FIELD REPORT #3 To: Nate Kaegebein, Kaegebein Fine Home Building Location: Job Site Job: Polen Residence, 1200-66 Date: February 22, 2013 Present: Nate Kaegebein, Kaegebein Fine Home Building Paul Stoffel E.I.T, Oddo Engineering Nate, oddo As you requested, on February 22, 2013 Oddo Engineering's personnel was present on site to inspect and review specific floor joist headers and penetrations in certain locations specified by the building inspector. Please see below for field observation notes: General: 1. Master Bedroom Ceiling - The existing 17'-0" long 11 7/8" TJI/210 and 11 7/8" MicrolIam (ML) @ 16" o.c. floor joist that support the 32" long 11 7/8" ML header at approximately 14'-6" from the exterior wall are acceptable as currently installed. Verify that either a Simpson ITT or MIT hangers are used for the cut floor joist as well as the 32" long header to the floor joist. Verify that each side of the 11 7/8"TJI shall have a web packout (backer block) with 3/4" x minimum of 12" long plywood and nailing per manufacturer. 2. Kitchen Ceiling - The existing 16'-6" long 11 7/8" TJI/210 floor joist at 16" o.c. that support the 32" long 11 7/8" ML header at approximately T-0" and 10'-0" from the exterior wall are acceptable as currently installed. Verify that either a Simpson ITT or MIT hangers are used for the cut floor joist as well as the 32" long header to the floor joist. Verify that each side of the 11 7/8"TJI shall have a web packout (backer block) with 3/4" x minimum of 12" long plywood and nailing per manufacturer and each header location. 3. Master Bathroom Ceiling - The existing 18'-6" long 11 718" TJI/210 floor joist at 12" o.c. that support the 32" long 11 7/8" ML header at approximately 2'-0", 7'-0". and 16'-6" from the exterior wall are acceptable as currently installed. Verify that either a Simpson ITT or MIT hangers are used for the cut floor joist as well as the 32" long header to the floor joist. Verify that each side of the 11 7i8"TJI shall have a web packout (backer block) with 3/" x minimum of 12" long plywood and nailing per manufacturer and each header location. Guest Master Bathroom Floor — Two of the adjacent 11 7/8 TJI/210 floor joist top flanges have been cut at the pump in the floor which is not permitted and not acceptable. Both of these damaged TJI's shall be removed and be replaced with either new 11 7/8" TJ1/210 or 11 7/8" ML. Verify that either a Simpson ITT or MIT hangers are used for the cut floor joist as well as the 32" long header to the floor joist,. Verify that each side of the 11 7/8"TJI shall have a web packout (backer block) with %" x minimum of 12" long plywood and nailing per manufacturer and each header location. Install the holes through the floor joist from the pump per manufacturer's specifications and locations. Notify Oddo Engineering if locations will differ. Temporary shore the existing framing at the floor as required during removal and installation of the new floor joist. Please let me know if you have any additional questions or comments. FIELD REPORT #3 ADDENDUM #1 Ed, oddo it engineering Per our phone conversation regarding the Polen Residence as -built conditions for the floor joist framing at the Master Bedroom Ceiling, see below for the addendum to note 3 on Field Report #3: 3. Master Bathroom Ceiling - The existing 18'-6" long 11 7/8" TJI/210 floor joist at 12" o.c. that support the 32" long 11 7/8" ML header at approximately 2'-0", 7'-0", and 16'-6" from the exterior wall are acceptable as currently installed. Verify that either a Simpson ITT or MIT hangers are used for the cut floor joist as well as the 32" long header to the floor joist. Verify that each side of the 11 7/8"TJI shall have a web packout (backer block) with 3/4" x minimum of 12" long plywood and nailing per manufacturer and each header location. However, the web packout (backer block) is not required at the 3" dia. hole at the 2'-0" mark. Addendum — The web packout (backer block) is only required at header locations where there are no penetrations through the joist. Verify that a Simpson ITT or MIT top flange hanger is installed per manufacturer specifications. If a face mount hanger is required or installed then install the web packout as noted in note #3. Master Bathroom Ceiling - The existing 18'-6" long 11 718" TJI/210 floor joist at 12" o.c. that support the 32" long 11 7/8" ML header at approximately 2'-0", 7'-0", and 16'-6" from the exterior wall are acceptable as currently installed. Verify that either a Simpson ITT or MIT hangers are used for the cut floor joist as well as the 32" long header to the floor joist. Verify that each side of the 11 718"TJI shall have a web packout (backer block) with 3/4" x minimum of 12" long plywood and nailing per manufacturer and each header location. However, the web packout (backer block) is not required at the 3" dia. hole at the 2'-0" mark. Addendum - The holes are actually 2 %z"0 and not 3"0 as noted in FR #3. The two 2'/2"0 holes are spaced 4'/z" apart at approximately the 2'-O" mark from the exterior wail and are acceptable as field installed with no additional reinforcement required to the TJI joist.
