Please review this document to determine if you have adequate information to design your project and to facilitate a plan review. SINGLE FAMILY OR DUPLEX BUILDING PERMIT APPLICATION CHECKLIST This checklist is to be used with all single family dwelling construction. APPLICATION FORM: Please make sure the following information is provided on the application. • Parcel number obtained from the County Assessor's Office. • Job address (assigned physical address) • Legal Description: lot, block, filing, subdivision or tract, section, township and range. • Owners name, mailing address, phone, fax, or cell. • Contactors name, mailing address, phone, fax, or cell. • Architect/Engineer's name, mailing address, phone, fax or cell. • Building size, height, number of stories, lot size. • Use of building (residential) • Class of Work (new, addition, alteration, repair, demo, move, other) • Description of work. • Type of heating (natural gas, propane, electric, other). • Sewer system (Community or ISDS), also see other items below. • Garage (attached or detached). • Driveway permit (please see other items below) • Valuation (labor and materials) see attached worksheet, OTHER ITEMS: • If you anticipate obtaining a water -tap from the City of Rifle, please provide a letter indicating that the City will provide water service. BUILDING PLANS: Two (2) sets of plans must be submitted. Plans must be 18"x24" minimum size, complete, identical, legible, and to scale. All plans will be checked at the counter for completeness. If any of the required information is missing, the building permit application cannot be accepted. SITE PLAN: Please make sure the following information is provided on the site plan. • Property address/legal description. • North Arrow and Scale on each page. • Show all property lines, building envelopes, flood plain, and easements. • Provide setback distances from boundaries, buildings, septic, well, and waterways. • Location and direction of the county or private road and driveway accessing the property. • Proposed and existing structures include sheds, barns, decks, patios, and any other buildings. • Existing and proposed grade, positive drainage around structure (to be contained on site). • Streams, rivers, creeks, springs, ponds and ditches. • Existing and proposed wells, septic tanks, leach fields, and other systems. Attn: Antony Cullwick 417 Main Street, Suite Y Carbondale, Colorado 81623 Job No. 108 323A Subject: Observation of Excavation, Proposed Residence, Lot 9, Ranch at Roaring Fork, Surrey Street, Garfield County, Colorado Dear Mr. Cullwick: As requested, a representative of 1-lepworth-Pawlak Geotechnical, Inc. observed the excavation at the subject site on August 3, 2011 to evaluate the soils exposed for foundation support. The findings of our observations and recommendations for the foundation design are presented in this report. The services were performed as additional services to our agreement for professional engineering services to Koru Ltd., dated June 12, 2008. We previously conducted a subsoil study for design of foundations at the site and presented our findings in a report dated January 22, 2009, Job No. 108 323A. At the time of our visit to the site, the foundation excavation had been cut in one level from 1 to 3 feet below the adjacent ground surface. The soils exposed in the bottom of the excavation consisted of medium dense, slightly clayey gravelly silty sand with rounded cobbles. The subgrade had been compacted with a vibratory double drum trench compactor and appeared to be firm. No free water was encountered in the excavation and the soils were slightly moist to moist. The soil conditions exposed in the excavation appeared to be somewhat finer -grained compared to those previously encountered on the site and spread footings designed for an allowable bearing pressure of 2,000 psf are recommended. In a phone conversation with Steve Millard of Rakowski -Millard, Steve stated that he would verify if there were any footings with bearing pressures higher than 2,000 psf. Other recommendations presented in our previous report which are applicable should also be observed. The recommendations submitted in this letter are based on our observation of the soils exposed within the foundation excavation and the previous limited subsurface exploration at the site. Variations in the subsurface conditions below the excavation could increase the risk of foundation movement. Dear Mr. Cullwick: As requested, Hepworth-Pawlak Geotechnical, Inc. performed a subsoil study for design of foundations at the subject site. The study was conducted in accordance with our agreement for geotechnical engineering services to Koru, Ltd. dated June 12, 2008. The data obtained and our recommendations based on the proposed construction and subsurface conditions encountered are presented in this report. Proposed Construction: The proposed residence will be a two story wood frame structure with an attached garage located in the building envelope shown on Figure 1. Ground floor will be slab -on -grade for the residence and garage. Cut depths are expected to range between about 3 to 5 feet. Foundation loadings for this type of construction are assumed to be relatively light and typical of the proposed type of construction. If building conditions or foundation loadings are significantly different from those described above, we should be notified to re-evaluate the recommendations presented in this report. Site Conditions: The site was vacant at the time of our field exploration, with 12 to 14 inches of snow cover. Blue Creek is located about 100 feet to the southwest of the building envelope. The ground surface on the site is relatively flat with a gentle slope down to the southwest. Vegetation consists of grass and weeds. There is marsh grass and willows to the southwest. Subsidence Potential: The Ranch at Roaring Fork is underlain by Pennsylvania Age Eagle Valley Evaporite bedrock. The evaporite contains gypsum deposits. Dissolution of the gypsum under certain conditions can cause sinkholes to develop and can produce Parker .303-841-7119 • Colorado Springs 719-633-5562 • Silverthorne 970-468-1989 -2 - areas of localized subsidence. Sinkholes were not observed in the immediate area of the subject Iot. The exploratory pits were relatively shallow, for foundation design only. Based on our present knowledge of the site, it cannot be said for certain that sinkholes will not develop. In our opinion, the risk of ground subsidence at Lot 9 is low and similar to other lots in the area but the owner should be aware of the potential for sinkhole development. Subsurface Conditions: The subsurface conditions at the site were evaluated by excavating two exploratory pits at the approximate locations shown on Figure 1. The logs of the pits are presented on Figure 2. The subsoils encountered, below about 1/2 foot of topsoil, consist of relatively dense, silty sandy gravel with cobbles and occasional boulders, overlying slightly silty sandy gravel with cobbles and occasional boulders to the pit depths of 8 to 9 feet. Results of gradation analyses performed on samples of the gravels (minus 5 and 6 inch fraction) obtained from the site are presented on Figure 3. No free water was observed in either Pit 1 or Pit 2 at the time of excavation but was thought to be probable in Pit 1 at a depth of 9 feet. The soils were slightly moist to wet in Pit 1. Foundation Recommendations: Considering the subsoil conditions encountered in the exploratory pits and the nature of the proposed construction, we recommend spread footings placed on the undisturbed natural gravel soil designed for an allowable soil bearing pressure of 3,000 psf for support of the proposed residence. Footings should be a minimum width of 16 inches for continuous walls and 2 feet for columns. Any existing fill, topsoil and loose disturbed soils encountered at the foundation bearing level within the excavation should be removed and the footing bearing level extended down to the undisturbed natural gravel soils. Voids created by the removal of large rock should be backfilled with compacted sand and gravel or with concrete. Exterior footings should be provided with adequate cover above their bearing elevations for frost protection. Placement of footings at least 36 inches below the exterior grade is typically used in this area. Continuous foundation walls should be reinforced top and bottom to span local anomalies such as by assuming an unsupported length of at least 10 feet. Foundation walls acting as retaining structures should be designed to resist a lateral earth pressure based on an equivalent fluid unit weight of at least 45 pcf for the on-site gravel soil, excluding oversized rock, as backfill. Floor Slabs: The natural on-site soils, exclusive of topsoil, are suitable to support lightly loaded slab -on -grade construction. To reduce the effects of some differential movement, floor slabs should be separated from all bearing walls and columns with expansion joints which allow unrestrained vertical movement. Floor slab control joints should be used to Job No.108 323A -3 - reduce damage due to shrinkage cracking. The requirements for joint spacing and slab reinforcement should be established by the designer based on experience and the intended slab use. A minimum 4 inch layer of free -draining gravel should be placed beneath slabs - on -grade to facilitate drainage and as a leveling course. This material should consist of minus 2 inch aggregate with Less than 50% passing the No. 4 sieve and less than 2% passing the No. 200 sieve. All fill materials for support of floor slabs should be compacted to at least 95% of maximum standard Proctor density at a moisture content near optimum. Required fill can consist ofthe on-site gravel soils devoid of vegetation, topsoil and oversized rock. Surface Drainage: The following drainage precautions should be observed during construction and maintained at all times after the residence has been completed: 1) Inundation of the foundation excavations and underslab areas should be avoided during construction. 2) Exterior backfill should be adjusted to near optimum moisture and compacted to at least 95% ofthe maximum standard Proctor density in pavement and slab areas and to at least 90% of the maximum standard Proctor density in landscape areas. 3) The ground surface surrounding the exterior of the building should be sloped to drain away from the foundation in all directions. We recommend a minimum slope of 6 inches in the first 10 feet in unpaved areas and a minimum slope of 3 inches in the first 10 feet in pavement and walkway areas. 4) Roof downspouts and drains should discharge well beyond the limits of all backfill. Limitations: This study has been conducted in accordance with generally accepted geotechnical engineering principles and practices in this area at this time. We make no warranty either express or implied. The conclusions and recommendations submitted in this report are based upon the data obtained from the exploratory pits excavated at the locations indicated on Figure 1 and to the depths shown on Figure 2, the proposed type of construction, and our experience in the area. Our services do not include determining the presence, prevention or possibility of mold or other biological contaminants (MOBC) developing in the future. If the client is concerned about MOBC, then a professional in this special field of practice should be consulted. Our findings include interpolation and extrapolation ofthe subsurface conditions identified at the exploratory pits and variations in the subsurface conditions may not become evident until excavation is performed. If conditions encountered during construction appear different from those described in this Job No.108 323A Gtech -4 - report, we should be notified at once so re-evaluation of the recommendations may be made. This report has been prepared for the exclusive use by our client for design purposes. We are not responsible for technical interpretations by others of our information. As the project evolves, we should provide continued consultation and field services during construction to review and monitor the implementation of our recommendations, and to verify that the recommendations have been appropriately interpreted. Significant design changes may require additional analysis or modifications to the recommendations presented herein. We recommend on-site observation of excavations and foundation bearing strata and testing of structural fill by a representative of the geotechnical engineer. For some projects it is also recommended that a concrete placement plan be submitted along with rebar shops. it is the contractor's responsibility to read the drawings and determine which shops are necessary and submit these to the S.E.R in a timely manner. S.E.R.'s review is intended only as an aid to the contractor in obtaining correct shop drawings. Responsibility for correctness shall rest with the contractor. 1. Shop drawings shall be submitted to the architect for all structural items in addition to items required by architectural specifications. 2. A plan layout drawing showing location and item marks for all loose items shall be submitted with individ- ual sub -assembly drawings. 3. The contractor shall review all shop drawings prior to submittal. Items not in accordance with the contract documents shall be flagged upon his review. 4. Verify all dimensions with the architect before submission to the S.E.R. 5. Any changes, substitutions, or deviations from original contract drawings shall be clouded by manufacturer or fabricator. Any of the aforementioned which are not clouded or flagged, by submitting parties, shall not be considered approved after S.E.R.'s review, U.N.O. 6. The shop drawings do not replace the original contract drawings. It is the contractor's responsibility to make sure items are constructed to original drawings. 7. The adequacy of engineering designs and layout performed by others rests with the designing or submitting authority. 8. The use of reproductions of these contract drawings by any contractor, sub -contractor, fabricator, erector or material supplier in lieu of preparation of shop drawings is expressly prohibited. E. GENERAL 1. All dimensions and details on structural drawings shall be verified against the architectural drawings and any discrepancies shall be brought to the attention of the architect before construction or fabrication com- mences. DO NOT SCALE DRAWINGS. 2. Coordinate all openings through floors, walls and roofs with the Mechanical and Electrical Contractors. Notify S.E.R. if any openings are to be cut in structural systems. 3. Provide all necessary temporary bracing, shoring, guying or other means to prevent excessive stresses and to hold structural elements in place during construction. 4. Details on the structural drawings are typical. Verify all dimensions and elevations with the architectural drawings. 5. The contract structural drawings and specifications represent the finished structure. They do not indicate the method of construction. The contractor shall provide all measures necessary to protect the structure during construction. Such measures shall include, but not be limited to, bracing, shoring for loads due to construction equipment, protection of existing structures. Observation visits to the site by the S.E.R. shall not include in- spection of the above items nor will the S.E.R. be responsible for the contractor's means, methods, techniques, sequences for procedure of construction, or the safety precautions and the programs incident there to. 6. Options are for the contractor's convenience. He shall be responsible for all changes necessary if he chooses an option and shall coordinate all details. The cost of additional design work necessitated by selection of an option shall be borne by the contractor. 7. Any engineering design provided by others and submitted for review shall bear the seal and signature of an engineer registered in Colorado. 8. Dimensions on the structural drawings are exact with the exception of masonry and sawn lumber dimen- sions which are nominal. Verify all dimensions with the architectural drawings. 9. Where required construction details are not shown or noted on these plans the contractor shall notify the S.E.R. and the S.E.R. shall provide sufficient details for the work to proceed. 10. Construction materials shall be spread out if placed on framed construction. Load shall not exceed the de- sign live Toad per square foot. FOUNDATION: A. EXCAVATION 1. Contractor shall provide all necessary sheeting, shoring and bracing where required to properly and safely complete the work. 2. Avoid excessive wetting or drying of the foundation excavation during construction, keep excavations rea- sonably free of water at all times and completely free of water during placement of the concrete. B. DESIGN 1. Foundation design is based on the soils report prepared by HP Geotech, project no. 108 323A, dated January 22, 2009 for the Girardot Residence in Garfield County, Colorado. A representative of HP Geotech should visit the site to observe excavations and verify soils conditions prior to placement of concrete form- work. 2. Footings shall bear on natural undisturbed soil or adequately compacted structural fill per geotechnical en- gineer, below the frost depth required by the applicable building code. Backfill on walls with fill on both sides shall be compacted in equal lifts each side of wall. Walls backfilled on one side only shall have all supporting slabs, permanent framing or temporary bracing in place prior to placement of the backfill (unless noted otherwise on plans). STRUCTURAL STEEL Structural steel shall be detailed, fabricated and erected in accordance with latest provisions of the AISC Manual of Steel Construction and AISC Code of Standard Practice. 1. Steel shapes and their properties shall be as follows, U.N.O.: a. All wide flange W shapes shall conform to ASTM A992 b. Channels, angles, plates, and S, M, and HP shapes shall conform to ASTM A36 c. Hollow structural shapes, HSS shall conform to ASTM A500 (Gr. B) d. Anchor rods shall conform to ASTM F1554 Grade 36, and anchor bolts ASTM A307 2. Provide shop applied paint in accordance with the Steel Structures Painting Council specifications as for all exterior members, architecturally exposed members, any members exposed to weather for an excessive period of time during construction, and where indicated on construction documents. Omit shop paint on surfaces to be welded, surfaces to receive welded stud, and contact surfaces of bolted connections. Provide field primer paint for painted members at welds, bolted connections, and areas of abraded shop paint. B. NON -SHRINK GROUT AND ANCHOR BOLTS 1. Anchor bolts used with steel shapes shall be 5/8"0 with minimum of 6" embedment and shall conform to ASTM A307 U.N.O. Anchor bolts embedded in concrete shall be placed with setting templates. Location and coordination of anchor bolt placement shall be the responsibility of the steel fabricator. Steel fabricator shall supply anchor bolt setting plan. 2. All column base plates shall have 1" min to 2" max grout pack. Grout pack shall be non -shrink, non corro- sive, nonmetallic, nonstaining product containing selected silica sands, portland cement, and water -reducing agents. A minimum compressive strength of 3,000 psi after one day and 7,000 psi after 28 days shall be developed. 3. Provide bolted connections for steel beams per the latest AJ.S.C. Manual of Steel Construction. Except where otherwise shown, provide connections to support loads equal to one-half the total uniform load capacity of the member, U.N.O. 2. A minimum of two bolts shall be used for all connections. Size and grade of bolts shall be determined by the fabricator, U.N.O. STRUCTURAL WOOD FRAMING: A. DIMENSIONAL LUMBER All timber construction light and heavy shalt conform to the latest edition of the National Design Specifications for Wood Construction. Provide hot or cold weather protection per ACI 305 & 306. 1. Concrete shall develop a minimum compressive strength at 28 days of: Walls & footings 3000 psi Slabs 3500 psi 2. Provide Type II cement with 4%-8% air entrainment and a 4" slump. Concrete aggregate shall be 3A4" normal weight stone except were lightweight concrete is specified. No admixtures shall be used without ap- proval by the S.E.R. 3. Lightweight concrete is concrete containing lightweight aggregate conforming to ASTM C-330 and has as air-dry unit weight (ASTMC-567) not exceeding 110 pcf. 4. Concrete shall be placed in a manner to prevent voids by using mechanical vibration and/or other approved method. Do not place pipes, ducts, or chases in the structural concrete without approval of the S.E.R. Thor- oughly horoughly clean all keyways and construction joints prior to placing concrete in adjacent area. Concrete shall not be in permanent contact with aluminum. B. REINFORCING Detailing, fabrication, and erection of reinforcing steel bars shall comply with the ACI Manual of Standard Practice for Detailing Reinforced Concrete Structures ACI 315. 1. Reinforcing steel shall be high strength deformed bars conforming to ASTM A615 grade 60. except weld- able reinforcement which shall be ASTM A706, Grade 60. 2. Concrete protection for reinforcement: Cast against and permanently exposed to earth 3 in. Exposed to earth or weather .. #6-#18 bar 2 in. #3-#5 bar 1 112 in. Not exposed to earth or weather . slabs & walls 3/4 in. beams & cols 1 1/2 in. 3. Re -bar lap splices shall be a minimum of 36 bar diameters, unless noted otherwise. 4. Discontinue 1/2 of horizontal steel across construction/control joints. 5. Provide corner bars of equal size and spacing around all corners and intersections. 6. Provide 2-115 bars with a minimum of 2'-0" projection beyond the sides of all openings in walls, beams and slabs. Provide 1--#5 x 4'-0" diagonally at all re-entrant comers of slabs. 7. Provide accessories necessary to properly support reinforcing at the positions shown on the plans. 2. All pressure treated lumber shall be Hem -Fir #2 or better, U.N.O. All wood attached to concrete or masonr} shall be pressure treated lumber. Pressure treated lumber may require heavier galvanizing treatments for fasten- ers and framing hardware. The contractor is to consult with the supplier, manufacturer, and local building offi- cial for current treatment requirements. The following design values were used for Hem -Fir. FASTENERS AND CONNECTORS Alternate fasteners and connectors to those listed below, which have I.C.C. approval, may be used with approval by the S.E.R. 1. All timber and wood framing nailing shall conform to the I.B.C., reference I.B.C. Table No. 2304.9.1 Fasten- ing Schedule, U.N.O.. Reference drawings for locations were additional nailing is required. 2. Nails shall meet the requirement of ASTM F 1667. All connectors for wood framing shown on the plans such as joist hangers, beam hangers, truss hangers, framing clips, hold-down anchors, etc., shall be manufactured by the Simpson Strong -Tie Company Inc, U.N.O. Install per manufacturer's specifications. 4. Bolts for wood member connections shall be 3/4"0, ASTM A307 with a minimum 1 1/4" washer each end U.N.O. Do not countersink bolts unless noted otherwise on structural drawings. 5. Lag screws shall meeting the requirements of ANSIIASME standard 13182.1 and they shall be installed per the latest NDS code. Each lag screw shall be installed with a washer. 6. Powder Actuated Fasteners (P.A.E's) for attaching wood framing to structural steel shall be Remington low velocity fasteners, length and diameter per plans. Install per manufacturer. 7. Screws called out as SDS are as manufactured by Simpson Strong -Tie Company Inc. Screws called out as OLYLOG,TLOK, LOG, LHOG or LL are as manufactured by FastenMasterAgawam, MA. 8. Screws called as #8 thru 1112 wood screws shall meet ANSI/ASME standard 18.6.1. C. WALL FRAMING AND SHEATHING 1. All exterior walls designated as hearing walls shall constructed with 2x6's @ 16".0.C. - U.N.O. All interior walls designated as bearing walls shall be constructed with 2x6's @ 16".0.C. - U.N.O. 2. All exterior walls shall be sheathed with wood structural panels certified by the APA for use as wall sheathing labeled, "EXTERIOR" OR "EXPOSURE 1", rated sheathing, 24/0, PZ" thick attached to studs with 8d common (0.131 0) or galvanized box nails (I" stud penetration) @ 6" O.C. at panel edges and 12" O.C. at intermediate supports.
