Please review this document to determine if you have adequate information to design your project and to facilitate a plan review. SINGLE FAMILY OR DUPLEX BUILDING PERMIT APPLICATION CHECKLIST This checklist is to be used with all single family dwelling construction, APPLICATION FORM: Please make sure the following information is provided on the application • Parcel number obtained from the County Assessor's Office. • Job address (assigned physical address) • Legal Description: lot, block, filing, subdivision or tract, section, township and range. • Owners name, mailing address, phone, fax, or cell. • Contactors name, mailing address, phone, fax, or cell • Architect/Engineer's name, mailing address, phone, fax or cell. • Building size, height, number of stories, lot size. • Use of building (residential) • Class of Work (new, addition, alteration, repair, demo, move, other) • Description of work. • Type of heating (natural gas, propane, electric, other). • Sewer system (Community,or ISDS), also see other items below. • Garage (attached or detached). • Driveway permit (please see other items below) • Valuation (labor and materials) see attached worksheet. OTHER ITEMS: • If you anticipate obtaining a water -tap from the City of Rifle, please provide a letter indicating that the City will provide water service. (Required to submit building permit application). • ISDS requires a septic permit application to be submitted with the building permit application. • A separate electrical permit must be obtained from the State of Colorado Electrical Board. • Prior to submittal of a building permit, you are required to show proof of legal and adequate access to the site. This may include proof of right to use a private easement, County Road & Bridge permit, or Colorado Department of Highway permit, including a Notice to Proceed. The County Road & Bridge Department can be reached at 970-625-8601. • If you anticipate excavating or grading prior to issuance of a building permit, you will be required to obtain a separate grading permit. • If you belong to a Homeowners Association (HOA), it is your responsibility to obtain written approval, if required. • Provide copies of any Resolutions and/or Land Use Permits associated with this property. • Additional Dwelling Units may require Administrative Review. • Minimum dwelling dimension is 20 ft wide x 20 feet long. • A Colorado State Licensed Electrician and Plumber must perform installation and hook-ups, unless the homeowner is performing the work. BUILDING PLANS: Two (2) sets of plans must be submitted. Plans must be 18"x24" minimum size, complete, identical, legible, and to scale. All plans will be checked at the counter for completeness. If any of the required information is missing, the building permit application cannot be accepted. SITE PLAN: Please make sure the following information is provided on the site plan. • Property address/legal description. • North Arrow and Scale on each page. • Show all property lines, building envelopes, flood plain, and easements. • Provide setback distances from boundaries, buildings, septic, well, and waterways. • Location and direction of the county or private road and driveway accessing the property. • Proposed and existing structures include sheds, barns, decks, patios, and any other buildings. • Existing and proposed grade, positive drainage around structure (to be contained on site). • Streams, rivers, creeks, springs, ponds and ditches. • Existing and proposed wells, septic tanks, leach fields, and other systems. (if applicable) • Retaining walls (engineer stamped detail required for walls over four (4) ft, measured from bottom of footing to top of wall). NOTE: Any site plan depicting the placement of any portion of the structure within fifty (50) feet of a property line or not within an established building envelope shall be prepared, stamped, and signed by a licensed surveyor. For structures to be built within a building envelope, provide a copy of the recorded subdivision final plat with the proposed structure located in the building envelope. ARCHITECTURAL PLANS: Please provide the following information on the architectural plans. • Minimum four (4) elevations (N,S,E,W) indicate height of building measured from existing grade to the midpoint between the ridge and eave of a gable or shed roof or to the top of a flat roof. • Floor plans for each level including dimensions and scale noted. • Clearly label each room or space (bedroom, bath, kitchen, closet, etc). • Show the location of the mechanical equipment. • Building cross sections with construction details. • Window sizes, types, and operation noted on the floor plans or elevations. • Specify roof slope/pitch, roof covering, and siding materials. • Stair and guard details specifying rise, run, height and spacing. • Attic, roof, and crawlspace ventilation details. Indicate size and location of ventilation openings. • Minimum insulation shown for walls, floors, roofs/ceilings, slabs, basements and crawlspaces. • Provide stove and fireplace make, model, EPA or Colorado Phase II Certification. Masonry fireplaces shall be designed to the International Residential Code (IRC) requirements. STRUCTURAL PLANS: Please provide the following information on the structural plans. • Design specifications to include roof and deck snow load, wind and seismic design, frost depth. • Foundation plan showing complete footing and foundation dimensions. • Footing and foundation details specifying reinforcement and referenced on plans. • Framing plans for each floor level and roof. All beams, columns, joists, rafters, and trusses specified. Pre -manufactured structures require the engineers stamp, signature, and date. • Framing /connection details, braced wall lines, shear wall schedule, location and hold downs. NOTE: If any required information is missing, delays in issuing the permit are to be expected. If determined by the Building Official that additional information is necessary to review the application and/or plans for compliance, the application may be placed on hold until the required information is provided. Work may not proceed without the issuance of a permit. The building department will collect a Plan Review fee at time of application submittal. The permit fee, as well as any Septic or Road Impact fees will be collected when the permit is issued. The permit application must be signed by the owner or by person having written authority from the owner to act as their representative. Building cannot be occupied until a Certificate of Occupancy (CO) is issued. Using the building without a CO will be considered an illegal occupancy and may be grounds for vacating the premises. May 4, 2004 Ron Vincent 119CAABC Aspen, CO 81611 RE: Vincent Property Barn/Recreation cabin -- Proposed OWS System, Lot 22, Lookout Mountain Ranch Subdivision, Garfield County, CO SE Job No. 24064.01 Dear Ron: Pursuant to your request this letter/report presents our findings in regard to the feasibility and design of an engineered Onsite Wastewater System (OWS) for the above referenced Site. This design is based on our evaluation of the site conditions with information provided by others for use in supporting your application to Garfield County. Our recommendations are in accordance with Garfield County and the State of Colorado ISDS Regulations. Garfield County must permit any proposed improvement to the site. We have reviewed the information forwarded to us, formulated art ©WS design and created a site sketch with construction details as part of our scope of work. Conclusions Based on our findings we believe that the design and installation of an approved OWS system is feasible in accordance with the Regulations of Garfield County and the State of Colorado. We recommend that a new 1000 gallon septic tank be installed that will discharge effluent to a 186 square foot soil absorption trench system. The soil absorption system can be installed in natural soils and will be located within the required setbacks down gradient of the proposed cabin, as delineated on the attached plan. The system will meet all required setbacks and be installed within the general boundaries indicated on the plan. Our Design is outlined below and delineated on the attached site plan. Site Location and Existing Conditions The subject site is located on Lot 22, Lookout Mountain Ranch Subdivision, Garfield County, CO off County Road 120, in Garfield County, Colorado. The Site comprises approximately 10.23 acres. The site is bounded in all directions by range lands and adjacents lots. The site has a moderate slope (15%-25%) eastward_ The site is covered with native and pasture grasses and scrub brush. Domestic water will be supplied by an onsite well. There are no wells within 100' of the general vicinity of the absorption field. Proposed Site Conditions It is our understanding that you intend to construct an 800 S.F. cabin adjacent to the existing barn. The maximum number of rooms to be utilized as bedrooms in the structure is 1. The proposed improvements will include a new OWS system with appropriate site grading. (HP). The HP report is dated May 8, 2004, Job Number 102 302_ The subsoil encountered at the site consists of topsoil overlying basalt fragments and cobbles in a silty sand matrix. Groundwater was not encountered in the excavations observed at the time of the observations. Percolation testing at the site was performed in June of, 2003. The results indicate a rate of 3-9 minutes per inch. We are using 15 minutes per inch for design purposes. Additional testing and evaluations will be performed during construction to confirm assumption made and assess any design changes that may be required. Design Criteria The design flow is calculated as follows: 1 bedroom recreation cabin --- design for equivalent 2 bedroom home population @ 2 person /bedroom = 4 persons_ From the Garfield County L S. D. S. Regulation: Max. Design flow (Qd) = # of people x (avg. flow) x 1.5 gal/person/day. Gallons per day per person for the subject house = 75 gal/person/day Assume 4 -person population. Qd = 4*7S*1.5* =450 gal/day Septic Tank Design Based on Design Flow Qd. A soil absorption trench systern, utilizing gravelless chambers is recommended. The State allows a 50% area reduction for the use of gravelless infiltration chambers in a trench configuration. The chamber units may be installed in native soils: Soil replacement with sand may be used in select locations if any scattered cobbles or rocks are encountered. We are using 15 rninures/inch for design to account for permeability through the biomat, Based upon the design percolation rate, the standard absorption area equation is: A (SF) = Qd *(t) 1.6 : where A = Area; Qd = Design flow (gal/day) 5 t = time in minutes This design calculation results in a recommended minimum absorption area: A = 450 *(15) = 349 sq. ff- 5 Apply a 50% reduction for utilizing gravelless infiltration chambers in a trench configuration. 349 x 0S = 175sq. ft. Assume 15S square feet per chamber: 175 sq.ft. = 11.2 chambers Use 12 chambers 15.5 sq.ft/charnber(50%) Use 186 SF of trench with gravelless chambers: Assume 3 -foot wide trenches with 6.25 -foot long chamber units. We recommend using a trench system composed of 2 trenches 38 feet long and 3 foot wide, containing 6 standard leaching chamber units in each trench. Excavate absorption trenches to accommodate a minimum 186 SF of interior surface arca. Effluent Distribution System A gravity distribution system will be utilized to transport effluent from the new concrete septic tank to a distribution tee and to the head chamber of each trench via individual distribution lines. An effluent filter will be installed in the secondary compartment of the septic tank to reduce suspended solid loading to the absorption trenches and extend the life of the treatrnent media in the trenches. A 4" PVC effluent line will discharge effluent to a distribution tee installed level, up gradient and adjacent to multiple trenches. The 4" PVC line will be installed with a minimum 2% slope. The discharge pipe(s) will connect to the head of the chamber row(s). Appropriate chamber end plates shall be installed on the inlet ends to prevent scouring of the trench bottom surface. An inspection port should be installed on the top mid -point cut out at the end chamber in each trench. This will allow for checking the performance of the system over time. OWS Operation and Maintenance Ron Vincent SE Job No. 24064.01 May 4, 2004 Page 4 The engineered system shall be inspected on a regular basis and be properly maintained. The responsibility for repair and maintenance of the system will remain with the Lot Owner. The owner shall inspect the OWS and perform maintenance and repairs necessary to ensure that the system is operating properly. The effluent filter shall be maintained when the tank is pumped or as needed. Suitable component handles and extensions on the filter assembly will be installed to provide easy maintenance. We recommend a periodic inspection be performed every 6 months. The OWS system should require minimal maintenance_ Several factors influencing the need for maintenance include: actual wastewater flows versus design flows, the volume of kitchen/domestic waste (excluding human waste and toilet paper), excessive household chemicals and other toxic liquids. The tank, dispersal field and other applicable treatment system components should be visually inspected bi- annually for debris, wear, damage, leaks, or other potential problems. In general, for a properly utilized system, septic tanks should be pumped and inspected every 2 - 4 years. The effluent filters should be cleaned every six months and at the time of pumping, Absorption fields should be maintained with suitable cover and kept free of root invasive plants. Positive surface drainage away from the absorption field should he maintained. Construction and inspections Prior to construction of the permitted system the engineer should be contacted by the contractor and owner well in advance to provide adequate time to discuss the system components with the contractor, answer questions, resolve any conflict issues and schedule inspection site visits based on construction progress. County Regulations require that the Design Engineers of record perform site inspections of the permitted system during construction and provide "As -Built" documentation of the installed system to the County after construction is complete. General Notes 1) Alt materials and installation practices shall conform to the Garfield County Individual Sewage Disposal Regulation. 2) All sewer lines and distribution lines in the system shall be 4" Schedule 40 or SDR -35 PVC unless specified otherwise on the plans. 3) Add a two-way clean out on the service lines from the house and detached barn. 4) The system shall be plumbed to distribution effluent into the trenches with equal distribution. 5) The contractor shall ensure that the concrete septic tank and sewer lines are watertight. 6) The trench area must be protected to prevent damage from vehicular or livestock traffic and must be crowned to divert drainage runoff away from the trenches to minimize surface infiltration. 7) The leaching chambers shall be installed level in each trench. A splash plate shall be installed on the trench bottom surface below the inlet in each trench to prevent scouring from the effluent. The lop of the backfill over the charnbers shall be covered with filter fabric or other suitable pervious material to prevent the migration of fines from the overlying topsoil layer. 8) The trenches must have a minimum cover of 12 inches. Sopris Engineering performed visits to inspect and document the as built conditions of the constructed system. We have coordinated our efforts with the system installer. The as built conditions and installation of the new OWS components is in compliance with the permitted design specifications for the system, The System was installed in accordance with Garfield County Regulations; the design presented in the Sopris Engineering design drawing dated May 4, 2004, A 1 000 -gallon dual compartment concrete septic tank was installed with appropriate sanitary tees and an effluent filter in the outlet tee. The absorption field was constructed with a total of 12 standard teaching chambers in two 38 Soot long trenches with 6 chambers per trench. The chambers were placed over approximately 12" of sand overlying approximately t-2 feet ofonsite-screened pit run type material in certain excavated areas, The chambers were backfilled with screened native soil consisting of basalt fragments and cobbles in a silty sand matrix. The trenches were installed level. Inspection ports were installed on each row at the end chamber. The chambers were installed in suitable soils across the field. No ground water was encountered to a depth of 10 feet. The field has individual distribution lines to the head of each chamber row from adistribution tee. The distribution tee was installed level approximately 4 inches above the chamber inlets. The construction was cornpteted utilizing specified materials installed according to design. The minimum setback distances have been maintained. OWS Operation and Maintenance All components of the engineered OWS shall be inspected on a regular basis and be properly maintained. The re establishment of vegetative cover over the field and responsibility for inspection and maintenance of the system will remain with the Lot Owner or the owners representative.
