The following items are required by Garfield County for a final inspection:
1. A final Electrical Inspection from the Colorado State Electrical Inspector;
2. Permanent address assigned by Garfield County Building Department posted where readily
visible from access road;
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water, non-absorbent
kitchen floor coverings, counter tops and finished walls, ready for stove and refrigerator, all
necessary plumbing;
4. A complete bathroom, with wash bowl, tub or shower, toilet stool, hot and cold running
water, non-absorbent floors and walls finished and a privacy door;
5. All steps outside or inside over three (3) steps must have handrails, guard rails on balconies or
decks over 30" high constructed to all 1994 UBC requirements;
6. Outside grading done to where water will detour away from the building;
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e. weather, but a Certificate of Occupancy will not be issued until all
the required items are completed and a final inspection made.

CONTRUCTION NOTES
1. GENERAL:
A. Engineer's approval must be secured for all structural
substitutions.
B. Contractor to verify all loyout dimensions prior
to placement of concrete forms.
2. FOUNDATIONS:
A. Maximum soil design pressure— 2000 Ib/4
B. All footings are to be placed on firm undisturbed, oved
natural soil or properly compacted backfill, app by
Engineer. Backfill shall be compacted to 95% (minimum)
Standard Proctor Density.
C. Do not place backfill against basement walls until
basement and first floor are in place.
D. Center all footings under walls, columns, or grid lines,
unless otherwise noted on plans.
E. Foundation Bolts— 1/2 x 7", or M.A.S., max 32" 0.C. and one within
12" of each end at sill plate joints, Embedded min. 7"
3. CONCRETE:
A. All cost—in—place concrete shall be made with type I1
Portland Cement, stone aggregate, and shall deveiop 3,000
PSI compressive strength in 28 days. Air entrainment 5%
(±1%).
B. Floor slobs on grade shall be poured in checker pattern
avoiding re—entrant corners with construction joints
located under partitions where practical,
C. Slobs, topping, footings, beams, and walls shall not hove
joints in a horizontal plane. Any stop in concrete work
must be made at center of span with verticol bulkheads
and horizontal keys, unless otherwise shown. All
construction joints shall be as detailed or as approved
by Engineer.
D. All concrete work and reinforcement detailing shall be in
accordance with ACI 318-89 Building Code. Unless noted
otherwise, all permonently exposed edges of concrete work
shall have a 3/4" chamfer.
E. No concrete is to be poured on frozen soil, and oll
footings, pads, and foundations must be adequately
protected from freezing during and after pouring of
concrete.
F. Ali exposed concrete shall be covered with 2 mil. PVC or
curing compound.
G. The contractor shall be responsible for taking concrete
samples and shall provide Engineer with copies of the
test results.
4. REINFORCEMENT:
A. Reinforcing shall be high strength deformed bars ASTM
designation A615 Grade 60, except ties, stirrups, and
where designated by engineer. All other reinforcing shall be
deformed bars ASTM designation A615 Grode 40. See note 30 above.
B. Welded Wire Fabric shall conform to ASTM M85-70, and
shall be lopped one full mesh at side and end splices.
See note 3D above.
C. Reinforcement protection: 3"
1. Concrete poured against eorth
2. Concrete poured in forms but exposed to weather..1 1/2"
D. AN bar lengths are drawn to scale unless noted. No
splices of reinforcement shall be made except as detailed
or authorized by the Engineer. Lap splices, where
permitted, shall be a minimum of 36 bar diameters. Make
all bars continuous oround corners.
E. Concrete reinforcing steel must be inspected by the
Engineer or local building official before placing of
concrete.

I, CECIL RASBERRY (herein called "Applicant"), hereby requests
permission and authority from the Board of County Commissioners to construct a
driveway approach(es) on the right-of-way of Garfield Coonty Road Number 30t, ,
adjacent to Applicant -s property located on the side of the road, a
distance of mile(s) from 3/10 SOUTH OF 7047 CR301 for the purpose of
obtaining access to CIRCULAR DRIVEWAY PERMIT
Applicant submits herewith for the consideration And approval
of the Board of County Commissioners, a sketch of the proposed instal-
lation showing all necessary specification detail including (1) front-
age of lot along road, (2) distance from centerline of road to
property line, (3) number of driveways requested, (4) width of pro-
posed driveway(s) and angle of approach, (5) distance from driveway
to road interseetion, if any, (6) size and shape of area separating
driveways if more than one approach, and (7) setback distance of
building(s) and other structures or improvements.
GENERAL PROVISIONS
FTRST: The Applicant represents all parties in interest, and affirms
that the driveway approach(es) is to be constructed by him for the
bona fide purpose of securing access t.o his property and not for the
purpose of doing business or servicing vehicles on the road right-of-way.
SECOND: The Applicant shall furnish all labor and materials, perform
all work, and pay all costs in connection with the construction of the
driveway(s) And its appurtenances on the right-of-way. All work shall
be completed within 30 days of the Permit date,
THIRD: The type of construction shall be As designated and/or approved
by the Beard of County Commissioners or their representative, And all
materials used shall he of satisfactory quality and suhject t.o inspec-
tion and approval of the Beard of County Commissioners or their
representative.
FOURTH: The traveling public shall be protected during the instal-
lation with proper warning signs and signals and the Board of County
Commissioners and their duly appointed agents and employees shall be
held harmless against any action for personal injury or property
damage sestained by reason of the exercise of the Permit.
FIFTH: The Applicant shall assume resoonsibilitv for thim
cjearance of snow, ice or sleet upon any portion of the driveway ap-

proach(es) even though deposited on the driveway(s) in the course of
the County snow removal operations,
STXTH: In the event it becomes necessary to remove any right-of-way
fence, the posts on either side of the entrance shall he securely
braced before the fence is cut to prevent any slacking of the remain-
ing fence, and all posts and wire removed shall be turned over to the
District Road supervisor of the Board of County Commissioners,
SEVENTH: No revisions or additions shall he made to the driveway(s)
or its appurtenances on the right-of-way without the written permis-
sion of the Board of County Commissioners,
ETGHTH: Provisions and specifications outlined herein shall apply on
all roads under the jurisdiction of the Board of County Commissioners
of Garfield County, Colorado, and the Specifications, set forth on the
attached hereof and incorporated heroin as conditions hereof.
