*CONDITIONS:
1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs,alters, or installs an individual sewage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine —6
months in jail or both).

Subject: Percolation Testing, Proposed Residence, Aggard Lane, Garfield County,
Colorado
Dear Mr. Timmer:
As requested by you, Hepworth - Pawlak Geotechnical, Inc., performed percolation
testing at the subject site. The testing was done in addition to our agreement for
professional services to you dated February 18, 2008. Hepworth-Pawlak Geotechnical,
Inc. performed a subsoil study for foundation design at the subject site and reported our
findings March 14, 2008, Job No. 108 053A.
A profile pit and three percolation test holes were excavated on May 7, 2008 at the
approximate locations shown on Figure 1. The subsoils exposed in the Profile Pit
consisted of about one foot of topsoil overlying silty clayey sand to the bottom pit depth
of 7 1/2 feet. The results of a gradation analysis performed on a sample of silty clayey
sand obtained from the pit are presented on Figure 2. The soil has a USDA soil texture
classification of sandy loam. No free water or evidence of seasonal perched water seepage
was observed in the pit and the soils were slightly moist to moist. Percolation test holes
were hand dug and soaked with water on May 7, 2008.
Percolation testing was conducted on May 8, 2008 by a representative of Hepworth -
Pawlak Geotechnical, Inc. The percolation test results are summarized on Table 1.
Based on the subsurface conditions encountered and the percolation test results, the tested
area should be suitable for an on-site waste disposal system. We understand our
Silverthome office will design the septic disposal system.
