The following items are required by Garfield County for a final inspection:
1, A final Electrical Inspection from the Colorado State Electrical Inspector.
2. Permanent address assigned by the Garfield County Building Department posted where
readily visible from access road.
3. A finished roof, a lockable house, complete exterior siding, exterior doors and windows
installed, a complete kitchen with cabinets, a sink with hot & cold running water, kitchen
floor coverings, counter tops and finished walls, ready for stove and refrigerator, all necessary
plumbing.
4. A complete bathroom, with wash bowl, tub or shower, toilet stool, hot & cold running, water,
floors and walls finished an a privacy door.
All steps outside or inside over three (3) steps must have handrails, guardrails on balconies
or decks over 30" high constructed to all 1994 UBC requirements.
6. Outside grading done to where water will detour away from the building.
7. Exceptions to the outside steps, decks and grading may be made upon the demonstration of
extenuating circumstances, i.e., weather, but a Certificate of Occupancy will not be issued
until all the required items are completed and a final inspection made.
A CERTIFICATE OF OCCUPANCY WILL NOT BE ISSUED UNTIL ALL THE ABOVE

[A.] Septic Tank Requirements .
L The effective liquid capacity shall be no less than 1250 gallons.
2. The inlet invert shall be 3 inches higher than the outlet invert,

. of the underside of the tank top and shall extend at least 14 inches below the outlet
invert. • •
4. The distance from the 'outlet • invert to the underside of the tank shall be at least 10
inches.

9. A septic. tank shall have two or more compartments (or more than one tank may be
used in series) to provide the following capacity arrangement, The first compartment
of a septic tank shall hold no less than 1/2 nor more than 213 of the required effective
capacity. .
10. The transfer, of liquid. from the first compartment to the second or successive
compartments shall be made at a liquid depth of at least 14 inches below the outlet
invert but not in the sludge zone.
11, At least one. access no less than 20 . inches across shall be provided to each
compartment of a septic tank_
12. The opening cover of a septic 'tank . access opening, inspection port, or • sampling
access port shall be no deeper than, 8 inches below the finished grade.
Risers manufacttned from concrete or other approved material shall be installed
over the, opening covers. of the access openings to a depth no greater than 8 inches
froin the finished grade if those, openings would be located greater than eight (8)
orches below finished grade.
IN Connection to. Septic Tanks:
Pipe meeting ASTM standard D 3034, property supported to prevent failure by
settling, shall extend from the dwelling to the septic tank and at least 6 feetfrom the
outlet invert of the septic tank Pipe meeting ASTM standard D1785, Schedule 40, shall
be required to be installed when the building sewer, or any portion of the outlet .pipe. from
.the septic tank to the distribution. or dispersal area, is. located under a driveway or. under
any other area subject to surface activity. There shall be at least two (2) feet of cover
over pipe of ASTM standard D1785, Schedule 40. Pipe of ASTM. standard D1785,
Schedule 40 may also be installed when there is less .than two (2) feet of cover pruvided
the pipe is encased within at least a 16 gauge corrugated pipe, or within concrete.

(CJ Installation of Septic Tanks:
1, Tams are to be installed on solid base and shall be level. The tank shall be installed
with removable covers or access openings extended to .within 8 inches of finished
grade in order to provide access to the inlet and outlet compartments for cleaning and
inspection. Roof drains, foundation drains, foundation drains, area drains, or cistern
overflows are not to enter the . tank or .any part of the individual sewage disposal
system.
2. The building sewer to septic tank pipe shall be laid with a minimum fall of 1/8 inch
per foot. Beads in the building sewer shall be limited to 45 degree, ells or long -sweep
:quarterds. Pipe meeting ASTM standard 3034 or pipe of equal or greater
strength of required, to be installed from the building to the septic tank. The building
sewer installation shall meet all the requirements of the current Uniform Plumbing
Code. The building sewer to septic tank pipe shall be at least three (3) inches in
diameter but shall be no. larger in diameter than the inlet invert as originally cast or
formed in the septie.tank. The inlet and outlet pipes shall be grouted and sealed to
the septic tank with waterproof materials.
3: Backfilling around a septic tank shall be accomplished in a manner t� prevent
settlement and to avoid undue strain on the tank and pipes entering and leaving the
tants,
4. .A cleanout shall be installed, outsideof the .building and as close to the building as
possible, to allow line clewing equipment to be utilized. A cleanout shall be
required for each 100 feed of sewer line between the building and the septic tank.

Subject: Subsoil Study For Foundation Design, Proposed Residence, Lot 17, -
Westbank Mesa. Garfield County, Colorado. •
Dear Mr. Douglas:
As requested, Hepworth-Pau/19.k Geotechnical, Inc. performed a subsoil study and
percolation test For design of foundations and septic disposal system at the subject site.
The study was conducted based on our professional services agreement letter to you,
dated March ]9. 1998. The data obtained and our recommendations based on the
proposed construction and subsurface conditions encountered are presented in this
report.
Proposed Construction: The proposed residence will he a one story wood frame
structure with a slab -on -grade floor. Cut depths are expected to range up 10 feet.
Foundation loadings for this type of construction are assumed to be relatively light.
If building conditions, dr foundation loadings are significantly different from chose
descrilbed above, we should be notified to reevaluate the recommendations presented in
this report.
Site Conditions: The lot is located uphill of Huebinger Drive. The grc,und surface is
strongly to moderately sloping down to the east and i;: covered with sagebrush, grass
and weeds.
Subsurface Conrhitions: The subsurface conditions at the site were evaluated by
excavating two exploratory pits, one near the northeast corner and the other near the
southwest corner of the proposed residence, Pi: 1 is located near the Percolation holes
to the northeast. The logs of the pits are presentee on Fig. 1. The subsoils
encountered. below about 11/ feet of topsoil, consist of sandy silty clay. Results of
swell -consolidation testing performed on relatively undisturbed samples of the silty clay
soil, presented nn Fig. 2, indicate low compressibility at natural low rnoisture and light
laadirg conditions and a ]ow collapse potential rsettlement under constant lid) when
wetted. The samples showed moderate to high compressibility under increased loading
after wetting. No free waxer was observed in the pits at the time of excavation and the
soils were slightly moist.
Foundation Recommendations: The sails encountered at the site are low dcn.ity and
compressible when wetted even under light residential loading. Considering the subsoil
conditions encountered in the exploratory pits and the nature or the proposed
construction, spread footings placed on the undisturbed natural soul can he used for

support of the proposed residence provided the risk of settlement and distress is
accepted by the owner. Footings should be designed for an allowable soil tearing
pressure of I.000 psf and have a minimum width of 20 inches for continuous walls and
2 feet for columns. Settlements could 'oe on the order of 1 to 2 inches dependiri on the
depth and extent of wetting and could cause post construction structural distress, Loose
and disturbed soils at the foundation bearing level within .the excavation should he
removed and the footing bearing level extended down to the undisturbed natural soils.
The exposed subgrade should be moistened and compacted in footing areas. Exterior
footings should be provided with adequate cover above their bearing elevations for frost
protection. Placement of footings at least 315 inches below the exterior grade,is typically
used in this area. Continuous foundation walls should he heavily reinforced cop and
bottom to spat local anomalies such as by assuming an unsupported length of at least 15.
feet. The foundation should be constructed in a "box like" shape and minimize rhe use
of isolated columns. Foundation walls acting as retaining structures should he designed
to resist a lateral earth pressure based on an equivalent fluid unit weight of at least 50
pc;f for the on-site soil as backfill.
Floor Slabs: The natural onsite soils, exclusive of topsoil, are suitable zo support
lightly loaded.slab-on-grade construction. There is a risk of post construction settlement
due to subgrade wetting. To reduce the effects of some differential movement. floor
slabs should be separated from all bearing walls and columns with expansion joints
which allow unrestrained vertical movement. floor slab control joints should he used to
reduce damage due to shrinkage cracking. The requirements for joint spacing and slab
reinforcement should be established by the designer based on experience and die
intended slab use. A minimum 4 -inch 1eyer of free -draining gavel should he placed
beneath slabs to act as a leveling course and reduce capillary moisture rise. This+
material should consist of minus 2 -inch aggregate with Tess than 50% passing the No. 4
sieve and less than 2% passing the No. 200 sieve.
All fill ma£erials for support of floor slabs should be compacted to at lest 95 % of
maximum standard Proctor density, at a moisture content near optimum. Rs c{uired fill
can consist of the on-site soils devoid of vegetation, topsoil and oversized rock.
Underdrain System: Although free water was nut encountered during our exploration,
it has been our experience in the area tha: local perched groundwater may develop
during times of heavy precipitation or seasonal runoff. Frozen ground during spring
runoff can create a perched condition. We recornme.nd below grade construction, such
as retaining waPs and below grade living areas more Olen 4 feet deep (if any), he
protected from wetting and hydrostatic pressure buildup by an underdrain system.

The drains should consist of drainpipe placed in the bottom of the wall backfill
surrounded above the invert level with free -draining granular anterial. The drain
should be placed at each level of excavation and at least I foot below lnwesi adjacent
finish grade and sloped at a minimum 1% to a suitable gravity outlet. Free -draining
granular material used in the underdrain system should contain less than 2% passing the
No. 200 sieve. less than 50% passing the No, 4 sieve and have a maximum siie of
2 inches. The drain gravel backfill should be at least 1 1 feet deep. An impervious
membrane such as 20 mil PVC should be placed beneath the drain gravel in a trough
shape and attached to the retaining wall with mastic to prevent wetting of the hearing
soils.
Surface Drainage: The fallowing drainage precautions should he observed during
construction and maintained at all times after the residence has been completed:
1) Inundation of the foundation excavations and underslab areas should he avoided
during construction_
2) Exterior backfill should be adjusted to near optimum moisture and compacted to
at least 95% of the maximum standard Proctor density in pavement and slab
areas and to at least 90% of the maximum standard Proctor density in landscape
areas, Free -draining wall backfill should be capped with at least 2 reef of te:
on-site soils to reduce surface water irrrihration.
3) The ground surface surrounding the exterior of the building shuulu hesloped to
drain away from the foundation in all directions. We recommend ;t minimum
slope of 12inches in the first 10 feet in unpaved areas and a minirnunslope of
3 inches in the first 10 feet in pavement and walkway areas. A swale will he
needed uphill to direct surface runoff around the residence.
4) Roof downspouts and drains should discharge well beyond the limits :if all
backfill.
5) Landscaping which requires regular heavy irrigation should he located xr least
1.0 feet from the building,
Percolation Testings Three percolation tests were conducted just clown I7ill to rh
northeast of the building site. The percolation test results, presented in Table I and the
subsurface profile a Pir 1 indicate the use of an infiltration septic disposal system
ghosild be feasible at the tested site. The percolation rates 'varied from 20 to 26 minutes
per inch. The average percolation rate was 23 minutes per inch.
Limitations: This report has been prepared in accordance with genern1ty accepted
acotechnieal engineering principles and practices in this arca at this time. We ;Hake no

other warranty either expressed or implied. The conclusions and recommendations
submitted in this report are based upon rite data obtained from the exploratory pit~
excavated at the locations indicated on Fig_ 1, the proposed type of construction and our
experience in the area. Our findings include interpolation and extrapolation of the
subsurface conditions identified at the exploratory pits and variation~ ie the subsurface
conditions may trot become evident until excavation is performed. If conditions
encountered during Construction appear different from those described in this report, we
Should he notified at once so re-evaluation of the recommendations may he made. The
depth of the compressible upper soils and the settlement risk to the huiidine could he
evaluated by drillin,g exploratory horinge. The site is underlain by the Eagle Valley
Evaporate and evaluation of the fixture subsidence risk due to dissolution or the bedrock
is beyond the scope of this study.
This report has been prepared for the exclusive use by our client for desier, purpoees.
We are not responsible for technical interpretations by others of our information. As
the project evolves. we should provide continued consultation and field services during
construction to review and monitor the implementation of our recommendations, dna to
verify that the recommendations have been appropriately interpreted, Significant design
changes may require additional analysis or .modifications to the recommendations
presented herein. We reetienmend on-site observation of excavations and foundation
bearing strata and testing of structural fill by a representative of the soil engineer.

Please rind enclosed the design of the sewage disposal system for your proposed residence on Lot
17, Huebinger Drive, Carbondale, Colorado. The design is based upon the soils report submitted by
Hepworth-Pawlak Geotechnical, Inc. dated April 24, 1998 and your residential house plans from Design
Traditions (job & tube It 0150.21).
The specific design calls fix the installation of a 1250 gallon septic tank- and 1404 square feet of
seepage bed_ This design, is based upon an average percolation rate of 23 minutes per inch and the State
Beath Department regulations for Individual Sewage Disposal Systems. I have included enough surface
area to account for four (4) bedrooms, a cloths washerand a garbage disposal system.
The septic tank is a standard 1250 gallon tank found at most companies that supply such
equipment. The tank must meet State Heath Department regulations and I have enclosed a set of
requirements for you to confirm with your supplier. I have also enclosed a set of requirements on how the
tank must be installed.

We will require you to have a Structial Engineer Registered in the State of Co., approve and
stamp the roof framing. It must meet or exceed a 40# Snow Load, and with -stand a 80 MPH
Wind Load.
Please note, the Family Room in the Basement, has no window. All habitable Rooms must have
10% Natural Light, 5%Ventilation. Any Bed Room any place, must have an Egress Window.This
will require the Stamped Approval of Mark I. Relph P.E. Who stamped the Basement Design.

In order to understand the scope of the work intended under a permit application and expedite the
issuance of a permit it is important that complete information be provided. Adequate and complete
information will prevent delays in the plan review process. Reviewing a plan and the discovery that
required information has not been provided by the applicant may result in the delay of the permit
issuance and in proceeding with building construction. The owner or contractor may be required to
provide this information before the plan review may proceed. This causes delays because other plans
that are in line for review may be given attention before the new information may be reviewed after
it has been provided to the Building Department.
Please review this document to determine if you have enough information to design your
project and provide adequate information to facilitate a plan review. If you do not, it may be
helpful to obtain a book titled "Dwelling Construction under the Uniform Building Code".
This book is available to you through this department at our cost. Also, please consider using
a design professional for assistance in your design and a construction professional for
construction of your project.
To provide for a more understandable plan in order to determine compliance with the building,
plumbing and mechanical codes, applicants are requested to review the following check list prior to
and during design. Applicants are required to indicate appropriately and to submit the completed
check list at time of application for a permit.
