Dear Jerry:
Per our discussion, the horizontal reinforcing bars shall have hook bars added to the top &
bottom bars only in the wall reinforcing. The center bar may remain without hooks. The
hooks shall be the same diameter as the horizontal bar.
The top bars shall have a 24" x 24" hook bar lapped to each of the top bars. The bottom
bars shall have either a 24" x 24" hook bar, or if access is limited, a 6" x 24" hook may be
installed from each direction with the 6" dimension overlapping in the comers. These hooks
again shall be placed on each horizontal bar.

Dear Jerry:
Per your request, I have reviewed the following items from the Building Department's
field report.
The continuous concrete strip & pad footings on the interior of the building may be
formed 9'/" thick in lieu of the 10" thickness shown in details 1/S3.1 & 2/53.1.
The exterior continuous footings may be formed with 2x10's with a 2x2 sistered on
top for a total depth of approximately 10 %". This would allow the exterior walls to be
formed with 2 courses of 16" tall ICF blocks for a total wall height of 32". The wall
reinforcing should remain unchanged.
The foundation was designed for an allowable bearing pressure of 1000 PSF as
specified by the Soils Report by HP Geotech # 107 0639 dated December 4, 2007.
The soils report recommends that a follow up site inspection be performed to
observe actual conditions but this is at the Owner's discretion.

Dear Jerry:
Per your request, I am writing to respond to the building department's question regarding the
assumed soils bearing pressures for this project. You provided us with soils report # 107
0639, by HP Geotech dated 12/14/07 that was provided for the High Mesa Development.
The report gave 2 different soils bearing pressures depending on which type of bearing soils
the building was founded on. We were not provided with a site specific location for the
building so we assumed the lower bearing pressure option of 1000 PSF. There were a few
borings which indicated high compression when wetted.
The soils report recommends that site specific bearing pressures be reviewed by a soils
engineer after excavation and before foundation placement, to verify the design pressures
given in the report. The Owner has the option to proceed without this verification as long as
the Owner is willing to accept the associated risk.

Jerry,
As requested, Schmueser Gordon Meyer reviewed your installation at the subject site. The
system was designed as a three bedroom dwelling, under Job No. 2009-368.001.
The installation included a new 1,250 two compartment septic tank with a Model 312 dosing
siphon installed in the second compartment of the tank. At the time of installation, the risers had
not been sealed allowing for groundwater infiltration and has now been corrected. The effluent
discharges to a five outlet distribution box equipped with speed levelers and at the time of
installation did not have a baffle. The original installation specified had a larger six outlet
distribution box to allow for additional settling prior to discharge. Upon a subsequent site visit it
was noted that a baffle had been installed in the original box. It was also noted that the speed
levelers were installed incorrectly and it was agreed that this would be corrected. The
absorption field is comprised of five trenches each with 17 units. There was a clean out
adjacent to the house septic for maintenance purposes.
The observed components of the OWTS were installed in general conformance with our design
and as per the attached as built drawing. The installation observations do not imply a guarantee
or warranty of materials or workmanship.
