The following items are required by Garfield County for a final Inspection:
I) A final Electricallnspection from the Colorado State Electrical Inspector.
2) Permanent address assigned by Garfield County Building Department and posted at the
structure and where readily visible from access road.
3) A finished roof; a lockable building; completed exterior siding; exterior doors and windows
installed; a complete kitchen with cabinets, sink with hot & cold running water, non-absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; all
necessary plumbing.
4) All bathrooms must be complete, with washbowl, tub or shower, toilet, hot and cold running
water, non-absorbent floors, walls finished, and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30" high must be constructed to all IBC and IRC requirements including guardrails.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8) A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY (C.O.) WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.
A C.O. MAY TAKE UP TO 5 BUSINESS DAYS TO BE PROCESSED AND ISSUED.
OWNER CANNOT OCCUPY OR USE DWELLING UNTIL A C.O. IS ISSUED.
OCCUPANCY OR USE OF DWELLING WITHOUT A C.O. WILL BE CONSIDERED AN
ILLEGAL OCCUPANCY AND MAY BE GROUNDS FOR VACATING PREMISES

In order to understand the scope of the work intended under a permit application and expedite the
issuance of a permit it is important that complete information be provided. When reviewing a plan and
it's discovered that required information has not been provided by the applicant, this will result in the
delay of the permit issuance and in proceeding with building construction. The owner or contractor shall
be required to provide this information before the plan review can proceed. Other plans that are in line
for review may be given attention before the new information may be reviewed after it has been
provided to the Building Department.
TWO (2) SETS OF CONSTRUCTION DRAWINGS & TWO (2) SITE PLANS MUST BE
SUBMITTED FOR ALL APPLICATIONS.
Please review this document to determine if you have enough information to design your project
and provide adequate information to facilitate a plan review. Also, please consider using a design
professional for assistance in your design and a construction professional for construction of your
project. Any project with more than ten (10) occupants requires the plans to be sealed by a
Colorado Registered Design Professional.
To provide for a more understandable plan in order to detennine compliance with the building, plumbing
and mechanical codes, applicants are requested to review the following checklist prior to and during
design. Applicants are required to indicate appropriately and to submit the completed checklist at
time of application for a permit.
Plans to be included for a Building Permit, must be on drafting paper at least 18"x24" and drawn
to scale.

Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with decks,
balcony, steps, hand rails and guard rails, windows and doors, including the finish grade line and
original grade. A section showing in detail, from the bottom of the footing to the top of the roof,
including re-bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house-rap, (which is required), siding or any approved building material. Engineered
foundations may be required.
A window schedule. A door schedule. A floor framing plan, a roof framing plan, roof must be
designed to withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind speed,
wind exposure B or C, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or your
plans will be returned.
All plans submitted must be incompliance with the 2003 IRC.
1. Is a site plan included that identifies the location of the proposed structure or addition and distances to
the property lines from each comer of the proposed structure(s) prepared by a licensed surveyor
and has the surveyors signature and professional stamp on the drawing? Properties with slopes of
30% or greater must be shown on the site plan. (NOTE Section: 106.2) Any site plan for the
placement of any portion of a structure within 50 ft. of a property line and not within a previously
surveyed building envelope on a subdivision final plat shall be prepared by a licensed surveyor
and have the surveyor's signature and professional stamp on the drawing. Any structure to be
built within a building envelope of a lot shown on a recorded subdivision plat shall include a
copy of the building envelope as it is shown on the fmal plat with the proposed structure located
within the envelope.

revocation of permit issued. Additionally, your Plan Review fee is not refundable if the plans
have been reviewed by the Building Department prior to any action by the HOA that requires either
revocation or substantial modification of the plans.

If any required infonnation is missing delays in issuing the permit are to be expected. Work may not
proceed without the issuance of a permit. If it is determined by the Building Official that additional
information is necessary to review the application and plans to determine minimum compliance with the
adopted codes, the application may be placed behind more recent applications for building pennits in the
review process and not reviewed until required information has been provided and the application rotates
again to first position for review, delay in issuance of the pennit or delay in proceeding with
construction.

Subject: Subsoil Study for Foundation Design and Percolation Test, Proposed
Residence, 5352 County Road 233 (Silt Mesa Road), Garfield County,
Colorado
Dear Mr. Collins:
As requested, Hepworth-Pawlak Geotechnical, Inc. performed a subsoil study and
percolation test for foundation and septic disposal designs at the subject site. The study
was conducted in accordance with our agreement for geotechnical engineering services to
you dated November 22, 2005. The data obtained and our recoi:mnendations based on the
proposed construction and subsurface conditions encountered are presented in this report.
Proposed Construction: A residence, detached garage and bam located on the site as
shown on Figure I are plam1ed. The residence will be a two story wood frame structure
.over crawlspace. The garage and shop will be single story structures with slab-on-grade
floors. Cut depths are expected to range between about 2 to 3 feet Foundation loadings
for this type of construction are assumed to be relatively light and typical of the proposed
. type of construction. An on-site septic disposal system is proposed to be located to the
southeast of the residence. .
Ifbuilding conditions or foundation loadings are significantly different from these
describeii above, we should be notified to re-:evaluate the recommendations presented in
this report.
Site Conditio~s: The property is about 5Yz acres in siz~. There is an existfug mobile
home and detached garage located in the northwest comer of the property. The terrain is
· generally low lying and relatively flat with a strong slope down to the south-so~theast at
grades from about 2Yz to 5%. Vegetation consists of thick grass and weeds. T!ie ground
Surface appears most! y natural except for the existing development in the northwest
portion. There are no irrigation ditches on the site but the property to the north across
County Road 233 is apparently irrigated.
Subsurface Conditions: The subsUrface conditions at the site were evaluated by
excavatfug four exploratory pits in the building areas and one profile pit in the septic
disposal area at the approximate locations shown on Figure 1. The logs of the pits are
presented on Figure 2. The subsoils encountered, below about Yz to 1 foot of organic
topsoil, consisted of medium stiff to soft (with depth), sandy silty clay that extended to
the pit depths of 7 to 8 feet. Results of swell-consolidation testing performed on

relatively undisturbed samples of silty clay, presented on Figures 3 and 4, indicate
moderate to high compressibility under conditions and loading and wetting. The
laboratory test results are summarized in Table 1. Groundwater seepage was observed in
the pits at the time of excavation at depths from about 5 to 6 feet. The subsoils were very
moist becoming wet near and below the seepage level.
Foundation Recommendations: Considering the subsoil conditions encountered in the
exploratory pits and the nature of the proposed construction, we recommend spread
footings placed on the upper natural soil designed for an allowable bearing pressure of
1,000 psffor support of the proposed buildings. The soils tend to compress under loading
and there could be some post-construction fuundation settlement. Footings should be a
minimum width of 18 inches for continuous walls and 2 feet for columns. All topsoil and
loose or disturbed soils encountered at the foundation bearing level within the excavation
should be removed and the footing bearing level extended down to the undisturbed
natural soils. The excavations should be kept as shallow as possible (within about 3 feet
of existing ground surface) to avoid encountering the soft soils. If soft soils are
encountered, stabilization such as by sub-excavating and replacing with coarse granular
soils ma:y be needed. Exterior footings should be provided with adequate cover above
their bearing elevations for frost protection. Placement of fuotings at least 36 inches
below the exterior grade is typically used in this area .. Continuous foundation walls
should be heavily reinforced top and bottom to span local anomalies such as by assuming
an unsupported length of at least 14 feet. Foundation walls acting as retaining structures
should also be designed to resist a lateral earth pressure based on an equivalent fluid unit
weight of at least 55 pcf for the on-site soil as backfill.
Flom: Slabs: Slab-on-grade construction may be preferred for !he residence due to the
potential for wet crawlspace. The natural on-site soils, exclusive of topsoil, are suitable
to support lightly loaded .slab-on-grade construction. To reduce the effects of some
differential movement, floor slabs should be separated from all bearing walls and
columns with expansion joints which allow unrestrained vertical movement Floor slab
control joints should be used to reduce damage due to shrinkage cracking. The ·
requirements for joint spacing and slab reinforcement should be established by the
designer l;Jased on experience and the intended slab use. A minimum 4 inch layer of
gravel should be placed beneath interior slabs for support and to facilitate drainage. This
material should consist of minus 2 inch aggregate with less than 50% passing !he No. 4
sieve and less than 12% passing the No. 200 sieve.
All fill materials for support of floor slabs should be compacted t:O at least 95% of
maximum standard Proctor density at a moisture content near optimum. Required fill can
consist of the on-site soils or suitable imported granular soils devoid of vegetation, topsoil
-and oversized rocks.
Underdrain System: Free water was encountered at relatively shallow depth and it has
been our experience in the area that the groundwater level may rise or local perched
groundwater can develop during times of heavy precipitation or seasonal runoff Frozen
ground during spring runoff can create also a perched condition. We recommend below-

grade construction, such as retaining walls and crawlspace areas, be protected from
wetting and hydrostatic pressure buildup by an underdrain system.
The drains should consist of drainpipe placed in the bottom of the wall backfill
surrounded above the invert level with free-draining granular material. The drain should
be placed at each level of exeavation and at least 1 foot below lowest adjacent finish
grade and sloped at a minimmn 1% to a suitable gravity' outlet or sump and pump.· Free-
draining granular material used in the underdrain system should contain less than 2%
passing the No. 200 sieve, less than 50% passing the No. 4 sieve and have a maXimum
size of2 inches. The drain gravel backfill should be at least 1 Yz feet deep.
Surface Drainage: The following drainage precautions should be observed during
construction and maintained at all times after the buildings have been completed:

In.undation of the foundation excavations and underslab areas should be
avoided during construction.
Exterior backfill should be adjusted to near optimum moisture and
compacted to at least 95% of the maximum stalldard Proctor density' in
pavement and slab areas and to at least 90% of the maximmn stalldard
Proctor density' in landscape areas.
The ground surface surrounding the exterior of the building should be
sloped to drain away from the foundation in all directions. We ·
recommend a minimum slope of6 inches in the :first 10 feet in unpaved
areas and a minimum slope of 3 inches in the :first 10 feet in pavement and
walkway areas.
Roof downspouts and drains should discharge well beyond the limits. of all
backfill.
Landscaping which requires regular heavy irrigation, such as sod, and
lawn sprinkler heads should be located at least 5 feet from the building.
Percolation Testing: Percolation tests were conducted on December 2, 2005 to evaluate
the feasibility' of an infiltration septic disposal system at the site. One profile pit and three
percolation holes were dug at the locations shown on Figure 1. The test holes (nominal ·.
12 inch diameter by 12 inch deep) were hand dug at the bottom of shallow backhoe pits
and were soaked with water one day prior to testing. The soils exposed in the percolation
holes are similar to those exposed in the Profile Pit shown on Figure 2 and consist of very
moist, sandy silty' clay with groundwater seepage at 5 Y, feet below ground surface. The
percolation test results, presented in Table 2, indicate percolation rates from about 15 to
180 minutes per inch. Based on the subsurface conditions encountered and the
percolation test results, the tested area should be suitable for an infiltration septic disposal
system. Due to the shallow groundwater condition, a civil engineer should design the
infiltration septic disposal system. ·
Limitations: This study bas been conducted in accordance with generally accepted
geotechnical engineering principles and practices in this area at this time. We make no
warranty' either expressed or implied .. The conclusions and recommendations submitted
in this report are based upon the data obtained from the exploratory pits excavated at the
locations indicated on Figure 1, the proposed type of construction and our experience in

the area. Our services do not include detemrining the presence, prevention or possibility
of mold or other biological contaminants (MOB C) developing in the future: If the client
is concerned about MOBC, then a professional in this special field of practice should be
consulted. Our findings include interpolation and extrapolation of the subsurface
conditions identified at the exploratory pits and ·variations in the subsurface conditions
may not become evident until excavation is performed If conditions encountered during ·
construction appear different from those described in this report, we should be notified at
· · once so re-evaluation of the reCommendations may.be made.
This report has been prepared for the exclusive use by our client for design purposes. We
are not responsible for technical interpretations by others of our information. All the
project evolves, we should provide continued consultation and field services during
construction to review and monl.tor the implementation of our recommendations, and to
verifY that the recommendations have been appropriately interpreted. Significant design
changes may require additional analysis or modifications to the recommendations
presented herein. We recommend on-site observation of excavations and foundation
bearing strata and testing of structural fill by a representative of the geotechnical
engineer.
