A final Electrical Inspection from the Colorado State Electrical Inspector; 2, Permanent address assigned by Garfield County Building Department posted where readily visible from access road; 3. Your request has been reviewed for the Board of Examiners of Water Well Construction and Pump Installation Contractors. The request is specifically for approval of the planned infiltration gallery well as required by Rule 10.4.8. Pursuant to Rules 10.4.8 and 18, and the authority granted by the Board, the construction plans are approved subject to the following conditions: 1. The infiltration gallery is constructed in accordance with the plan submitted and revisions required by the conditions of the approval. 2. Backfill above the gravel bed must be compacted to a density equal to or greater than the natural soil material. 3. It is recommended that an impermeable material be placed over the gravel bed to prevent the entrance of contaminants. 4. The ground surface must be properly graded to provide positive drainage away from the well head. 5. The well must be constructed in compliance with the disinfection standards provided in Rules 6.9 and 17. 6. The well must be constructed by a licensed water well contractor or by the owner with equipment both owned and operated by the owner. If the well is to be constructed by a licensed water well contractor, the Board required that the contractor be identified prior to construction and that a standard Well Construction and Test Report must be submitted to document the well construction. Approval of this request does not relieve the owner of potential responsibility or liability in the event contamination of the water source results from construction, nor does the grantor assume any responsibility or liability should contamination occur. Adequate and complete information will prevent delays in the plan review process. Reviewing a plan and the discovery that required information has not been provided by the applicant may result in the delay of the permit issuance and in proceeding with building construction. The owner or contractor may be required to provide this information before the plan review may proceed. This causes delays because other plans that are in line for review may be given attention before the new information may be reviewed after it has been provided to the Building Department. Please review this document to determine if you have enough information to design your project and provide adequate information to facilitate a plan review. If you do not, it may be helpful to obtain a book titled "Dwelling Construction under the Uniform Building rode". This book is available to you through this department at our cost. Also, please consider using a design professional for assistance in your design and a construction professional for construction of your project. To provide for a more understandable plan in order to determine compliance with the building, plumbing and mechanical codes, applicants are requested to review the following check list prior to and during design.
