The following items are required by Garfield County for a final inspection:
1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assigned by Garfield County Building Department posted where readily
visible from access road;
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent
kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all
necessary plumbing;
4. A complete bathroom, with washbowl, tub or shower, toilet stool, hot and cold running water,
non-absorbent floors and walls finished and a privacy door;
5. All steps outside or inside over three (3) steps must have handrails, guard rails on balconies or
decks over 30" high constructed to all 1997 UBC requirements;
6. Outside grading done to where water will detour away from the building;
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until
all the required items are completed and a final inspection made.

decks, balcony, steps, hard rails and guard rails, windows and doors, including the finish grade I. A

Dear Dick and Judi,
The Mountain Springs Ranch Architectural Committee has approved the architectural
plans and site plan you submitted April 13 for Lot 26.
These approvals are subject to the following conditions:
1). You are required to install proper drainage systems for the retaining wall and house
foundation including, but not limited to, perforated drain pipe on the uphill and downhill
sides of the retaining wall and perforated pipe around the footers of the house that will
be taken into a non -perforated pipe past the septic system to the northeast of the house
and into the drainage from the uphill spring;
2). You shall use the "Applewood" color for the exterior logs and the "Bronze" color for
the roof and trim, as submitted to the committee April 20;
3). You are required to properly design, construct and maintain the uphill ditch on your
driveway.
Design shall include, but not be limited to, provision of a ditch on the uphill side capable
of accommodating high-water, spring or storm run-off without flowing on to slide areas
and otherwise endangering the integrity of the Mountain Springs Road;
-ConSiruction shall include, but not be limited to, building -up the present road to create a
barrier and proper drainage to the uphill ditch before the final road base and gravel are
added and the option of reconnecting the old drainage pipe as an overflow back-up;
Maintenance shall include, but not be limited to, clearing, plowing, diversion and other
maintenance and repair on the road and ditch so as to prevent water during the spring
run-off from flowing on to Mountain Springs Road;
4). You are required to establish a bond in the amount of $20,000 with the Mountain
Springs Homeowners Association named as an additional insured and the bond shall
remain in effect until June 30, 2006, or until the Mountain Spring Ranch Homeowners
Association Road Committee deems the ditch system on the driveway of Lot 26
adequate to accommodate water run-off.

As requested we have investigated subsurface conditions and designed an onsite wastewater system (OWS)
for the proposed residence at the subject site. The subsurface investigation was conducted May 2, 2002.
SITE CONDITIONS
The lot is located in a rural area in Garfield County, Colorado. The proposed drain field area is located on a
shelf of Mancos Shale that has been cut to accommodate the residence and OWS. The proposed drain field
location is relatively flat and has no vegetation. This is the most suitable location for the drain field. Water
will be provided by a community well.
PROPOSED CONSTRUCTION
A three-bedroom residence is proposed at the location presented on Figure 1. The OWS was based on three
bedrooms and 450 gallons per day (GPD) using Garfield Environmental Health Department loading of 75
gallons per day per person. The sewage loading with a peaking factor of 1.5 is 675 GPD. This loading
includes washing machines and garbage grinders.
SUBSURFACE CONDITIONS
A subsurface investigation was conducted by digging three percolation holes and observing the cut slopes
directly west of the proposed drain field. The percolation holes were 36 to 22 inches deep, as indicated on
Figure 1. Subsurface conditions consisted of highly fractured Mancos Shale to the maximum depth explored
of 3 feet. Percolation tests indicate percolation rates of 10 to 40 minutes per inch (MPI), with an average
percolation rate of 23 MPI. We are sizing the drain field based on a percolation rate of 28 MPI due to the
proposed imported sand filter material. Percolation data is presented in Table 1.
RECOMMENDATIONS
The installation must include a septic tank, with an effluent pump, and four chamber -trenches with imported
sand -filter material, at the locations presented on Figure 2. The OWS design is based on an average sewage
load of 450 GPD and an application rate to the top of the sand of 28 MPI. The installation must include
one 1250 -gallon, two-compartment, precast concrete septic tank, with an effluent pump in a Biotube®

pump vault, and four chamber -beds with four feet of imported sand filter material. Garfield County sand
filter specifications as indicated on Figure 3 must be met. Each bed must have one row of six chambers for a
total of 24 chambers. A I.25 -inch PVC lateral with 'A inch holes drilled on three-foot centers must be
Suspended within chambers. Holes should face upward with every fifth facing down. A pump with a total
flow rate of 37.9 Gallons Per Minute (GPM) and a Total Dynamic Head (TDH) of 20.4 feet is required to
pressurize the distribution system at the field.
Vehicle movement over the drain field area during construction should be kept at a minimum to
minimize compaction of the receiving fractured bedrock. The bottom of the trench should be scarified
with a pick, shovel, or backhoe bucket teeth prior tothe addition of sand.
The surface of the drain fields should be re-established and then be seeded using a native seed mix. These
mixes do not require irrigation and develop a growth 10 to 15 inches high. No automatic sprinkler system
should be installed over the fields.
OPERATION AND MAINTENANCE
The owner must realize an OWS is different from public sewer service. The owner must be aware of
and assume responsibility for continued maintenance of the system. In the absence of a monitoring
program, the septic tank should be pumped every two years. Effluent filters and screened vaults, as

not putting plastic or other nonbiodegradable materials into the septic system. Garbage disposal use
should be minimized.
The backwash of a water softener may be harmful to the OWS. A separate drywell should be constructed
for the backwash waste, if a softener is installed. Chemically treated water from a swimming pool or spas
should not be introduced to the OWS.
LIMITATIONS
A contractor who is experienced in OWS installation should install the OWS design. Our investigation,
layout, design and recommendations are based on data submitted. If subsurface conditions considerably
different from those described in this report are encountered, we should be notified to evaluate the effect on
the proposed OWS. If the health department makes modifications to this design, we should be contacted to
evaluate the impact on the performance of the OWS. The county and our firm must be contacted upon
completion of the OWS.
