I. BACKGROUND
The Applicant was approved for a Certificate of Designation by the Garfield County Board
of County Commissioners for the South Canyon Landfill on a 31.9 acre portion of a
2,518.87 acre parcel located west of the City of Glenwood Springs. The landfill has been
in operation since 1956. This approval is memorialized by a Certificate of Designation
signed on December 15, 1997 and amended on March 9, 1999.The facility and property
are owned by the City of Glenwood Springs. Access to the facility is off County Road
134.
II. GENERAL APPLICATION DESCRIPTION
The Applicant is requesting a correction to the legal description for the approved
Certificate of Designation. The Applicant has provided the following explanation for the
request to correct the legal description:

Project Purpose/Statement of Need: This project description addresses the previously
approved, erroneously defined Certificate of Designation (CD) boundary and requests approval
of a corrected CD boundary for the South Canyon Landfill (SCLF) owned and operated by the
City of Glenwood Springs, Colorado (Glenwood). The boundary change is required so filling
can continue in the permitted disposal filling area outside of the erroneously placed CD
boundary.
Background: The South Canyon Landfill (SCLF) is located approximately 5 miles west of
Glenwood Springs, Colorado, approximately one mile south of 1-70, along the west side of South
Canyon Road and has been operational since 1956. The only access to the site is via the landfill
entrance road off of South Canyon Creek Road which enters the site at its southeast comer. With
regard to the facility's legal description, the site is contained within approximately 3,000 acres of
property owned by Glenwood and the solid waste activities take place on portions of Sections 2, 3,
10, and 11, Township 6 South, Range 90 West of the 6th Principal Meridian in Garfield County,
Colorado. The filling area of the landfill is approximately 40.9 acres and will not change as a result
of this boundary change request. In fact, there arc no changes requested for the solid waste
operations within the revised CD boundary at this time.
The facility is permitted and operates in accordance with the State of Colorado Solid Waste
Regulations. The CD was issued in March 9, 1999.
The City owns approximately 3,000 acres, of which, approximately 193 acres have been zoned
by Garfield County for waste disposal and composting operations. South Canyon currently
consists of approximately 31.9 acres for Subtitle D permitted waste disposal and approximately 9
acres of historical waste. Approximately 23.1 acres of the approved Subtitle D landfill filling
area have been developed and the remaining 8.8 acres are outside the CD boundary. The change
in the CD boundary will not change these acreages, but will allow Glenwood to construct
previously approved fill acreages.
The filling area (approximately 31.9 acres of approved Subtitle 1) fill and 9 acres of historic 1111) is
located within a much larger property (approximately 3,000 acres) owned by Glenwood and, as
such; the fill limit is not adjacent to any other land owners in the area. Additionally, since the
submittal of the original EDOP, Glenwood acquired properties adjacent to the north side of the
permitted landfill from the U.S. Bureau of Land Management so any adjacent property owners
are far from the filling limit and will not be impacted by the CD boundary change.
The review and approval of this correction is governed by Section 30-20-103 of the State
Statutes. This Section requires that an application be tendered to the governing body
having jurisdiction over the area. The Statutes lay out the process for notification as well
as review standards, which will be considered within this Report.

The parcel is currently developed with the South Canyon Landfill. The landfill area is
approximately 40.9 acres within a larger 2500+ acre parcel owned by the City of
Glenwood Springs.
III. PUBLIC COMMENTS AND REFFERAL AGENCY COMMENTS
Public Notice was provided for the Board of County Commissioner's public hearing in
accordance with State Statute. No public comments have been received on this
application.

The Division is very interested in, and in support of, the requested
amendment to the CD area legal description. It is typical that the CD
boundary would encompass the entire approved landfill footprint. This isn't
true at South Canyon for a variety of reasons but the City has operated in
compliance (with the CD) by remaining inside the CD boundary. However
many operational problems have resulted because of this boundaty
limitation. These have included re -designing the landfill along the south
edge to provide additional waste airspace by expanding both laterally and
vertically in the past 4 years. Another expansion has recently been
proposed to the Division. With the relatively recent purchase of the Bureau
of Land Management land to the north by the City, a CD boundaty can be
approved that does encompass the already approved landfill footprint,
covers only lands owned by the City and meets the intention of the currently
approved Design and Operations Plan.
The Division strongly supports the requested boundary change that would
add approximately 40 acres. This request does not represent a landfill
expansion or re -design but rather a correction, or maybe update, to the
official CD boundary. County approval of this request will give the City more
options in the near term with regards to landfill operations within the design
of the currently approved permit.
The Application was also referred to the following agencies and departments, however,
no comments were received: Garfield County Public Health, Garfield County
Environmental Health, Bureau of Land Management, and the City of Glenwood Springs.
IV. STAFF COMMENTS AND ANALYSIS
In accordance with State Statute, in considering an application for a proposed solid
wastes disposal site and facility, the governing body having jurisdiction shall take into
account:
(a) The effect that the solid wastes disposal site and facility will have on the surrounding
property, taking into consideration the types of processing to be used, surrounding
property uses and values, and wind and climatic conditions;
The South Canyon Landfill has been in operation since 1956 while a formal
Certificate of Designation from the Garfield County Board of County
Commissioners was issued and recognized in 1997. The City of Glenwood
Springs has substantial land holdings in the area surrounding the Landfill of
approximately 2500 acres. In addition, most adjacent property is owned by
the Bureau of Land Management (BLM). The nearest property that is not
owned by the City of Glenwood Springs or the BLM is a property
approximately 0.75 miles to the east of the Landfill area. In addition, the fill

area to be utilized was contemplated in the original Certificate of
Designation, however was inadvertently excluded from the legal description
when the Certificate boundary was created. To this end, while this
correction will allow the Landfill to continue to expand, it will not be
expanding beyond the limits that were originally mapped and contemplated.
And as a result, no additional impacts to surrounding property is expected
above and beyond the original approval.
(b) The convenience and accessibility of the solid wastes disposal site and facility to
potential users;
The access to the property and the facility will remain unchanged from what
exists today. In addition, no change in traffic or road impacts are expected
as a result of this correction.
(c) The ability of the applicant to comply with the health standards and operating
procedures provided for in this part 1 and such rules and regulations as may be
prescribed by the department;
No changes to the operating procedures or compliance with health
standards are anticipated as a result of this correction. The application was
referred to the Colorado Department of Public Health and Environment
(CDPHE) (See Exhibit 3), which states that "The Division strongly supports
the requested boundary change that would add approximately 40 acres.
This request does not represent a landfill expansion or re -design but rather
a correction, or maybe update, to the official CD boundary." To this end, the
Landfill will continue to comply with the applicable health standards and
operating procedures.
(d) Recommendations by county, district, or municipal public health agencies.
The application was referred to Garfield County Public Health, however no
comments were received. As a result, no recommendations from Garfield
County Public Health have been considered with this application.
In regards to compliance with the LUDC and Comprehensive Plan, since the facility has
been in operation since 1956, it has been in operation since before the County had
zoning or a Comprehensive Plan. As the Garfield County Board of County
Commissioners issued a Certificate of Designation for this facility in 1997 and then
amended that Certificate in 1999, the Board has recognized the facility and reviewed its
operations in relation to the Zoning Resolution and Comprehensive Plan in place at that
time. As this request is for a correction to that existing approval and not for an expansion
or other alteration, it is Staff's opinion that the existing Certificate of Designation and the
review conducted at that time regarding zoning and Comprehensive Plan compliance
remains intact.

APPROVAL
SUGGESTED FINDINGS
1. That proper public notice was provided as required for the hearing before the
Board of County Commissioners.
2. The hearing before the Board of County Commissioners was extensive and
complete, that all pertinent facts, matters and issues were submitted and that
all interested parties were heard at that meeting.
3. That for the above stated and other reasons the proposed amendment to the
approved Certificate of Designation is in the best interest of the health, safety,
convenience, order, prosperity and welfare of the citizens of Garfield County.
4. That the application is in general conformance with the 2030 Comprehensive
Plan, as amended.
5. That the application is in general conformance with the Garfield County Land
Use and Development Code of 2013, as amended.
6. That the application has adequately met the requirements of the Section 30-
20-103 of the Colorado Revised Statutes.
Conditions of Approval
1. The legal description of the Certificate of Designation boundary as approved
by the Garfield County Board of County Commissioners on December 15,
1997 and amended on March 9, 1999 shall be amended to the following
description:

the Landfill access road
The SWI/4 SW1/4; the SEI/4 SW1/4; the S1/2 SW1/4SE1/4; the S1/2SE1/4 SE1/4; the
SW1/4NW1/4SE1/4, the NW1/4SW1/4SE1/4, the W1/2NE1/4SW1/4SE1/4, the
SE1/4NE1/4SW1/4, and the E1/2SW1/4NE1/4SW1/4 all in Section 3
N1/2NE1/4NW1/4,the N1/2NW1/4NE1/4,the N1/2NE1/4NE1/4 all in Section 10
That portion of the N1/2 NW1/4 NW1/4 in Section '11, lying southerly of the northerly

The Hazardous Materials and Waste Management Division of the Colorado Department of Public Health and Environment
Solid Waste and Materials Management Program (Division) received the Referral form dated June 30, 2016 pertaining to
the request by the City of Glenwood Springs to amend a previously approved Certificate of Designation (CD) for the South
Canyon Landfill. The amendment would administratively correct the legal description in the CD to be consistent with the
previously approved landfill footprint as presented in the Facility Design and Operation Plan (1994).
The Division is very interested in, and in support of, the requested amendment to the CD area legal description. It is
typical that the CD boundary would encompass the entire approved landfill footprint. This isn't true at South Canyon for a
variety of reasons but the City has operated in compliance (with the CD) by remaining inside the CD boundary. However
many operational problems have resulted because of this boundary limitation. These have included re -designing the

American Environmental Consulting, LLC (AEC), on behalf of the City of Glenwood Springs
(City), is providing this construction -level design plan for the previously permitted and approved
fill area to the north and east of the existing fill at the South Canyon Solid Waste Disposal Site
(SCLF) located near the City of Glenwood Springs, Colorado. As capacity is quickly dwindling,
construction is expected to begin as soon as possible. Initially, the Phase 4A area, approximately
0.47 acres to the north of the current fill area, will be constructed. A construction report will be
submitted for approval to begin disposal. At the same time as the construction report is prepared,
submitted, and reviewed for approval, construction will begin in the 1.38 -acre Phase 4B area (4B
design drawings to follow). This will allow a sufficient capacity for design and construction
activities to take place on the remainder of the Phase 4C area and a larger permit expansion to be
prepared and submitted. No filling can begin until a Certificate of Designation (CD) boundary
amendment is approved by Garfield County. The CD boundary change request will be heard by
Garfield County in a hearing on August 8, 2016.
Background
The SCLF plan was originally prepared, dated September 30, 1994, and submitted to Garfield
County by Steffen, Robertson, and Kirsten (SRK). The plan was modified in a document dated
December 26, 1996 and submitted by KRW. The KRW addendum includes the Construction
Quality Assurance Plan (CQAP) that guides construction of the one -foot of recompacted soil
liner in Phases 4A, 4B, and 4C.
South Canyon currently consists of five phases, Phase 1 through Phase 5. The CD boundary was
incorrectly identified in those early documents, which limits the permitted phase development
since portions of the permitted landfill are outside the boundaries of the CD. The City has

submitted a request to amend the CD boundary to include all the originally permitted areas of the
landfill. That request will be heard by the Garfield County Commissioners on August 8, 2016.
Assuming that boundary is corrected, the SCLF can be filled as permitted.
Phase 4 Construction
Construction activities can begin prior to the approval of the amended CD boundary; however,
no filling can take place in the areas outside of the CD boundary until the boundary amendment
is approved.
The permit requires that the base of the landfill be constructed with one -foot of recompacted soil.
The originally outlined base (floor) of the landfill has changed since the plan was first approved.
The current base is larger as a result of soil excavation activities conducted since 1994. The
intent of the plan to place a recompacted soil liner on the base of the landfill will not change and
the base of the landfill will continue to incorporate this liner. A survey was conducted to define
the base of the landfill by surveying points at the base of the existing fill and at the base of slope
of the existing ground surface north and east of the fill. The plates (1 through 4) show the extent
of the landfill base in Phase 4A and the permitted fill limit.
Construction in the permitted Phase 4 area will take place sequentially in modules 4A, 4B, and
4C in order to quickly develop landfill capacity and continue uninterrupted disposal. The entire
permitted remaining unused filling area includes approximately 11.23 acres with approximately
5.49 acres being on the base (floor) of the landfill and the remaining area including the sidewalls
of the fill area. The approved disposal plan includes placing a one -foot thick compacted soil
barrier on the base of the landfill prior to beginning filling.
The initial plans include constructing approximately 1.85 acres of the base of the landfill by
installing the permitted one -foot thick liner over two areas, Phases 4A (0.47 acres, Plates 3 and 4
attached) and 4B (1.38 acres, Plates 5 and 6, to follow). The construction will take place in two
consecutive events in order to ensure capacity for continued filling at the landfill. The initial
construction, Phase 4A, will take place on the landfill base north of the existing fill area (-0.47
acres) in a portion of the landfill property that was once owned by the U.S. Bureau of Land
Management (BLM). The City acquired this property in 2013 after the refuse filled on this
portion of the landfill was removed. Once liner over this area is installed, construction will
immediately move to the next area, Phase 4B, which is a 1.38 -acre area east of the existing fill.
During construction of Phase 4B, the certification report for the Phase 4A area will be completed
and submitted for review and approval.
It should be noted that the intent is construct one foot of recompacted clay on the floor area of
the remaining, permitted landfill. It is likely that some of the subgrade and top of liner points
may have to be field fit against the toes of the fill area and toes of the natural ground slopes.
Therefore, some of the acreages may change and there may be additional survey points added or
survey point moved slightly. These will be documented within the liner construction area to
ensure a minimum of one -foot of clay is placed over the subgrade floor.

Construction of the base floor areas will include the following:
1. Preparation of the subgrade including surveying the floor and field fitting survey points as
necessary.
2. Placement of one foot of recompacted soil to a permeability of 1 x 10"7 cm/sec or less
3. Installation of a termination berm along the downgradient limit of each constructed area
4. Installation of a temporary leachate collection area and conveyance pipe in Phase 4B that will
tie into the existing leachate collection pipe (detail Plate 7 attached).
Construction will be completed according to the attached Construction Quality Assurance Plan
(CQAP). The CQAP includes preconstruction soil testing and construction soil testing. The
plan does not include the preparation of a test pad since that was completed during the Phase 5
construction and materials from the same borrow area will be used.
The temporary leachate conveyance pipe will be installed following completion of the
recompacted soil liner placement in Phase 4B. The pipe will be corrugated or smooth high
density polyethylene (HDPE) pipe that will extend from a low point inside Phase 4B to tie into
the existing leachate pipe located near the manhole. It will tie in before the valve so the valve
can continue to be used to segregate and remove clean runoff and leachate. The pipe will be
placed in a trench of a minimum of one -foot deep and covered with lightly compacted soil for
protection. A termination berm will be constructed on the edge of the liner as shown in the
details (Plate 7). Care will be taken to lightly compact the berm in the vicinity of the leachate
pipe. Inside the berm, a four -foot section of the leachate pipe will be perforated and wrapped
with a geotextile to facilitate leachate flow from the Phase 4A and 4B areas. Upon completion of
Phase 4C, the termination berm and pipe will be removed.
Phase 4C has not been designed as yet since it will require the construction of a leachate sump
that will tie into the existing leachate removal system. Once it is designed, the plan will be
submitted and construction will begin.

1.0 INTRODUCTION
This Construction Quality Assurance (CQA) Plan outlines the documentation activities
associated with the construction of the one -foot thick recompacted soil liner over Phases 4A, 4B,
and 4C (Phase 4) at the South Canyon Landfill (SCLF) which includes a total of 5.49 -acres. The
phases will be constructed at the discretion of the SCLF as capacity needs arise. Initially, Phase
4A (0.47 acres) and Phase 4B (1.38 acres) are expected to be constructed sequentially.
The activities outlined in this document will serve as the basis for the preparation of a
construction certification report(s) documenting the work performed as part of the Phase 4
construction, which include the following:
• Preparation of the subgrade including excavation and placement of general fill in areas.
• Installation of a 1 -foot thick cohesive soil liner.
• Installation of a leachate collection pipe for Phase 4 tying into the existing leachate
conveyance system.
• Construction of a temporary termination berm.
The design and construction of Phase 4 will be completed according to the SCLF-approved
design plan. The SCLF plan was originally prepared, dated September 30, 1994, and submitted
to Garfield County by Steffen, Robertson, and Kirsten (SRK). The plan was modified in a

2.0 GENERAL CONDITIONS
2.1 Responsibility and Authority
The City of Glenwood Springs will be responsible for the implementation of this CQA Plan. The
following is a list of responsible personnel:
Owner
The Owner (which can also be a representative of the Owner) shall be responsible for
coordination between the Owner, the construction crew, and the third -party CQA Engineer. The
Owner shall delegate authority, and correspondingly, shall be responsible to see that the CQA
Plan is followed.
CQA Engineer
A professional engineer licensed to practice in Colorado shall be retained by the Owner to
provide on-site construction oversight, quality assurance testing, and a final report demonstrating
that the requirements of this CQA Plan are met. In addition, the CQA Engineer or his/her
designee shall coordinate with the contractor(s) and/or installer(s) and their personnel for the
purposes of sharing information. Should it become apparent to the CQA Engineer or his/ her
designee that construction quality is substandard, the CQA Engineer shall inform the Owner's
Representative of the apparent deficiencies such that adjustments can be made. The CQA
Engineer must be employed by an organization that operates independently of the landfill
contract operator, construction contractor, Owner, and permit holder. The CQA Engineer will be
responsible for certifying that construction was completed in accordance with the approved
engineering design plans and specifications of the construction permit
CQA Monitor
If the CQA Engineer cannot serve to provide on-site inspection of all liner and/or cover
construction activities and reporting, the CQA Engineer shall designate a CQA Monitor to
perform those duties. The CQA Monitor shall be an individual that represents the CQA Engineer
and provides on-site construction oversight, quality assurance testing, and general observance
and documentation of construction. The CQA Monitor will document on-site construction
activities on a Daily Field Activities Report. An example of this report is included in Appendix
A.
2.2 Construction Meetings
Under all circumstances, the Colorado Department of Public Health and Environment
(CDPHE) will be given seven calendar days advance notification prior to the initiation of landfill
construction projects.

2.2.1 Preconstruction Meeting
A meeting involving the Owner/operator, CQA personnel, and the contractor(s) shall take place
prior to the start of construction. This meeting should include discussion of:
• Each party's responsibilities
• Lines or means of communication
• Procedures for changes or problems
• QA/QC procedures and requirements
• Level of the CDPHE's involvement
• Safety Issues on during construction
• Other issues as they pertain to the construction project
2.2.2 Daily Progress Meetings
Regularly scheduled, daily meetings between CQA personnel and the contractor(s) shall take
place to review and discuss such topics as the following:
• Previous work
• Future work
• Construction problems
• Specification or CQAP Revisions
• Schedule revisions
• Other issues that require attention
2.2.3 Other Meetings
Unscheduled meetings shall take place as required to address issues such as construction
progress and changed conditions as circumstances dictate.

3.0 SOIL LINER CONSTRUCTION
This section covers material conformance testing, general construction procedures, and
testing requirements for the compacted soil liner system for Phase 4 construction.
3.1 Subgrade Preparation
3.1.1 Verification of Subgrade Continuity
When the preparation of the subgrade in the construction area of the landfill is completed, the
CQA Engineer will:
• Inspect the subgrade on the side slopes and base of the landfill and note areas of weak or
excessively weathered subgrade material, and
• Direct and observe the proof rolling of the base of the landfill, if necessary, and note areas
that exhibit excessive rutting, heaving, or softening. No more than 2 inches of deflection will
be allowable based on visual observations.
Backfill material in the excavation will be structural fill material that will be placed and
compacted. The CQA Resident Engineer will observe any excavation and backfilling operations.
3.1.2 Structural Fill Placement and Construction Quality Assurance Evaluation
If backfilling of the excavation subgrade is required, then the minimum frequency of soils testing
for CQA purposes will conform to the minimum frequencies presented in Table 1. Unless
otherwise required by the CQA Monitor, the structural fill shall be compacted a moisture content
sufficient to achieve a minimum density of 95 percent of maximum density (generally 2 percent
dry of optimum moisture to 4 percent wet of optimum moisture) as defined by Standard Proctor
Compaction testing (ASTM D698). The maximum particle size for structural fill material is 6
inches unless otherwise approved by the CQA Monitor.
Nuclear density meter test methods will be used for the field testing of the in-situ dry unit weight
and moisture content of the in-place, compacted structural fill. Drive cylinder tests and oven
moisture content tests may be conducted if deemed necessary by CQA personnel to calibrate the
results of the nuclear density meter and in cases of uncertainty with the nuclear density meter test
results.
During structural fill placement the CQA Monitor will observe and document that:
• Individual fill lifts of structural fill material shall be placed and compacted at a loose
thickness such that the maximum thickness of the compacted lift is no greater than
approximately 12 inches.

mud, ice, or frozen material.
If an area in the fill is discovered that does not meet specification, the CQA Monitor will
determine the extent and nature of the area. The CQA Monitor will determine the extent of the
area by additional tests, observations, a review of records, or other means that the CQA Monitor
deems appropriate. After the extent of the area is determined, the CQA Monitor will observe and
document that the deficiency is corrected by the Contractor before any additional work is
performed in the area of the deficiency.

A Professional Land Surveyor licensed in the State of Colorado will perform the CQA survey.
The Surveyor will survey the subgrade excavation at all designed survey points to confirm that
the grades and elevations in the field agree with those shown on the Drawings. Additionally,
some survey points may be added or moved to field fit the area of the prepared subgrade. These
field fit points will still meet the survey requirements in Section 5. CQA Surveys will be
conducted in accordance with the requirements described in Section 5 of this document.
The CQA Engineer will review and approve the survey results before the next phase of the lining
system is constructed.
3.2 Construction Quality Assurance for Cohesive Soil Liner
Field density testing and clay sampling for quality assurance will be performed during cohesive
soil liner construction as discussed in this section. Cohesive liner will only be placed on the
floor of the permitted landfill that is included in the construction drawings.
3.2.1 Preconstruction Materials Testing
Prior to construction of the compacted soil liner system for Phase 4, representative samples of
the materials proposed for use will be collected and tested to verify that the soils to be used for
construction meet the project specifications. The following tests outlined in Table 1 will be
performed for this project prior to placing any cohesive soil liner.

In order for a material to qualify for use as soil liner material, it must have a group symbol of
CL, CH, SC or ML according to the Unified Soil Classification System. In addition, each soil
used for construction must also meet the following criteria:
• Allow more than 30 percent passage through a No. 200 sieve;
• Have a liquid limit equal to or greater than 20;
• Have a plasticity index equal to or greater than 10; and

• Have a coefficient of permeability of 1 x 10-7 centimeters per second (cm/ sec) or less for soil
liner construction when compacted to a density and moisture content specification outlined in
this document.
3.2.2 Cohesive Soil Liner Construction
Prior to construction of the cohesive soil liner, the subgrade will meet the elevations specified on
the construction drawings as verified by a surveyor as discussed in Section 3.1.3 above and be
approved by the CQA Engineer or CQA Monitor. Construction progress will be monitored by use
of the initial subgrade survey in combination with grade stakes or intermediate surveying during
construction, as necessary. No test pad will be constructed since the same material will be used
as was previously used during the 2013 Phase 5 construction.
The cohesive soil liner shall be constructed in accordance with design criteria of the South
Canyon Landfill Phase 4 Design Plan. This section of the CQAP outlines the procedures to be
followed during placement of the cohesive soil liner
1. The minimum thickness of the cohesive soil liner at all locations shall be at least 1.0 foot,
measured perpendicular to the liner surface.
2. The cohesive soil -lined base at all locations shall be constructed in lift heights no greater than
approximately 6 inches after compaction and no greater than the depth of the compactor
teeth, whichever is less.
The cohesive soil liner will be constructed in 8 -inch thick loose lifts compacted to 6 inches. A
24 -inch diameter disc (or similar equipment approved by the CQA Engineer) will be used to
break up the clay clods, expose stones in clay to allow for removal and to assist in raising the
moisture content to 0 to +4 percent above optimum moisture content as determined by Standard
Proctor test (ASTM -D698). The soils will be compacted with equipment that kneads, compacts,
and inter -bonds the soil from the bottom of the lift up, such as a sheep's foot compactor or high
profile pad foot compactor. Compaction will be performed using a Caterpillar 815 tamping foot
compactor or equivalent. As compaction is achieved and the feet of the compactor walk out of
the lift, the uppermost surface (except the final lift) will be left in a rough condition, or will be
scarified by disking, to promote bonding of the soil. Material conditioning procedures and
compaction equipment rolling patterns may be evaluated and modified as necessary to yield a
workable, consistent, and suitable liner material.
3.2.3 CQA Testing
A CQA Monitor, under the supervision of the CQA Engineer, shall be present on-site to monitor
the placement and compaction of the soil liner.
Density testing of cohesive soil liner shall be performed randomly but on a frequency of four
density tests per acre per compacted lift of cohesive soil liner placed and spaced to provide

complete coverage over the constructed area. This frequency is approximately equivalent to
testing on 100 -foot centers.
The nuclear density gauge shall be calibrated in accordance with manufacturer's instructions.
Unstable or erratic gauges shall not be used for quality assurance testing.
The testing shall be offset on each subsequent lift. Additional density tests shall be obtained in
confined areas where equipment movement is hindered or hand compaction is necessary. The
number of density tests in confined areas shall be recommended by the CQA Engineer based on
the size of the area. If areas are encountered which do not meet the specified moisture content or
percent compaction, the area will be reworked; moisture conditioned, and recompacted as
necessary. The area to be reworked shall be bounded by the additional passing moisture/density
test locations and the material shall be reworked from the failing test location halfway to the
passing test locations. Drying, wetting, additional compaction, or a combination there of shall be
used to bring the nonconforming area to an acceptable level. Retests will be performed
following the rework activities. All tests, whether passing or failing, shall be documented.
Each penetration made into the cohesive soil liner for moisture/density testing purposes shall be
repaired after testing. The test hole shall be repaired by backfilling the test hole with bentonite
and hydrated or other material approved by the CQA Resident Engineer.
Density tests will be reported on the Field Moisture/Density Test Report. Completed Field
Moisture/Density Test Reports shall be included in an appendix to the report. Prior to the
preconstruction meeting, the CQA Consultant shall provide the Owner with a sample of the field
moisture/density form for review and approval.
3.2.4 CQA Construction Sampling
The material used to construct this liner shall conform to the following specifications:

Each Shelby tube must collect at least 4 inches of cohesive soil liner to provide sufficient
material for permeability testing in accordance with these specifications. Each void remaining
after extracting the undisturbed sample will be backfilled with bentonite and hydrated. Shelby
tube sample locations will be included on the as -built drawing in the construction certification
report (Section 6.4).
A soil sample inventory log will be maintained as samples are acquired. All completed
inventory logs shall be included in an appendix to the final construction certification report.
3.2.5 Thickness Verification
The thickness of the compacted cohesive soil liner shall be verified by a surveyor licensed to
practice in the State of Colorado. The surveyor may be employed by the CQA Engineer. Prior
to construction of the compacted soil liner, a survey shall be completed on a minimum of 50 -foot
grid system to document the top of subgrade elevations. Additional survey points shall be taken
at the toes and crests of slopes and grade changes. At the conclusion of placement of the
compacted soil liner, a survey will be completed at the same 50 -foot grid point locations as well
as the toes and crests of slopes and grade changes to verify the required soil component thickness
was achieved. Acceptable tolerances for surveying the top of the cohesive soil liner are included
in Section 5 of this document.
3.3 CQA for the Termination Berms
Unspecified soil materials shall be used to construct termination berms. The material shall be
placed and compacted in individual lifts no thicker than 12 inches after compaction. Compaction
will be completed using a minimum of three passes with suitably heavy equipment. Care will be
taken when compacting in the area of the temporary leachate conveyance pipe.

4.0 LEACHATE CONVEYANCE SYSTEM
This section covers the general construction and oversight necessary to verify that the installation
of the leachate conveyance system is constructed in accordance with the construction documents.
The leachate conveyance system is a temporary piping system designed to gravity flow leachate
from Phases 4A and 4B to the existing leachate system. The system will consist of a 6 -inch
diameter high density polyethylene (HDPE) pipe that is perforated from the upgradient base of
the termination berm to a length of four feet.
4.1 Leachate Conveyance Piping
The temporary leachate conveyance pipe will be constructed per the design drawings. Pipe
material will be high density polyethylene (HDPE) standard dimension ratio (SDR) 17 and will
be smooth. A four -foot section of the pipe on the upgradient side of the termination berm will be
perforated with approximately quarter -inch holes placed at 60 degrees around the pipe every 4
inches and offset from each other. The perforation pattern will be approved by the CQA
Engineer. The perforated portion of the pipe will be wrapped with an 8 -ounce per square yard
nonwoven geotextile.
The leachate conveyance pipe shall be placed in a trench approximately one -foot deep and
covered with soil to protect the pipe. The soil will be lightly compacted after backfilling.
The alignment will be marked with stakes.
4.2 Quality Assurance Testing
The CQA Monitor shall observe the placement of the piping to verify that the appropriate slope
on the piping has been achieved. Additionally, visual observation of piping connections shall be
completed to document proper connection of pipe segments and orientation of perforated pipe,
where applicable.
The CQA certification report will document the elevations of the leachate pipe to the nearest 0.1
foot at least every 100 feet along the top of the pipe and at grade breaks. Additionally, the pipe
will be surveyed at the tie in to the existing leachate conveyance pipe. Additional pertinent
components or junctions will also be recorded.
4.3 Piping Protection
The pipe will be wrapped with an 8 ounce nonwoven geotextile (as selected by the CQA
Engineer) to minimize the potential for clogging the perforations. The pipe will be covered by
gravel that will extend 2 feet above the top of the clay and extend at least two feet on either side
of the pipe with the gravel extending one foot in front (upgradient) of the pipe mouth. Refuse
will not be placed within three feet of the gravel to enable access to the pipe if necessary. The

gravel can be any coarse material approved by the CQA Engineer since the primary purpose of
the material is to protect the pipe.
The pipe will be placed in a trench a minimum of one foot deep outside of the termination berm
and the trench will extend to the point where the temporary pipe ties into the permanent leachate
collection pipe. It will be backfilled with lightly -compacted, unspecified soils to protect it from
daily operations at the landfill. Stakes will be placed on the alignment at a minimum of 50 -foot
centers to identify the location of the pipe and trench.

The Surveyor will be required to survey the subgrade and the top of liner and the leachate pipe,
for the landfill in accordance with the requirements of the project. The surveys will be
conducted on a minimum 50 -foot grid to ensure compliance with all project lines, grades,
elevations, and thicknesses. All grade break lines will be surveyed in addition to the grid points.
All pipes for leachate detection, collection and/or removal will be surveyed at their inverts at
start and end points and grade breaks. The survey will include enough information to confirm
that the following features of the landfill are constructed in accordance with the Drawings:
• Toe of slope;
• Crest of slope;
• Grade breaks;
• Temporary termination berms and liner terminations;
• Temporary leachate collection pipe.
The following survey tolerances for the layer components of the construction shall be adhered to:
Vertical Tolerances
• Subgrade: +0.00 feet; -0.20 feet
• Top of soil liner -0.00 feet; +0.20 feet
(Minimum design thickness must be constructed; 12.0")
Horizontal Tolerances
• ± 0.50 feet of design
The top of leachate collection pipe will be surveyed every 100 feet and will be accurate to a
tolerance of 0.1 feet.
All final certification surveys shall be accompanied by a certification by a Professional Land
Surveyor licensed in Colorado that the construction was completed in compliance with project
survey requirements. Generally, deviations from these tolerances will not be approved by the
CQA Engineer; however, some circumstances may dictate a deviation from these tolerances with
pre -approval of the CQA Engineer.

Proper documentation of the construction process is an important aspect of construction
documentation. In addition to the completion of the forms mentioned previously, the following
reports shall be completed.
6.1 Daily Reports
The CQA Monitor is to prepare daily written reports which should be made available to the CQA
Engineer as required. These reports will include information about the work accomplished each
day, tests and observations that were made, and descriptions of the adequacy of the work
performed. At a minimum, the reports should include the following:
• Date, project name, location, area involved in construction, equipment utilized, and personnel
involved in major activities;
• Description of weather conditions, including temperature, cloud cover, and precipitation
• Description of the type of construction, inspection, and testing activity for the day;
• Location of construction activity for the day;
• Location of tests completed;
• Discussion of construction methods (i.e., equipment make/model, number of compactor
passes, etc.) as they relate to previous cell or final cover construction;
• Results of construction activity (i.e., first lift completed, sump completed, etc.);
• Description of construction materials used including reference to certifications, test results,
etc.,
• Location of observation activity or location from which the sample(s) were obtained;
• Results of testing performed (passing or failing);
• Construction or testing problems and required actions;
• Photographic documentation of construction progress including date, location, and name of
photographer; and
• Signature of the CQA Monitor
Appendix A includes example CQA forms including the Daily Activities Field Report and the
Nuclear Moisture/Density Gauge Test Record.
6.2 Design Change Documentation
On occasion it may be necessary to modify the design during construction activities. Changes to
the design or deviation from the permit documents must be approved by the Owner and the
CDPHE.

6.3 Deviation from CQA Plan
During the course of construction, deviations from the approved CQA Plan may be necessary
due to various construction issues, permit modifications, regulatory changes, new technology, or
changes to accepted standards. Deviations from this CQA Plan must be documented and
approved by the Owner and the CDPHE prior to their implementation.
6.4 Final Certification Report
At the completion of the construction, the CQA Engineer will prepare the final CQA certification
report for submittal to the CDPHE. This report will include the CQA Engineer's Colorado
Professional Engineer's seal and date. The certification report will contain the following:
• A certification (signed, sealed, and dated) by the CQA Engineer stating that the landfill
construction has been completed in accordance with the engineering design and CQA Plan.
• As -built drawings (signed, sealed, and dated) and survey certification (signed, sealed, and
dated) by a Colorado -registered land surveyor or a Colorado -licensed Professional Engineer.
• Field data and laboratory test results.
• Inspection reports, certifications, and photographs
