I understand I need to choose a time between 8am-9am or 4pm-5pm to pick up and/or drop off my
dog(s) and that there is no pick ups or drop offs on Sunday until 4-5pm. Initials
I agree to pay the suggested rate for the boarding being effective from the day the dog(s) are dropped
off at Cozy Little Kennel until they are picked up. I will be charged $20 per dog on the day of pick up if
my dog(s) are being picked up after 11am. Payment will be given to Cozy Little Kennel before or on the
day of pick up. Initials
I understand my dog will be commingling with other dogs in playgroups. I give authorization for my
dog(s) to interact with other dogs under human supervision. Initials
If my dog becomes ill or injured, my dog will either be taken to the veterinarian I provided the Cozy Little
Kennel when my dog was checked in, furthermore upon their unavailability to be contacted I give
permission to Cozy Little Kennel to call a veterinarian of choice, given the circumstances of emergency
and my dog's vets availability to treat or tend to my dog(s) illness or injury. I am responsible for all
charges resulting from veterinary care. Initials
My dog(s) has been treated for fleas and ticks and upon arrival to Cozy Little Kennel. If fleas or ticks
are found on my dog(s), I am responsible for the cost of treatment and give permission to Cozy Little
Kennel to administer the treatment. Initials
I understand that if my dog dies while in the care of Cozy Little Kennel my dog will be taken to either my
own veterinarian or possibly another local veterinarian and maintained there for pick-up or further
instruction. Initials
I understand that while my dog(s) is staying under the care of Cozy Little Kennel, like any other care
facility, there are potential risk arising from canine cough, viruses, punctures, nicks, scratches, bites,
other injuries, or illness. Cozy Little Kennel is not responsible for payment of treatment for any of these
problems concerning my dog's well-being. Initials
I have announced to Cozy Little Kennel all things pertaining to my dog's character and/or behavior
toward other dogs or humans, that can put them at risk of attack, injury or biting. Initials
I have announced to Cozy Little Kennel all medical issues pertaining to my dogs needs and/or any old
injuries that may recur while my dog(s) is in their care. Initials
I announce to Cozy Little Kennel that my dog(s) are fully up to date on all vaccinations that the kennel
requires (Rabies,Parvo, Distemper and Bordetella) Initials
My dog(s) has been treated for fleas and ticks and upon arrival to Cozy Little Kennel. If fleas or ticks
are found on my dog(s), I am responsible for the cost of treatment that and give permission to Cozy
Little Kennel to administer the treatment. Initials
I give Cozy Little Kennel permission to use a smaller pen than state regulated under the instances of
either unavailable or cage fighting is occurring. Initials
I give permission to Cozy Little Kennel to use a noise control device (such as a shock collar) if my
dog(s) is being what is considered out of control and disturbing, causing the kennel to not stay in
compliance with state and county regulations concerning the noise ordinance the kennel is required to
follow. Initials
I agree that Cozy Little Kennel will not be responsible for damaged, lost, or stolen personal items
belonging to me. Initials
As the owner of the dog(s) being boarded at Cozy Little Kennel, I release to them the records on my
dog(s) from my veterinarian and is free for filing, documenting, and retrieving them when necessary.
I, (print please) , release all liability from all staff,
volunteers, and/or Cori Rider, Owner of Cozy Little Kennel. Located at 500 County Road 259 Rifle, CO
81650. In case of accident, illness, or injury, I fully understand that Cozy Little Kennel will take the very
best care of my dog(s) and every precaution necessary for the welfare and health of my dog(s),
however I am fully aware and understand Cozy Little Kennel is not liable for any accident, illness, or
injury of my dog(s). By signing this contract, I certify to the accuracy of all information I have given to
Cozy Little Kennel about my dog(s). I have read, understand and agree with the policies stated on this
agreement with Cozy Little Kennel, all that is contained on this page and agree with the all conditions
and all statements of this agreement.
