When reviewing a plan and it's discovered that required information has not been provided by the applicant, this will result in the delay of the permit issuance and in proceeding with building construction. The owner or contractor shall be required to provide this information before the plan review can proceed. Other plans that are in line for review may be given attention before the new information may be reviewed after it has been provided to the Building Department. Please review this document to determine if you have enough information to design your project and provide adequate information to facilitate a plan review. Also, please consider using a design professional for assistance in your design and a construction professional for construction of your project. Any project with more than ten (10) occupants requires the plans to be sealed by a Colorado Registered Design Professional. To provide for a more understandable plan and in order to determine compliance with the building, plumbing and mechanical codes, applicants are requested to review the following checklist prior to and during design. Plans to be included for a Building Permit must be on draft paper at least 18"x 24'"' and drawn to scale. Plans must include a floor plan, a concrete footing and foundation plan, elevations all sides with decks, balcony steps, hand rails and guard rails, windows and doors, including the finish grade and original grade 1 line. A section showing in detail, from the bottom of the footing to the top of the roof, including re-bar, anchor bo!ts, pressure treated plates, floor joists, wall studs and spacing, insulation, sheeting, house-rap, (which is required), siding or any approved building material. Engineered foundations may be required. Check with the Building Department. A window schedule. A door schedule. A floor framing plan, a roofing framing plan, roof must be designed to withstand fl40 pound per square foot up to 7,000 feet in elevation, a 90 M.P.H. windspeed, wind exposure B or C, and a 36 inch frost depth. All sheets need to be identified by number and indexed. All of the above requirements must be met or your plans will be returned. All plans submitted must be incompliance with the 2003 IBC, IPC, IMC, IFGC, and IFC. Applicants are required to indicate appropriately and to submit completed checklist at time of application for a permit: 1. Is a site plan included that identifies the location of the proposed structure, additions or other buildings, setback easements, and utility easements showing distances to the property lines from each corner of the proposed structure prepared by a licensed surveyor and has the surveyors signature and professional stamp on the drawing? Slopes of 30% or more on properties must be show on site plan. (NOTE: Section 106.2) Any site plan for the placement of any portion of a structure within 50 ft. of a property line and not within a previously surveyed building envelope on a subdivision final plat shall be prepared by a licensed surveyor and have the surveyors signature and professional stamp on the drawing. Any structure to be built within a building envelope of a lot shown on a recorded subdivision plat, shall include a copy of the building envelope as it is shown on the final plat with the proposed structure located within the envelope. The :findings of our observations and recommendations for the foundation design are presented in this report. The services were performed in accordance with our agreement for geotechnical engineering services to Legacy Homes, dated March 9, 2009. We previously observed a foundation excavation at the subject site fur a proposed addition to the existing animal hospital building and reported our findings in a letter dated October 13, 1999, Job No. 199 793. The proposed building will be a tall one story m10tal frame structure with slab on grade floor. A spread footing f'Oundation imposing a maximum soil bearing pressure of 2,500 psfwas assumed .fur des.ign. At the time of our frrst visit to the site, the foundation excavation had been cut in one level about 3 feet below the adjacent ground surface and was about 70% completed. We observed the completed. excavation on March 16, 2009. The soils exposed in the bottom ofthe excavation consisted of silty sandy gravel with cobbles and small boulders. No fi·ee water was encountered in the excavation and the soils were slightly moist to moist. The topsoil had been removed within the floor slab area and up to 12 inches of the on-s.ite granular soil had been placed and compacted. The soil was too rocky for compaction testing and we observed proof rolling of the area. The proof roll was perforro.ed using a I 0-wheel dump truck with a full load of% aggregate base course material. The area that was proof rolled appeared to be firm and stable under the applied load. Considering the conditions exposed in the excavation and the nature of the proposed construction, spread fuotings placed on the undisturbed natural soil designed for an allowable soil bearing pressure of2,500 psf can be used for support ofthe proposed bt1ilding. Footings should be a minimum width of 16 inches for continuous walls and 2 feet for columns. Loose and disturbed soils in footing areas should be removed and the bearing level extended down to the undisturbed natural soils. Exterior footings should be provided with adequate soil cover above their bearing elevations fur frost protection. Structural.fill placed within floor slab areas can consist ofthe on-site granular soils, devoid of oversized rock, ~ompacted to at least 95% of standard Proctor density at a moistnre content near optimum. Backfill placed around the structure should be compacted and the surface graded to prevent ponding within at least 1 0 feet of the building. The recommendations submitted in this letter are based on our observation of the soils exposed within the fuundation excavation and do not include subsurfuce exploration to evaluate the subsurface conditions within the loaded depth of foundation influence. This study is based on the assumption that soils beneath the footings have equal or better support than those exposed. The risk of fuundation movement may be greater than indicated in this report because of possible variations in the subsurfuce ~ond.itions. In. order to reveal the nature and extent of variations in the subsurface conditions below the excavation, drilllng would be required. As we discussed, there are three pertinent sections of the 2003 IBC: 402.11 Security Grilles and Doors #3: Horizontal sliding or vertical security grilles or doors that are part of a required means of egress shall conform to the following: 3. The doors or grilles shall be openable from within without the use of any special knowledge or effort where the space is occupied. 1008.1.2 Door Swing Exception #1: Egress doors shall be side hinged swinging. Exceptions: 1. Private garages, office areas, factory and storage areas with an occupant load of 10 or less. 1008.1.3.5 Security Grilles: In Groups B, F,M and S, horizontal sliding or vertical security grilles are permitted at the main exit and shall be openable from the inside without the use of a key or special knowledge or effort during periods that the space is occupied. The grilles shall remain secured in the full-open position during the period of occupancy by the general public. Where two or more means of egress are required, not more than one-half the exits or exit access doorways shall be equipped with horizontal sliding or vertical security grilles. 1 Since the occupant load in the pharmacy is less than 10 and we are not required to have more than one egress door from that space, then the security grilles would be permitted on the condition that they are openable from within the room at all times. Changes in use or design not approved by lhe permit or the issuing authority may cause the revocation or suspension of the permit. APPEALS 1. Should the permitlee or applicant object to the denial of a permit application by the Department or object lo any of the terms or conditions of a permit placed there by the Department, lhe applicant and pennittee (appellant) have a right to appeal lhe decision lo lhe [Transpmtalion] Commission [of Colorado]. To appeal a decision, submit a request for administrative hearing to the Transportation Commission of Colorado within 60 days of transmitlal of notice of denial or transrnitlal of the permit for signature. Submit lhe request to the Transportation Commission of Colorado, 4201 East Arkansas Avenue, Denver, Colorado 80222-3400. The request shall include reasons for the appeal and may indude changes, revisions, or conditions that would be acceptable to the permittee or applicant. 2. Any appeal by the applicant or permittee of action by a local issuing authority shall be filed with the local authority and be consistent with the appeal procedures of the local authority. 3. In submitting the request for administrative hearing, the appellant has the option of including within the appeal a request for a review by the Department's internal administrative review committee pursuant to [Code] subsection 2.10. When such commi!tee review is requested, processing of the appeal for formal administrative hearing, 2.9(5) and (6), shall be suspended until the appellant notifies the Commission to proceed with the administrdtive hearing, or the appellant submits a request to the Commission or the administrative law judge to withdraw the appeal. The two administrative processes, the internal administrative review committee, and the administrative hearing, may not nm concurrently. 4. Regardless of any communications, meetings, administrative reviews or negotiations with the Oepartinent or the inlemal administrative review Committee regarding revisions or objections to the permit or a denial, if lhe permittee or applicant wishes to appeal the Department's decision to the Commission for a hearing, the appeal must be brought to the Commission within 60 days of transmittal of notice of denial or transmittal of the permit. PERMIT EXPIRATION 1. A permit shall be considered expired if the access is not under construction within one year of the permit issue date or before the expiration of any authorized extension. When the permittee is unable to commence construction within one year after the permit issue dale, the pennitlee may request a one year extension from the issuing authority. No more than two one-year extensions may be granted under any circumstances. If the access is not under construction within three years from date of issue the penmit will be considered expired. Any request for an extension must be in V.Jriting and submitted to the issuing authority before the permit expires. The request should slate the reasons why the extension is necessary, when construction is anticipated. and include a copy of page 1 (face of permit) of lhe access permit. Extension approvals shall be in writing. The local issuing authority shall obtain the concurrence of the Department prior to the approval of an extension, and shall notify the Department of all denied extensions within ten days. Any person wishing to reestablish an access permit that has expired may begin again with the application proo~~dores. An approved Notice to Proceed, automatically renews the ae<:ess permit for the period of the Notice to Proceed. CONSTRUCTION 1. Construction may not begin until a Notice to Proceed is approved. (Code subsection 2.4] 2. The construction of the access and its appurtenances as required by the terms and conditions of the permit shall be completed at the expense of the permittee except as provided in subsection 2.14. All materials used in the construction of the access within the highway right-of-way or on permanent easements, become public property. Any materials removed from the highway right-of-way will be disposed of only as directed by the Department. All fencing, guard rail, traffic control devices and other equipment and materials removed in the course of access construction shall be given to the Department unless otherwise instructed by the permit or the Department inspector. 3. The pennittee shall notify the individual or the office specified on the permit or Notice to Proceed at least two working days prior to any construction within state highway right-of-way. Construction of the access shall not proceed until both the access permit and the Notice to Proceed are issued. The access shall be completed in an expeditious and safe manner and shall be finished within 45 days from initiation of construction within the highway right-of-way. A construction time extension not to exceed 30 working days may be requested from the individual or office specified on the permit. 4. The issuing authority and the Department may inspect \he access during construction and upon completion of the access to ensure that all terms and conditions of the permit are met. Inspectors are authorized to enforce the conditions of the permit during conslruclion and to halt any activities within slate right-of-way that do not comply with the provisions of the permit, that conflict with concurrent highway construction or maintenance work, that endanger State Highway Access Permit Form 101, Page 3 highway property, natural or cultural resources protected by law, or the health and safety of workers or the public. 5. Prior to using the access, the permittee is required to complete the construction according to the terms and conditions of the pennit. Failure by the permittee to abide by all permit terms and conditions shall be sufficient cause for the Oeparlrnent or issuing authority to initiate action to suspend or revoke the penni! and dose the access. If in the determination of the Department or issuing authority the failure to comply with or complete the construction requirements of the permit create a highway safety hazard, such shall be sufficient cause for the summary suspension of the permit. If the permittee \Nishes to use the access prior to completion, arrangements must be approved by the issuing authority and Department and induded in the permit. The Department or issuing authority may order a halt to any unauthorized use of the access pursuant to statutory and regulatory powers. Reconstruction or improvement of the access may be required when the permittee has failed to meet required specifications of design or materials, If any oonstruction element fails within two years due to improper construction or material specifications, the permittee shall be responsible for aU repairs. Failure to make such repairs may result in suspension of the permit and dosure of the access. 6. The permittee shall provide construction traffic control devices at a!l times during access construction, in conformance with the M.U.T.C.D. as required by section 42-4-104, C.R.S., as amended. 7. A utility permit shall be obtained for any utility work within highway right-of-way. VVhere necessary to remove, relocate, or repair a traffic control device or public or private utilities for the construction of a permitted access, the relocation, removal or repair shall be accomplished by the pennittee wf\hout cost to the Department or issuing authority, and at the direction of the Department or ulih'ty company. Any damage to the state highway or other public right-of-way beyond that which is allowed in the permit shaH be repaired immediately. The permittee is responsible for the repair of any utility damaged in the course of access construction, reconstruction or repair. 8. In the event it becomes necessary to remove any right-of- way fence, the posts on either side of the acress shall be securely braced with an approved end post before the fence is cut to prevent any slacking of the remaining fence. All posts and \oVire removed are Department property and shall be turned over to a representative of the Department. 9. The permittee shall ensure that a copy of the permit is available for review at the construction site at all times. The penni! may require the contractor to notify the individual or office specified on the permit at any specified phases in construction to allow the field inspector to inspect various aspects of construction such as concrete forms, subbase, base course compaction, and materials spedfications. Minor changes and additions may be ordered by the Department or local authority field inspector to meet unanticipated site conditions. 10. Each access shall be constructed in a manner that shall not cause water to enter onto the roadway or shoulder. and shall not interfere with the existing drainage system on the right-of~ way or any adopted municipal system and drainage plan. 11. The Pennittee is responsible for obtaining any necessary additional Federal, State and/or City/County permits or clearances required for construction of the access. Issuance of this access permit does not constitute verification of the above required actions by the Permittee. By accepting the permit. the permlt1ee stipulates and agrees to fully protect, save, defend, indemniry, and hold harmless, to the extent allowed by law, the issuing Authority, and each of the Authority's directors, officers, employees, agents and representatives, from and against any and all claims, costs {including but not limited to all reasonable rees and charges of engineers, architects, attorneys, and other professionals or expert witnesses and all court or other dispute resolution costs directly incurred by reason of daims directly brought against lhe Authorily), losses, damages, pre-or post~ judgment interest, causes of action, suits, or liability of any nature whatsoever by reason of liability imposed due to Permittee's failure to obtain, or disregard of, any applicable federal, state or local environmental permits, approvals, authorizations, or dearances, or in meeting or complying wilh any applicable federal, state or local environmental law, regulation, condition or requirements in connection with any activities authorized by this Access Permit. CHANGES IN ACCESS USE AND PERMIT VIOLATIONS 1, It is the responsibility of the property owner and permittee to ensure that the use of the access to the properly is not in violation of the Code, permit terms and condilions or the Act. The terms and conditions of any permit are binding upon all assigns, successors-in-interest, heirs and occupants. If any significant changes are made or will be made in the use of the property which will affect access operation, traffic volume and or vehicle type, the permittee or property owner shall contact the local issuing authority or the Department to determine if a new access permit and modifications to the access are required. 2. When an access is constructed or used in violation of the Code, section 43-2-147(5)(c), C.R.S., of the Act applies. The Department or issuing authority may summarily suspend an access permit and immediately order closure of the access when its continued use presents an immediate threat to public health, INelfare or safety. Summary suspension shall comply with article 4 of title 24, C. R.S. MAINTENANCE 1. The permittee, his or her heirs, successors~in-interest, assigns, and occupants of the property serviced by the access shall be responsible for meeting the terms and conditions of the permit, the repair and maintenance of the access beyond the edge of the roadway induding any caHie guard and gate, and the removal or clearance of snow or ice upon the acress even though deposited on the access in the course of Department snow removal operations. Wtlhfn unincorporated areas the Department will keep access culverts dean as part of maintenance of the highway drainage system. Ho~Never, the permittee is responsible for the repair and replacement of any access-related culverts within the right-of-way. Within incorporated areas, drainage responsibilities for municipalities are determined by statute and local ordinance. The Department will maintain the roadway including auxiliary lanes and shoulders, except in those cases where the access installation has failed due to improper access construction and/or failure to follow permit requirements and specifications in which case the permittee shall be responsible for such repair. Any significant repairs such as culvert replacement, resurfacing, or changes in design or specifications, requires authorization from the Department. These areas have security and attic requirements that require a plywood framed rafter ceiling. These areas have long spans and have been checked for a very light attic load of 20 PSF. This load is much less than the required load of 125 PSF which the code recommends for storage in commercial buildings. Therefore these areas shall be treated as attic areas with limited access & storage abilites. The Owner needs to understand this requirement. If the Owner wants more storage ablility, the framing sizes would need to increase in depth and cost. The areas of ceilings with plywood floors shall be framed with 11 7/8 T Jl 210 joists @ 16" o.c. w/ 3/4" T&G plywood glued & screwed. The joists shall span the shortest direction across the rooms. The maximum span checked in the Surgery area was 20'-2" for the 20 PSF attic load. The walls in the area of wood framed ceilings shall be 6" steel studs, 20 GA, @ 16" o.c. with either plywood or drywall on both faces for bracing. I recommend that the bottom wall tracks be attached to the cone floor with power actuated fasteners@ 16" o.c. The typical non load bearing walls in other areas of the building may be framed with typical interior metal studs (usually 25 GA studs). The top of these stud walls will be braced by a dropped ceiling to be provided by the wall installer.
