The following items are required by Garfield County for a final inspection:
1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assigned by Garfield County Building Department posted where readily
visible from access road ;
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent
kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all
necessary plumbing;
4. All bathrooms must be complete, with washbowl, tub or shower, toilet stool , hot and cold
running water, non-absorbent floors and walls finished and a privacy door;
5. All steps outside or inside over three (3) steps must have handrails, guard rails on balconies or
decks over 30" high constructed to all IBC and IRC requirements;
6. Outside grading done to where water will detour away from the building;
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until
all the required items are completed and a final inspection made;
8. A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable .
9. If you will be connecting to a public water and /or sewer system, proof of the tap fees have been
paid and the connections inspected by the service provider prior to issuance of a C.O .
A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL TilE
ABOVE ITEMS HAVE BEEN COMPLETED.

In order to understand the scope of the work intended under a permit application and expedite the
issuance of a permit it is important that complete information be provided. When reviewing a plan
and it 's discovered that required information has not been provided by the applicant, this will result
in the delay of the permit iss uance and in proceeding with building construction. The owner or
contractor shall be required to provide this information before the plan review can proceed. Other
plans that are in line for review may be given attention before the new information may be reviewed
after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your
project and provide adequate information to facilitate a plan review. Also, please consider
using a design professional for assistance in your design and a construction professional for
construction of your project. Any project with more than ten (10) occupants requires the
plans to be sealed by a Colorado Registered Design Professional.
To provide for a more understandable plan in order to determine compliance with the building,
plumbing and mechanical codes, applicants are requested to review the following checklist prior to
and during design. Applicants are required to indicate appropriately and to submit the
completed checklist at time of application for a permit.
Plans to be included for a Building Permit, must be on drafting paper at least 18"x24" and
drawn to scale.
Plans must include a floor plan , a concrete footing and foundation plan, elevation s all side s with
decks, balcony, steps, hand rails and guard rails, windows and doors, including the finish grade line
and original grade. A section showing in detail, from the bottom ofthe footing to the top of the roof,
including re-bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house-rap, (which is required), siding or any approved building material. Engineered
foundations may be required.
A window schedule. A door schedule. A floor framing plan, a roof framing plan, roof must be
designed to withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind
speed, wind exposure B or C, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or
your plans will be returned.
All plans submitted must be incompliance with the 2003 IRC.
I. Is a site plan included that identifies the location of the proposed structure or addition and
distances to the property lines from each corner of the proposed structure(s) prepared by
a licensed surveyor and has the surveyors signature and professional stamp on the
drawing? Properties with slopes of 30% or greater must be shown on the site plan.
(NOTE Section: 106.2) Any site plan for the placement of any portion of a structure
within 50 ft. of a property line and not within a previously surveyed building envelope on
a subdivision final plat shall be prepared by a licensed surveyor and have the surveyor's
signature and professional stamp on the drawing. Any structure to be built within a
building envelope of a lot shown on a recorded subdivision plat shall include a copy of
the building envelope as it is shown on the final plat with the proposed structure located
within the envelope.

Delays in issuing the permit are to be expected. Work may not proceed without the issuance of a
permit. I fit is determined by the Building Official that additional information is necessary to review
the application and plans to determine minimum compliance with the adopted codes, the application
may be placed behind more recent applications for building permits in the review process and not
reviewed until required information has been provided and the application rotates again to first
position for review, delay in issuance of the permit or delay in proceeding with construction.

As of October I, 2007 all required construction observations for the Post Tensioned Slab-on-Grade Foundation
System at the subject site have been completed by this office.
From our observations, we have concluded that this foundation has been constructed in accordance with the
approved construction documents and guidelines set forth by the Post Tensioning Institute. All required
observations and their subsequent letters by this office for the Post Tensioned Slab-on-Grade Foundation System
which includes the Trench Beam and Steel Observation including Ufer Rod, Tendon Placement (pre-pour)
Observation, and Tendon Stressing Observation have been approved by this office. We also note that the
required insulation has been installed per our structural details and the International Residential Code, 2003
Edition. No further observations are required.

Per your request, we visited the subject site on October I, 2007 to observe the tendon stressing operations for
post-tensioned slab-on-grade design.
The stressing of the tendons were conducted within acceptable tolerances and appeared to be in general
conformance with the construction. At this time, tendon tails may be cut off and the anchor cavities grouted with
a high strength, non-shrink grout. This concludes the construction of the post-tensioned foundation system.

Per your request, we visited the subject site on September 14, 2007 to observe the tendon placement and interior
trench beams of the post-tensioned slab-on-grade. The dimensions, geometry, and required options have not
been verified at this time. The contractor assumes responsibility for the accuracy of all building dimensions,
locations on lot, foundation elevations, and the slab step layout.
The installation of the tendons and interior stiffened beams appeared to be in general conformance with the
construction plans. We observed the top of grade appeared to be at the correct elevation to maintain the slab
thickness per plan. It is the responsibility of the contractor to verify slab thickness and tendon layout remain as
observed during concrete placement. No other items were observed at this time.

beams of the post-tensioned slab-on-grade. The dimensions, geometry, and required options have not been
verified at this time. The contractor assumes responsibility for the accuracy of all building dimensions, locations
on lot, foundation elevations, and the slab step layout.
The trench beam depth, width, and required steel placement appeared to be in general conformance with
approved construction documents. Also, we observed the required l-inch polystyrene foam (extruded) insulation
was placed in the trenches at the exterior of the building footprint per plan and is temporarily to the trench wall.
We also observed that an Ufer rod was placed at the time of the wall steel installation. No other items were
observed at this time.

Per our telephone conversation, we understand that the covered porch option was not placed with the
foundation. Since the foundation at the above subject site is a Post Tensioned Slab-on-Grade system,
we recommend incorporating the optional porch with the existing foundation (Option 1). We
understand that the existing tendons for the foundation have not been cut and could be spliced at the
porch location. Trench beams should be placed at the perimeter of the optional porch. The trench
beams should be 12 inches wide by 4 feet deep with (2) rows of #4 continuous reinforcing steel located
at the bottom of the trench beam. The vertical insulation attached to the existing trench beams should
be removed adjacent to the added trench beams. Embed and epoxy (I) row of #4 bars, 12 inches long,
into the bottom of the existing trench beams and tie to a continuous #4 bar. Embedded bars should be
located 6 inches from the bottom of the existing trench beams. Dowels are to be embedded 4"
minimum and epoxy with Simpson AT structural epoxy or equivalent. Install vertical insulation at the
trench beams per the plan details on Sheet F2.0 for the Optional Covered Porch.
The porch slab should be 9 inches thick with 2" of horizontal insulation (XPS Type IV) under the slab.
New tendons should be spliced to the existing tendon tails. Install a standard splice chuck at the
existing tendon tails. Install a 3 inch diameter PVC sleeve, 12 inches long, around the splice chuck.
Install the end of the sleeve at the opposite end of where the stressing equipment will elongate the
tendon. Seal off the ends of the PVC to tendons using duct tape prior to pouring concrete. We
recommend that the concrete be in place and cured a minimum of I 0 days prior to stressing the new
cables. Refer to Figure I and 2 for clarification.
If some movement in the porch slab is not a concern, we recommend installing 4 foot foundation walls
with footings at the perimeter of the Optional Covered porch (Option 2). The foundation walls should
be 8 inches wide and bear on continuous footings which are 20 inches wide by 8 inches deep. Provide
(2) rows of #4 continuous reinforcing steel at the top and bottom of the foundation walls. The existing
tendons should be cut and patched as required. Provide a 4 inch thick structural concrete slab for the
added porch. lnstall4x4-W2.9xW2.9 wire welded fabric at the mid-depth of the slab and provide 6"
thick void material under the porch slab. Cantilever the structural porch slab over the added foundation
wall to the existing foundation and provide an expansion material. Note that the new porch foundation

walls and slab should not be connected to the existing Post Tensioned Slab-on-Grade foundation. Refer
to Figure 3 for clarification.
A representative from this office must observe the repairs to the added porch prior to placement of new
concrete. For Option 2 only, the soils engineer of record should be called out to observe the footing
placement and the on-site soil conditions for the new foundation walls. If a third party engineer is
contacted to observe the foundation walls and structural slab for Option 2, please provide our office
with a copy of the approval letter.
