The following items are required by Garfield County for a final inspection:
1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assigned by Garfield County Building Department posted where readily
visible from access road;
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent
kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all
necessary plumbing;
4. All bathrooms must be complete, with washbowl, tub or shower, toilet stool, hot and cold
running water, non-absorbent floors and walls finished and a privacy door;
5. All steps outside or inside over three (3) steps must have handrails, guard rails on balconies or
decks over 30" high constructed to all IBC and IRC requirements;
6. Outside grading done to where water will detour away from the building;
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until
all the required items are completed and a final inspection made;
8. A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.

In order to understand the scope of the work intended under a permit application and expedite the
issuance of a permit it is important that complete information be provided. When reviewing a plan
and it's discovered that required information has not been provided by the applicant, this will result
in the delay of the permit issuance and in proceeding with building construction. The owner or
contractor shall be required to provide this information before the plan review can proceed. Other
plans that are in line for review may be given attention before the new information may be reviewed
after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your
project and provide adequate information to facilitate a plan review. Also, please consider
using a design professional for assistance in your design and a construction professional for
construction of your project. Any project with more than ten (10) occupants requires the plans
to be sealed by a Colorado Registered Design Professional.
To provide for a more understandable plan in order to determine compliance with the building,
plumbing and mechanical codes, applicants are requested to review the following checklist prior to
and during design. Applicants are required to indicate appropriately and to submit the
completed checklist at time of application for a permit.
Plans to be included for a Building Permit, must be on drafting paper at least 18"x24" and
drawn to scale.

Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with
decks, balcony, steps, hand rails and guard rails, windows and doors, including the finish grade line
and original grade. A section showing in detail, from the bottom of the footing to the top of the roof,
including re -bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house -rap, (which is required), siding or any approved building material. Engineered
foundations may be required.
A window schedule. A door schedule. A floor framing plan, a roof framing plan, roof must be
designed to withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind
speed, wind exposure B or C, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or
your plans will be returned.
All plans submitted must be incompliance with the 2003 IRC.
1. Is a site plan included that identifies the location of the proposed structure or addition and
distances to the property lines from each corner of the proposed structure(s) prepared by a
licensed surveyor and has the surveyors signature and professional stamp on the drawing?
Properties with slopes of 30% or greater must be shown on the site plan. (NOTE
Section: 106.2) Any site plan for the placement of any portion of a structure within 50 ft.
of a property line and not within a previously surveyed building envelope on a
subdivision final plat shall be prepared by a licensed surveyor and have the surveyor's
signature and professional stamp on the drawing. Any structure to be built within a
building envelope of a lot shown on a recorded subdivision plat shall include a copy of
the building envelope as it is shown on the final plat with the proposed structure located
within the envelope.

Delays in issuing the permit are to be expected. Work may not proceed without the issuance of a
permit. If it is determined by the Building Official that additional information is necessary to review
the application and plans to determine minimum compliance with the adopted codes, the application
may be placed behind more recent applications for building permits in the review process and not

PURPOSE AND SCOPE OF STUDY
This report presents the results of a subsoil study for a proposed residence to he located at
Lot 12, Sun Meadows Estates, County Road 216, Garfield County, Colorado. The project
site is shown on Figure 1. The purpose of the study was to develop recommendations for
the foundation design. The study was conducted in accordance with our agreement for
geotechnical engineering services to Caribou Construction dated May 3, 2005. Hepworth-
Pawlak Geotechnical Inc., previously perfonned a preliminary geotechnical study for Sun
Meadow Estates (formerly Mamms View) and reported our findings under Job No. 100
169, dated March 28, 2000.
A field exploration program consisting of one exploratory boring in the building area and
one profile boring in the septic disposal area was conducted to obtain information on the
subsurface conditions. Samples of the subsoils obtained during the field exploration were
tested in the laboratory to determine their classification, compressibility or swell and
other engineering characteristics. The results of the field exploration and laboratory
testing were analyzed to develop recommendations for foundation types, depths and
allowable pressures for the proposed building foundation. This report summarizes the
data obtained during this study and presents our conclusions, design recommendations
and other geotechnical engineering considerations based on the proposed construction and
the subsurface conditions encountered.
PROPOSED CONSTRUCTION
The proposed residence will be one story wood frame construction above a crawlspace
with an attached garage. Garage floor will be slab -on -grade. Grading for the structure is
assumed to be relatively minor with cut depths between about 2 to 4 feet. We assume
relatively light foundation loadings, typical of the proposed type of construction.

If building loadings, location or grading plans change significantly from those described
above, we should be notified to re-evaluate the recommendations contained in this report.
SITE CONDITIONS
Lot 12 is located on the north side of County Road 216 and on the west side of North
Meadow Drive. The site was vacant at the time of our field visit. The lot is 2.008 acres
and the building area is located near the center of the property. Vegetation consists of
grass and weeds. The ground surface is relatively flat with a slight slope down to the
north.
FIELD EXPLORATION
The field exploration for the project was conducted on July 13, 2005. One exploratory
boring was drilled in the building area and a profile boring was drilled in the septic
disposal area at the locations shown on Figure 1 to evaluate the subsurface conditions.
The borings were advanced with 4 inch diameter continuous flight augers powered by a
truck -mounted CME -45B drill rig. The borings were logged by a representative of
Hepworth-Pawlak Geotechnical, Inc.
Samples of the subsoils were taken with a 2 inch I.D. spoon sampler. The sampler was
driven into the subsoils at various depths with blows from a 140 pound hammer falling 30
inches. This test is similar to the standard penetration test described by ASTM Method
D-1586. The penetration resistance values are an indication of the relative density or
consistency of the subsoils. Depths at which the samples were taken and the penetration
resistance values are shown on the Logs of Exploratory Borings, Figure 2. The samples
were returned to our laboratory for review by the project engineer and testing.

SUBSURFACE CONDITIONS
Graphic logs of the subsurface conditions encountered at the site are shown on Figure 2.
The subsoils consisted of about '/s foot of topsoil overlying sandy silty clay. Silty sand
with gravel was encountered below the clay at a depth of 6 to 9 feet to the depth drilled of
26 feet at Boring 1. Boring 8 from the preliminary geotechnical study, located at the
south end of Lot 12, encountered silty sand with gravel at a depth of 14 feet and no
groundwater level to the drilled depth of 21 feet.
Laboratory testing performed on samples obtained from Boring 1 included natural
moisture content, dry density and percent finer than #200 sieve. Results of swell -
consolidation testing performed on relatively undisturbed drive samples, presented on
Figure 4, indicate low to moderate compressibility under conditions oflight loading and
existing moisture conditions with a minor swell potential when wetted. The laboratory
testing is summarized in Table 1.
No free water was encountered in the borings at the time of drilling and the subsoils were
slightly moist to moist.
DESIGN RECOMMENDATIONS
FOUNDATIONS
Considering the subsurface conditions encountered in the exploratory borings and the
nature of the proposed construction, we recommend the building be founded with spread
footings bearing on the natural soils.
The design and construction criteria presented below should be observed for a spread
footing foundation system.

1) Footings placed on the undisturbed natural soils should be designed for an
allowable bearing pressure of 1,500 psf Based on experience, we expect
initial settlement of footings designed and constructed as discussed in this
section will be about 1 inch or less. Additional differential settlements
could be on the order of inch if the bearing soils became wetted.
2) The footings should have a minimum width of 16 inches for continuous
walls and 2 feet for isolated pads.
3) Exterior footings and footings beneath unheated areas should he provided
with adequate soil cover above their bearing elevation for frost protection.
Placement of foundations at least 36 inches below exterior grade is
typically used in this area.
4) Continuous foundation walls should be reinforced top and bottom to span
local anomalies such as by assuming an unsupported length of at least 12
feet. Foundation walls acting as retaining structures should also be
designed to resist a lateral earth pressure corresponding to an equivalent
fluid unit weight of at least 50 pcf.
5) The topsoil and any loose or disturbed soils should be removed and the
footing bearing level extended down to the undisturbed natural soils. The
exposed soils in footing area should then be moistened and compacted.
6) A representative of the geotechnical engineer should observe all footing
excavations prior to concrete placement to evaluate bearing conditions.
FLOOR SLABS
The natural on-site soils, exclusive of topsoil, are suitable to support lightly loaded slab -
on -grade construction. To reduce the effects of some differential movement, floor slabs
should be separated from all bearing walls and columns with expansion joints which
allow unrestrained vertical movement. Floor slab control joints should be used to reduce
damage due to shrinkage cracking. The requirements for joint spacing and slab
reinforcement should be established by the designer based on expenence and the intended
slab use. A minimum 4 inch layer of free -draining gravel should be placed beneath

basement level slabs to facilitate drainage. This material should consist of minus 2 inch
aggregate with at ]east 50% retained on the No. 4 sieve and less than 2% passing the No.
200 sieve.
All fill materials for support of floor slabs should be compacted to at least 95% of
maximum standard Proctor density at a moisture content near optimum. Required fill can
consist of the on-site soils devoid of vegetation and topsoil.
UNDERDRAIN SYSTEM
Although free water was not encountered during our exploration, it has been our
experience in the area that local perched groundwater can develop during nines of heavy
precipitation or seasonal runoff. Frozen ground during spring runoff can also create a
perched condition. We recommend below -grade construction, such as retaining walls and
crawlspace areas, be protected from wetting and hydrostatic pressure buildup by an
underdrain system.
The drains should consist of drainpipe placed in the bottom of the wall backfill
surrounded above the invert level with free -draining granular material. The drain should
be placed at each level of excavation and at least 1 foot below lowest adjacent finish
grade and sloped at a minimum 1% to a suitable gravity outlet, sump and pump or
drywell. Free -draining granular material used in the underdrain system should contain
less than 2% passing the No. 200 sieve, less than 50% passing the No. 4 sieve and have a
maximum size of 2 inches. The drain gravel backfill should be at least 1'/: feet deep.
SURFACE DRAINAGE
The following drainage precautions should be observed during construction and
maintained at all times after the residence has been completed:
1) Inundation of the foundation excavations and underslab areas should be
avoided during construction.

_) Exterior backfill should be adjusted to near optimum moisture and
compacted to at least 95% of the maximum standard Proctor density in
pavement and slab areas and to at least 90% of the maximum standard
Proctor density in landscape areas.
3) The ground surface surrounding the exterior of the building should be
sloped to dram away from the foundation in all directions. We
recommend a minimum slope of 6 inches in the first 10 feet in unpaved
areas and a minimum slope of 3 inches in the first 10 feet in paved areas.
Free -draining wall backfill should be capped with about 2 feet of the on-
site soils to reduce surface water infiltration.
4) Roof downspouts and drains should discharge well beyond the limits of all
backfill.
5) Landscaping which requires regular heavy irrigation should be located at
least 10 feet from foundation walls. Consideration should be given to use
of xeriscape to reduce the potential for wetting of soils below the building
caused by irrigation.
PROFILE BORING
A profile boring was drilled in the proposed septic disposal area at the location shown on
Figure I. The subsoils encountered, below about six inches of topsoil, consisted of sandy
silt/clay to 6 feet overlying silty sand to the bottom hole depth of 9 feet. The log of the
profile boring is shown on Figure 2. No fi-ee water was observed at the time of drilling
and the subsoils were slightly moist to moist.
LIMITATIONS
This study has been conducted in accordance with generally accepted geotechnical
engineering principles and practices in this area at this time. We make no wan-anty either
express or implied. The conclusions and recommendations submitted in this report are
based upon the data obtained from the exploratory borings drilled at the locations

indicated on Figure 1, the proposed type of construction and our experience in the area.
Our services do not include determining the presence, prevention or possibility of mold or
other biological contaminants (MOBC) developing in the future. If the client is
concerned about MOBC, then a professional in this special field of practice should he
consulted. Our findings include extrapolation of the subsurface conditions identified at
the exploratory borings and variations in the subsurface conditions may not become
evident until excavation is performed. If conditions encountered during construction
appear different from those described in this report, we should be notified so that re-
evaluation of the recommendations may be made.
This report has been prepared for the exclusive use by our client for design purposes. We
are not responsible for technical interpretations by others of our information. As the
project evolves, we should provide continued consultation and field services during
construction to review and monitor the implementation of our recommendations, and to
verify that the recommendations have been appropriately interpreted. Significant design
changes may require additional analysis or modifications to the recommendations
presented herein. We recommend on-site observation of excavations and foundation
bearing strata and testing of structural fill by a representative of the geotechnical
engineer.
