In order to understand the scope of the work intended under a permit application and expedite th e
iss uance of a p ermit it is importa nt that complete info rmation be provided. When reviewing a plan
and it's discovered that required informatio n has not been provided by the applicant, thi s will re s ult
in the delay of th e permit issuanc e and in proceeding with building construction. The owner or
contractor shall be req uired to provide thi s information before the plan review can proceed. Other
plans that are in line for rev iew may be given attention before the new information may be reviewed
after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your
project and pr·ovide adequate information to facilitate a plan review. Also, please consider
using a design pr·ofessional for· assistance in your design and a construction professional for
construction of your project. Any pr·oject with more than ten (10) occupants requires the
plans to be sealed by a Colorado Register·e d Design Pr·ofessional.
To provide for a more und e rstandable plan in order to determine compliance with the building,
plumbing and mechanical codes, applicants are reque sted to review the following checklist prior to
and during design. Applicants are required to indicate appropriately and to submit the
completed checklist at time of application for a permit.
Plans to be included for a Building Permit, must be on drafting paper· at least 18"x24" and
drawn to scale.
Pl ans mu st include a floor plan , a concrete footing and foundation plan , e levation s a ll si des with
decks, balcony, steps, hand rails and guard rails, windows and doors, including the finish grade line
and original grade. A section showing in detail, from the bottom of the footing to the top of the roof,
including re-bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house-rap, (which is required), siding or any approved building material. Engineered
foundations may be required.
A window schedule. A door schedule. A floor framing plan, a roof framing plan, roof must be
designed to withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind
speed, wind exposure B or C, and a 3 6 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or
your plans will be returned.
All plans submitted must be incompliance with the 2003 IRC.
I. Is a site plan included that identifies the location of the proposed structure or addition and
distances to the property lines from each comer of the proposed structure(s) prepared by
a licensed surveyor and has the surveyors signature and professional stamp on the
drawing? Properties with slopes of 30% or greater must be shown on the site plan.
(NOTE Section: 106.2) Any site plan for the placement of any portion of a structure
within 50 ft. of a property line and not within a previously surveyed building envelope on
a subdivision final plat shall be prepared by a licensed surveyor and have the surveyor's
signature and professional stamp on the drawing. Any structure to be built within a
building envelope of a lot shown on a recorded subdivision plat shall include a copy of
the building envelope as it is shown on the final plat with the proposed structure located
within the e~lope.

if the property you are applying for a building permit on is located on a comer lot. Special

Please refer to your building permit number 10343 , issued 3/9/2007
A routine review of our open building permits indicates that we have not received a request for an
inspection on this permit since 1/18/2008
Please be advised that building permits expire, per Uniform Building Code, section 106.4.4 and
International Code Section 105.5 " Every permit issued by the building official under the provisions of
this code shall expire by limitation and become null and void if the building or work authorized by
such permit is not commenced within 180 days from the date of such permit, or if the building or work
work authorized by such permit is suspended or abandoned at any time after the work has commenced
for a period of 180 days. Before such work can be recommenced, a new permit shall first be obtained
to do so, and the fee therefore shall be one half the amount required for a new permit for such work,
provided no change have been made or will be made in the original plans and specifications for such
work, and provided further that such suspension or abandonment has not exceeded one year. In order
to renew action or a permit after expiration, the permittee shall pay a new full permit fee."
Please contact this office at your very earliest convenience, in writing, to advise us about the status of
your permitted work.
If you have not responded within thirty (30) days from the receipt of this notice the permit will
be closed and a permanent notation will be placed in the property records at the County Clerk
and Recorders office that indicates that the work permitted per Colorado Revised Statues
30-28-205 & 210, was not inspected, is not approved and the building or area is illegal to
occupy and is subject to legal abatement action.

Dear Dr. and Mrs. Hutton:
As requested, Hepworth-Pawlak Geotechnical, Inc. performed a subsoil study for design
of foundations at the subject site. The study was conducted in accordance with our
proposal for geotechnical engineering services to you dated May 2, 2006. The data
obtained and our recommendations based on the proposed construction and subsurface
conditions encountered are presented in this report. Hepworth-Pawlak Geotechnical, Inc.
previously conducted a preliminary geotechnical study for development of Roaring Fork
Preserve and presented our findings in a report dated November 30,2000, Job No. 100
627.
Proposed Construction: The proposed residence will be one and two story wood frame
structure with an attached garage located on the site as shown on Figure 1. Ground floor
will be slab-on-grade. Cut depths are expected to range up to about 3 feet. Foundation
loadings for this type of construction are assumed to be relatively light and typical of the
proposed type of construction.
Ifbui1ding conditions or foundation loadings are significantly different from those
described above, we should be notified to re-evaluate the recommendations presented in
this report.
Site Conditions: The site was vacant at the time of our field exploration. The ground
surface is relatively flat with about 2 to 3 feet of elevation difference across the building
area. There is an existing pond and irrigation ditch to the east of the building area.

Vegetation consists of cottonwood trees with an understory of grass and weeds. Scattered
cobbles are exposed on the ground surface.
Subsidence Potential: Roaring Fork Preserve is underlain by Pem1sylvania Age Eagle
Valley Evaporite bedrock. The evaporite contains gypsum deposits. Dissolution of the
gypsum under certain conditions can cause sinkholes to develop and can produce areas of
localized subsidence. Sinkholes were not observed in the immediate area of the subject
lot. The exploratory pi.ts were relatively shallow, for foundation design only. Based on
our present knowledge of the site, it camwt be said for certain that sinkholes will not
develop. In our opinion, the risk of ground subsidence at Lot 3 throughout the sen'ice life
of the proposed residence is low and similar to other lots in the area but the owner should
be aware of the potential for sinkhole development.
Subsurface Conditions: The subsurface conditions at the site were evaluated by
excavating two exploratory pits at the approximate locations shown on Figure 1. The
Jogs of the pits are presented on Figure 2. The subsoils encountered, below about I foot
of topsoil and 2 to 2Yo feet of medium stiff, sandy silty clay, consist of relatively dense,
silty sandy gravel with cobbles to the pit depths of 4 to 5 feet. Results of a gradation
analysis performed on a sample of the gravel soils (minus 3 inch fraction) obtained from
the site are presented on Figure 3. Groundwater was observed in the pits at depths of 4
and 5 feet and the soils were slightly moist to wet at the groundwater level.
Foundation Recommendations: Groundwater was encountered in the exploratory pits
and the excavation depths for the residence should be kept relatively shallow to avoid
groundwater impacts. Considering the subsoil conditions encountered in the exploratory
pits and the nature ofthe proposed construction, we recommend spread footings placed
on the undisturbed natural gravel soil or compacted structural fill designed for an
allowable soil bearing pressure of2,000 psffor support of the proposed residence.
Footings should be a minimum width of 16 inches for continuous walls and 2 feet for
colurm1s. Topsoil, clay and loose disturbed soils encountered at the foundation bearing
level within the excavation should be removed and the footing bearing level extended

down to the undisturbed natural gravel soils. As an alternative, design bearing levels can
be re-established with compacted structural fill. The fill should consist of granular soils
compacted to at least 98% of standard Proctor density at a moisture content near
optimum. The fill should extend laterally beyond the footings at least equal to the depth
of fill below the footings. Exterior footings should be provided with adequate cover
above their bearing elevations for frost protection. Placement of footings at least 36
inches below the exterior grade is typically used in this area. Continuous foundation
walls should be reinforced top and bottom to span local anomalies such as by assuming
an unsupported length of at least 10 feet. Foundation walls acting as retaining structures
should be designed to resist a lateral earth pressure based on an equivalent fluid unit
weight of at least 45 pcf for the on-site gravel soil, excluding oversized rock, as backfill.
Floor Slabs: The natural on-site soils, exclusive of topsoil, are suitable to support lightly
loaded slab-on-grade construction. To reduce the effects of somecii_ffer~11tial movement,
floor slabs should be separated from all bearing walls and coluums with expansion joints
which allow unrestrained vertical movement. Floor slab control joints should be used to
reduce damage due to shrinkage cracking. The requirements for joint spacing and slab
reinforcement should be established by the designer based on experience and the intended
slab use. A minimum 4 inch layer of free-draining gravel should be placed below slabs-
on-grade to facilitate drainage and limit capillary moisture rise. This material should
consist of minus 2 inch aggregate with less than 50% passing the No. 4 sieve and less
than 2% passing the No. 200 sieve.
All fill materials for support of floor slabs should be compacted to at least 9 5% of
maximum standard Proctor density at a moisture content near optimum. Required fill can
consist of the on-site soils devoid of vegetation, topsoil and oversized rock.
Surface Drainage: The following drainage precautions should be observed during
construction and maintained at all times after the residence has been completed:
1) Inundation of the foundation excavations and underslab areas should be
avoided during construction.

2) Exterior backfill should be adjusted to near optimum moisture and
compacted to at least 95% of the maximum standard Proctor density in
pavement and slab areas and to at least 90% of the maximum standard
Proctor density in landscape areas.
3) The ground surface surrounding the exterior of the building should be
sloped to drain away from the foundation in all directions. We
recommend a minimum slope of 6 inches in the first 1 0 feet in unpaved
areas and a minimum slope of 3 inches in the first I 0 feet in pavement and
walkway areas.
4) Roof downspouts and drains should discharge well beyond the limits of all
backfill.
Limitations: This study has been conducted in accordance with generally accepted
geotechnical engineering principles and practices in this area at this time. We make no
warranty either express or implied. The conclusions and recommendations submitted in
this report are based upon the data obtained from the exploratory pits excavated at the
locations indicated on Figure 1 and to the depths shown on Figure 2, the proposed type of
construction, and our experience in the area. Our services do not include determining the
presence, prevention or possibility of mold or other biological contaminants (MOBC)
developing in the future. If the client is concerned about MOBC, then a professional in
this special field of practice should be consulted. Our findings include interpolation and
extrapolation of the subsurface conditions identified at the exploratory pits and variations
in the subsurface conditions may not become evident until excavation is performed. If
conditions encountered during construction appear different from those described in this
report, we should be notified at once so re-evaluation of the recommendations may be
made.
This report has been prepared for the exclusive use by our client for design purposes. We
are not responsible for technical interpretations by others of our information.· As the
project evolves, we should provide continued consultation and field services during
construction to review and monitor the implementation of our recommendations, and to

verity that the recommendations have been appropriately interpreted. Significant design
changes may require additional analysis or modifications to the recommendations
presented herein. We recommend on-site observation of excavations and foundation
bearing strata and testing of structural fill by a representative of the geotechnical
engineer.

as required by CRS 30-28-136 (h) (I) Garfield County does not override the State Engineers
opinion regarding injury to decreed water rights.
Prior to the approval any Final Plat, each well permit will be approved by the State
Division of Water Resources and each well will be drilled and meet the following criteria:

That a four ( 4) hour pump test be performed on the well to be used;
A well completion report demonstrating the depth of the well, the characteristics of the
aquifer and the static water level;
The results of the four ( 4) hour pump test indicating the pumping rate in gallons per
minute and information showing drawdown and recharge;
A written opinion of the person conducting the well test that this well should be adequate
to supply water to the number of proposed lots;
An assumption of an average of no less than 3. 5 people per dwelling unit, using 100
gallons of water per person, per day;
If the well is to be shared, a legal, well sharing declaration which discusses all easements
and costs associated with the operation and maintenance of the system and who will be
responsible for paying these costs and how assessments will be made for these costs;
7. The water quality be tested by an independent testing laboratory and meet State guidelines
concerning bacteria and nitrates.
D. Fire Protection: The site is located in the Carbondale and Rural Fire Protection District
(CRFPD). The CRFPD noted in a letter during the sketch plan process that the proposed
access should be adequate for fire protection purposes. Additionally they asked for
30,000 gallons of backup water storage for fire protection and that each unit have
sprinklers.
Zancanella & Associates (ZA) noted in a letter dated 8/23/00 that the proposed water
system improvements for the Mayfly Bend subdivision will include a joint 30,000 gallon
storage tank, to be shared with the proposed Roaring Fork Preserve Subdivision (RFP).
There was a fire protection plan submitted at the Planning Commission hearing for the
Roaring Fork Preserve subdivision application .. Water for fire protection will be limited
to water carried on fire apparatus and tanker shuttle, since there is no other fire protection
water included in either this application or the Mayfly Bend subdivision application. The
applicants propose to place three 10,000 gallon water storag~tanks at a location within
the Roaring Fork Preserve subdivision. (See plans pgs. 2... -r-25 ) Additionally, the
covenants have been amended to require NFP ABD compliant fire sprinklers in the houses.
The Carbondale & Rural Fire Protection District has reviewed the plans and stated that the
design and installation should meet the following standards:

ALL SERVICE septic, LLC performed installation observations during August and September of
2008 for the subject property.
The installation included a 2000-gallon, two-compartment precast concrete septic umk, with an AX-
20 AdvantexTM Unit and a 50()-gallon concrete tank to dose the mounded drain Held. The tanks and
the AdvantcxTM system were installed per plans. The sewage qjcetor system, directing sewage from
the house to the septic tank, was installed per specifications,
The mounded drain lield bed consisted of 3 mws with 14 chamber~ per row for a total of 42
chambers. A 1.5-ineh level maniti.))d was installed to distribute ellluent.
The OWS was installed according to specifications. Tllis observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic. LLC should be notified if changes

ALL SERVICE septic, LLC performed a subsurface investigation and an onsite wastewater system
(OWS) design for the subject residence. The property is located outside of Carbondale Colorado, in
an area where OWS are necessary. Water will be provided by the local wate r service. According to
the Homeowners Association By-laws the OWS must be sized based on a minimum of 4-bedrooms,
and must incorporate a secondary treatment process.
SITE CONDITIONS
A single-family residence is proposed as indicated on Figure 1. There is a pond proposed to the north
of the residence. The slope in the area of the proposed drain field is relatively flat, and vegetation
consists of a full cover of native grasses and wildflowers.
SUBSURFACE
The subsurface was investigated by excavating an 8-foot test pit, and three percolation holes, in the area
of the proposed drain field , as indicated on Figure I. The test pit indicated the site has a 6-inch root
zone, underlain by sand with gravel and cobbles to the maximum depth explored of 8 feet.
Groundwater or bedrock was not encountered. A piezometer was installed. Percolation rates ranged
from 8 to 10 minutes per inch (MPI). A 10 MPI rate will be used to design the OWS.
DESIGN SPECIFICATIONS
The OWS design is based on 4-bedrooms and an average sewage load of 600 GPD. The installation
must include a 2000-gallon, two-compartment, precast concrete septic tank, with an AX-20
Advantex™ Unit. Gravity discharge to the drain field is proposed. If gravity discharge cannot be
achieved, and drain field depths maintained, our office should be contacte d. Tank alternatives may be
considered.

Three gravelless 'Quick-4' trenches must be installed as indicated on plans. Each trench must have
14 chambers, for a total of 42 chambers and 415 square feet (SF) of infiltrative area. A concrete
distribution box with a baffle must be installed. Construction must be according to the county ISDS
regulations, the septic permit provided by Garfield County Environmental Health Department, and this
design.
ADVANTEXOPERATION AND INSTALLATION
A qualified installer should install The Advantex ™ Unit. A two-year operation and maintenance
contract should be provided by the installer. Additional construction notes are provided on construction
figures attached.
I. The control panel should be mounted in 'line-of-site' to tank and Advantex ™ unit.
2. A dedicated !SA service for control breaker (in OSI panel) is required.
3. F.L.A. is 6.0 for P-30 pump, and 8.0 for P-50 pump.
4. Conduit should be used for electrical lines.
5. Electrician must provide phone line, if applicable, and terminate in control panel.
OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Geo-fabrics or plastics should not be used over the drain field. Livestock
should not graze on the drain field. Plumbing fixtures should be checked to ensure that no additional
water is being discharged to OWS. For Example, a running toilet or leaky faucet can discharge
hundreds of gallons of water a day and harm a drain field.
The homeowner should pump the septic tank every two years and clean the effluent filter as needed.
Garbage disposal use should be minimized, and non-biodegradable materials should not be placed
into the OWS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWS. No hazardous wastes should be directed into the OWS.
Mechanical room drains should not discharge into the OWS. The OWS is engineered for domestic
waste only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes should be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain
field due to the potential to compact soil. Extensions should be placed on all septic tank components
to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be
used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices
is optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWS during construction. The OWS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
installation.

conditions described in report, ALL SERVICE septic, LLC should be notified . AllOWS construction

Control Panel, Pumps and Alarms: During each visit, all electrical components shall be tested for proper
function. Correct control settings will be verified. Operations data from panel will be recorded.
Processing Tank: Dming each visit, the scum and sludge layers in the processing tank (septic tank) will be
measured. A recommendation for pumping will be made when the scum thickness exceeds four inches, or the
sludge thi~kness exceeds. 21! inches.
Pumping System: During each visit all pumps shall be observed to verify proper performance. The pumps will be
operated manually. The Biotube screen will be removed and cleaned.
Recirculating Filter: The filter will be observed for signs of excess organic build-up or improper performance.
Tbe media will be cleaned as necessary. The system will be observed to verify proper distribution across the media
and distribution laterals will be flushed.
Drain Field: The surface area around the drain field will be observed for sigJlS offailure such as lush vegetation
growth or effluent ponding. Liquid levels within the field will be observed through the observation pipe and levels
recorded.
Items that are not included in this contract include:
1. Corrective and/or repair majntenance if necessary 1 other than covered under warranty.
2. Effluent sampling and analysis if applicable.
3. The cost of tank pumping.
Alarms: VPI will respond to alarm conditions at no cost during the agreement period.
Service Calls: Service calls will be billed at a rate of$70.00 per hour and $4~.00 per hour for travel time. A visit
to the site other than the routine vis. its provided by this agreement or a system watnnty issue will be deemed a
service call. Examples would include (but are not limited to) damage caused by acts of vandalism, failure of tank or
failure of another party's work, i.e. electrician, plumber, excavator, etc.
The goal ofVPl is to observe the operation and perform maintenance for the Onsite Wastewater System to allow for
proper long-term function ofthe system and compliance with the conditions of the installation permit and
t'ecommcndations of the manufacturer.
VPI will perform O&M services as outlined for $26:5 per visit with two visits per year ($:530 per year). Payment in
full for the first ( 3) years is required prior to initiation of the agreement. This agreement period will be for the
( 3 )-year period subsequent to placing the system into operation.
