1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1964.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs,alters, or installs an individual sewage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine —6
months in jail or both).

thority granted in 66-44-4, CRS 1963, amended 66-3-14, CRS 1963.
2. This permit is valid only for connection to structures which have fully complied .with County zoning and building requirements.
Connection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a viola-
tion of a requirement of the permit and cause for both legal action and revocation of the permit.
3. Section III, 3.24 requires any person who constructs, alters, or installs an individual sewage disposal system in a manner which in-
volves a knowing and material variation from the terms or specifications contained in the application of permit commits a Class I,
Petty Offense ($500.00 fine — 6 months in jail or both).

A site visit was performed Thursday June 19, 2008 at the Brenner residence. The following
information is offered:
Under Permit No. 4452 issued May 27, 2008, the existing seepage pit access manhole was
pumped and carefully excavated to evaluate the reason for failure. The pit showed the typical
black anaerobic soil staining beginning approximately 4 -feet below ground surface. A small
area (12 -inches wide) containing gravel surrounded the perforated manhole barrel sections (2-3
foot sections, 4 feet in diameter). The area was excavated approximately 5 -feet around the
manhole for a depth of 6 feet and baddilled with 1%" drain rock (645 SF Absorption area total,
420 SF sidewall area only). A geotextile fabric was installed over the drain rock (approximately
24 -inches below finish grade) and backfilled with native soil and top -dressed with gravel '. Said
manhole and pit is located adjacent to the existing driveway as shown on the plot plan submitted
with the application.
The system appears to be functioning normally. No evidence of effluent surfacing or odors were
present at the time of the site visit. The septic tank is accessible for periodic maintenance. The
repair is complete and has been performed under my direction by the owner and his contractor
and is in substantial conformance to the State ISDS Regulations.
