Application Requirements: These items must be submitted with the anplicatioq
v1] Plans and specifications for the proposed use including the hours of operation, the amount of vehicles
accessing the site on a daily, weekly and/or monthly basis, and the size of any existing or proposed
structures that will be tttiiiled in conjunction with the proposed use. Please submit this information
in narrative form and be specific.

detail the amount of water that would be used and the type of wastewater treatment. If you will be
utilizing well water, please attach a copy of th appropriate well permit and any other legal water
-supply information, including a water allotment contract or an approved water augmentation plan.

The consideration of this proposed Special Use will require at least one (1) public hearing, for which public
notice must be provided. The Planning Department will mail you information concerning this hearing(s),
approximately 30 days prior to the scheduled hearing. You will then be required to notify, by certified return
receipt mail, all adjacent landowners and publish the notice provided by the Planning Department, in a
newspaper of general circulation. Both these notices must be mailed/published at least 15 days prior to the
public hearing. The applicant shall bear the cost of mailing and publication and proof of mailing and
publication must be submitted at the time of the public hearing.

I currently live in a 560 sq. ft. cabin that I propose to designate as an accessory dwelling
so that I may build a primary residence of approx. 3000 sq. ft. at a later date. The cabin
and the proposed residence will be located on my 40 - acre lot at 5175 Grass Mesa Rd.
Rifle.
Vehicles accessing the property would number 4 round trips a day.
I estimate the combined water usage of both dwellings to be approx. 1000 gallons a
day. The well is currently producing 6 gallons per minute [ see enclosed flow test] which is
more than adequate for both dwellings.
The cabin uses a permitted and approved septic system. The primary residence will
have it's own septic system.

ISSUANCE OF THIS PERMIT DOES NOT CONFER A WATER RIGHT
CONDITIONS OF APPROVAL.
1) This well shall be used in such a way as to cause no material injury to existing water rights. The issuance of
the permit does not assure the applicant that no injury will occur to another vested water right or preclude
another owner of a vested water right from seeking relief in a civil court action.
21 The construction of this well shall be in compliance with the Water Well Construction Rules 2 CCR 402-2,
unless approval of a variance has been granted by the State Board of Examiners of Water Well Construction
and Pump Installation Contractors in accordance with Rule 18.
3) Approved pursuant to CRS 37-92-602(3)(b)1111(A) as the only well on a tract of land of 40 acres described as
Lot 58, Grass Mesa Ranches division of land, Garfield County.
4) The use of ground water from this well is limited to fire protection, ordinary household purposes inside 3 single
family dwellings, the irrigation .of not more than one (1) acre of home gardens and lawns, and the watering of
domestic animals.
5) The maximum pumping rate shall not exceed 15 GPM.
6) The return flow from the use of the well must be through an individual waste water disposal system of the
non -evaporative type where the water is returned to the same stream system in which the well is located.
