The following items are required by Garfield County for a final inspection:
1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assigned by Garfield County Building Department posted where readily
visible from access road;
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent
kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all
necessary plumbing;
4. A complete bathroom, with washbowl, tub or shower, toilet stool, hot and cold running water,
non-absorbent floors and walls finished and a privacy door;
5. All steps outside or inside over three (3) steps must have handrails, guard rails on balconies or
decks over 30" high constructed to all 1997 UBC requirements;
6. Outside grading done to where water will detour away from the building;
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until
all the required items are completed and a final inspection made;
8. A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, where applicable.
A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.

MINIMUM APPLICATION REQUIREMENTS
for
MANUFACTURED HOMES
In order to understand the scope of the work intended under a permit application and expedite the issuance of
a permit it is important that complete information be provided. Adequate and complete information will
prevent delays in the plan review process. Reviewing a plan and the discovery that required information has
not been provided by the applicant may result in the delay of the permit issuance and in proceeding with
building construction. The owner or contractor may be required to provide this information before the plan
review may proceed. This causes delays because other plans that are in line for review may be given attention
before the new information may be reviewed after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your project and
provide adequate information to facilitate a plan review. If you do not, it may be helpful to obtain a
book titled "Dwelling Construction under the Uniform Building Code". This book is available to you
through this department at our cost. Also, please consider using a design professional for assistance in
your design and a construction professional for construction of your project.
To provide for a more understandable plan in order to determine compliance with the building, plumbing and
mechanical codes, applicants are requested to review the following checklist prior to and during design.
Applicants are required to indicate appropriately and to submit the completed checklist at time of application
for a permit.
All plans submitted must be incompliance with the 1997 UBC, UMC, and 1997 UPC.

1. Is.a site plan included that identifies the location of the proposed structure or addition and distances to
the property lines from each corner of the proposed structure prepared by a licensed surveyor and has
the surveyors signature and professional stamp on the drawing? Properties with slopes of 30% or more
must be shown on site plan. NOTE: 106.3.1(7.) Any site plan for the placement of any portion of a
structure within 50 ft. of a property line and not within a previously surveyed building envelope on a
subdivision final plat shall be prepared by a licensed surveyor and have the surveyors signature and
professional stamp on the drawing. Any structure to be built within a building envelope of a lot
shown on a recorded subdivision plat shall include a copy of the building envelope as it is shown on
the final pl&twith the proposed structure located within the envelope.

2. Does the site plan when applicable include the location of the I.S.D.S. (Individual Sewage
Disposal System) and the distances to the property lines, wells (on subject property and adjacent
properties), streams or water courses? This information must be certified by a licensed surveyor
with thei gnature and professional stamp on the design.

refer to Section 5.05.03 in the Garfield County Zoning Resolution if the property you are applying

If you have answered "No" on any of the questions, you may be required to provide this information at the
request of the Building Official prior to beginning the plan review process. Delays in issuing the permit are to
be expected. Work may not proceed without the issuance of a permit. If it is determined by the Building
Official that additional information is necessary to review the application and plans to determine minimum
compliance with the adopted codes, the application may be placed behind more recent applications for
building permits in the review process and not reviewed until required information has been provided and the
application rotates again to first position for review, delay in issuance of the permit or delay in proceeding
with construction.

Subdivision, Garfield County, Colorado.
Dear Clasen:
As requested, a representative of Hepworth-Pawlak Geotechnical, Inc. observed the
excavation at the subject site on March 21, 2004 to evaluate the soils exposed for
foundation support. The findings of our observations and recommendations for the
foundation design are presented in this report. We previously conducted a subsoil study
for design of foundations at the site and presented our findings in a report dated
November 7, 2003, Job No. 103 694.
The proposed residence will consists of a 1'/: story wood frame structure over a full
basement level. Spread footings sized for an allowable soil bearing pressure of 1,200 psf
were recommended in our previous report. We understand that the foundation has been
designed as 101/4 inch thick Superior Walls placed on a 10 inch crushed gravel mat.
At the time of our visit to the site, the foundation excavation had been cut in one level
from 10 to 11'1 feet below the adjacent ground surface. The soils exposed in the bottom
of the excavation consisted of medium dense. sand and silt. The upper several inches
appeared to be loose from the excavation equipment. No free water was encountered in
the excavation and the soils were slightly moist.
The soils exposed in the excavation appear consistent with those previously encountered
on the site and hydrocompressive. Spread footings placed on the natural soils and
designed for an allowable bearing pressure of 1,200 psf were recommended with a risk of
settlement and distress if the underlying bearing soils become wetted. The proposed
crushed gravel mat footing will provide a conduit for subsurface water to penetrate the
bearing soils which could result in large differential settlements damaging to the
structure. We also recommended the foundations be designed to span on unsupported
length of 12 feet. As an alternative to a concrete footing, a well graded material, such as
v. inch road base, could be used to help limit free flow of water to the bearing soils.

Regardless of the type of footing used, loose disturbed soils should be moistened and
compacted. The structural engineer for the residence should review the foundation plans
for potential impacts to the structure from differential settlements. Other
recommendations presented in our previous report which are applicable should also be
observed.
The recommendations submitted in this letter are based on our observation of the soils
exposed within the foundation excavation and the previous subsurface exploration at the
site. Variations in the subsurface conditions below the excavation could increase the risk
of foundation movement. We should be advised of any variations encountered in the
excavation conditions for possible changes to recommendations contained in this letter.
Our consulting services do not include determining the presence, prevention or
possibility of mold or other biological contaminants (MOBC) developing in the future. If
the client is concerned about MOBC, then a professional in this special field of practice
should be consulted.
