Permanent address assigned by Garfield County Building Department posted where readily visible from access road; 3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all necessary plumbing; 4. Outside grading done to where water will detour away from the building; 7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until all the required items are completed and a final inspection made. Fry Comments: Subgrade was native material, sidewall was over excavated approximately one foot. Very minor sloughing and were present in the excavation. Contractor removed loose material in excavatio, prior to concrete placement. Ambient air temperature was . approximately 90 degrees Fahrenheit. Bottom mat of #8 bar placed on 4 -inch mortar cubes to maintain the 3 -inch minimum clearance. Both top and bottom mats of #8 bar have 17 bars each way, placed. 16 -inch center to center. Top and bottom mats were tied more than every -other bar (approximately 80%), all mat edges were tied 100%. Top That supported by pier rebarr and by several #4 bars bent and evenly spaced throughout the placement to maintain consistent separation between mats. A minimum of 8 -inches was Maintained between mat edges and excavation sidewalllf. Each of the piers (3) were made up of nine vertical #8 bar and six #3 bar hoops more or Iess every 12 -inches. Vertical #G bar had a minimum of 30 -inch bend on bottern, Beeps were over -lapel a minimum. of 18 -inches. All pier bars were tied 100%. Tower bolts and bolt plate placed on cinder blocks for support and tied to pier bar. Tower bolts extended to the bottom bar mat with a minimum clearance of 12 -inches between holt and sidewall of excavation. Minimum to no rust present on reinforcing steel in placement. Grade 60 reinforcing steel used. Contractor used orae -inch stinger to consolidate concrete during placement. No movement of reinforcing steel was noted during concrete placement_ Concrete was placed on non -frozen soils. Inspector: . The existing 125' ower was designed and manufactured by LeBlanc Communications Inc. of Sioux City;, IA In 1997. All the existing tower legs are made of 50 ksi solid rods while all the braces are made of 313 ksi structural!steel angles. All the existing tower sections are bolted and X -braced. The details of the bower sections, member sizes and allowable capacities, forces and reactions, twi is and sways, antenna loading and configuration of the tower are shown on the printout of the-compu er structural analysis !which is a part of this report. The most updated 'information about the antenna loading and the existing self-supporting tower or LeBlanc Drawing No. 4570241.01 dated 4-22-97 was furnished by your office. The required basic; design wind speed for the Colorado site is 80 mph wind with 112" radial ice (w th wind load reduction and 1/3 Increase in allowable stresses taken) per EIA -222-F-1996 Standard_ The required antenna loading is as follows. (12) PCS Panel. antennas (assumed 5 sq. ft. each) on mounts with (12) PCS Panel antennas (assumed 5 sq. ft. each) on mounts with (12) PCS Panel antennas (assumed 5 sq. ft. each) on mounts with (12) PCS Panel antennas (assumed 5 sq. ft. each) on mounts with (2) 6' Dishes with radomes and (2) 1-518" lines at 85' Featlllnes are assumed to be stacked on 2 rows or attached to tower face. The most stressed point of the tower legs occurs al EL. 40' in which 55.5% of the allowable capacitt4' of the existing tower legs has been used. The most stressed point of the tower diagonals occurs at EL. 80" In which 40.8% of the allow capacity of the existing tower diagonals has been used. Also, the most stressed point of the tower girls occurs at EL. 0' in which 21.O% of the allow le capacity of the existing tower girt* has been used. Furthermore, the a fisting tower meets the rigidity requirements per EIA -222-F-1998 Standard. Per my structural analysis dated 5-12-01, the maximum tower base reactions are as follows: Total! shear = 24 kips Axial down load = 34 kips Uplift per leg al 144 kips Compression per leg - 187 kips Overturning momeht ■ 1873 ft -kips A new tower foundation will be designed based upon the updated base reactions as soon' as a s Is report for a specified tower site located in the State of Colorado Is available.
