The following items are required by Garfield County for a final Inspection:
I) A final Electrical Inspection from the Colorado State Electrical Inspector.
2) Permanent address assigned by Garfield County Building Department and posted at the
structure and where readily visible from access road.
3) A finished roof; a lockable building ; completed exterior siding; exterior doors and windows
in stalled ; a complete kitchen with cabinets, sink with hot & cold running water, non-absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; all
neces sa ry plumbing.
4) All bathrooms must be co mplete , with washbowl, tub or shower, toilet, hot and cold running
water, non-absorbent floors, walls fini s hed, and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30" high must be constructed to alllBC and lRC requirements includin g guardrails.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8) A final inspection sign off by the Garfield County Road & Bridge Department for driveway
in stallation , where applicable; as well as any fina l s ign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY (C.O.) WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.
A C.O. MAY TAKE UP TO 5 BUSINESS DAYS TO BE PROCESSED AND ISSUED.
OWNER CANNOT OCCUPY OR USE DWELLING UNTIL A C.O. IS ISSUED.

MINIMUM APPLICATION REQUIREMENTS
For
MANUFACTURED HOMES
In order to understand the scope of the work intended under a permit application and expedite the issuance of a
permit it is important that complete information be provided. When reviewing a plan and it's discovered that
required information has not been provided by the applicant, this will result in the delay of the permit issuance
and in proceeding with building construction. The owner or contractor shall be required to provide this
information before the plan review can proceed. Other plans that are in line for review may be given attention
before the new information may be reviewed after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your project and
provide adequate information to facilitate a plan review. Also, please consider using a design professional
for assistance in your design and a construction professional for construction of your project.
To provide for a more understandable plan in order to determine compliance with the building, plumbing and
mechanical codes, applicants are requested to review the following checklist prior to and during design.
Applicants are required to indicate appropriately and to submit the completed checklist at time of application for a
permit.
All plans submitted must be incompliance with the 2003 IRC for Residences and 2003 IBC for Commercial
Offices and Storage.
I. Is a site plan included that identifies the location of the proposed structure or addition and distances to the
property lines from each corner of the proposed structure prepared by a licensed surveyor and has the
surveyors signature and professional stamp on the drawing? Properties with slopes of 30% or more must
be shown on site plan. (NOTE Section 106.2) Any site plan for the placement of any portion of a
structure within 50 ft. of a property line and not within a previously surveyed building envelope on a
subdivision final plat shall be prepared by a licensed surveyor and have the surveyor's signature and
professional stamp on the drawing. Any structure to be built within a building envelope of a lot shown on
a recorded subdivision plat shall include a copy of the building envelope as it is shown on the final plat
with the proposed structure located within the envelope.

2. Does the site plan when applicable include the location of the !.S.O.S. (Individual Sewage Disposal System)
and the distances to the property lines, wells (on subject property and adjacent properties), streams or
water courses? This information must be certified by a licensed surveyor with their signature and
professional stamp on the design.

3. Does the site plan include any other buildings, setback easements and utility easements? Please refer to
Section 5.05.03 in the Garfield County Zoning Resolution if the property you are applying for a building
permit on is located on a corner lot. Special setbacks do apply.

Photographs of the Colorado Division of Housing certification tags are provided for the
housing units that immediately available to OXY. For those units that have not been
delivered yet, detailed specifications from the manufacturer are provided (see attached).
All trailers used for temporary housing are all built to meet or exceed Garfield County
building requirements. The manufacturers of these units will certify each trailer
according to Colorado Division of Housing (CDOH) regulations. For the units identified
in this application but not yet available to MOC or it's contractors, evidence of the
CDOH certification will be provided to the Garfield County Building and Planning
Department prior to occupancy at well pad 697-09-61 (parcel 216921400026).
All trailers, potable water systems and wastewater systems will be located within the
confines of the well pad as approved by the COGCC. The site plan for the 697-09-61
well pad is provided along with this submittal.

Thanks for the help on my other permits, I am preparing some more permits, this time for OXY.
I am preparing Building Permits for Temporary Employee Housing Units for 2 OXY locations. These are COGCC approved
well pad locations, one is in Section 16, township 6 south, range 97 west, the other is in Section 9, township 6 south,
range 97 west. Both of these pads are accessed via the Garden Gulch road.
Attached are maps of the roads leading to the locations.
I need a driveway permit exemption letter from you. Could you send me an email that I can use for my Building
Permits?

I spoke with Ken Smith after receipt of your questions and he agreed with my comment that there was a bit of
redundancy imbedded in his questions.
After getting clarification from him on his questions I told him I would provide both an email and hard copy response.
In response to your questions specific to the Garfield County Building Permit applications for Temp Housing Locations
697-09-61 and 697-16A2, please see the following:
Question 1: How are the units connected together? Is it a structural connection skid unit to skid unit or just butted up
together?
Response: These units are butted up together.
Question 2: (Provide) .. Foundation and tie down details .... Foundation plan indicating support of the buildings, tie down
locations ..... Do the three "main" rails bear on grade, on cross rails, on needle beams supported by piles, or other?
Response: Please see the attached diagram and email from the OXY Compliance Officer indicating that the trailers will
be tied down near the corners. This is demonstrated in the attached drawing of the typical tie-down locations.
Foundation systems will consist of trailer skids bearing directly on grade. These units sit directly on the ground.
Question 3: (Provide) .. assumed soil properties and/or soils report.
Response: OXY places soil layers in lifts. The resultant compaction is a 90% modified proctor for the entire pad site,
which includes the areas supporting the Temporary Housing Units. OXY utilizes sheepsfoot compactors and water trucks,
as necessary, to achieve the necessary soil compaction. The maximum loading for the soils supporting the Temp Housing
units is approximately 4180 PSF. Please see attached OXY soils document which demonstrates the typical soil loading
capacities for OXY well pads. (F45-Foundation Loading).
Question 4: (Provide) .. A "key/master plan" indicating a plan of all the different types of buildings to be joined together,
showing all doors, including paths of egress and/or travel between different buildings.

Possibly having the option to do both a single permit per pad of several units and also the option of issuing a single permit
for each building would be a good way to go. If you guys are doing work for Company A and they move the same units
with the rig every time the rig moves, it would make sense to issue one permit for all the units and just amend the permit
with a new site plan when the rig moves. Company B may mix and match rigs with units and a single permit per building
would make sense. Based on that, submit your application based on the practice the company will be following and let's
hope they do not mix the two options. If it appears a company may mix the two options, then one complete application
per building would be the best way to go.

Thanks again for answering my questions this morning, I have another one:
If I am applying for building permits for a number of housing units on a single COGCC approved drill site, I know I have
to submit an application for each unit but, do I have to provide a set of site plans for each unit as well or
can I group all the units specific to the pad site and just provide 2 site plans for the entire pad site. The site plan would
reflect all the housing units for the pad site.
I'm trying not to double up on the paperwork since the site plan would be the same for all the units on the pad site.

Tested the automatic release system with power to appliances. Ansul system activated at all
spray nozzles including upper hood and duct. Power cut off to all appliances. Hood fan
continued to run to exhaust all smoke out the duct.

Tested the manual pull station. Fire alarm sounded during both tests . The hood and duct
system is approved .
FIRE ALARM SYSTEM:
Tested all smoke and heat detectors. Tested all pull stations . Tested all emergency lights
installed in kitchen , recreation room and corridors . All devices activated properly and all
horn/strobe units functioned properly . Horn/strobes and manual pull stations are located in
accordance with NFPA 72 .
Fire alarm system is approved .
AUTOMATIC FIRE SPRINKLER SYSTEM:
Hydraulic data plate states that 3 sprinkler heads were calculated at a .05 density.
Hydraulic demand for this system is 38 .9 17 gpm@ 22 .7 p.s.i.
NFPA 13R requires a 30 minute delivery of calculated water. Water storage on site dedicated
to sprinkler system is 1400 gallons. 30 minute requirement is 1,168 gallons. There are 1400
gallons of dedicated water on site .
A hydrostatic test at 200 p.s.i. for two hour duration was conducted on December 19, 2008 and
the system passed with no loss of pressure .
Fire Pump Test-Static Pressure on riser was 35 p.s.i .
An inspectors test valve was i nstalled on 1/9/2009 and the pump tested at 22 p .s .i . residual
pressure .
Fire pump is an approved Aqua Tank NFPA 13R package .
T he line from the pump to the water tank (plastic) has been replaced with the prope r braided
stainless steel flex line .
Hold open devices were installed on all interior doors. I stated that they must be removed
because all interior doors are self-closing 20 minute fire doors and frames. This task was
completed in my presence .
I informed Outpost Office Division that no storage would be allowed on shelves within 18
inches of the fire sprinkler deflectors . They must maintain an 18 inch clear space below
sprinkler heads to ensure proper operation.
The fire sprinkler system and fire pump operation is approved .

It appears they still need to pass their sprinkler system test. Please file . Hopefully, it is under Oxy or the property
owner and has the well site number HMP 290. It would be better for the State to sign off before we go up to do
our final. That way we are the last ones in and in theory they can occupy with our final review.

gallons short.

We could not perform inspector's test because no restricted orifice was installed in the line.
We conducted a main drain test and residual pressure dropped to
0 p.s.i. The system must flow at a minimum of 33.84 p.s.i. Fire pump is an approved Aqua
Tank NFPA 13R package. Jim from Tri County Fire Protection will install an inspector's test
line in the riser and retest the pump.
I stated that the line from the pump to the water tank (plastic) must be replaced with a braided
stainless steel flex line or a copper line.

The following list provides more detailed descriptions of the requirements in Section 4 of the Mechanical Compliance
Certificate.
Requirements Specific To: HVAC System 1 :
1. The specified heating and/or cooling equipment is covered by ASH RAE 90.1 Code and must meet the following minimum efficiency:
Rooftop Package Unit: 9.7 SEER
Generic Requirements: Must be met by all systems to which the requirement is applicable:
1. Design heating and cooling loads for the building must be determined using procedures in the ASHRAE Handbook of Fundamentals or
ali approved equivalent calculation procedure.
2. All equipment and systems must be sized to be no greater than needed to meet calculated loads. A single piece of equipment providing
both heating and cooling must satisfy this provision for one function with the capacity for the other function as small as possible, within
available equipment options.
Exception: The equipment and/or system capacity may be greater than calculated loads for standby purposes. Standby equipment
must be automatically controlled to be off when the primary equipment and/or system is operating.
Exception: Multiple units of the same equipment type whose combined capacities exceed the calculated load are allowed if they are
provided with controls to sequence operation of the units as the load increases or decreases.
3. Each heating or cooling system seiVing a single zone must have its own temperature control device.
4. Each humidification system must have its own humidity control device.
5. Thermostats controlling both heating and cooling must be capable of maintaining a 5 degrees F dead band {a range of temperature
where no heating or cooling is provided).
• Exception: Deadband capability is not required if the thermostat does not have automatic changeover capability between heating and
cooling.
6. The system or zone control must be a programmable thermostat or other automatic control meeting the following criteria:a) capable
of setting back temperature to 55 degrees F during heating and seWng up to 85 degrees F during coolingb) capable of automatically
setting back or shutting down systems during unoccupied hours using_ 7 different day schedulesc) have an accessible 2-hour occupant
overrided) have a battery back-up capable of maintaining programmed settings for at least 10 hours without power.
-Exception: A setback or shutoff contrails not required on thermostats that control systems serving areas that operate continuously.
• Exception: A setback or shutoff control is not required on systems with total energy demand of 2 kW (6,826 Btu/h) or less.
7. Outdoor-air supply systems with design airflow rates >3,000 cfm of outdoor air and all exhaust systems must have dampers that are
automatically closed while the equipment is not operating.
8. The system must supply outside ventilation air as required by Chapter 4 of the International Mechanical Code. If the ventilation system is
designed to supply outdoor-air quantities exceeding minimum required levels, the system must be caPable of reducing outdoor-air flow to
the minimum required levels.
9. Air ducts must be insulated to the following levels:a) Supply and return air ducts for conditioned air located in unconditioned spaces
(spaces neither heated nor cooled) must be insulated with a minimum of R-5. Unconditioned spaces include attics, crawl spaces,
unheated basements, and unheated garages.b) Supply and return air ducts and plenums must be insulated to a minimum of R-8 when
located outside the building.c) When ducts are located within exterior components (e.g., floors or roofs), minimum R-8 insulation is
required only between the duct and the building exterior.
Exception: Duct insulation is not required on ducts located within equipment.
-Exception: Duct insulation is not required when the design temperature difference between the interior and exterior of the duct or
plenum does not exceed 15 degrees F.
10. All joints, longitudinal and transverse seams, and connections In ductwork must be securely sealed using weldments; mechanical
fasteners with seals, gaskets, or mastics; mesh and mastic sealing systems; or tapes. Tapes and mastics must be listed and labeled in
accordance with UL 181A or UL 181 B.
-Exception: Continuously welded and locking-type longitudinal joints and seams on ducts operating at static pressures less than 2
inches w.g. pressure classification.
11. Mechanical fasteners and seals, mastics, or gaskets must be used when connecting ducts to fans and other air distribution equipment,
including multiple-zone terminal units.

12. Operation and maintenance documentation must be provided to the owner that includes at least the following information:a) equipment
capacity (input and output) and required maintenance actionsb) equipment operation and maintenance manualsc) HVAC system
control maintenance and calibration information, including wiring diagrams, schematics, and control sequence descriptions; desired
or field-determined set points must be permanently recorded on control drawings, at control devices, or, for digital control systems, in
programming commentsd) complete narrative of how each system Is Intended to operate.
13. Each supply air outlet or diffuser and each zone terminal device (such as VAV or mixing box) must have its own balancing device.
Acceptable balancing devices include adjustable dampers located within the ductwork, terminal devices, and supply air diffusers.
14. Stair and elevator shaft vents must be equipped with motorized dampers capable of being automatically closed during normal building
operation and interlocked to open as required by fire and smoke detection systems. All gravity outdoor air supply and exhaust hoods,
vents, and ventilators must be equipped with motorized dampers that will automatically shut when the spaces served are not in use.
Exceptions: ·Gravity (non-motorized) dampers are acceptable in buildings less than three stories in height above grade. -Ventilation
systems serving unconditioned spaces.
