Dear Tamra:
On behalf of Colorado Rocky Mountain School ("CRMS"), enclosed are three paper copies and a
CD of a Land Use Change Permit Application Form for a proposed Active Center to be constructed on the
CRMS campus. This Application contains the following:
1. The County's Land Use Change Permit Application Form, which has been completed and
which has been executed by Chad Lee as attorney for CRMS and as authorized representative;
2. Letter of Authorization from CRMS;
3. A conformed copy of a Statement of Authority indicating that Mr. McCoy has the authority
to act on behalf of CRMS;
3. Ownership documentation (deeds);
4. Legal description of CRMS;
4. An executed Payment Agreement form;
5. Copies of the approved 1999 Conditional Use
Amendment to Conditional Use Permit Application;
Permit Application and the 2010
6. A current vicinity map and a vicinity map from 2011;
