Onsite Wastewater Treatment System (OWTS) Installation Observations
450 Peterson Lane
Garfield County, Colorado
Permit Number SEPT-05-18-5260
Bill,
ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system (OW TS)
on July 9, 2018 for the subject property. Holmes Excavation installed the system.
The sewer line exiting the residence was replaced and a double-sweep clean out installed.
The system installation included a 1500-gallon, two-compartment Roth® poly septic tank with an effluent
filter on the outlet tee. This filter must be cleaned annually, or as needed. Risers were installed to
bring manhole lids to grade for access.
A Bull Run valve was installed on the 4-inch sewer line existing the septic tank and allows alternating the
STAs. We recommend keeping the tool for alternating the Bull Run Valve in housing pipe for the valve.
Each zone of the STA consists of a distribution box. Speed Levelers were installed on each sewer line
existing the distribution box to assure equal flow to each bed.
Zone A consists of two beds; one bed with 3 rows of 10 ‘Quick 4’ Infiltrator® chambers and one bed with
6 rows of 5 ‘Quick 4’ Infiltrator® chambers for a total of 60 chambers and 720 square feet of infiltrative
area. The bed with 6 rows of 5 ‘Quick 4’ Infiltrator® chambers was necessary to avoid the buried trash in
the yard.
Zone B consists of two beds; each with 3 rows of 10 ‘Quick 4’ Infiltrator® chambers for a total of 60
chambers and 720 square feet of infiltrative area.
Beds are connected with level manifolds. There was at least 6-feet of undisturbed soil between each
bed. Inspection ports were installed in all four corners of each bed.
The property owner or occupant is responsible for keeping records on the frequency of
alternating each STA. We recommend rotating the valve annually. The in-use STA must be
monitored to assure saturation does not occur. If saturation is observed, the rotation frequency
must be adjusted. Records must be available for review by Garfield County, if requested. We
have provided a document to assist in record keeping (enclosed).
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are
made to the OWTS in the future. Any additional OWTS construction must be according to the county
regulations.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.
Please call with questions.
Sincerely,
