Authority. This application for a must be property, described above, or an authorized agent. If the signature below is not that of the Owner, a separate

The following items are required by Garfield County for a final Inspection:
1) A final Electrical Inspection from the Colorado State Electrical Inspector.
2) Permanent address assigned by Garfield County Building Department and posted at the
structure and where readily visible from access road.
3) A finished roof; a lockable building; completed exterior siding; exterior doors and windows
installed; a complete kitchen with cabinets, sink with hot & cold running water, non-absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; all
necessary plumbing.
4) All bathrooms must be complete, with washbowl, tub or shower, toilet, bot and cold running
water, non-absorbent floors, walls finished, and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30" high must be constructed to all IBC and IRC requirements including guardrails.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8) A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY (C.O.) WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.
A C.O. MAY TAKE UP TO 5 BUSINESS DAYS TO BE PROCESSED AND ISSUED.

In order to understand the scope of the work intended under a permit application and expedite the
issuance of a permit it is important that complete information be provided. When reviewing a plan and
it's discovered that required information has not been provided by the applicant, this will result in the
delay of the permit issuance and in proceeding with building construction. The owner or contractor shall
be required to provide this information before the plan review can proceed. Other plans that are .in line
for review may be given attention before the new information may be reviewed after it has been
provided to the Building Department.
TWO (2) SETS OF CONSTRUCTION DRAWINGS & TWO (2) SITE PLANS MUST BE
SUBMITTED FOR ALL APPLICATIONS.
Please review this document to determine if you have enough information to design your project
and provide adequate information to facilitate a plan review. Also, please consider using a design
professional for assistance in your design and a construction professional for construction of your
project. Any project with more than ten (10) occupants requires the plans to be sealed by a
Colorado Registered Design Professional.
To provide for a more understandable plan in order to determine compliance with the building, plumbing
and mechanical codes, applicants are requested to review the following checklist prior to and during
design. Applicants are required to indicate appropriately and to submit the completed checklist at
time of application for a permit.
Plans to be included for a Building Permit, must be on drafting paper at least 18"x24" and drawn
to scale.

Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with decks,
balcony, steps, hand rails and guard rails, windows and doors, including the finish grade line and
original grade. A section showing in detail, from the bottom of the footing to the top of the roof,
including re-bar, anchor bolts, pressure treated plates, floor joists, wall studs and spacing, insulation,
sheeting, house-rap, (which is required), siding or any approved building material. Engineered
foundations may be required.
A window schedule. A door schedule. A floor framing plan, a roof framing plan, roof must be
designed to withstand a 40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. wind speed,
wind exposure B or C, and a 36 inch frost depth.
All sheets to be identified by number and indexed. All of the above requirements must be met or your
plans will be returned.
All plans submitted must be incompliance with the 2003 IRC.
l. Is a site plan included that identifies the location of the proposed structure or addition and distances to
the property lines from each comer of the proposed structure(s) prepared by a licensed surveyor
and has the surveyors signature and professional stamp on the drawing? Properties with slopes of
30% or greater must be shown on the site plan. (NOTE Section: 106.2) Any site plan for the
placement of any portion of a structure within 50 ft. of a property line and not within a previously
surveyed building envelope on a subdivision fmal plat shall be prepared by a licensed surveyor
and have the surveyor's signature and professional stamp on the drawing. Any structure to be
built within a building envelope of a lot shown on a recorded subdivision plat shall include a
copy of the building envelope as it is shown on the final plat with the proposed structure located
within the_pnvelope.

Notes:
If any required information is missing delays in issuing the permit are to be expected. Work may not
proceed without the issuance of a permit. If it is determined by the Building Official that additional
information is necessary to review the application and plans to determine minimum compliance with the
adopted codes, tbe application may be placed behind more recent applications for building permits in the
review process and not reviewed until required information has been provided and the application rotates
again to first position for review, delay in issuance of tbe permit or delay in proceeding with

After doing some investigating (and searching everyone's memory) this is
what appears to have happened back in November, 2008:
After failing 2 foundation wall form inspections, I asked Ruben Vargas of
RMS concrete to meet with the inspector personally as I had a previously
scheduled long weekend vacation. He did so (he thinks it was you) and
received permission to pour, provided the structural engineer approved
drilling and epoxy-setting the missing dowels.
Since a letter from that time seems to be missing, it may very well have
been I who dropped the ball after returning from that short trip. I
apologize. Attached is the engineer's approval of the epoxy-set dowels.
Ruben is searching for any documentation he may have. I'll call the
Department when I have everything together and hopefully this issue can
be resolved.

Subsoil Study for Foundation Design, Proposed Residence, Lot M-43,
Roaring Fork Mesa at Aspen Glen, 54 Golden Stone Drive, Garfield
County, Colorado
As requested, Hepworth-Pawlak Geotechnical, Inc. performed a subsoil study for design
of foundations at the subject site. The study was conducted in accordance with our
agreement for geotechnical engineering services to DB Construction Management dated
March 12, 2008. The data obtained and our recommendations based on the proposed
construction and subsurface conditions encountered are presented in this report.
Proposed Construction: The proposed residence will be two story wood frame structure
over a walkout level basement with an attached garage, located on the site as shown on
Figure I. Ground floors will be slab-on-grade. Cut depths are expected to range between
about 2 to 8 feet. Foundation loadings for this type of construction are assumed to be
relatively light and typical of the proposed type of construction.
If building conditions or foundation loadings are significantly different from those
described above, we should be notified to re-evaluate the recommendations presented in
this report.
Site Conditions: The property was vacant of structures and free of snow cover at the
time of our exploration. Vegetation consists of grass and weeds, with trees along the
property lines. The ground surface slopes down to the east at a grade of about 8 percent
becoming steeper at the rear of the building envelope and sloping down at about 16
percent. There has been grading in the area for the subdivision development.

Subsidence Potential: The Aspen Glen subdivision is underlain by Pennsylvania Age
Eagle Valley Evaporite bedrock. The evaporite contains gypsum deposits. Dissolution of
the gypsum under certain conditions can cause sinkholes to develop and can produce
areas oflocalized subsidence. During previous work in the area, sinkholes were observed
in the Aspen Glen subdivision, mainly east of the Roaring Fork River. Sinkholes were
not observed in the immediate area of the subject lot. The exploratory pits were relatively
shallow, for fOundation design only. Based on our present knowledge of the site, it
cannot be said for certain that sinkholes will not develop. In our opinion, the risk of
ground subsidence at Lot M-43 is low and similar to other lots in the area but the owner
should be aware of the potential for sinkhole development.
Subsurface Conditions: The subsurface conditions at the site were evaluated by
excavating two exploratory pits at the approximate locations shown on Figure I. The
logs ofthe pits are presented on Figure 2. The subsoils encountered, below about Yz foot
of topsoil, consisted of stiff, sandy sihy clay overlying relatively dense, sandy gravel and
cobbles with boulders at a depth of about 1 y, feet. Results of swell-consolidation testing
performed on a relatively undisturbed sample ofthe sandy silty clay, presented on Figure
3, indicate low compressibility and a low hydro-compression potential when wetted.
Results of a gradation analysis perfonned on a sample of the sandy gravel and cobbles
soils (minus 5 inch fraction) obtained from the site are presented on Figure 4. No free
water was observed in the pits at the time of excavation and the soils were moist to
slightly moist.
Foundation Recommendations: Considering the subsoil conditions encountered in the
exploratory pits and the nature o fthe proposed construction, we recommend spread
footings placed entirely on the undisturbed natural course granular soils designed for an
allowable bearing pressure of3,000 psffor support of the proposed residence. The sandy
silty clay soils may tend to compress after wetting and should be removed from beneath
footing areas. Footings should be a minimum width of 16 inches for continuous walls
and 2 feet for columns. The clay soils and any loose and disturbed soils encountered at
the foundation bearing level within the excavation should be removed and the footing
bearing level extended down to the undisturbed natural granular soils. Exterior footings
should be provided with adequate cover above their bearing elevations for frost
protection. Placement of footings at least 36 inches below the exterior grade is typically
used in this area. Omtinuous foundation walls should be reinforced top and bottom to
span local anomalies such as by assuming an unsupported length of at least 12 feet.

Foundation walls acting as retaining structures should also be designed to resist a lateral
earth pressure based on an equivalent fluid unit weight of at least 50 pcf fur the on-site
soil, excluding topsoil and oversized rocks, as backfill. Some settlement of deep
foundation wall backfill should be expected even though the material is properly
compacted.
Floor Slabs: The natural on-site soils, exclusive of topsoil, are suitable to support lightly
loaded slab-on-grade construction. Removing the sandy silty clay soils from below slabs
would provide a relatively low risk of slab settlement and can be further evaluated at the
time of construction. To reduce the effects of some differential movement, floor slabs
should be separated from all bearing walls and columns with expansion joints which
allow unrestrained vertical movement. Floor slab control joints should be used to reduce
damage due to shrinkage cracking. The requirements for joint spacing and slab
reinforcement should be established by the designer based on· experience and the intended
slab use. A minimum 4 inch layer of free-draining gravel should be placed beneath
basement level slabs to facilitate drainage. This material should consist of minus 2 inch
aggregate with less than 50% passing the No.4 sieve and less than 2% passing the No.
200 sieve.
All fill materials for support of floor slabs should be compacted to at least 95% of
maximum standard Proctor density at a moisture content near optimum. Required fill can
consist of the on-site granular soils devoid of topsoil and oversized rocks.
Underdrain System: Although fi·ee water was not encountered during our exploration, it
has been our experience in the area that local perched groundwater can develop during
times of heavy precipitation or seasonal runoff. Frozen ground during spring runoff can
also create a perched condition. We recommend below-grade construction, such as
retaining walls and basement areas, be protected from wetting and hydrostatic pressure
buildup by an underdrain system.
The drains should consist of drainpipe placed in the bottom of the wall backfill
surrounded above the invert level with free-draining granular material. The drain should
be placed at each level of excavation and at least 1 foot below lowest adjacent finish
grade and sloped at a minimum I% to a suitable gravity outlet. Free-draining granular
material used in the underdrain system should contain less than 2% passing the >fo. 200

sieve, less than 50% passing the No.4 sieve and have a maximum size of2 inches. The
drain gravel backfill should be at least I 'l2 feet deep.
Surface Drainage: The following drainage precautions should be observed during
construction and maintained at all times after the residence has been completed:
1) Inundation of the foundation excavations and underslab areas should be
avoided during construction.
2) Exterior backfill should be adjusted to near optimum moisture and
compacted to at least 95% of the maximum standard Proctor density in
pavement and slab areas and to at least 90% of the maximum standard
Proctor density in landscape areas. Free-draining wall backfill should be
capped with about 2 feet of the on-site, finer graded soils to reduce surfuce
water infiltration.
3) The ground surface surrounding the exterior of the building should be
sloped to drain away from the foundation in all directions. We
recommend a minimum slope of 12 inches in the first 10 feet in unpaved
areas and a minimum slope of3 inches in the first 10 feet in pavement and
walkway areas.
4) Roof downspouts and drains should discharge well beyond the limits of all
backfill.
Limitations: This study has been conducted in accordance with generally accepted
geotechnical engineering principles and practices in this area at this time. We make no
warranty either express or implied. The conclusions and recommendations submitted in
this report are based upon the data obtained from the exploratory pits excavated at the
locations indicated on Figure 1 and to the depths shown on Figure 2, the proposed type of
construction, and our experience in the area. Our services do not include determining the
presence, prevention or possibility of mold or other biological contaminants (MOBC)
developing in the future. If the client is concerned about MOBC, then a professional in
this special field of practice should be consulted. Our findings include interpolation and
extrapolation of the subsurface conditions identified at the exploratory pits and variations
in the subsurface conditions may not become evident until excavation is performed. If
conditions encountered during construction appear different from those described in this
report, we should be notified at once so re-evaluation of the recommendations may be
made.

Thank you for submitting and shuttling a final set of plans to the DRC for review on July
9, 2008. The DRC has completed final review of the submitted set of plans. Receipt is
hereby acknowledged for the Builder's Completion Deposit of $20,000 and the W -9
form, the $3,000 SAF payment and the $1,644 CRAF payment. As we discussed at the
meeting I still need the Soils Analysis and please send that to me electronically.
The DRC requires a building height verification survey to be completed prior to the
rough framing inspection. The owner or owner's representative owner shall submit a
completed height survey to Aspen Glen DRC. Height survey shall be performed by a
Colorado professional land surveyor and shall include a statement by the surveyor stating
whether the building is/is not in compliance with Garfield County building height rules
and explaining how building height was determined. The top of sheathing shall conform
to the height limit. The standard form that shall be used is attached to this letter.
In the event there is a dispute the DRC reserves the right to verify height with their
surveyor. Cost of the survey will be deducted from the builder's completion deposit.
The DRC has developed a standard form for the surveyor to use when conducting this
survey. Please contact me before that survey is scheduled.
A pre-construction meeting shall be scheduled prior to the start of construction. Please
call me to schedule that meeting. Two additional site inspections need to occur during
construction of the residence. These are ultimately the responsibility of the owner and
bear upon release of the Builder's Completion Deposit. Please contact the DRC
Administrator at (970) 963-3362, upon completion of the dry-in phase of construction
and for a final site inspection by the DRC. Details of these procedures are contained in
the Design Review application packet. Failure to call for any of these inspections will
result in $1000 per violation which will be deducted from the Builder's Completion
Deposit. It will be necessary for the builder to arrange reviews in accordance with
the DRC's scheduled sessions.

Certificate of Occupancy and an Improvement Location Certificate must be provided to
