Re: Delinquent Discharge Monitoring Report, NPDES Permit No.00-0030970,
Garfield County
Dear Permittee:
The National Pollutant Discharge Elimination System Waste Discharge Permit
issued for your facility requires that you submit a Discharge Monitoring
Report, BA Form 3320-1, on a regular schedule. Each permit that has been
issued in the State of Colorado specifies monthly, quarterly, semi-annual,
or annual discharge monitoring reports. Reports for the previous
reporting period are due by the 28th day of the month following that
period. Your permit specifies the self-monitoring requirements, in terms
of sample types and frequencies and the reporting schedule. These reports
are to be sent to the Water Quality Control Division, Colorado Department
of Health and to the Environmental Protection Agency. If no discharge
occurred from your facility during the previous reporting period, "No
Discharge" should be reported.
According to our records, we have not received a Discharge Monitoring
Report from you for the period of October 1 thru December 31, 1981.
We request that you comply with this permit requirement within fifteen
(15) days after receipt of this letter. Failure to comply constitutes a
violation of your permit and may subject you to enforcement action by this
Department and/or the Environmental Protection Agency.
