*CONDITIONS:
1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs,alters, or installs an individual sewage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine —6
months in jail or both).

The ISDS system for the 5 bedroom single family Mackley residence at the Spruce Creek
Ranch is a 2000 gallon capacity, two chamber, pre -fabricated septic tank by Copeland Concrete
Inc. and a gravel -less leach field system in bed configuration. The leach field uses 42 Egnali7er
36TM chambers by Infiltrator Systems, Inc.
The system was installed by a qualified and experienced ISDS installer in accordance with the
design.

The following information summarizes the site evaluation and design of the individual sewage.
disposal system for 0436 County Road 329, Rifle, Colorado 81650. In addition to the application,
a map to the property, a site map, percolation test data, calculations and vendor information are
attached.
Overview and Introduction
The sewage disposal system serves a four (4) bedroom single family residence with detached
one (1) bedroom apartment located on a large parcel of property located in Sections 3 and 4,
Township 7 South, Range 94 West of the 6th P.M. The onsite disposal system will consist of a septic
tank followed by a chamber system leach field configured in a bed. A site map of the property is
attached.
Five (5) bedrooms are to be constructed at the residence. A garbage disposal is not proposed
to be installed in the residence. The calculated wastewater flow based on Garfield County standards
is 1250 gpd for the first 4 bedrooms and 250 gpd for the 5th bedroom for a total of 1500 gpd.
Percolation Tests
Based on the test pit excavated at the disposal site, the soils consist of silty sand. Three (3)
percolation holes were constructed and tested in accordance with the Garfield County Building and
Sanitation Department (County) requirements on April 30, 2004. The percolation rates averaged 9
minutes per inch.
Proposed Individual Sewage Disposal System
All constructed components and appurtenances associated with the Individual Sewage
Disposal System have been designed to meet or exceed County standards. The closest waterway is
Spruce Creek which is 60 feet away. A dry gulch is over 50 feet away. See attached site map.
The septic tank shall be a 1500 gallon capacity precast concrete, two chamber tank. The
proposed location of the septic tank is shown on the site plan. The leach field shall be a gravel -less
chamber system in a bed configuration. It will consist of 35 units constructed in 7 rows of 5
chambers. The chamber shall be Equalizer 36 units by Infiltrator Systems, Inc. (See attached Vendor
Information) The location of the leach field is shown on the site plan.
