As requested, we have designed an onsite wastewater system (OWS) for the proposed residence.
CHURCH and Associates, Inc. (CHURCH) performed the subsurface investigation on May 7, 2003,
under Job No. 15706W.
SITE CONDITIONS
The property is in a rural residential area of Garfield County, where OWS are required. The area of the
proposed drain field has a full cover of native grasses and sagebrush and an approximate 15% slope to
the south. The residence will be served water by an onsite well located I 00+ feet northwest of the drain
field.
PROPOSED CONSTRUCTION
A 4-bedroom residence is proposed at the location presented on Figure I. The OWS was sized based on
four bedrooms and 600 gallon per day (GPD) using Garfield County Building and Sanitation
Department (GCBSD) loading of 75 GPD per person. The sewage loading with a peaking factor of 1.5
is 900 GPD. This loading includes washing machines and garbage grinders.
SUBSURFACE CONDITIONS
The subsurface investigation was performed by excavating a profile pit and three percolation holes at the
locations indicated on Figure 2. Subsurface materials in the profile pit consisted of a sandy, clay root
zone to 1.5 feet, underlain by slightly gravelly, sandy, silty clay with basalt boulders generally less than
6-inches in diameter to the maximum depth explored of 9 feet. No ground water was encountered.
Percolation tests indicate percolation rates ranging from 27 minutes per inch (MP!) to 53 MPI. The
average percolation rate is 37 MP!. Percolation data is presented in Table I.
RECOMMENDATIONS
The OWS installation is to include a septic tank and a gravelless chamber-trench drain field installed at
the locations presented on Figure 2. The OWS design is based on an average sewage load of 600 GPD

and a percolation rate of 37 MPI. One 1500-gallon, two-compartment, precast concrete septic tank with
an eflluent filter on the outlet of the second compartment and three gravelless chamber-trenches must be
installed. Each trench must have one row of 12 chambers for a total of 36 chambers and 558 ft2 of
infiltrative area. A concrete distribution box with a minimum of three outlets is required to distribute
eflluent evenly throughout the field.
Vehicle movement over the drain field area during construction should be kept at a minimum to
minimize compaction of the receiving soils. The bottom of the trenches should be scarified with a
pick, shovel, or backhoe bucket teeth prior to the addition of chambers.
The surface of the drain field should be seeded after installation of the system. A native seed mix from
the area available at seed stores should be used. These mixes do not require irrigation and develop a
growth I 0 to 15 inches high. No automatic sprinkler system should be installed over the field.
If additional bedrooms in unfinished areas are possible, it is suggested that the OWS be constructed to
handle the additional loading. The installation of a properly engineered OWS to serve future build-out
can be cost effective.
OPERATION AND MAINTENANCE
The owner must realize an OWS is different from public sewer. The owner must assume responsibility
for maintenance of the system. The system is a low maintenance system, but the owner must have the
septic tanks pumped and filters cleaned (hosed oft). Unless there is a monitoring program, the septic
tank should be pumped and filter cleaned every two years. There are daily considerations, such as not
putting plastic or other non-biodegradable material into the system. Water use must be monitored so
toilets are not allowed to run when seals malfunction. To illustrate the point, a running toilet will
consume in excess of I 000 gallons per day if allowed to flow. An excess 1000 GPD loading may flood
and irreparably harm the OWS.
We caution against installation of a water softener. The hydraulic loading from the backwash of a water
softener can be detrimental to OWS and a separate drywell should be constructed for the backwash
waste, if a water softener is installed. No landscaping or plastic can be used over the field, which will
reduce evapotranspiration from the field. Chemically treated water from a swimming pool or spas
should not be introduced into the OWS.
LIMITATIONS
An OWS design requires installation by a contractor who is an experienced installer. Our investigation,
layout, design and recommendations are based on data submitted. If subsurface conditions different
from those described are encountered, CHURCH should evaluate the effect of the changes on the
proposed OWS. If the health department makes modifications to this design, we should be contacted to
evaluate the impact on the performance of our OWS design.
All construction is to be in accordance with GCBSD ISDS regulations. Pipe type and size, burial
requirements, septic tank construction, and other specifications not depicted in our report, are to
conform to ISDS regulations. The system installer is to have demonstrated knowledge of the ISDS
regulations and requirements.

As requested, we observed the installation of the onsite wastewater system for the subject site on
September 19, October 2, 2003 and March 5, 2004. The system was designed under our Job No. 15706W,
dated September 3, 2003 with a clarification letter dated August 15, 2003. Deficiencies listed in our last
Installation Observation Letter, dated December 5, 2003, have been corrected.
The system includes the installation of a 1500-gallon, two-compartment, concrete septic tank with an
effluent filter on the outlet, and three gravelless chamber-trenches. Each trench has one row of 12
chambers for a total of 36 chambers and 558 ft2 of infiltrative area. An as-built drawing is included as
