*CONDITIONS:
1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements, Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine --- 6
months in jail or both).

At your request, on October 29, 2008, we visited the construction site on Lot 12, Teller Springs,
152 Spur Drive, Garfield County, Colorado. The purpose of our site visit was to inspect the installation
of the septic system.
The system was installed in overall compliance with the septic system design, Drawing No.
05186SD-ADV.DWG, dated November 16, 2005 and as shown on the attached as -built sketch. They
installed 2000 gallons, two compartments, precast, concrete septic tank, and. a 1500 -gallons Advantex
Treatment systetn. 2AX20 series. Two building sewer Lines were connected to the first septic tank with
one cleanout on each. of the lines near the building and one cleanout adjacent to the inlet of the tank.
Another clean-out was constructed between the two concrete tanks. Advantex filters were placed at
about 3 feet south from the Advantex septic tank. Th.e h.eadworks box was constructed adjacent to the
Geoflow dosing tank.
The Geoflow leachfield area was constructed west from the residence and is approximately
2250 square feet. About 1125 L.F. of 4 inch diameter, wasteflow dripline was installed within the
leachfield area with 2 feet minimum spacing between each dri.pl.ine and between drip emitters.
A letter of the start up of the Advantex system by Septic Smart, LLC is attached to this report.

Dear Louiza,
Please accept this letter as confirmation that the AdvanTex equipment was installed as
specified on the drawings from LKP Engineering for the following project:
Reichardt Residence, 0152 Spur Lane, Glenwood Springs, Garfield County Colorado.
Septic Smart LLc, delivered and connected the AdvanTex system as shown on the site
drawings. The system has been wired and the phone line connected. The system is currently
operational.
The septic tanks and drip field were installed by Clay Reichardt, the septic tanks and drip
tubing and connections appear to have been installed per manufactures recommended
procedures.

This office performed a structural framing inspection at the above mentioned project on January
21, 2009. The inspected structural framing differed from the original approved plan set. Without
exception, the observed alternative framing (wood or steel) was more than adequate for the design
loading. All connections were welded, bolted or hangered per code. This engineer has deemed the
inspected framing per code and adequate. Please call us if you have questions.
