Dear Permittee or APPticant
A site inspection of the construction requirements for the above permit reveated it to be
acceptabte and in general conformity with the terms and conditions of the permit. The
inspection conducted by Cotorado Department of Transportation (CDOT) is onty for the
general conformance of permit construction with CDOT's design standards and Code
iequirements. lt does not release the Permittee of the responsibitity to futty comply with
the appl.icabte provisions of the access permit and State Highway Access Code.
ln accordance with the Subsection 2.5(6) of the State Highway Access Code, "lf any
construction element faits within two-years due to improper construction or material
specifications, the Permittee shatl be responsible for alt repairs. Failure to make such
repairs may resutt in suspension of the permit and closure of the access".
You are advised that your |-year warranty period begins from the date of project
acceptance, in accordance with the permit terms and conditions and the Access Code. You
(the permittee/owner) are advised that the date of acceptance is: September 6, 20,l8
