A final Electrical Inspection from the Colorado State Electrical Inspector; 2. Pennanent address assigned by Garfield County Building Department posted where readily visible from access road; 3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all necessary plumbing; 4 . A complete bathroom, with washbowl, tub or shower, toilet stool, hot and cold running water, non-absorbent floors and walls finished and a privacy door; 5. All steps outside or inside over three (3) steps must have handrails, guard rails on balconies or decks over 30" high constructed to alll997 UBC requirements; 6. Outside grading done to where water will detour away from the building; 7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until all the required items are completed and a final inspection made; 8. A final inspection sign off by the Garfield County Road & Bridge Department for driveway installation, where applicable; as well as any final sign off by the Fire District, where applicable. A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL THE ABOVE ITEMS HAVE BEEN COMPLETED. ****CANNOT OCCUPY OR USE DWELLING UNTIL A CERTIFICATE OF OCCUPANCY (C.O .) IS ISSUED. Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with decks, balcony, steps, hand rails and guard rails, windows and doors, including the finish grade line. A driveway approach is understood to be that portion of the county road right-of way between the pavement edge and the property line that is designed and used for the interchange of traffic between the roadway and abutting property. 2. At any intersection, a driveway shallqe restricted for a sufficient distance from the intersection to preserve the normal and safe movement of traffic. (It is recommended for rural residence entrances that a minimum intersection clearance of 50 feet be provided and for rural commercial entrances a minimum of 100 feet be provided .) 3. All et.trances and exits shall be so located and constructed that vehicles approaching or using them will be able to obtam adequate sight distance in both directions along the county road in order to maneuver safely and without interfering with county road traffic. 4 . The construction of parking or servicing areas on the county road right-of-way is specifically prohibited. Commerci al establishments for customer vehicles should provide off-the-road parking facilities . 10. The grade of entrance and exit shal1 slope downward and away from the road surface at the same rate as the nonnal shoulder slope and for a distance equal to the width of the shoulder but in no case less than twenty (20) feet from the pavement edge. Further, any frozen material will be removed from site by permittee. 2) When a paved or chipped surface is cut, it is to be replaced with a minimum three-inch (3") hot mix asphalt patch. Temporary patch with cold mix asphalt would be allowed, which would be replaced with hot mix asphalt when the weather or availability of materials aJJow the work to be completed. An asphalt or chipped surface cut would require the road to be straight cut and squared by means of a pavement cutter or saw. A chipped surface cut will requir~ a seal coat of the patch, which will be full width or half width of roadway when trenches run with the road. !'or trenches across the road, the edges of the seal coat will overlap the existing edge. A 3 foot high orange plastic fence will be installed daily to keep livestock out of the open trench after hours . 7) In areas being disturbed where vegetation is established, reseeding with a mixture either by broadcasting, drilling and mulching with seed mixtures suitable to the climatic conditions or existing vegetation wiii be performed. The findings of our observations are presented in this report. The services were performed in accordance with our agreement for geotechnical engineering services to you, dated February 16, 2004. The site is located in the bottom of the Roaring Fork River Valley, east of Carbondale and north of County Road 100, northwest of Mayfly Bend Subdivision and south of the Roaring Fork River. Vegetation consists of cottonwood trees and brush with a ground cover of grass and weeds. There was about 6 inches of snow cover at the time of our ~ site visit. The ground surface is relatively flat with a slight slope down to the north. The Oxford irrigation ditch crosses through the site from east to west about 40 feet north of the Profile Pit. The proposed building site is about 120 feet to the east of the Profile Pit. A Profile Pit had been excavated at the location shown on Figure 1. The subsoils exposed in the pit consisted of 1 foot of topsoil overlying slightly silty to silty, fine to medium sand to the bottom pit depth of 7 feet. Garfield County, CO SE Job No. 24020.01 Dear George & Andy: Pursuant to County Regulations, this Jetter provides documentation that the new Onsite Wastewater System (OWS), installed approximately 3 years ago. is in general compliance with the permitted design. This letter also documents the As-built conditions of the Garage-ADU Structure in tenns of the Finish floor elevation. with respect to the base flood elevation (BFE). Sopris Engineering has petformed site visits to measure, inspect. and document the as -built conditions of the constructed system. We have coordinated our efforts with the treatment system installer, and the contractors that constructed the system. The system was inspected prior to backfilline and after all installations was completed . The as-buHt conditions and installation of the new OWS components is in compliance with the permitted design specifications for the system. The as-built field dimensions and location of the new OWS treatment components were installed with respect to existing site conditions and are marked in the field by the component risers and inspection ports installed to the finish surface grades. In addition we have checked the system in coordination with the Service provider that has performed required standard maintenance. bi-annually, pursuit to the ongoing maintenance agreement contract The System was installed in accordance with Garfield County Regulations; the design parameters presented in the Sopris Engineering design drawing, dated ApriJ 30, 2004 and the design report, dated May 6, 2004. A l 500-gallon dual compartment septic tank was installed with an Orenco Systems, Tnc . Advantex AX-20 treatment system installed with the septic tank. The Advantex re-circulating packed bed filter assembly was instaJied by a qualified installer. The system has a 24" diameter by 7' deep Orenco dosing pump basin installed adjacent to the septic tank. The simplex pump assembly is designed to pressure dose treated effluent to the perforated field laterals through a distribution manifold. The systems control box has a remote telemetry unit that shares a connection to a phone line. A Three-year inspection and maintenance agreement is in place, which provides for routine maintenance and inspections every 6 months by a qualified service provider. The maintenance agreement contract is due for renewal next year. Flow is split equally between the chamber rows through the pressure manifold to the individual distribution laterals running the full length of the chamber rows. Inspection ports were installed on each end chamber and on the head comer chambers. The overall dimension of the dispersal field is 75 ' by 9' mounded approximately 3.25-feet above the natural surface grade. The field was installed level with the chambers installed over 24-inches of filter sand overlying suitable native soils. Suitable native soil is backfilled over and around the chambers. The 24-inch layer of filter sand below the leaching chambers provides a basal area ofapproximately1400 S.F . Treated effluent from the septic tank/ Advantex treatment system is pressure dosed to a manifold at the head of the bed. The flow is equally split to each row in the field. Each chamber row has a 4" inch perforated PVC distribution pipe running the length of the row. The construction was completed utilizing specified materials installed according to design. The minimum setback distances have been maintained. Advantex AX-20 The advanced treatment system installed is an Orenco Systems Advantex-AX·20 unit (re·circulating trickling packed bed filter). The system is capable of producing effluent quality which meets or exceeds the requirements of the US EPA for secondary wastewater treatment (30mg/L BOD and 30 mg/L suspended solids). The dispersal field is sized to adequately service (4) bedrooms. AllOWS components are accessible from the surface to facilitate system testing and maintenance. The return flow is discharged back to the secondary compartment for nitrogen removal. The Advantex AX-20 is designed to provide treatment resulting in effluent water quality parameter levels of 30 mg/L BOD and 20 mg!L TSS based on a design flow of 1125 gpd. Maintenan~e Protocol The engineered system shall continue to be inspected on a regular basis and be properly maintained . Ownership of the system and responsibility for construction! repair and maintenance will remain with the Lot Owner. The Owner shall continue to retain the services of qualified persotulel to inspect the OWS and to perfonn all maintenance and repairs necessary to ensure that the system is functioning properly, is in good operating condition and is in compliance with the manufacturer's performance requirements. The operating components of the OWS system will be serviced and inspected bi-annually. Garage-ADU Finish Floor Elevation & Base Flood Elevation The Garage -ADU structure was constructed that includes a 1-bedroom studio apartment over a single garage with adjacent accessory space. An adjacent 3-bedroom single-family home will be built in the future. The new garage-ADU structure is generally located as shown on the design plan. Applicable floodplain information for the site was referenced from the FEMA; FIRM map (panel No., 080205 1880 B) dated January 3, 1986: which indicates that a portion of the site is in Zone A. The existing grades shown on the plan are based on a recent topographical survey by Sopris Engineering, using the 1929 vertical datum as referenced on the FEMA map.
