The following items are required by Garfield County for a final inspection:
I. A final El cctrical Inspection from the Colorado Slate Electrical Inspector,
2. Pennanent address assigned by Garfield County Building Department posted where readily
visible from access road;
3. A finished roof. a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen v.ith cabinets, a sink with hot & cold running water, non-absorbent
kitchen floor coverings, counter lops and finished walls, ready for stove and refrigerator, all
necessary plumbing:
4. All bathrooms must be complete, with washbowl, tub or shower, toilet stool, hot and cold
running water, non·absorbent floors and waits finished and a privacy door;
5. All steps outside or inside over three (3) steps must have handrails, guard mils on balconies or
decks over 30" high constructed to all IBC and IRC requirements;
14. Outside grading done to where water will detour away from the building:
Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificah: of Occupancy will not be issued until
all the required items arc completed and a final inspection made:
8. A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign olfby the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED .

In order to understand the scope of the work intended under a permit application and expedite the
issuance of a permit it is important that complete information be provided. When reviewing a plan
and it's discovered that required information has not been provided by the applicant, this will result
in the delay of the permit issuance and in proceeding with building construction. The owner or
contractor shall be required to provide this information before the plan review can proceed. Other
plans that are in line for review may be given attention before the new information may be reviewed
after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your
project and provide adequate information to facilitate a plan review. Also, please consider
using a design professional for assistance in your design and a construction professional for
construction of your project. Any project with more than ten (10) occupants requires the
plans to be sealed by a Colorado Registered Design Professional.
To provide for a more understandable plan in order to determine compliance with the building,
plumbing and mechanical codes, applicants are requested to review the following checklist prior to
and during design. Applicants are required to indicate appropriately and to submit the
completed checklist at time of application for a permit.
Plans to be included for a Building Permit, must be on drafting paper at least l8"x24" and
drawn to scale.
Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with

decks, balcony, steps, hand rails and guard rails, windows and doors, including the finish grade line
and original grade. A section showing in detail, from the bottom of the footing to the top of the roof,
including re-bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house-rap, (which is required), siding or any approved building material. Engineered
foundations may be required.
A window schedule. A door schedule. A floor framing plan, a roof framing plan, roof must be
designed to withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind
speed, wind exposure B or C, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or
your plans will be returned.
All plans submitted must be incompliance with the 2003 IRC.
1. Is a site plan included that identifies the location of the proposed structure or addition and
distances to the property lines from each comer of the proposed structure(s) prepared by
a licensed surveyor and has the surveyors signature and professional stamp on the
drawing? Properties with slopes of30% or greater must be shown on the site plan.
(NOTE Section: 106.2) Any site plan for the placement of any portion of a structure
within 50 ft. of a property line and not within a previously surveyed building envelope on
a subdivision final plat shall be prepared by a licensed surveyor and have the surveyor's
signature and professional stamp on the drawing. Any structure to be built within a
building envelope of a lot shown on a recorded subdivision plat shall include a copy of
the building envelope as it is shown on the final plat with the proposed structure located
within the envelope.

Delays in issuing the permit are to be expected. Work may not proceed without the issuance of a
permit. Ifit is determined by the Building Official that additional information is necessaty to review

INSTRUCTIONS:
This Application & Check1ist is to be used for all project submittals to the Eagles Point Architectural
Conunittee (EP/AC). This form is intended to assist in ensuring that the application submittal is
complete. This wi11 help provide a quicker, more thorough review process.
It is recommended that each Applicant thoroughly review the requirements of the current
Architectural Standards, the DecJaration of Covenants {including Resolutions) and the recorded
Final Plat prior to preparing a submittaL
Applicants preparing a submittal should compare the information they are about to submit with the
requirements of this form and Acheck off@ each required item on the form. If any item is not
checked, the Application submittal is not complete and should not be submitted. Incomplete
Applications will not be processed and will be returned to the Applicant.
In addition to the checklist, Applicants should fill in all of the places on the Application Form where
information is requested. The Architectural Committee wi11 use this information in evaluating your
Application. If information is requested but is not applicable to your specific project submittal, write
'AN/A@ in the space provided. If some required information is yet to be determined (such as exterior
paint colors you may not have yet selected) write A TBD@ in the appropriated space provided. You
will then be required to make a fo11ow-up submittal with these selections at a later date but before
installation in the home.

Dear Mr. Perry:
As requested, Hepworth-Pawlak Geotechnical. Inc. performed a s ubsoil study for design
of foundations at the subject site. The study was conducted in accordance with our
agreement for geotechnical engineering sen·ices to you dated December 9, 2005. The
data obtained and our recommendations based on the proposed construction and
subsurface conditions encountered are presented in this report. Hepworth-Pawlak
Geoteclmical. Inc. previously performed a preliminary geotechnical study for
development of the subdivision and presented our findings in u report dated November
21 , 2003 , Job No. I 03 680.
Proposed Construction: The proposed residence will he a two story wood frame
structure with an attached garage localed on the site between the exploratory pits shown
on Figure I . Ground floor will be structural over a crawlspace tor the residence and slab·
on-grade in the garage. Cut depths are expected to range up to about 3 feet. Foundation
loadings for this type of construction are assumed to be relatively light and typical of the
proposed type of construction.
If building conditions or foundation loadings arc sigmficantly d i fferent from those
described above, we should be notified to re·C\'aluatc the recommendations presented in
this report.
Site Conditions: The site was v acant at the time of our tield exploration. The ground
surface of the lot is relatively flat. The vegetation has been remo\'ed from the lot.
Subsurface Conditions: The subsurfac e conditions at the site were evaluated by
excavating two exploratory pits at the approximate loc ations shown on Fibrure 1. The
logs of the pits nrc presented on Figure 2. The subsoils encountered, below about 1 toot
oftopsoil and 1 to 2'h ofmedtum stiff, sandy clay and silt. consist of relatively dense,

basalt !:,'Tavel. cobbles and boulders in a sandy clay matrix t o the pit depths of2'l'2 to 4~
feet. Results of swell-consolidatio n resting perfimned on a relatively undisturbed sample
of the clay. presented on Figure 3. indicate low compressibility under existing moisture
conditions and light loading and a minor expans10n potential when wetted. No free water
was observed in the pits at the time of excavation and the soils were slightly moist. A
boring drilled to the east of the lot as part of the preliminary study encountered about 5Yl
feet of clay soils O\'erlying the ba'\alt rock soils and no groundwater to the drilled depth of
12 feet in Octob er 1003 .
Foundation Recommendations: Considering the subsoil conditions encountered in the
exploratory pits and the nature of the proposed construction. we recommend spread
footings placed on the undisturbed natural soil designed for an allowable soil bearing
pressure of 1 ,500 psf for support of the proposed residence. The clay soils in the area tend
to have variable settlement/heave potential when wetted and there could be some post-
construction foundation movement if the bearing soils become wetted. Bearing entirely
on the basalt rock soils will help mitigate the pot~ntial expansiorusettlemcnt of the clay
soils. The settlement/heave potential should be further e\'aluated at the time of
construction. Footings should be a minimum width of 18 inches for continuous walls and
2 feet for columns. The topsoil and loose disturbed soils encountered at the foundatwn
bearing level within the excavation should be removed and the footing bearing level
extended down to the undisturbed natural soils. Voids created by the removal oflarge
rocks should be backfill with compacted sand and ~'t'avel, such as road base, or with
concrete. Exterior footings should be provided with adequate cover above their bearing
elevations for frost protection. Placement of footings at least 36 inches below the exterior
grade is typically used in this area. Continuous foundation walls should be reinforced top
and bottom to span local anomalies such as by assuming an unsupported length of at least
12 feet. Foundatio n walls acting as retaining structures should be designed to resist a
lateral earth pressure based on an equivalent fluid unit weight of at least 50 pcf for the on-
site soil, excludin g v egetation. topsoil or oversized rock. as backfill.
Floor Slabs: The natural on-si t e soils. exclusive of topsoil, are suitable to support lightly
loaded slab-on-b'l'ade construction. The clay soils have 'ariable settlement/heave
potential which co uld result in some slab movement if the bearing soils become wetted.
To reduce the effect s of some differential movement, floor slabs should be separated from
all bearing walls and co lumns with expansion joints which allow unrestrained vertical
movement. Floor slab control joints should be used to reduce damage due to shrinkage
cracking. The requirements for joint spacing and slab rei nforcement should be
established by the designer based on experience and the intended slab use. A minimum 4
inch layer of sand and gravel. such as r oad base should be placed below interior slabs-on-

~'fade as a leveling course and for subgrade suppon. This material should consist of
minus 2 inch aggregate with Jess than 50% passing the "''o . 4 steve and Jess than 12%
passing the No. 200 sieve.
All fiJI materials tor support of floor slabs should be compacted to at least 95% of
maximum standard Proctor density at a moisture content near optimum. Required fill can
consist of the on-site soils devoid of\·egetation. topsoil and oversized rock.
Surface Drainage: The followmg drainage precautions should ht! observed during
construction and maintained at all times after the residence has been completed:
I) Inundation of the foundation excavations and underslab areas should be
avoided during construction. Drying could increase the expansion
potential of the clay soils.
2) Exterior backfill should be adjusted to ncar optimum moisture and
compacted to at least 95% of the maximum standard Proctor density in
pa,·cment and slab areas and to at least 90% of the maximum standard
Proctor density in landscape areas.
3) The ground surface surrounding the exterior of the building should be
sloped to drain away from the foundation in all directions. We
recommend a minimum slope of 6 inches m the first 1 0 feet in unpaved
areas and a minimum slope of 3 inches in the first 10 feet in pavement and
walkway areas.
4) Roof downspouts and drains should discharge well beyond the limits of all
backfill.
5) Irrigation sprinkler heads and landscaping which requires regular heavy
irrigation, such as sod, should be located at least 5 teet from foundation
walls.
Limitations: This study has been conducted in accordance with generally accepted
geotechnical engineering principles and practices in this area at this time. We make no
warranty either express or implied. The conclusions and recommendations submitted in
this report are based upon the data obtained from the exploratory pits excavated at the
locations indicated on Figure 1 and to the depths shown on Figure 2, the proposed type of
construction, and our experience in the area. Our sen·ices do not mclude determining the
presence, prevention or possibility of mold or other biological contaminants (MOBC)
developing in the future. If the client is concerned about MOBC, then a professional in
this special field of practice should be consulted. Our findings include interpolation and
extrapolation of the subsurface conditions identified at the exploratory pits and variations
in the subsurface conditions may not become evident until excavation is perfonned. If

conditions encountered during construction appear dlfrerent from those described in this
report, we should be notified at once so re-evaluation of the recommendations may be
made.
This report has been prepared for the exclusive use b y our client for design purposes. We
are not responsible for technical mterpretations by others of our intbnnation. As the
project evolves, we should provide continued consultation and 1ield services during
construction to review and monitor the implementation of our recommendations, and to
verifY that the recommendations have been appropriately interpreted. Significant design
changes may require additional analysis or modifications to the recommendations
presented herein. We recommend on-site observation of cxcavatwns and foundation
bearing strata and testing of structural fil) by a representative of the geotechnical
engineer.
