•CONDITIONS:
1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Arlicle 10 C.R.S. 1973, Revi"ti 1984.
2. This permit is valid only for connei:tion to structures which have fully complied with County zoning and building requirements. Con·
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs, alters, or installs an individual se'Wage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine -6
months in jail or both).

Re: As-Built Drawings for the Sherick Individual Sewage Disposal System
(ISDS) for the Commercial Building "E" located at 38150 River Frontage Road,
New Castle, Colorado 81647.
Dear Andy:
Attached please find a two-sheet set of As-Built plans for this system. The
system was installed in substantial compliance with the design plans. Final
locations and details shown on these plans reflect what was constructed.
I do not know the permit number for this system, but it should have been
submitted sometime in April, I believe.
The drywell was installed and inspected in April and May of this year, and the
septic tank was installed later, and inspected in July.
If possible, I would like to get a copy of the final permit, for my files.
If you have any questions, please do not hesitate to call.
