either in
law or equity, of, in and to the Properties, together with all improvements
situated thereon and all water rights, permits and applications, and
reservoir and ditch rights, apper;alntng or belonging thereto or used in
connection therewith, and other appurtenances thereunto belonging.
This deed shall extend to, be binding upon, and inure to the benefit of the
successors, and assigns of Grantee.

Permit for Temporary Employee Housing, all as more specifically set out in the Garfield
County's Building and Planning Department report dated August 13, 2007.

faithfully perform its duties under the terms of the permit, then this obligation shall be null and
void, otherwise to remain in foll force and effect.

Description and Location
The discharge shall be made into the wet well located at 3209 D Road inside the
building as shown in Attachment I.
Waste Description: The Permittee is authorized to discharge wastewater that is solely domestic waste.
The domestic waste is generated from the temporary living quarters of gas and oil field personnel. The
wastewater is generated through the use of showers, sinks, toilets and washing machines. _It is collected
on site in a holding tank that is pumped by the Permittee and brought to the discharge location.
II. EFFLUENT MONITORING AND LIMITATIONS
A. Dilution Prohibition
The Permittee shall not increase the use of clean water or process water or, in any way, attempt
to dilute a discharge as a partial or complete substitute for adequate treatment to achieve
compliance with any applicable limitation, standard or requirement.
B. Specific Effluent Limitations
I. Effective immediately, the Permittee shall not discharge wastewater containing any of the
materials and substances in excess of the concentrations listed in the Table below. The
Permittee shall monitor at the frequency shown.

I. GENERAL PROJECT DESCRIPTION
The Applicant (Chevron) seeks a Land Use Permit for
the Skinner Ridge Minor Temporary Housing facility
(TEH) to house 9 to 24 personnel for the purpose of
natural gas drilling operations. Chevron obtained
approval from the Colorado Oil and Gas Conservation
Commission (COGCC) for four new underground
injection wells for the disposal of produced water. The
site selected for drilling cannot accommodate TEH for
drilling crews without significant enlargement and
disturbance and therefore employees are proposed to
be housed off-site.
The Applicant has represented that the facility will be
on-site for less than one year, will have water hauled
to the site, and a vault-and-haul scenario for handling
sewage and wastewater.
II. LOCATION AND SITE DESCRIPTION
The TEH facility will be situated on an existing 1.7
acre well pad (Chevron Shale W11-34, API: 05-045-
6682) located approximately 0.6 miles north of the
intersection of County Roads 204 and 211 and approximately 10 miles south of the active drilling
location. The site has access to County Road 211 by a private driveway.
The proposed facility will be located entirely within Chevron property and will be operated by
Chevron. The Applicant owns all the surface and minerals of the subject site. The facility will be on-
site for approximately four weeks; commencing when a County permit is issued. The employees will
occupy the facility in 12 hour shifts (six on duty/six off duty) such that six individuals will be housed at
the facility at any given time.
The site will contain the following:
• Two 14 foot by 60 foot trailers;
• One 3,440 gallon water tank;
• Two 1,900 septic tanks; and,
• 30 cubic yard dumpster.

Housing Facility: The Applicant represents that Mountain West Oil Field Services and Supply
(Mountain West) will supply and set up the two 14' x 60' skid-mounted trailers on-site. Each trailer
will have a common living and kitchen area, two bedrooms (each with three beds), and two
bathrooms.
Estimated Dates of Operation: The facility is anticipated to be on-site September 10, 2013 and
removed on September 24, 2013.

Water:
The Applicant is proposing one 3,440 gallon potable water tank that will be stored inside a metal sea
box container. The potable water will be hauled to the facility by Mountain West who is a certified
potable water hauler registered with the Colorado Department of Public Health and Environment
(Permit No. CO 0252516). Mountain West will install and service the potable water system and
maintain records of any water testing (source and on-site) and will make those records available
upon request.
Mountain West will obtain water from an approved water source. The primary and secondary
water sources the Applicant proposes are:
• Primary: Ute Water District located at 20 Road Fill Station, 1275 20 Road, Grand
Junction, CO
• Secondary (if necessary): City of Grand Junction, Fill .Station at City Shops located at 333
West Avenue, Grand Junction, CO
The Applicant has provided a letter from Mountain West stating that they will provide potable water
hauling services for Chevron.
Sewage and Wastewater: The facility will have two 1,900 gallon tanks, one for each trailer, equipped
with beacon light alarms for overflow protection. Each of the two systems is designed for a
minimum of 75 gallons of wastewater per person per day or 450 gallons per person six per day for
each unit. Each wastewater tank has capacity to service six occupants for about four days. Actual
usage will vary depending on individual crew members. The system will be serviced every Monday,
Wednesday, and Friday, or emptied as necessary, for the duration of occupancy. The wastewater
will be disposed by vault and haul services provided by Mountain West and disposed at the Clifton
Sanitation District located at 217 D Road, Grand Junction, CO (NPDES Permit C00033791).
The Applicant has provided a letter indicating that Mountain West will provide the sewage hauling
service for this facility.
Solid Waste/Trash: One 30 cubic yard wildlife proof refuse container will be brought on-site to
manage miscellaneous solid waste, refuse and trash. Trash services will be provided by Redi
Services, LLC located at 2143 Airport Road, Rifle, CO. Trash will be disposed atthe Garfield County
Landfill. No food will be stored outdoors and all food wastes will be disposed in the designated trash
receptacle.
No "Will Serve" from Redi Services, LLC was provided in the application. As a condition of
approval, staff recommends that this be provided prior to the issuance of a Land Use Change
Permit.
Fire: Each trailer is equipped with smoke alarms at each of the sleeping areas. ABC type fire
extinguishers are provided with each unit. Smoking is only permitted in designated smoking areas at
least 35 feet from any vegetated areas. Each housing unit is equipped with carbon monoxide
monitors.
Utilities: Electrical service is provided by an existing power pole and electrical box situated at the
northeast corner of the well pad. An underground electric line was installed previously from the
electrical box to the west side of the pad where the proposed housing units will be installed. State
electrical permitting and hookups will be managed by Mountain West.

Ill. ZONING AND ADJACENT LAND USES
The subject property and Permitted Site are zoned Resource Lands -Gentle Slopes and are
adjacent to other parcels that are zoned Resource Lands and Public Lands. Surrounding land uses
include natural gas development, agriculture, and wildlife.
IV. REFERRAL AGENCY COMMENTS
The application was referred to the following County Departments for their review and comment as
per Section 4-301 (A)(4) and notification was given to all adjacent property owners and mineral
owners of the proposed application as per Sections 4-103 (B)(2)(c), 4-103 (8)(3). Comments
received are noted below.
• Garfield County Sheriff -No issues with the application.
• DeBeque Fire Protection District -No issues with the application.
• Bureau of Land Management -No comments received.
V. REVIEW STANDARDS & STAFF COMMENTS
The Minor Temporary Employee Housing application is being processed in accordance with Section
4-107, Minor Temporary Housing Facility and Section 4-103, Administrative Review of the Land Use
and Development Code (LUDC). The criteria used to review the application are set forth in Section
7-706.
Section 7-706 Temporary Employee Housing Facilities, Minor
A. Federal, State, and Local Laws and Regulations
Staff Comment: The Applicant states they will adhere to this section of the LUDC.
B. Notification of Facility Installation and Removal
Staff Comment: The Applicant will notify the Garfield County Sheriff's Office and DeBeque
Fire Department at least 24 hours prior to installation and removal of the proposed Minor
Facility. The Garfield County Development Department will be copied on all such notification.
C. Water Systems
Staff Comment: The Applicant indicates as follows:
1. The water system will be compliant with all applicable State and local laws and
regulations and abide by all standards set forth in Section 7-705.B. of the Garfield
County Land Use and Development Code.
2. Not applicable.
3. Mountain West will install and service the potable water system at this facility.

"Will Serve" letter in application. The Clifton Sanitation District will receive the
wastewater.
a. All roads to the proposed location, including the pad access, are well maintained
year around by Mesa and Garfield Counties and/or Chevron and are easily
accessible 24/7 and 365 days per year. Access to the well pad is secured by locked
gate to prevent public access. A Garfield County Driveway Permit can be found in
the application.
b. Mountain West agreement to provide and service the systems is found in the
application.
c. Mountain West will maintain all records associated with wastewater maintenance
and disposal.
d. Mountain West will maintain all records and provide those records to Garfield County
upon request.
e. The proposed TEH and associated wastewater system is not expected to be on
location for approximately four weeks. In no case shall the TEH be on-site for more
than one year.
f. This is addressed in Section II of this report.
E. Clean, Safe, and Sanitary Condition
Staff Comment: The Applicant indicates that the facility will be maintained in a clean, safe,
and sanitary condition at all times. No hazardous or noxious materials would be used or
stored at the site.
F. Trash and Food Storage
Staff Comment: This is addressed in Section II of this report.
G. Fire Protection

H. No Domestic Animals Allowed
Staff Comment: No domestic animals will be permitted on-site.
I. Removal of Facility.
Staff Comment: The Applicant will remove all housing structures, potable water and
wastewater tanks, and trash bins immediately following completion of drilling activities.
Electrical infrastructure will remain on location for future use.
Upon removal of the facility, Chevron will provide Garfield County Community Development
Department with photographs, signed and dated by Chevron's Compliance Office (Julie
Justus) as evidence that all housing structures have been removed with the specified 10 day
timeframe.
VI. RECOMMENDED FINDINGS
Should the Community Development Department Director approve the request for a Minor
Temporary Employee Housing, staff suggests the following findings:
1. That the proper public notice was provided in accordance with Section 4-103 of the Land
Use and Development Code.
2. That the review of this application was extensive and complete, that all pertinent facts,
matters and issues were submitted or could be submitted and that all interested parties had

an opportunity to be heard before the given Director's Determination approval date of August
30, 2013.
3. That for the above stated and other reasons the proposed amended final plat are in the best
interest of the health, safety, convenience, order, prosperity, and welfare of the citizens of
Garfield County.
4. That with the recommended conditions, the application will be in general conformance with
the 2030 Comprehensive Plan.
5. That with the recommended conditions, the application will adequately meet the
requirements of the Garfield County Land Use and Development Code, as the same has
been amended at the time the application was deemed by the Community Development
Department to be technically complete.
IV. STAFF RECOMMENDATION
Staff supports the finding that the Minor Temporary Employee Housing satisfies the
requirements and standards of the Garfield County LUDC and recommends that the Director of
Community Development approve the requested Skinner Ridge Minor Temporary Employee
Housing with the following conditions:
1. That all representations of the Applicant shall be considered conditions of approval unless
amended herein.
2. The Minor Facility shall comply with all applicable Federal, State, and local laws and
regulations.
3. Prior to the issuance of a Land Use Change Permit, the Applicant shall provide to the
Community Development Department a "Will Serve" letter from the hauler that picks up and
disposes of the solid waste/trash for the facility.

6) In the event it becomes necessary to remove any right-of-way fence, the posts on either side of the entrance shall
be surely braced before the fence is cut to prevent any slacking of the remaining fence and all posts and wire
removed shall be turned over to the District Road Supervisor of the Board of County Commissioners.
7) No revisions or additions shall be made to the driveway(s) or its appurtenances on the right-of-way without
written permission of the Board of County Commissioners.
8) Provisions and specifications outlined herein shall apply on all roads under the jurisdiction of the Board of County
Commissioners of Garfield County, Colorado, and the Specifications, set forth on the attached hereof and
incorporated herein as conditions hereof.
9) Final inspection of driveway will be reqnired upon completion and must be approved by person issuing
permit or representative of person issuing permit.
The inspection and sign off must be done prior to any CO from the Bnilding and Planning
Department being issued.

Specifications
I. A driveway approach is understood to be that portion of the county road right-of way between the pavement edge
and the property line that is designed and used for the interchange of traffic between the roadway and abutting
property.
2. At any intersection, a driveway shall be restricted for a sufficient distance from the intersection to preserve the
normal and safe movement of traffic. (It is recommended for rural residence entrances that a minimum
intersection clearance of 50 feet be provided and for rural commercial entrances a minimum of I 00 feet be
provided.)
3. All entrances and exits shall be so located and constructed that vehicles approaching or using them will be able to
obtain adequate sight distance in both directions along the county road in order to maneuver safely and without
interfering with county road traffic.
4. The Applicant shall not be permitted to erect any sign or display material, either fixed or movable, on or
extending over any portion of the county road right-of-way.
5. Generally, no more than one approach shall be allowed any parcel or property the frontage of which is less than
one hundred (100) feet. Additional entrances or exits for parcels having a frontage in excess of one hundred 100)
feet shall be permitted only after showing of actual convenience and necessity.
6. All driveways shall be so located that the flared portion adjacent to the traveled way will not encroach upon
adjoining property.
7. No commercial driveway shall have a width greater than thirty (30) feet measured at right angles to the centerline ·
of the driveway except as increased bypermissible radii: No noncommercial driveway shall have a width greater
than twenty (20) feet measured at right angles to the· centerline of the.driveway, except as increased by
permissible radii. .., ·
8. The axis of an approach to the road may be at a right angle to the centerline of the county road and of any angle
between ninety (90) degrees and sixty (60) degrees but shall not be less than sixty (60) degrees. Adjustment will
be made according to the type of traffic to be served and other physical conditions.
9. The construction of parking or servicing areas on the county road right-of-way is specifically prohibited.
Commercial establishments for customer vehicles should provide off-the-road parking facilities.
l 0. The grade of entrance and exit shall slope downward and away from the road surface at the same rate as the
normal shoulder slope and for a distance equal to the width of the shoulder but in no case less than twenty (20)
feet from the pavement edge. Approach grades are restricted to not more than ten percent (10%).
11. All driveways and approaches shall be so constructed that they shall not interfere with the drainage system of the
street or county road. The Applicant will be required to provide, at his own expense, drainage structures at
entrances and exits, which will become an integral part of the existing drainage system. The Board of County
Commissioners or their representative, prior to installation, must approve the dimensions and types of all drainage
structures.
Note: This permit shall be made available at the site where and when work is being done. A work sketch or
drawing of the proposed driveway(s) mnst accompany application. No permit will be issued without drawing,
blueprint, or sketch.
