The following items are required by Garfield County for a final Inspection:
I) A final Electrical Inspection from the Colorado State Electrical Inspector.
2) Permanent address assigned by Garfield County Building Department and posted at the
structure and where readi ly visible from access road .
3) A finished roof; a lockable building; completed exterior siding; exterior doors and windows
installed; a comp lete kitchen with cabinets, sink with hot & cold running water, non-absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; all
necessary plumbing.
4) All bathrooms must be complete, with w ashbowl, tub or shower, toilet, hot and cold runn in g
water, non-absorbent floors , walls finished, and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30" high must be constructed to aiiiBC and IRC requirements in cluding guardrai ls.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks , grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8) A final in s pection sign off by the Garfie ld County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire Di strict , and/or State

In order to understand the scope of the work intended under a permit application and expedite the
issuance of a permit it is important that complete information be provided. When reviewing a plan and
it's discovered that required information has not been provided by the applicant, this will result in the
delay of the permit issuance and in proceeding with building construction. The owner or contractor shall
be required to provide this information before the plan review can proceed. Other plans that are in line
for review may be given attention before the new information may be reviewed after it has been
provided to the Building Department.
TWO (2) SETS OF CONSTRUCTION DRAWINGS & TWO (2) SITE PLANS MUST BE
SUBMITTED FOR ALL APPLICATIONS.
Please review this document to determine if you have enough information to design your project
and provide adequate information to facilitate a plan review. Also, please consider using a design
professional for assistance in your design and a construction professional for construction of your
project. Any project with more than ten (10) occupants requires the plans to be sealed by a
Colorado Registered Design Professional.
To provide for a more understandable plan in order to determine compliance with the building, plumbing
and mechanical codes, applicants are requested to review the following checklist prior to and during
design. Applicants are required to indicate appropriately and to submit the completed checklist at
time of application for a permit.
Plans to be included for a Building Permit, must be on drafting paper at least 18"x24" and drawn
to scale.
Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with decks,
balcony, steps, hand rails and guard rails, windows and doors, including the finish grade line and
original grade. A section showing in detail, from the bottom of the footing to the top of the roof,
including re-bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house-rap, (which is required), siding or any approved building material. Engineered
foundations may be required.
A window schedule. A door schedule. A floor framing plan, a roof framing plan, roof must be
designed to withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind speed,
wind exposure B or C, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or your
plans will be returned.
All plans submitted must be incompliance with the 2003 IRC.
I. Is a site plan included that identifies the location of the proposed structure or addition and distances to
the property lines from each comer of the proposed structure(s) prepared by a licensed surveyor
and has the surveyors signature and professional stamp on the drawing? Properties with slopes of
30% or greater must be shown on the site plan. (NOTE Section: 106.2) Any site plan for the
placement of any portion of a structure within 50 ft. of a property line and not within a previously
surveyed building envelope on a subdivision final plat shall be prepared by a licensed surveyor
and have the surveyor's signature and professional stamp on the drawing. Any structure to be
built within a building envelope of a lot shown on a recorded subdivision plat shall include a
copy of the building envelope as it is shown on the final plat with the proposed structure located
within the envelope.

If any required information is missing delays in issuing the permit are to be expected. Work may not
proceed without the issuance of a permit. If it is determined by the Building Official that additional
information is necessary to review the application and plans to determine minimum compliance with the
adopted codes, the application may be placed behind more recent applications for building permits in the
review process and not reviewed until required information has been provided and the application rotates
again to first position for review, delay in issuance of the permit or delay in proceeding with
construction.

Subject: Observation of Excavation, Proposed Residence, Lot 9, Filing 4, Oak
Meadows, 0129 Old Midland, Garfield County, Colorado
Dear Keith:
As requested, a representative of Hepworth-Pawlak Geotechnical, Inc. observed the
excavation at the subject site on May 19, 2008 to evaluate the soils exposed for
foundation support. The fmdings of our observations and recommendations for the
foundation design are presented in this report. The services were performed in
accordance with our agreement for professional engineering services to you, dated May 6,
2008. Hepworth-Pawlak Geotechnical previously performed a geologic site assessment
and preliminary geotechnical study for the Oak Meadows Filing 4 development,
submitting our findings in a report dated April 15, 1998, Job No. 196 420.
The residence will be a two story wood frame structure over crawlspace. The attached
garage will have a slab-on-grade ground floor. The building has been designed to be
supported on spread footings assuming an allowable soil bearing pressure of I ,500 psf
The ground surface of the Jot has a strong slope down to the northeast.
At the time of our site visit, the foundation excavation was nearly complete and had been
cut in one level fi·om about I to I 0 feet below the adjacent ground surface. The eastern
side frost trench for the footings had not been dug. The soils in the bottom of the
excavation consisted primarily of stiff, sandy silty clay. In the shallow cuts along the
eastern side of the excavation, there was some black topsoil at subgrade. Results of
swell-consolidation testing performed on a sample of the silty clay, shown on Figure I,
indicate the soils are moderately compressible under conditions of loading and wetting.

The laboratory testing is summarized in Table I. No free water was encountered in the
excavation and the silty clay soils were generally moist.
Considering the conditions exposed in the excavation and the nature of the proposed
construction, spread footings placed on the undisturbed natural soils designed for an
allowable bearing pressure of I ,500 psf can be used for support of the proposed residence.
The soils may tend to compress when wetted which could result in some post-
construction movement of the foundation and precautions should be taken to prevent
wetting. Footings should be a minimum width of 18 inches for continuous walls and 2
feet for columns. All topsoil and loose disturbed soils in footing areas should be removed
to expose the stiff silty clay soils and the sub grade should be compacted prior to placing
footing concrete. Exterior footings should be provided with adequate soil cover above
their bearing elevations for frost protection. Continuous foundation walls should be well
reinforced top and bottom to span local anomalies such as by assuming an unsupported
length of at least 12 feet. Foundation walls acting as retaining structures should also be
designed to resist a lateral earth pressure based on an equivalent fluid unit weight of at
least 55 pcf for on-site soil as backfill. A perimeter foundation drain should be provided
to prevent temporary buildup of hydrostatic pressure behind the crawlspace walls and
wetting of the lower level. Structural fill placed within floor slab areas can consist of the
on-site soils, excluding topsoil and oversized rocks, or base course, compacted to at least
95% of SPD (standard Proctor density) at a moisture content near optimum. Backfill
placed around the structure should be compacted to at least 90% SPD and the surface
graded to prevent ponding within at least 10 feet of the building. Landscaping that
requires regular heavy irrigation, such as sod, and sprinkler heads should not be located
within 5 feet ofthe foundation.
The recommendations submitted in this letter are based on our observation of the soils
exposed within the foundation excavation and do not include subsurface exploration to
evaluate the subsurface conditions within the loaded depth of foundation influence. This
study is based on the assumption that soils beneath the footings have equal or better
support than those exposed. The risk of foundation movement may be greater than
indicated in this report because of possible variations in the subsurface conditions. In

We understand that a representative of the Garfield County Building and Planning
Department inspected and approved the concrete footings and footing reinforcement prior
to placement of concrete for the footings .
We also understand , based upon statements from Keith Wittenberg , that the grade
wall reinforcement for the foundation grade walls was in accordanc e with the construction
documents issued by our office . We understand that the reinforcement was placed in
accordance with those docum ents to include placement of ' corner ' bars at the wall
corners, and to include app ropriate lap lengths for the horizontal bars (24" minimum lap
length). Note that the typical grade wall height is 3 '-4", and typical wall reinforcement
was specified as #4 vertical @ 16" on center, (2)-#5 horizontal , top and bottom of the
grade wall , and (2) -#4 horizontal intermediate .
This specified foundation wall reinforcement is structurally adequate and
appropriate for the soil conditions and building design loads. The foundation system
constructed in accordance with this specification is structurally adequate and safe .
