1. All construction, backlillhig and patching is to comply with Article 090.045 of the Municipal Code.
2. It will be the contrnctor's responsibility to notify all utility companies (1-800-922-1987) 48 hours in advance of excavation.
3. All work in the City's right of ways must have an attached Traffic C'ontal Plan prepared by a Traffic Control Supervisor. The Traffic Control Plan must
address pcvfestriau and vehicle safety where appropriate. The contractor shall furnish sufficient barricades, flashers, signs, and, if needed, certified flag
person(s) in ordrr to prevent persons and vehicles frame sustaining injury.
4. This penttil shall he kept available on the job site fpr the duration ofwaak.
5. Repairs to damaged existing utilities will be backcharged to the contractor doing the excavation.
6. "Ilio permittee shall be responsible for the repair of any deficiencies in the right-of-way caused by the exeasatiou foi two years after plelion date. Failure
to respond within 48 howl will authorize City forces to make necessary repairs and backcharge those repairs to dee permittee. Emergency conditions will be
repaired by the City immediately and buckehai ed,
7. Asphalt and concrete restoration shall be completed within 10 working days of completion of trench backfill operations. If pavement cannot be repaired with
Trot mix, it shall be mid patched (silvex or equivalcnt) and maintained as necessary throughout the winter. When hot mix is available, old patch shall be
removed and the trench restored with hog mix.
8. Work shall not be accepted by the City until all provisions arc met by the contractor.
9, Contractors are required to maintain a current City license with proof of workman's compensation and public liability and properly damage insurance,

12. If it is deteltmined at a later date that the building will not he constructed, the applicant Shull h11e011, grade and revegetate the site prior to expiration of the
permit (Sec UBC Section 106.4 for expiration date).
13. Issuance of this permit does not represent or guarantee the location or existence of any public right of way, The pennittee is responsible fin verifying the
location of all right of ways.

15. The permittee is responsible for obtaining all neces's'ary private or public easements for the proposed project.
16. Contactor is to contact the City Engineering Department 24 hours prior to preparing patch area, for size determination and final pilling. call 384-6435 to
schedule inspection.

This permit is issued with the specific understanding that it is subject to suspension or revocation for failure to comply with the terms and conditions of
the Glenwood Springs Municipal Code. Representatives of the City of Glenwood Springs are authorized to inspect the site at any time.
