The following items are required by Garfield County for a final inspection:
1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assigned by Garfield County Building Department posted where readily
visible from access road;
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent
kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all
necessary plumbing;
4. All bathrooms must be complete, with washbowl, tub or shower, toilet stool, hot and cold
running water, non-absorbent floors and walls finished and a privacy door;
5. All steps outside or inside over three (3) steps must have handrails, guard rails on balconies or
decks over 30" high constructed to all IBC and IRC requirements;
6. Outside grading done to where water will detour away from the building;
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until
all the required items are completed and a final inspection made;
8. A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
9. If you will be connecting to a public water and/or sewer system, proof of the tap fees have been
paid and the connections inspected by the service provider prior to issuance of a C.O.
A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.

MINIMUM APPLICATION REQUIREMENTS
For
MANUFACTURED HOMES
In order to understand the scope of the work intended under a permit application and expedite the issuance of
a permit it is important that complete information be provided. When reviewing a plan and it's discovered
that required infonnation has not been provided by the applicant, this will result in the delay of the permit
issuance and in proceeding with building construction. The owner or contractor shall be required to provide
this information before the plan review can proceed. Other plans that are in line for review may be given
attention before the new information may be reviewed after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your project and
provide adequate information to facilitate a plan review. Also, please consider using a design
professional for assistance in your design and a construction professional for construction of your
project.
To provide for a more understandable plan in order to determine compliance with the building, plumbing and
mechanical codes, applicants are requested to review the following checklist prior to and during design.
Applicants are required to indicate appropriately and to submit the completed checklist at time of application

All plans submitted must be incompliance with the 2003 IRC for Residences and 2003 IBC for

Is a site plan included that identifies the location of the proposed structure or addition and distances to
the property lines from each corner of the proposed structure prepared by a licensed surveyor and has
the surveyors signature and professional stamp on the drawing? Properties with slopes of30% or more
must be shown on site plan. (NOTE Section 1 06.2) Any site plan for the placement of any portion of
a structure within 50 ft. of a property line and not within a previously surveyed building envelope on a
subdivision final plat shall be prepared by a licensed surveyor and have the surveyor's signature and

shown on a recorded subdivision plat shall include a copy of the building envelope as it is shown on

If you have answered "No" on any of the questions, you may be required to provide this information at the
request of the Building Official prior to beginning the plan review process. Delays in issuing the permit are to
be expected. Work may not proceed without the issuance of a permit. If it is determined by the Building
Official that additional information is necessary to review the application and plans to determine minimum
compliance with the adopted codes, the application may be placed behind more recent applications for
building permits in the review process and not reviewed until required information has been provided and the

hereby requests permission and authority from the Board of County Commissioners to construct a driveway approach ( es)
on the right-of-way off of County Road, 309, 2482 ft East of CR 309 CR 301, located on the North side of road for the
purpose of obtaining access to property.
Applicant submits herewith for the consideration and approval of the Board of County Commissioners, a sketch of the
proposed installation showing all the necessary specification detail including:
1. Frontage oflot along road.
2. Distance from centerline of road to property line.
3. Number of driveways requested
4. Width of proposed driveways and angle of approach.
5. Distance from driveway to road intersection, if any.
6. Size and shape of area separating driveways if more than one approach.
7. Setback distance ofbuilding(s) and other structure improvements.
8. No unloading of equipment on county road, any damage caused to county road will be repaired at subdivision
expense.
9. Responsible for two years from the date of completion.
General Provisions
I) The applicant represents all parties in interest, and affirms that the driveway approach (es) is to be constructed by
him for the bona fide purpose of securing access to his property and not for the purpose of doing business or
servicing vehicles on the road right of way.
2) The applicant shall furnish all labor and materials, perform all work, and pay all costs in connection with the
construction of the driveway(s). All work shall be completed within thirty (30) days of the permit date.
3) The type of construction shall be as designated and/or approved by the Board of County Commissioners or their
representative and all materials used shall be of satisfactory quality and subject to inspection and approval of the
Board of County Commissioners or their representative.
4) The traveling public shall be protected during the installation with proper warning signs and signals and the Board
of County Commissioners and their duly appointed agents and employee shall be held harmless against any action
for personal injury or property damage sustained by any reason of the exercise of the Permit.
5) The Applicant shall assume responsibility for the removal or clearance of snow, ice, or sleet upon any portion of
the driveway approach (es) even though deposited on the driveway(s) in the course of the County snow removal
operations.
Specifications
1. A driveway approach is understood to be that portion of the county road right-ofway between the pavement edge
and the property line that is designed and used for the interchange of traffic between the roadway and abutting
property.
2. At any intersection, a driveway shall be restricted for a sufficient distance from the intersection to preserve the
normal and safe movement of traffic. (It is recommended for rural residence entrances that a minimum
intersection clearance of 50 feet be provided and for rural commercial entrances a minimum of 100 feet be
provided.)
3. All entrances and exits shall be so located and constructed that vehicles approaching or using them will be able to
obtain adequate sight distance in both directions along the county road in order to maneuver safely and without
interfering with county road traffic.
4. The Applicant shall not be permitted to erect any sign or display material, either fixed or movable, on or
extending over any portion of the county road right-of-way.
5. Generally, no more than one approach shall be allowed any parcel or property the frontage of which is less than
one hundred (1 00) feet. Additional entrances or exits for parcels having a frontage in excess of one hundred 1 00)
feet shall be permitted only after showing of actual convenience and necessity.
6. · All driveways shall be so located that the flared portion adjacent to the traveled way will not encroach upon
adjoining property.
7. No commercial driveway shall have a width greater than thirty (30) feet measured at right angles to the centerline
of the driveway except as increased by permissible radii. No noncommercial driveway shall have a width greater
than twenty (20) feet measured at right angles to the centerline of the driveway, except as increased by
permissible radii.
8. The axis of an approach to the road may be at a right angle to the centerline of the county road and of any angle
between ninety (90) degrees and sixty (60) degrees but shall not be less than sixty (60) degrees. Adjustment will
be made according to the type of traffic to be served and other physical conditions.
9. The construction of parking or servicing areas on the county road right-of-way is specifically prohibited.
Commercial establishments for customer vehicles should provide off-the-road parking facilities.
I 0. The grade of entrance and exit shall slope downward and away from the road surface at the same rate as the
normal shoulder slope and for a distance equal to the width of the shoulder but in no case less than twenty (20)
feet from the pavement edge. Approach grades are restricted to not more than ten percent (10%).
11. All driveways and approaches shall be so constructed that they shall not interfere with the drainage system of the
street or county road. The Applicant will be required to provide, at his own expense, drainage structures at
entrances and exits, which will become an integral part of the existing drainage system. The Board of County
Commissioners or their representative, prior to installation, must approve the dimensions and types of all drainage
structures.

· 6) In the event it becomes necessary to remove any right-of-way fence, the posts on either side of the entrance shall
be surely braced before the fence is cut to prevent any slacking of the remaining fence and all posts and wire
removed shall be turned over to the District Road Supervisor of the Board of County Commissioners.
7) No revisions or additions shall be made to the driveway(s) or its appurtenances on the right-of-way without

1. The footer and perimeter walls are to use measurements tram the attached foundation
print Measuring the home is not a reliable method for determining foundation size. The
contractor should check the attached print to any other prints (such as engineered prints)
to verify both match, prior to beginning foundation. Variations from tl1is print, that result
in installation problems will be the responsibility of the contractor or the home buyer.
2. The attached foundation design has not been engineered to meet specific soil conditions.
3. There are three basic types of foundation designs that the home buyer and contractor
must be aware of:
a. Non-vented and insulated crawlspace-The home on this foundation would not
have floor insulation, but the foundation walls must be insulated to local code
and no vents would be installed in the perimeter of the foundation. If used this
foundation requires that any fuel burning appliances have extenders to draw air
from outside the crawlspace.
b. Vented crawlspace-This home must hove floor insulation installed at the factoty
or on site. Vents must be installed to manufacturers specifications.
c. Non-vented basement-This home must have floor insulation if the basement is
unfinished or no floor insulation if basement is heated. This scenario can differ,
depending upon State and Local Codes. Buyer must verify what is needed based
upon local building codes.
4. Golden Villa Homes does not make the following connections, unless specifically noted
in writing in your home purchase conlmct. Buyer is responsible for the following
connections.
a. Sewer lines under home. Factory stubs drain line through floor only and they
must be interconnected.
b. Water lines. Water lines are run to all fixtures. There is one central connection
that must be made.
c. Gas lines. The gas Jines are not present and must be connected to individual
appliances.
d. Electrical. Power lines from utility company to interior meter panel are the
responsibility of others. Wiring is complete from home fixtures to panel box,
except hard wire cross over, which must be connected by otl1ers.
e. Dryer vent connections to exterior of home.
f. Water heater pan and pop off drains to exterior of home.
g. Furnace and A/C drain Jines to sewer or exterior of home.
5. Most Cities and Counties require restroom facilities to be available at all times during
construction. Our Contractors have the same requirements. Buyer is responsible to
supply restroom facilities from start of construction until construction is complete.
6. The contractor must provide a crawlspace access so setup materials can be removed from
under the home. Minimum dimensions are 36"x 30"x 36" net. Ell.ierior access will need
to be insulated as to not effect the U value of the home. Jfthe contrnctor or customer
chooses to use an interior crnwlspaces access, all modifications and finish of the access is
the responsibility of others, unless specifically noted in writing in the final Home Order.
This method could result in additional dirt and wear in the homes interior.

9. FOUR SEANSONS HOMES DO NOT HAVE FLOOR INSULATION FROM THE
MANUFACTURER. BONN A VILLA HOMES OFFERS FLOOR INSULATION
FOR HQMES AS AN OPTION. If there is no floor insulation present from tl1e
manufacturer of the home and any portion on the foundation is a crawlspace, the
crawlspace portion must be insulated by the home buyers foundation contractor to meet
State, County or City requirements.
I 0. The customer or contractor are required to install a 2 x 10 treated sill plate. The bolts
used to anchor this sill plate must not be recessed into the plate. The threads must be cut
off at the top of the nut. The sill plate will be used to anchor the home to the foundation,
so a strong connection is imperative. Anchor bolts should be centered or set slightly
toward the interior of the foundation, not toward the exterior of the foundation.
11. The Ridge Beam on BonnnviUn Modular Homes is al2" laminate beam. The floor
joists are 2xl 0. This results in a 2.79' height difference that must he accounted for
in the center line support construction. The laminate beam is notched at each end of
the home, to eliminate perimeter foundation modification. If purchasing a Four Seasons
Modular, there is no height difference between ridge beam and floor joists, so tile pony
wall should be same height as sill plate or slightly lower.
12. If a crane is to be used, the site must be inspected prior to construction to verify specific
placement requirements for the crane and the home sections. Unless specifically stated in
writing, crane costs are not included in the standard setup pricing included with the
homes price.
13. Stem wall height is to be a minimum of31 %''including sill plate, but not including the
foorter. Buyer must notify seller if this measurement of wall height exceeds 48".
14. Rollover Setup Requirements-There must be 30" ofthe foundation exposed in order to
roll a home onto a foundation. The pad on the rolling side or sides must be flat and leveL
The foundation wall must be backfilled and compacted leaving the 30" exposed waiL
The rollover pad is considered the area extending 22' out from the foundation and the full
length of the foundation, plus 5' for the transport hitches. Accommodations must be
made for the delivery truck at the end of the pad site. Grading of site is the responsibility
of others. Ramping too steep of an approach will result in setup delays and downtime
expenses. It is preferable to access both sides of the foundation for a rollover setup. One
sided roll-ons can be done, but greater coordination of delivery and roll-on is required
(see figure ]a. attached). If30" wall is not exposed, the set crew may require a crane to
set the home at the home buyers expense.
15. The transport platforms must be able to be moved out ofthe way, without impeding other
sections to be setup. Generally platforms are picked up every 3 to 4 weeks. Ifthis time
frame is a problem, please notify Golden Villa Homes installation or service department
at (970) 245-0741
16. AFTER INSTALLATION OF HOME. When backfilling the crawlspace, there must
be a minimum 2% grade for 1 0' to channel water away from the foundation. Backfill
must be at least 4" below crawlspace vents in order to prevent water draining into the
crawlspace. Proper grading is the contractor or customers responsibility. The setup
installer has the right to refuse to set any home that appears to not have adequate drainage
on site.

and the backfill is a minimum of 6" away from bottom of siding on the
home. The installers will not remove setup equipment through interior basement access
using portable ladders, other arrangements must be made by customer or contractor. The
basement floor must be completed prior to installation of tbe home, unless buyer is
willing to hlre n crane for tbe installation. If the event of a day-light basement, the
home must be installed from the backfilled side of the foundation. If that is not possible a
crane will be required to complete the set of the home.
18. If you purchased a Bonnavilla Modular Home with a deck, there should be a porch
support detail attached to this information. Bonnavilla Modular decks require specific
support noted on the home print and the above mentioned detail.

1. Hot Water Heater-Pan Drain and Pop Off drain lines must not drain into
crawlspace or basement. Those drain lines must be dt·ained outside building and
foundation.
2. 1-l.E.F. Fumaces/Air Conditioners-High Efficiency Furnaces/Air Conditioners

Homes with open slat porches are not to be enclosed with concrete perlmeter walls, unless the contractor has an
engineer design a way to drain water that gets into the crawlspace area. The home portion must still be fully
enclosed wlth welL
The preferred method Is to install footers at required frost depth, than pour concrete to 4' or 6" above finished
grade and Installing a decl< post mount braclcet for a wood post of adequate size to carry loads. The roundaUon
contractor would then Ins tell treated or approved wood posts to support dec!<. This should give a nicer finished
loolt than concrete columns or blocl<. This is a preferred melhod, not a required method. Steps coming off these
decl<s will more than lilcely be wood instead of concrete If the preferred method Is used.
