Dear Mr. Olson Hepworth-Pawlak Geotechnical, Inc. (HP Geotech) observed the installation of the Onsite Wastewater Treatment System (OWTS) for the proposed residence at the subject property. The OWTS components observed include: sewer pipe, cleanout, septic and dose tanks, pump operation, effluent transport pipe, automatic distribution valve, Infiltrator chambers, inspection ports and the locations of components. We previously designed an OWTS for the subject site and presented our recommendations in a report dated April 11, 2014, Job No. 113 161B. Our observations and recommendations are presented in the following paragraphs. We performed our As -Built site evaluation on October 28th and December 18th, 2014. The installation and Iocations of system components were verified as shown on Figure 1. The tank and field locations and orientation were modified slightly from our design as shown on Figure 1; however, this will not alter the effectiveness of the OWTS. Photographs of the OWTS installation are provided in the attached Photograph Log. 1t is our opinion that the OWTS was installed in general conformance with our April 11, 2014 design. The observations and recommendations presented above are based on our site observations and our experience in the area. We make no warranty either express or implied. Alterations to the system or improper backfill and site grading could affect the operation and effectiveness of the OWTS.
