ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system (OWTS)
on October 14 and December 2, 2015 for the subject property. Avalanche Excavation installed the
system. The existing septic tank and STA were abandoned.
The installation included a new 1000 -gallon, two-compartment Roth® ploy septic tank with an effluent
filter or screen on the outlet tee of the septic tank.
Effluent gravity flows from the septic tank to a distribution box, which is accessible from grade. The STA
consists of four trenches, each with 11 `Quick 4' Infiltrator® chambers for a total area of 528 square feet
of infiltrative area. The infiltrative area was placed below the Silty Clay Loam horizon, and did not exceed
4 -feet below native grade.
A minimum of 6 -feet of undisturbed soil was present between each trench. Inspection ports were placed
at both the beginning and end of each trench.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are
made to the OWTS in the future. Any additional OWTS construction must be according to the county
regulations.
LIMITS:
Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.

and have the tank pumped, when necessary, generally every three to five years.

drainfield, roots can grow into your
system and clog it.

advise you of the proper distance for
planting trees and shrubs, depending on
your septic tank location.

your dishwasher, shower, washing
machine, and toilet at the same time.
All that extra water can really strain
your septic system.
Do:
• Stagger the use of water -generating
appliances. This can be helpful
especially if your system has not been
pumped in a long time.
• Become more water efficient by fixing
plumbing leaks and consider installing
bathroom and kitchen faucet aerators
and water -efficient products.
