*CONDITIONS:
1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs, alters, or installs an individual seWage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine -6
months in jail or both).

ALL SERVICE septic, LLC prepared an onsite wastewater system (OWS) design for the subject
residence. The property is located in a rural residential area where OWS and wells are necessary. HP
Geotech prepared the Subsoil study dated December 4, 2003, and is included in the Appendix.
SITE CONDITIONS
The site is vacant and has moderate slopes to the northwest. A 4-bedroom residence is proposed.
Vegetation consists of juniper and pinon trees with sagebrush. A site plan is included as Figure I.
SUBSURFACE
The subsurface was investigated by HP Geotech on December 2nd 2003 by excavating a test pit, and
three percolation holes, near the proposed drain field location as shown on Figure I. HP Geotech
indicated the test pit at the site ha.~ a 6·inch topsoil layer, underlain sandy silty clay to 2.5 feet,
underlain by silty gravelly sand with scattered basalt cobbles to the maximum deptb explored of 9
feet. Groundwater or bedrock was not encountered. Percolation rates ranged from 11 to 30 minutes
per inch (MPI). The average percolation rate of24 MPI will be used to design the OWS.
DESIGN SPECIFICATIONS
The OWS design is based on 4-bedrooms and an average sewage load of 600 GPD. Ihe installation
must include a 1250-gallon, two-compartment, precast concrete septic tank, with an eftluent filter on
the outlet. Tank alternatives may be considered. Three gravelless chamber trenches must be installed
as indicated on plans provided. Three rows of IS 'Quick 4' chambers, for a total of 45 chambers, and
444.1 ~~quare feet (SF) of infiltrative area, is required. If standard thambers are preferred, 30
ehambers are required. We should be notified If this option is taken. Serial distribution should be
used to distribute etlluent. A shallow installation is preferred due to the presence of large basalt

boulders at a depth of 2.5 feet. If a boulder is encountered during construction, the boulder should be
removed, and concrete sand or onsite sandy soils should placed in void. The sand should be
adequately compacted to minimize settling. Construction must be according to the county ISDS
regulatiollli, the septic permit provided by Garfield County Environmental Health Department, and this
design.
OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Oeo~fabrics or plastics should not be used over the drain field. Livestock
should not graze on the drain field. Plumbing fixtures should be checked to ensure that no additional
water is being discharged to OWS. For Example, a running toilet or leaky faucet can discharge
hundreds of gallons of water a day and harm a drain field.
The homeowner should pump the septic tank every two years and clean the effluent filter as needed.
Garbage disposal use should be minimized, and non-biodegradable materials should not be placed
into the OWS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWS. No hazardous wastes should be directed into the OWS.
Mechanical room drains should not discharge into the OWS. The OWS is engineered for domestic
wasteouly.
ADDITIONAL CONSTRUCTION NOTES
If applicable, air release valves and weep holes should be installed to allow pumping systems to drain
and minimize risk of freezing. Extensions should be placed on all septic tank components to allow
access to them from existing grade. Precast concrete tanks and distribution boxes should be used,
unless plastic or fiberglass is required. Access to all tank compartments and distribution devices is
optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWS during construction. The OWS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
illlitallation.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction
must be according to the county regulatiollli. Requirements not specified in this report must follow
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulatio!lli ofthe county in which they are working.
