The following items are required by Garfield County for a final inspection:
1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assigned by Garfield County Building Department posted where readily
visible from access road;
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent
kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all
necessary plumbing;
4. All bathrooms must be complete, with washbowl, tub or shower, toilet stool, hot and cold
running water, non-absorbent floors and walls finished and a privacy door;
5. All steps outside or inside over three (3) steps must have handrails, guard rails on balconies or
decks over 30" high constructed to all IBC and IRC requirements;
6. Outside grading done to where water will detour away from the building;
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until
all the required items are completed and a final inspection made;
8. A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, anchor State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.

ADDITIONS
ALTERATIONS
and
MOVED BUILDINGS
In order to understand the scope of the work intended under a permit application and
expedite the issuance of a permit it is important that complete information be provided.
Adequate and complete information will prevent delays in the plan review process.
Reviewing a plan and the discovery that required information has not been provided by
the applicant may result in the delay of the permit issuance and in proceeding with
building construction. The owner or contractor may be required to provide this
information before the plan review may proceed. This causes delays because other
plans that are in line for review may be given attention before the new information may be
reviewed after it has been provided to the Building Department.
Please review this document to determine if you have enough information to
design your project and provide adequate information to facilitate a plan review. If
you do not, it may be helpful to obtain a book titled "Dwelling Construction under
the Uniform Building Code". This book is available to you through this department
at our cost. Also, please consider using a design professional for assistance in
your design and a construction professional for construction of your project.
To provide for a more understandable plan in order to determine compliance with the
building, plumbing and mechanical codes, applicants are requested to review the
following checklist prior to and during design. Applicants are required to indicate
appropriately and to submit the completed checklist at time of application for a
permit.
Plans to be included for a Building Permit, must be on drafting paper at least
18"x24" and drawn to scale.
Plans must include a floor plan, a concrete footing and foundation plan, elevations all
sides with decks, balcony, steps, hand rails and guard rails, windows and doors, including
the finish grade line. A section showing in detail, from the bottom of the footing to the top
of the roof, including re -bar, anchor bolts, pressure treated plates, floor joists, wall studs
and spacing, insulation, sheeting, house -rap, (which is required), siding or any approved
building material.
A window schedule. A door schedule.
A floor framing plan, a roof framing plan, roof must be designed to withstand a 40 pound
per square foot up to 7,000 feet in elevation, an 80 M.P.H. windshear, wind exposure l3,
windload of 15 pounds per square foot, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be
met or your plans will be returned.
All plans submitted must be incompliance with the 1997 UBC, UMC and 1997 UPC.
1. Is a site plan included that identifies the location of the proposed structure or
addition and distances to the property lines from each corner of the proposed
structure(s) prepared by a licensed surveyor and has the surveyors signature and
professional stamp on the drawing? Properties with slopes of 30% or greater must
be shown on the site plan. NOTE: 106.3.1 (7.) Any site plan for the placement of any
portion of a structure within 50 ft. of a property line and not within a previously
surveyed building envelope on a subdivision final plat shall be prepared by a
licensed surveyor and have the surveyors signature and professional stamp on the
drawing. Any structure to be built within a building envelope of a lot shown on a
recorded subdivi ion plat, shall include a copy of the building envelope as it is shown
on the final plat with the proposed structure located within the envelope.

information at the request of the Building Official prior to beginning the plan review
process. Delays in issuing the permit are to be expected. Work may not proceed without
the issuance of a permit. If it is determined by the Building Official that additional
information is necessary to review the application and plans to determine minimum

Projects or Landscaping Construction within the time constraints noted in Conditions 2. & 3. On Page
6 of this Application will result in a Notice and Opportunity for a Hearing before the BMSA Board of
Directors. If I/We are found in violation of these Conditions at the Hearing it will result in a fine to the
Owner of not less than $500.00 per month for each full month in excess of the allotted completion time
frame.
UWe hereby acknowledge that Home Construction or Improvement Projects or Landscape Construction
features which are built in non-conformance with this full application, including the Conditions of
Approval noted on Page 6 of 6 herein, I/We will be cited by the BMSAJAC in written citation form at the
time of discovery. In extenuating circumstances a Variance may be requested from the BMSA/AC,
provided such Variance request item is within the purview and approval jurisdiction of the BMSA/AC.
If a Variance is not granted by the BMSA/AC and if the Non -Conforming Items are not corrected within
60 days after the date of written citation, I/We will be served Notice of a Hearing before the BMSA Board
of Directors, and if l/We are found in violation of these Non -Conforming Items at the Hearing UWe will
be fined a minimum of $500.00 per Non -Conforming Item. Notwithstanding the foregoing, I/We
acknowledge that the BMSA Board of Directors and/or Architectural Committee may pursue any legal
remedy available in the event of a non-compliance.

Failure to pay fines levied as determined at the Hearing may also result in a Lien being placed on your
Property or litigation Process initiated by the BMSA Board of Directors to secure conformance with this
full Application and its respective Conditions Of Approval.

department, drainage design or structural soundness, nor does it constitute assurance that the plans
comply with all applicable codes, regulations, ordinances and laws.

CONDITIONS OF APPROVAL or DISAPPROVAL
1. No sitework or building construction may proceed on any Lot (including no preliminary site clearing,
grubbing, grade alteration, storing of material, foundation excavation, extension of underground utilities,
etc.) until the Project has received a Project approval from the Architectural Committee and has been
issued a Building Permit (as required by Garfield County for new home or Renovation projects).
2. All general building and site improvements construction for any Project shall be completed and shall have
issued a Certificate of Occupancy from the Garfield County Building Department no later than one (1)
Year after the issuance date of this Architectural Committee Approval.
3. The complete landscaping and revegetation of the improved Lot, to specifically include the front side and
rear yard areas, shall be completed no later than six (6) months from the date of the Certificate of
Occupancy.
4. If the selection of colors and /or landscaping plans are not included with this application they must be
submitted for approval before any action is taken to install the missing items.

INSTRUCTIONS:
This Application & Checklist is to be used for all project submittals to the Battlement Mesa Service Association
Architectural Committee (BMSA/AC). This form is intended to assist in ensuring that the application submittal
is complete. This will help provide a quicker, more thorough review process.
It is recommended that each Applicant thoroughly review the requirements of the current Architectural
Standards the Amended and Restated Declaration of Covenants (including Resolutions) and the recorded
Final Plat for their respective neighborhood prior to preparing a submittal.
Applicants preparing a submittal should compare the information they are about to submit with the requirements
of this form and "check off' each required item on the form. If any item is not checked, the Application submittal
is not complete and should not be submitted. Incomplete Applications will not be processed and will be returned
to the Applicant.
In addition to the checklist, Applicants should fill in all of the places on the Application Form where information
is requested. The Architectural Committee will use this information in evaluating your Application If information
is requested but is not applicable to your specific project submittal, write "N/A" in the space provided. If some
required information is yet to be determined (such as exterior paint colors you may not have yet selected) write
"TBD" in the appropriated space provided. You will then be required to make a follow-up submittal with these
selections at a later date but before installation in the home.
DO NOT fill in any information in the "Conditions for Approval" section of this form. This portion of the form
is for Architectural Committee use only.

hereby requests permission and authority from the Board of County Commissioners to construct a driveway approach (es)
on the right-of-way off of County Road, CR 372W (Snowberry Place), At Address At Address of Lot 14 -Block 5 CR
372W(Snowberry Place), located on the East side of road for the purpose of obtaining access to property.
Applicant submits herewith for the consideration and approval of the Board of County Commissioners, a sketch of the
proposed installation showing all the necessary specification detail including:
1. Frontage of lot along road.
2. Distance from centerline of road to property line.
3. Number of driveways requested
4. Width of proposed driveways and angle of approach.
5. Distance from driveway to road intersection, if any.
6. Size and shape of area separating driveways if more than one approach.
7. Setback distance of building(s) and other structure improvements.
8. No unloading of equipment on county road, any damage caused to county road will be repaired at subdivision
expense.
9. Responsible for two years from the date of completion.
General Provisions
1) The applicant represents all parties in interest, and affirms that the driveway approach (es) is to be constructed by
him for the bona fide purpose of securing access to his property and not for the purpose of doing business or
servicing vehicles on the road right of way.
2) The applicant shall furnish all labor and materials, perform all work, and pay all costs in connection with the
construction of the driveway(s). All work shall be completed within thirty (30) days of the permit date.
3) The type of construction shall be as designated and/or approved by the Board of County Commissioners or their
representative and all materials used shall be of satisfactory quality and subject to inspection and approval of the
Board of County Commissioners or their representative.
4) The traveling public shall be protected during the installation with proper warning signs and signals and the Board
of County Commissioners and their duly appointed agents and employee shall be held harmless against any action
for personal injury or property damage sustained by any reason of the exercise of the Permit.
5) The Applicant shall assume responsibility for the removal or clearance of snow, ice, or sleet upon any portion of
the driveway approach (es) even though deposited on the driveway(s) in the course of the County snow removal
operations.
6) fn the event it becomes necessary to remove any right-of-way fence, the posts on either side of the entrance shall
• be surely braced before the fence is cut to prevent any slacking of the remaining fence and all posts and wire
removed shall be turned over to the District Road Supervisor of the Board of County Commissioners.
7) No revisions or additions shall be made to the driveway(s) or its appurtenances on the right-of-way without
written permission of the Board of County Commissioners.
8) Provisions and specifications outlined herein shall apply on all roads under the jurisdiction of the Board of County
Commissioners of Garfield County, Colorado, and the Specifications, set forth on the attached hereof and
incorporated herein as conditions hereof.
9) Final inspection of driveway will be required upon completion and must be approved by person issuing
permit or representative of person issuing permit.
The inspection and sign off must be done prior to any CO from the Building and Planning
Department being issued.
