Box 1252 Carbondale, CO 81623 Subject: Final OWTS Inspection 544 County Road 100 Carbondale, CO 81623 Final Acceptance Letter OWTS Permit ##: SEPT -6171 September 8, 2020 Mr. Rice, On August 20, 2020, we visited the site at 544 County Road 100 near Carbondale in Garfield County, Colorado to observe the Onsite Wastewater Treatment System (OWTS) installation pursuant to OWTS Permit # SEPT -6171. At the time of our August 20 site visit, the contractor had installed a 1,000 -gallon two compartment concrete septic tank with an effluent filter in the outlet tee, the new sewer line, distribution box with access to grade, and the two chamber trenches. The tank and distribution box were installed level, with access brought to grade. The two gravelless chamber trenches consist of 8 chambers per trench (16 chambers total). The original design specified 20 Infiltrator Quick 4 chambers. The contractor installed 16 Arc36 chambers. However, the resultant area of the STA is equal, so GCPH has no objections to this substitution. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Please provide an As -Built drawing of the system, when available, for our files.
