Enclosed for your review and processing is a copy of LINN Operating's (*LINN")
Application for an Onsite Wastewater Treatment System Permit, as well as all required
supplemental information. The Application requests approval from Garfield County for
operation of the wastewater system for the temporary employee housing quarters to
accommodate drilling personnel working on LINN's L-l5 well pad, on lands owned by
Encana Oil and Gas, Inc.

Addendum to Onsite Wastewater Treatment System (OWTS) Permit for the [-15 well pad:
Potable water for the site would be stored in one (1) 3,000 gallon potable water tank, which would be
weather-proof and insulated to prevent freezing. The water would then be piped from this tank to the
internal plumbing systems of the individual mobile home units. The 3,000 gallon tank would senre all four
mobile units. Potable water would be pressurized by a pump that will deliver water at a pressure of 50 to
60 pounds per square inch to all fixtures in the housing quarters. All plumbing meets the 2009
lnternational Plumbing Code and Regulation 43 OWTS as amended by Garfield County. LINN anticipates
5 individuals to be staying at the housing facility at any one time. Calculations for water usage are for 5
people using 75 gallons of water per day per person, which would be 375 gallons per day. LINN anticipates
and has contracted for twice weekly deliveries of potable water.
Wastewater would be temporarily stored in one (1) 4,000 gallon waste water holding tank at the
employee housing quarters. The tank would be heated to prevent freezing and would be equipped with
overflow alarms that would be triggered when the tank reaches 80% of capacity. The tank and piping
would be made of proper materials, would be water tight, properly sealed or screened to prevent entry
and exit of insects and other vermin. All plumbing meets the 2009 lnternational Plumbing Code and
Regulation 43 OWTS as amended by Garfield County. The holding tanks would be pumped by vacuum
trucks (United Site Services) two times per week to maintain ample storage capacity. Wastewater haul
trips would typically remove up to 4,000 gallons from each holding tank. The wastewater would then be
hauled and disposed of (as appropriate) at the Garfield County Landfill. Records of all wastewater haul
trips will be maintained on-site to verifli the waste was hauled by an approved service and receipts from
the hauler will be kept on record. Letters from the potable water supplier and wastewater hauler
certifying that they will provide service to the temporary employee housing quarters are attached.
