Dear Mr. Jarman:
Per the conditions of approval listed in the Resolution 2010-08 associated with the PA 41-9 pad
location:
9. The Applicant shall be required to submit a report annually, until such time as the well pad is
released from the COGCC reclamation bond, of the extraction and processing operation, for staff
review. Upon review of any deficiencies pursuant to conditions of approval or other local, state,
or federal permits, Staff niay forward the report to the County Commissioners for fill review of
the Special Use Permit.
On the PA 41-9 location, normal operations occurred this year which include the following:
• 418;'2014: Bareground Weed Spray Application
• 6/6/2014: Noxious weed spraying for Musk Thistle by WPX.
• 7116/2014: Noxious weed spraying for Scotch Thistle by WPX.
• There was no workover activity to report on the PA 41-9 Pad during 2014.
• WPX personnel and contractors continually monitor and maintain production equipment,
water removal, and production as needed.
WPX has reviewed the conditions set forth in the Major Impact Review and remains compliant
under the conditions of the resolution listed above.
