Garfield County Building Department. My wife, Cynthia Calvin, and I are planning to build a

determined by the Building Official that additional information is necessary to review the application
and/or plans for compliance, the application may be placed on hold until the required information is
provided. Work may not proceed without the issuance of a permit.
The building department will collect a Plan Review fee at time of application submittal. The permit fee, as
well as any Septic or Road Impact fees will be collected when the permit is issued.
The permit application must be signed by the owner or by person having written authority from the owner
to act as their representative.
Building cannot be occupied until a Certificate of Occupancy (CO) is issued. Using the building without a
CO will be considered an illegal occupancy and may be grounds for vacating the premises. (Final/CO
inspection requirements will be attached to your approved field set of plans.)

9/26/11 Review plans, called contractor Doug Hayes, informed him each set of architectural plans differ
and structural plans do not reference architectural plans. Request revised plans.
10/5/11 Received/reviewed revised plans, forwarded to front counter for permit issuance.
1/27/12 A stop work order was issued by the building inspector. Need to verify setbacks and building
height due to relocation of structure.
2/8/12 Meeting with architect Dick Hampleton to discuss submittal requirements/documentation to
satisfy the height limitations.
2/10/12 A partial stop work order was lifted for areas not affecting the height restriction.

Subject: Observation of Excavation, Proposed Residence, Lot 13, Stirling Ranch,
0486 Schooner Lane, Garfield County, Colorado
Dear Mr. McShave & Ms. Calvin:
As requested by Doug Hayes, a representative of Hepworth-Pawlak Geotechnical, Inc.
observed the excavation at the subject site on October 13, 2011 to evaluate the soils
exposed for foundation support. The findings of our observations and recommendations
for the foundation design are presented in this report. We previously conducted a subsoil
study for design of foundations at the site and presented our find ngs in a report dated
July 26, 2010, Job No. 110 171A. The services were performed as additional services in
accordance with our proposal for professional engineering services to you, dated May 20,
2010.
At the time of our visit to the site, the foundation excavation had been cut in multiple
levels from 2 to 6 feet below the adjacent ground surface. The soils exposed in the
bottom of the excavation consisted of basalt gravel, cobbles and boulders in a calcareous
sandy clay matrix. Results of swell -consolidation testing performed on a sample of the
matrix soils taken from the site, shown on Figure 1, indicate the matrix soils have low to
moderate compressibility under conditions of loading and wetting with a minor collapse
potential (settlement under constant Ioad) when wetted under a light load. No free water
was encountered in the excavation and the soils were slightly moist.
The soil conditions exposed in the excavation are consistent with those previously
encountered on the site and suitable for support of spread footings designed for the
recommended allowable bearing pressure of 2,000 psf. Loose and disturbed soils should
be moistened and compacted or removed in the footing areas to expose the undisturbed
natural soils. Other recommendations presented in our previous report which are
applicable should also be observed.
The recommendations submitted in this letter are based on our observation of the soils
exposed within the foundation excavation and the previous limited subsurface exploration
at the site. Variations in the subsurface conditions below the excavation could increase

the risk of foundation movement. We should be advised of any variations encountered in
the excavation conditions for possible changes to recommendations contained in this
letter. Our services do not include determining the presence, prevention or possibility of
mold or other biological contaminants (MOBC) developing in the future. If the client is
concerned about MOBC, then a professional in this special field of practice should be
consulted.
