The folljlwing items are required by Garfield County for a final Inspection:
i.) A final Electrical Inspection from the Colorado State Electrical Inspector.
2) Permanent address assigned by Garfield County Building Department and posted at the
structure and where readily visible from access road.
3) A finished roof; a lockable building; completed exterior siding; exterior doors and windows
installed; a complete kitchen with cabinets, sink with hot & cold running water, non-absorbent
kitchen floor covering, counter tops and finished walls, ready for stove and refrigerator; all
necessar; plumbing.
4) All bathrooms must be complete, with washbowl, tub or shower, toilet, hot and cold running
water, non-absorbent floors, walls finished, and privacy door.
5) Steps over three (3) risers, outside or inside must be must have handrails. Balconies and decks
over 30" high must be constructed to all IBC and IRC requirements including guardrails.
6) Outside grading completed so that water slopes away from the building;
7) Exceptions to the outside steps, decks, grading may be made upon the demonstration of
extenuating circumstances., i.e. weather. Under such circumstances A Certificate of
Occupancy may be issued conditionally.
8) A final inspection sign off by the Garfield County Road & Bridge Department for driveway
installation, where applicable; as well as any final sign off by the Fire District, and/or State
Agencies where applicable.
A CERTIFICATE OF OCCUPANCY (C.O.) WILL NOT BE ISSUED UNTIL ALL THE
ABOVE ITEMS HAVE BEEN COMPLETED.
A C.O. MAY TAKE UP TO 5 BUSINESS DAYS TO BE PROCESSED AND ISSUED.
OWNER CANNOT OCCUPY OR USE DWELLING UNTIL A C.O. IS ISSUED.

In order to understand the scope of the work intended under a permit application and expedite the issuance of
a permit it is important that complete information be provided. When reviewing a plan and it's discovered that
required information has not been provided by the applicant, this will result in the delay of the permit issuance
and in proceeding with building construction. The owner or contractor shall be required to provide this
information before the plan review can proceed. Other plans that are in line for review may be given attention
before the new information may be reviewed after it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your project and
provide adequate information to facilitate a plan review. Also, please consider using a design
professional for assistance in your design and a construction professional for construction of your
project. Any project with more than ten (10) occupants requires the plans to be sealed by a Colorado
Registered Design Professional.
To provide for a more understandable plan and in order to determine compliance with the building, plumbing
and mechanical codes, applicants are requested to review the following checklist prior to and during design.
Plans to be included for a Building Permit must be on draft paper at least 18"x 24'"' and drawn to
scale.
Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with decks,
balcony steps, hand rails and guard rails, windows and doors, including the finish grade and original grade

line. A section showing in detail, from the bottom of the footing to the top of the roof, including re-bar,
anchor bo!ts, pressure treated plates, floor joists, wall studs and spacing, insulation, sheeting, house-rap,
(which is required), siding or any approved building material. Engineered foundations may be required.

A window schedule. A door schedule. A floor framing plan, a roofing framing plan, roof must be designed to
withstand fl40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. windspeed, wind exposure B
or C, and a 36 inch frost depth.
All sheets need to be identified by number and indexed. All of the above requirements must be met or your
plans will be returned.
All plans submitted must be incompliance with the 2003 IBC, IPC, IMC, IFGC, and IFC.
Applicants are required to indicate appropriately and to submit completed checklist at time of
application for a permit:
1. Is a site plan included that identifies the location of the proposed structure, additions or other buildings,
setback easements, and utility easements showing distances to the property lines from each corner of
the proposed structure prepared by a licensed surveyor and has the surveyors signature and
professional stamp on the drawing? Slopes of 30% or more on properties must be show on site plan.
(NOTE: Section 106.2) Any site plan for the placement of any portion of a structure within 50 ft. of a
property line and not within a previously surveyed building envelope on a subdivision final plat shall
be prepared by a licensed surveyor and have the surveyors signature and professional stamp on the
drawing. Any structure to be built within a building envelope of a lot shown on a recorded
subdivision plat, shall include a copy of the building envelope as it is shown on the final plat with the
proposed structure located within the envelope.

Subject: Observation of Excavation, Proposed Veterinary Clinic Building, Alpine
Animal Hospital, 17776 Highway 82, Garfield County, Colorado
Dear Mr. Sabo
As requested, a representative ofHcpworth·Pawlak Geotechnic~;~.l, Inc. observed the
excavation at the subject site on March 13 and 16, 2009 to evaluate the soils exposed fur
foundation support. The :findings of our observations and recommendations for the
foundation design are presented in this report. The services were performed in
accordance with our agreement for geotechnical engineering services to Legacy Homes,
dated March 9, 2009. We previously observed a foundation excavation at the subject site
fur a proposed addition to the existing animal hospital building and reported our findings
in a letter dated October 13, 1999, Job No. 199 793.
The proposed building will be a tall one story m10tal frame structure with slab on grade
floor. A spread footing f'Oundation imposing a maximum soil bearing pressure of 2,500
psfwas assumed .fur des.ign.
At the time of our frrst visit to the site, the foundation excavation had been cut in one
level about 3 feet below the adjacent ground surface and was about 70% completed. We
observed the completed. excavation on March 16, 2009. The soils exposed in the bottom
ofthe excavation consisted of silty sandy gravel with cobbles and small boulders. No
fi·ee water was encountered in the excavation and the soils were slightly moist to moist.
The topsoil had been removed within the floor slab area and up to 12 inches of the on-s.ite
granular soil had been placed and compacted. The soil was too rocky for compaction
testing and we observed proof rolling of the area. The proof roll was perforro.ed using a
I 0-wheel dump truck with a full load of% aggregate base course material. The area that
was proof rolled appeared to be firm and stable under the applied load.
Considering the conditions exposed in the excavation and the nature of the proposed
construction, spread fuotings placed on the undisturbed natural soil designed for an
allowable soil bearing pressure of2,500 psf can be used for support ofthe proposed
bt1ilding. Footings should be a minimum width of 16 inches for continuous walls and 2
feet for columns. Loose and disturbed soils in footing areas should be removed and the
bearing level extended down to the undisturbed natural soils. Exterior footings should be
provided with adequate soil cover above their bearing elevations fur frost protection.

such as by assuming an unsupported length of at least l 0 fuet. Structural.fill placed
within floor slab areas can consist ofthe on-site granular soils, devoid of oversized rock,
~ompacted to at least 95% of standard Proctor density at a moistnre content near
optimum. Backfill placed around the structure should be compacted and the surface
graded to prevent ponding within at least 1 0 feet of the building.
The recommendations submitted in this letter are based on our observation of the soils
exposed within the fuundation excavation and do not include subsurfuce exploration to
evaluate the subsurface conditions within the loaded depth of foundation influence. This
study is based on the assumption that soils beneath the footings have equal or better
support than those exposed. The risk of fuundation movement may be greater than
indicated in this report because of possible variations in the subsurfuce ~ond.itions. In.
order to reveal the nature and extent of variations in the subsurface conditions below the
excavation, drilllng would be required. It is possible the data obtained by subsurface
exploration could change the recommendations contained in this letter.

Thank you for taking the time to discuss the security grilles at the Alpine Animal
Hospital with me today. As we discussed, there are three pertinent sections of the 2003
IBC:
402.11 Security Grilles and Doors #3:
Horizontal sliding or vertical security grilles or doors that are part of a required means
of egress shall conform to the following:
3. The doors or grilles shall be openable from within without the use of any special
knowledge or effort where the space is occupied.
1008.1.2 Door Swing Exception #1:
Egress doors shall be side hinged swinging.
Exceptions:
1. Private garages, office areas, factory and storage areas with an occupant load
of 10 or less.
1008.1.3.5 Security Grilles:
In Groups B, F,M and S, horizontal sliding or vertical security grilles are permitted at
the main exit and shall be openable from the inside without the use of a key or special
knowledge or effort during periods that the space is occupied. The grilles shall remain
secured in the full-open position during the period of occupancy by the general public.
Where two or more means of egress are required, not more than one-half the exits or exit
access doorways shall be equipped with horizontal sliding or vertical security grilles.

Since the occupant load in the pharmacy is less than 10 and we are not required to have
more than one egress door from that space, then the security grilles would be permitted on
the condition that they are openable from within the room at all times. We will adhere to
this requirement and specify hardware for the doors which complies with it.
I will also send you our hardware specification for the County's records. Please let me
know if this is not your understanding of our conversation.

accordance with the above referenced Stale Highway Access Permit and this Notice to Proceed.
This Notice to Proceed is valid only if the referenced Access Permit has not expired. Access Permits expire
one year from date of issue if not under construction, or completed. Access Permits may be extended in
accordance with Section 2.3(11)(d), of the Access Code.
Adequate advance warning is required at all times during access construction, in conformance with the Manual
on Uniform Traffic Control Devices for Streets and Highways.

The following paragraphs are excerpts of the Stale Highway
Access Code. rh$se are provided for your convenience but
do not alteviate compliance with all ~ections of the Access
Code. A copy of the Slate Highway Access Code is
available from your local Issuing authority (local government)
or !he Colorado Department of Transportation (Department).
When this permit was issued, the issuing authority m•de its
decision based in part on information submitted by the
applicant, on the ao<:$sS category which is assigned to the
highway, what allernatille access to other public roads and
streets is available, and safety and design standards.
Changes In use or design not approved by the permit or the
issuing authority may causa the revocation or suspension of
the permit
APPEA~S
1. Should the permittee or applicant object to lhe denial of
a permil application by tile Department or object to any of
the terms or conditions of a permit placed there by the
Department, the applicant and permittee (appollanQ have a
right to appeal the decision to the [Transpoltatlon)
Commi~slon (of Colorado}, To appeal a declolon, submit a
request for administrative hearing ·to the Transportation
Commission of Colorado within 60 days of transmi«al of
notfee or denial or transmittal of the permit for signature.
Submit the request to the Transportation Commission of
Colorado, 4201 East Arl<ansas Avenue, Denver, Colorado
80222-3400. The request s~all include reasons for the
appeal and may indude change•, revision•, or conctition5
that would be acceptable to the permlttee or applicant.
2. Any appeal by the applicant or permiUee of aclion by a
local issuing authority ~hall be filed with the local authority
and be consistent with the appeal procedures of the local
authority.
3. In subrnining the reQuest for administrative healing, the
appellant has the option of including within the appeal a
request for a review by the Department's Internal
administrative review ool'l'\mlttoe pursuant to (Code]
subsection 2.1 0. When such committee review is requested,
processing of the appeal for formal administr~tlve hearing,
2.9(5) and (6), shall be suspended until the appellant notifies
the Commission to proceed with the adl'l'\lnistralive hoartng,
or the appeil~nt submits a request lo \he Commission or the
administrative law judge to withdraw lhe appeal. The two
administrative proces~es, the Internal administrative review
committee, a11d the administrative hearing, may not run
concurrently.
4. Regardless of any communicatfons, meetings.
administrative reviews or negotiations With the Department
or the inlemal admtnislrotive review Committee regarding
revisions or objections to the pennit or a denial, if the
permittee or applicant wishes to appeal the Department's
decision to the Commission for a hearing, the appeal must
be brought to the Commission within 60 days ol transMittal
of notice of denial or transmittal or the p.,mlt.

PERMIT EXPIRATION
1. A penni! shall be considered expired If the access Is not
under construction within one year or the permit issue date
or before the expiration or any authorized e><tension. When
the permittee is unable to commence oonslruction within one
year after the permit Issue dale, the permittee may request a
one year extension fi'om the iss~lng authority. No more than
two one·year extensions may be granted under any
cirournstances. tr the access is not under construction within
three years from date of ls;ue the permit will be considered
expired. Any request for an extension must be in writing and
$Ubmitted to the issuing authority before the permit expires.
The req~est should stale the reasons why the extension Is
necessary, when construction is anticipated, and include ~
copy of page 1 (face of permit) oF lhe access permit.
Extensic>n approvals shall he In writing. The local Issuing
•uthor~y shall obtain the concurrence of the Oepartment
prior to the approval of an eJ<tenslon, and shall notify the
Oepartl'l'\ant of all denied extensions within ten days; Any
person wishing to reestablish an access permit that has
expired may begin again with the application procedures. An
approved Notice to Proceed, automatically renews the
access penmit for the period of the Notice to Proceed.
CONSTRUCTION
1. Construction may not begin until a Notice to Proceed is
approved. (Code subsection 2.41
2. The construclion of the access and its appurtenanl'>'ls
as required by the term• and conditions of the permit shall
be completed at the expense of the penmlttee except as
provided In subseolion 2.14. All materials used in the
C<>nstruction of the access within the highway right-of-way or
on perManent easements, become public pmperty. Any
materials removed from the highway right-of-way will be
disposed of only as directed by the Oepat\1'1'\ent. All fencing,
guard rail, traffic control devices and other equipment and
materials removed to the course of a<:<:e$$ construction shall
be given lo the O..par1ment unless othe<Wise instructed by
tile permit or the Department inspeclor.
3. The peominee shall notify the individual or the offi<.e
specified on the permit or Notice to Proceed at least two
working days prior to any construction within slate highway
rlghl·of-way. Construclion of the ae<>ess shall not proceed
unm both the access permit and tho Notice to Proceed are
issued. rhe access shall be completed in an expeditious and
sare manner and shall be finished within 45 clays from
initiation of construclion within the hi9hway right-of.way. A
con$tfuction tirne exten.sfon not to e:-:ceed 30 working days
may be r~quested from the individual or office specified on
the permit.
4. The Issuing authority and the Department may inspect
the access during construction and upon completion of the
aa:ess to ensure that all te~rn• and cond~ions of lhe permit
. are met. Inspectors are authorized to enforce tile conditions
of the permit during construction and to hafi any activities
within slate right·of·way that do not comply with the
provisions of the permit, !hat conHict wilh concunent

construction of a permitted ac.ces:s, the re1o-catlon, removal or
repair shall bo accomplished by the pem>Jttee wilhout cosl to
the Oep~rtment or lssvln9 authority, and at lhe direction of the
Oepartmenl or utility company. Any damage lo the state
highway or other pUbtlo right-of-way beyond that wtolch is
allowed In the penntt shall be repaired immedlalely. The
permittee: i5 respon$ible for the repair of an~ U1ility damaged in
the course of acc:.e$8 amstrucUon, reconstruction or repair.
6. In the event it becomes necessary to remove any right·of·
way fence, the posts on •iltler side of the access shall be
securely bta.eed with an approved end poS"f before tho !ence is
cut to prevent any slacking of the remaining .fence. All posls
al'\d wire remov1;3d are Oepartmen1 property and shall be turned
over to a representative of the Department.
B. · The permittee shall ensure that a copy of the permit l$

construction lo allow the field inspector to. inspect various

CHANGeS IN ACCESS USE AND PERMIT VIOl.ATIONS
1. It Is the responsibility of \lle p(l)peny owner and permittee
to ensure that the use of the access to the property is not in
violation of the Code, permit terms and conditions or the Aet.
The terms and condiflons of any permit are binding upon all
tl$Signs, succes.sor.r!nwlntere$(, heirs and occupants. If any
significant changes ore made or will be mada in lhe use of the
property which will affect access operalit~n, traffic volume and or
vehiole type, lhe permittee or property owner shall contact the
locatlssuin~ authority or the Dffpartment to determine If a Mw
aceess permit and mOdlncations to the acoess are required,
2. When an access is constructed or tJsed in violation of the
Code, section 43·2·147(5)(c), C.R.S., or the Act applieo, The
Department or Issuing aulhority may summarily suspend an
a.cce$$ permit and immediately order closure of the access

MAINTENANCE

!lepartmenl will keep acces• culverts clean as part of
mainten.anC13 of 1he highWay drainage system. However, the
permittee is responsible for the repair and replacement of any
access·relaled culverts within the oighl-of·way. Within
· inco~poratad areas 1 drainage r~spons!bilities for munlclpaliUes
are dl!!termined by statute and local ordinance. The Department
will maintain lhe roadway Including auxiliary lane• and
shoulders, except In those cases where the a-ecess Installation

i. This access is approved as constructed in accordance with Section 2.6(3) of the Access Code.
2. This permit replaces any and all additional access permits that may be In existence. All other
State Highway access to the property shall be removed.
3. This permitted access is only for the use and purpose stated in the Application and Permit.
This Permit Is issued in accordance with the State Highway Access Code (2 CCA 60i·1), and
· is based in part upon the Information submitted by the Permittee. Any subsequent relocation,
reconstrucllon, or modifications to the access or changes in the traffic volume or traffic nature
using the access shall be requested for by means of a new application. Any changes causing
non"compliance with the Access Code may render this permit void, requiring a new permit.

The following paragraphs are excerpts of the State Highway
Ac...cess Code. These are provided for your convenience but
do not a!leviale compliance wilh all sections of the Access
Code. A copy of the State Highway Access Code is
available from your local issuing authority (local government)
or the Colorado Department of Transportation (Department).
\Mlen this permit was issued, the issuing authority made its
decision based in part on information submitted by the
applicant, on the access category which is assigned to the
highway, what alternative access to other public roads and
s1reels is available, and safety and design standards.
Changes in use or design not approved by lhe permit or the
issuing authority may cause the revocation or suspension of
the permit.
APPEALS
1. Should the permitlee or applicant object to the denial of
a permit application by the Department or object lo any of
the terms or conditions of a permit placed there by the
Department, lhe applicant and pennittee (appellant) have a
right to appeal lhe decision lo lhe [Transpmtalion]
Commission [of Colorado]. To appeal a decision, submit a
request for administrative hearing to the Transportation
Commission of Colorado within 60 days of transmitlal of
notice of denial or transrnitlal of the permit for signature.
Submit lhe request to the Transportation Commission of
Colorado, 4201 East Arkansas Avenue, Denver, Colorado
80222-3400. The request shall include reasons for the
appeal and may indude changes, revisions, or conditions
that would be acceptable to the permittee or applicant.
2. Any appeal by the applicant or permittee of action by a
local issuing authority shall be filed with the local authority
and be consistent with the appeal procedures of the local
authority.
3. In submitting the request for administrative hearing, the
appellant has the option of including within the appeal a
request for a review by the Department's internal
administrative review committee pursuant to [Code]
subsection 2.10. When such commi!tee review is requested,
processing of the appeal for formal administrative hearing,
2.9(5) and (6), shall be suspended until the appellant notifies
the Commission to proceed with the administrdtive hearing,
or the appellant submits a request to the Commission or the
administrative law judge to withdraw the appeal. The two
administrative processes, the internal administrative review
committee, and the administrative hearing, may not nm
concurrently.
4. Regardless of any communications, meetings,
administrative reviews or negotiations with the Oepartinent
or the inlemal administrative review Committee regarding
revisions or objections to the permit or a denial, if lhe
permittee or applicant wishes to appeal the Department's
decision to the Commission for a hearing, the appeal must
be brought to the Commission within 60 days of transmittal
of notice of denial or transmittal of the permit.
PERMIT EXPIRATION
1. A permit shall be considered expired if the access is not
under construction within one year of the permit issue date
or before the expiration of any authorized extension. When
the permittee is unable to commence construction within one
year after the permit issue dale, the pennitlee may request a
one year extension from the issuing authority. No more than
two one-year extensions may be granted under any
circumstances. If the access is not under construction within
three years from date of issue the penmit will be considered
expired. Any request for an extension must be in V.Jriting and
submitted to the issuing authority before the permit expires.
The request should slate the reasons why the extension is
necessary, when construction is anticipated. and include a
copy of page 1 (face of permit) of lhe access permit.
Extension approvals shall be in writing. The local issuing
authority shall obtain the concurrence of the Department
prior to the approval of an extension, and shall notify the
Department of all denied extensions within ten days. Any
person wishing to reestablish an access permit that has
expired may begin again with the application proo~~dores. An
approved Notice to Proceed, automatically renews the
ae<:ess permit for the period of the Notice to Proceed.
CONSTRUCTION
1. Construction may not begin until a Notice to Proceed is
approved. (Code subsection 2.4]
2. The construction of the access and its appurtenances
as required by the terms and conditions of the permit shall
be completed at the expense of the permittee except as
provided in subsection 2.14. All materials used in the
construction of the access within the highway right-of-way or
on permanent easements, become public property. Any
materials removed from the highway right-of-way will be
disposed of only as directed by the Department. All fencing,
guard rail, traffic control devices and other equipment and
materials removed in the course of access construction shall
be given to the Department unless otherwise instructed by
the permit or the Department inspector.
3. The pennittee shall notify the individual or the office
specified on the permit or Notice to Proceed at least two
working days prior to any construction within state highway
right-of-way. Construction of the access shall not proceed
until both the access permit and the Notice to Proceed are
issued. The access shall be completed in an expeditious and
safe manner and shall be finished within 45 days from
initiation of construction within the highway right-of-way. A
construction time extension not to exceed 30 working days
may be requested from the individual or office specified on
the permit.
4. The issuing authority and the Department may inspect
\he access during construction and upon completion of the
access to ensure that all terms and conditions of the permit
are met. Inspectors are authorized to enforce the conditions
of the permit during conslruclion and to halt any activities
within slate right-of-way that do not comply with the
provisions of the permit, that conflict with concurrent
highway construction or maintenance work, that endanger

highway property, natural or cultural resources protected by
law, or the health and safety of workers or the public.
5. Prior to using the access, the permittee is required to
complete the construction according to the terms and conditions
of the pennit. Failure by the permittee to abide by all permit
terms and conditions shall be sufficient cause for the
Oeparlrnent or issuing authority to initiate action to suspend or
revoke the penni! and dose the access. If in the determination
of the Department or issuing authority the failure to comply with
or complete the construction requirements of the permit create
a highway safety hazard, such shall be sufficient cause for the
summary suspension of the permit. If the permittee \Nishes to
use the access prior to completion, arrangements must be
approved by the issuing authority and Department and induded
in the permit. The Department or issuing authority may order a
halt to any unauthorized use of the access pursuant to statutory
and regulatory powers. Reconstruction or improvement of the
access may be required when the permittee has failed to meet
required specifications of design or materials, If any
oonstruction element fails within two years due to improper
construction or material specifications, the permittee shall be
responsible for aU repairs. Failure to make such repairs may
result in suspension of the permit and dosure of the access.
6. The permittee shall provide construction traffic control
devices at a!l times during access construction, in conformance
with the M.U.T.C.D. as required by section 42-4-104, C.R.S., as
amended.
7. A utility permit shall be obtained for any utility work within
highway right-of-way. VVhere necessary to remove, relocate, or
repair a traffic control device or public or private utilities for the
construction of a permitted access, the relocation, removal or
repair shall be accomplished by the pennittee wf\hout cost to
the Department or issuing authority, and at the direction of the
Department or ulih'ty company. Any damage to the state
highway or other public right-of-way beyond that which is
allowed in the permit shaH be repaired immediately. The
permittee is responsible for the repair of any utility damaged in
the course of access construction, reconstruction or repair.
8. In the event it becomes necessary to remove any right-of-
way fence, the posts on either side of the acress shall be
securely braced with an approved end post before the fence is
cut to prevent any slacking of the remaining fence. All posts
and \oVire removed are Department property and shall be turned
over to a representative of the Department.
9. The permittee shall ensure that a copy of the permit is
available for review at the construction site at all times. The
penni! may require the contractor to notify the individual or
office specified on the permit at any specified phases in
construction to allow the field inspector to inspect various
aspects of construction such as concrete forms, subbase, base
course compaction, and materials spedfications. Minor changes
and additions may be ordered by the Department or local
authority field inspector to meet unanticipated site conditions.
10. Each access shall be constructed in a manner that shall not
cause water to enter onto the roadway or shoulder. and shall
not interfere with the existing drainage system on the right-of~
way or any adopted municipal system and drainage plan.
11. The Pennittee is responsible for obtaining any
necessary additional Federal, State and/or City/County permits
or clearances required for construction of the access. Issuance
of this access permit does not constitute verification of the
above required actions by the Permittee.
By accepting the permit. the permlt1ee stipulates and
agrees to fully protect, save, defend, indemniry, and hold
harmless, to the extent allowed by law, the issuing Authority,
and each of the Authority's directors, officers, employees,
agents and representatives, from and against any and all
claims, costs {including but not limited to all reasonable rees
and charges of engineers, architects, attorneys, and other
professionals or expert witnesses and all court or other dispute
resolution costs directly incurred by reason of daims directly
brought against lhe Authorily), losses, damages, pre-or post~
judgment interest, causes of action, suits, or liability of any
nature whatsoever by reason of liability imposed due to
Permittee's failure to obtain, or disregard of, any applicable
federal, state or local environmental permits, approvals,
authorizations, or dearances, or in meeting or complying wilh
any applicable federal, state or local environmental law,
regulation, condition or requirements in connection with any
activities authorized by this Access Permit.
CHANGES IN ACCESS USE AND PERMIT VIOLATIONS
1, It is the responsibility of the property owner and permittee
to ensure that the use of the access to the properly is not in
violation of the Code, permit terms and condilions or the Act.
The terms and conditions of any permit are binding upon all
assigns, successors-in-interest, heirs and occupants. If any
significant changes are made or will be made in the use of the
property which will affect access operation, traffic volume and or
vehicle type, the permittee or property owner shall contact the
local issuing authority or the Department to determine if a new
access permit and modifications to the access are required.
2. When an access is constructed or used in violation of the
Code, section 43-2-147(5)(c), C.R.S., of the Act applies. The
Department or issuing authority may summarily suspend an
access permit and immediately order closure of the access
when its continued use presents an immediate threat to public
health, INelfare or safety. Summary suspension shall comply
with article 4 of title 24, C. R.S.
MAINTENANCE
1. The permittee, his or her heirs, successors~in-interest,
assigns, and occupants of the property serviced by the access
shall be responsible for meeting the terms and conditions of the
permit, the repair and maintenance of the access beyond the
edge of the roadway induding any caHie guard and gate, and
the removal or clearance of snow or ice upon the acress even
though deposited on the access in the course of Department
snow removal operations. Wtlhfn unincorporated areas the
Department will keep access culverts dean as part of
maintenance of the highway drainage system. Ho~Never, the
permittee is responsible for the repair and replacement of any
access-related culverts within the right-of-way. Within
incorporated areas, drainage responsibilities for municipalities
are determined by statute and local ordinance. The Department
will maintain the roadway including auxiliary lanes and
shoulders, except in those cases where the access installation
has failed due to improper access construction and/or failure to
follow permit requirements and specifications in which case the
permittee shall be responsible for such repair. Any significant
repairs such as culvert replacement, resurfacing, or changes in
design or specifications, requires authorization from the
Department.

1. This access is approved as constructed in accordance with Section 2.6(3) of the Access Code.
2. This permil replaces any and all additional access permits that may be in existence. All other
State Highway access to the property shall be removed.
3. This permitted access is only for the use and purpose stated in the Application and Permit.
This Permit is issued in accordance with the State Highway Access Code (2 CCR 601-1), and
is based in part upon the information submitted by the Permittee. Any subsequent relocation,
reconstruction, or modifications to the access or changes in the traffic volume or traffic nature
using the access shall be requested for by means of a new application. Any changes causing
non-compliance with the Access Code may render this permit void, requiring a new permit.
4. The traffic limitations listed on the face of this permit shall be adhered to. The traffic volumes
are listed in Design Hourly Volume (DHV) trips or Average Daily Trips (ADT) where entering
the site and returning counts as two trips.
5. The traffic volume is for 48 ADT.
6. The existing culvert shall be kept free of blockage to maintain proper flow and drainage .

Hazardous Materiai§.1Jl..olid Waste-The'Sofid Wastes Disposal Sites and Facilities Act C.RS. 30-20-100, et al, and Regulations
Pertaining to SoHd Waste Disposal Sites and Facilities (6 CCR 1007 -2). prohibit solid waste disposal without an approved Certificate
of Designation (a landfill permit). The Colorado Hazardous Waste Act C.R.S. 25-15-301 et al, and lhe Colorado Hazardous Waste
Regulations (6 CCR 1007-3) prohibit the transfer, storage or disposal (TSD} of hazardous waste except at permitted TSD sites. There
are no permitted landfills or TSD sites within the State Highway Right of Way. Therefore, all solid or hazardous wastes that might be
generated by the activities or entities entering the Stale Highway Right or Way most be removed from lhe ROW and disposed of at a
permitted facility or designated collection point (e.g., for solid waste, a utility or construction company's own dumpster). If pre~existing
solid waste or hazardous materials contamination (including oil or gasoline contaminated soil, asbestos, chemicals, mine tailings, etc.)
is encountered during the performance of wort<, the permittee shall halt work in the affected area and immediately contact the COOT

Asbestos Containing Materials. Asbestos Contaminated Soil-All work on asbestos containing materials (ACM} must comply with
the applicable requirements of the COPHE Air Pollution Control Division's (APCD) Regulation 8. Disposal of ACM. and work done in
asbestos-contaminated soil, must comply with the CDHPE Hazardous Materials and Waste Management Division's (HMWMD) Solid

MuniciQal SeQarate Storm Sewer System {MS4} Discharge Permit-Discharges from the slorrn sewer systems of larger
municipalities, and from the COOT highway drainage system that hes within those municipalities, are subject to MS4 Permits issued
by the COP HE WQCO. For facilities that lie within the boundaries of a municipality that is subjec' to a MS4 permit, the owner of such
facility should contact the municipality regarding stormwater related clearances that may have been established under that
municipality's MS4 permit. AU discharges to the COOT highway drainage system must comply with the applicable provisions of the
Colorado Water Quality Control Act and lhe Colorado Discharge Permit Regulations, and are subject to inspection by the COOT and

Discharge of Dredge~ or Fill Mi!terial-404 Permits Administered By the U.S. Army: CorQs of Engineers, and Section 401
Water Quality Certifications Issued by the COP HE WQCD-Corps of Engineers 404 Permits are required for the discharge of
dredged or fill materiars into waters of the Uni1ed States, including wet!an~s. There are various types of 404 Permits. including
Nationwide Permits, which are issued for activities with relatively minor impacts. For example, there is a Nationwide Permit for Utillly
Line Activities (NWP #12). However, depending upon the specific circumstances, it is possible that either a "General" or "lndtvidual''

Erosion and Sediment Control Practices~ For activities requiring a Construction Stormwater Permit, erosion control requirements
will be specified through that permit In those situations where a stormwater permit is not required, all reasonable measures should be
taken in order to minimize erosion and sedimentation. In either case, the COOT Stormwater Qualily and Erosion Control Guide (2002)

Disposal of Drilfing Fluids· Drilling fiuids used in operations such as Horizontal Directional Drilling may be classified as ''discharges~
or "solid wastes'', and in general, should be pumped or vacuumed from the construction area, removed from the State Highway Right
of Way, and disposed of a1 pennitted facilities that specifically accept such wastes. Disposal of drilling Ouids inlo stonn dr~ins, storm
sewers, roadside ditches or any other type of man-made or natural waterway is prohibited by Water Qua lily Control and/or Solid
Waste regulations. Small quantities of drilling fluid solids (less I han 1 cubic yard of sollds) may be left on-site after either being
separated from fluids or after infiltration of the water. provided: 1) the drilling fluid consists of only water and bentonite clay, or, if

Per your request I reviewed the areas of ceiling framing over the Phamnacy and Surgery areas.
These areas have security and attic requirements that require a plywood framed rafter ceiling.
These areas have long spans and have been checked for a very light attic load of 20 PSF. This
load is much less than the required load of 125 PSF which the code recommends for storage in
commercial buildings. Therefore these areas shall be treated as attic areas with limited access
& storage abilites. The Owner needs to understand this requirement. If the Owner wants more
storage ablility, the framing sizes would need to increase in depth and cost.
The areas of ceilings with plywood floors shall be framed with 11 7/8 T Jl 210 joists @ 16" o.c.
w/ 3/4" T&G plywood glued & screwed. The joists shall span the shortest direction across the
rooms. The maximum span checked in the Surgery area was 20'-2" for the 20 PSF attic load.
The walls in the area of wood framed ceilings shall be 6" steel studs, 20 GA, @ 16" o.c. with
either plywood or drywall on both faces for bracing. I recommend that the bottom wall tracks be
attached to the cone floor with power actuated fasteners@ 16" o.c.
The typical non load bearing walls in other areas of the building may be framed with typical interior
metal studs (usually 25 GA studs). The top of these stud walls will be braced by a dropped ceiling
to be provided by the wall installer. It should be field verified that the bracing provides adequate
support & perfomnance for the walls in all conditions.

Per your request, we have modeled the Alpine Animal Hospital (AAH) using the Department of Energy's
ComCheck program to demonstrate compliance with 2006 International Energy Conservation Code
(IECC), as adopted by Garfield County Building Department. Although the AAH does not have
vestibules at all entrance doors as required per 2006 IECC 502.4.6, according to Exception 4 of this
section, doors that open directly from a space less than 3000 square feet are exempt from the requirement
to have a vestibule. At the AAH, all entrance doors open directly from spaces less than 3000 square feet.
Approximate areas of spaces with exterior doors are as follows:
Reception/Main Entry (S-138, S-130): 795 ff including hallways to enclosing doors

We have based the energy model on most recent architectural drawings, window schedule, elevations, and
electrical drawings and lighting fixture information provided. According to the attached calculations, the
proposed Alpine Animal Hospital design complies with minimum energy standards for envelope, interior
lighting, exterior lighting, and mechanical systems. The following values were used in our calculations
and these minimum values for conditioned spaces must be attained in actual construction:

The following list provides more detailed descriptions of the requirements in Section 4 of the Mechanical Compliance
Certificate.
Requirements Specific To: FC-1 • Single Zone :
1. The specified heating equipment is covered by Federal minimum efficiency requirements. New equipment of this type can be assumed to
meet or exceed ASHRAE 90.1 Code requirements for equipment efficiency.
Requirements Specific To: FC-1 ·Multiple Zones :
1. The specified heating equipment is covered by Federal minimum efficiency requirements. New equipment of this type can be assumed to
meet or exceed ASHRAE 90.1 Code requirements for equipment efficiency.
2. Each zone of a multiple-zone system must have Its own temperature control device.

4. The specified multip!e~zone system is equipped with single-ductVAV terminals. These terminals must be equipped with dampers, air
valves, or other means to reduce the supply of primary supply air to a minimum prior to reheating.
5. Multiple-zone systems must include controls capable of resetting the supply air temperature by at least 25% of the difference between
the design supply alr temperature and the design room temperature.
Requirements Specific To: Storage Water Heater 1 :
1. Service water heating system design loads for the purpose of sizing systems and equipment must be determined in accordance with
manUfacturers' published sizing guidelines.
2. Service water heating equipment used solely for heating potable water, pool heaters, and hot water storage tanks must meet the
following mlnlumum efficiency: Gas Storage Water Heater efficiency>= 0.52 EF
3. Insulation must be provided for recirculating system piping, including the supply 'and ret~rn piping of a circulating tank type water heater.
4. Temperature controls must be provided that allow for storage temperature adjustment from 120 degrees For lower to a maximum
temperature compatible with the intended use except when the manufacturer's installation instructions specify a higher m!nlmum
thermostat setting to minimize condensation and resulting corrosion. Documentation of the installation instructions must be provided to
be exempted from this requirement.
5. Systems designed to maintain usage temperatures in hot water pipes, such as recirculating hot water systems or heat trace, must
be equipped with automatic time switches or other controls that can be set to switch off the temperature maintenance system during
extended periods when hot water Js not required.
6. When used to mahita!n storage tank water temperature, recirculating pumps must be equipped with controls limiting operation to the start
of the heating cycle to a maximum of 5 minutes after the end of the heating cycle.
Generic Requirements: Must be met by all systems to which the requirement is applicable:
1. Design heating and cooling loads for the building must be determined using procedures in the ASHRAE Handbook of Fundamentals or
an approved equivalent calculation procedure.
2. All equipment and systems must be sized to be no greater than needed to meet calculated loads. A single piece of equipment providing
both heating and cooling must satisfy this provision for one function with the capacity for the other function as small as possible, within
available equipment options.
Exception: The equipment and/or system capacity may be greater than calculated loads for standby purposes. Standby equipment
must be automatically controlh~d to be off when the primary equipment and/or system is operating.

• Exception: Multiple units of the same equipment type whOse combined capacities exceed the calculated load are allowed if they are
provided with controls to sequence operation of the unit~ as the load Increases or decreases.
3. Each heating or cooling system serving a single zone must have Its own temperature control device.
4. Each humidification system must have its own humidity control device.
5. The system or zone control must be a programmable thermostat or other automatic control meeting the following criteria: a) capable
of setting back temperature to 55 degrees F during heating and setting up to 85 degrees F during cooling, b) capable of automatically
setting back or shutting down systems during unoccupied hours using 7 different day schedules, c) have an accessible 2·hour occupant
override, d) have a battery back·up capable of maintaining programmed settings for at least 10 hours without power.
~ Exception: A setback or shutoff control is not required on thermostats that control systems serving areas that operate continuously.
• Exception: A setback or shutoff control is not required on systems with total energy demand of2 kW (6,826 Btu/h) or less.
6. The system must supply outside ventilation air as required by Chapter 4 of the International Mechanical Code. If the ventilation system is
designed to supply outdoor·air quantities exceeding minimum required levels, the system must be capable of reducing outdoor·air flow to
the minimum required levels.
7. Air duels must be insulated to the following levels: a) Supply and return air duels for condilioned air located in uncondilioned spaces
(spaces neither heated nor cooled) must be insulated with a minimum of R-5. Unconditioned spaces Include attlcs 1 crawl spaces,
unheated basements~ and unheated garages. b) Supply and retum air ducts and plenums must be Insulated to a minimum of R-8 when
located outside the building. c) When ducts are located within exterior components (e.g., floors or roofs), minimum R-8 insulation is
required only between the duct and the building exterior.
Exception: Duct Insulation Is not required on ducts located within equipment.
Exception: Duct insulation is not required when the design temperature difference between the interior and exterior of the duct or
plenum does not exceed 15 degrees F.
Exception: Continuously welded and locking-type longitudinal joints and seams on ducts operating at static pressures less than 2
inches w.g. pressure classification.
8. Mechanical fasteners and seals, mastics, or gaskets must be used when connecting ducts to fans and other air distribution equipment,
Including multiple·zone terminal units.
9. All joints, longitudinal and transverse seams, and connections in ductwork must be securely sealed using weldments; mechanical
fasteners with seals, gaskets, or mastics; mesh and mastic sealing systems; or tapes. Tapes and mastics must be listed and labeled in
accordance wHh UL 181A and shall be marked '181A·P' for pressure sensitive tape, '181A-M' for mastic or '181A·H' for heal-sensitive
tape. Tapes and mastics used to seal flexible air ducts and flexible air connectors shall comply with UL 1818 and shall be marked
'181 8-FX' for pressure-sensitive tape or '1818-M' for mastic. Unlisted duct tape Is not permitted as a sealant on any metal ducts.
10. Operation and maintenance documentation must be provided to the owner that Includes at least the following information: a) equipment
capacity (input and output) and required maintenance actions b) equipment operation and maintenance manuals c) HVAC system
control maintenance and calibration information, including wiring diagrams, schematics, and control sequence descriptions; desired
or field-determined set points must be permanently recorded on control drawings, at control devices, or, for digital control systems, in
programming comments d) complete narrative of how each system is intended to operate.
11. Service hot water piping, where required, must be Insulated to 1/2 in. If pipe less than 1.5 ln. nominal diameter. Larger pipe must be
insulated to 1 in .. Pipe insulation will have a conductivity of less than 0.28 Btu.ln/(h-ft2-degrees F).
12. Temperature controlling means must be provided to limit the maximum temperature of water delivered from lavatory faucets in public
facility restrooms to 110 degrees F.

14. Outdoor air supply and exhaust systems must have motorized dampers that automatically shut when the systems or spaces served
are not in use. Dampers must be capable of automatically shutting off during preoccupancy building warm~up, cool-down, and setback,
