Onsite Wastewater Treatment System (OWTS) Installation Observations
Valley View Business Center
7780 Hwy 82
Garfield County, Colorado
Permit Number SEPT-07-20-6396
Iris,
ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system (OWTS)
on November 20, 2020 for the subject property. Miles Rippy Construction installed the system.
The OWTS was sized to accommodate 525 gallons per day (GPD). Development on the property
consists of two buildings with commercial office businesses and a one -bedroom apartment in the upper
level of the southern -most office. The subject OWTS serves a portion of the development. We
recommend tracking water use to better identify actual wastewater flow.
The existing septic tank was abandoned by pumping the tank, crushing the bottom, top, and sides of the
septic tank, and filling the remaining void space. The existing seepage pit was also abandoned by
pumping and filling the remaining void space.
A new, 1500-gallon, two -compartment concrete septic tank with an H-20 rated lid and cast iron manhole
lids were installed. An Orenco® Biotube Effluent Filter must be installed on the outlet tee. This filter
must be cleaned annually, or as needed.
Excavation for the new seepage pit was 14-feet long, 14-feet wide, and a minimum of 10-feet deep
below the inlet pipe. The bottom of the excavation was filled with 6-inches of 3/4-inch diameter cleaned
screened rock. Four -foot diameter perforated manhole rings were stacked in the center of the
excavation. Manhole rings were surrounded by 3/4-inch diameter cleaned screened rock, compacted in 2-
foot lifts with a vibratory plate compactor.
Cast iron extensions and cast iron lids were utilized to bring the manhole rings of the seepage pit to
grade for access.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are
made to the OWTS in the future. Any additional OWTS construction must be according to the county
regulations.
LIMITS:
Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.
Please call with questions.
Sincerely,
ALL SERVICE septic, LLC
Carla Ostberg, MPH, REHS

Permit Conditions:
This OWTS Permit is approved on the condition of compliance with the report and design as
submitted by All Service Septic dated July 25, 2020. Changes to the design and permit must be
approved by Garfield County Public Health and the designer prior to construction.
The new OWTS will serve a portion of the Valley View Business Center and replace a failed existing
seepage pit. The new OWTS will consist of a 1,500-gallon, two -compartment pre -cast septic tank set
level and accessible at grade with an effluent filter in the outlet tee of the tank. The tank lid will have
an H-20 rating, as the tank will be in a parking lot. Effluent will gravity flow to the new seepage pit,
which consists of an excavation of 14-feet long by 14-feet wide by 10-feet deep (756 ft2)with a 4-
foot diameter structure in the center of the excavation consisting of 2-foot tall perforated concrete
rings, surrounded by %-inch diameter screened gravel. The structure will be placed on 6-inches of
gravel on the bottom of the excavation. Per Garfield County OWTS Regulations, "Schedule 40 or
pipe of equivalent or greater strength must be used for the placement of piping under driveways
or roadways and in instances where sewer line setback distances are granted a variance for any
reason." (43.9.D.1.d.) All piping for this OWTS shall be Schedule 40 or stronger. Maintain all
setbacks and elevations as designed.
Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final
inspection of the installed system prior to backfill.
Agreement
All installations must comply with all requirements of the Garfield County OWTS regulations
including but not limited to the conditions specified above.
In consideration of the issuance of the permit, the applicant hereby agrees to comply with all
laws and regulations related to the zoning, location, construction and erection of the proposed
OWTS and associated structures for which this permit is granted. The applicant further agrees
that if the above said regulations are not fully complied with in the zoning, location, erection
and construction of the above described OWTS and structure, the permit may then be revoked
by notice from the Health Department and immediately become null and void.
The issuance of a permit and specifications of terms and conditions therein will not constitute
assumption of liability, nor create presumption that the local public health agency or its

satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated
in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from
the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and inspection of the system must be completed by GCPH before final
approval of tJe system will be issued.

On November 20, 2020, we visited the site at 7780 Highway 82 near Glenwood Springs
in Garfield County, Colorado to observe the Onsite Wastewater Treatment System (OWTS)
installation pursuant to OWTS Permit # SEPT-6396. At the time of our site visit, the contractor
had installed a 1,500-gallon two compartment concrete septic tank, the new sewer line, and the
new drywell. The tank and drywell rings were installed level, with access brought to grade. The
contractor was backfilling the excavation around the drywell with washed gravel while we were
on site. In our opinion, this system was installed in general conformance with Garfield County
OWTS Regulations. No further action or review from Garfield County Environmental Health is
required regarding this permit. Please provide an As -Built drawing of the system, when
available, for our files.

Do Your Part, Be SepticSmart:
The Do's and Don'ts of Your Septic System
Learn these simple steps to protect your home, health, environment and property value:
septicsmart
U.S. Environmental Protection Agency
Do:
• Have your system inspected (in general) every three years by a licensed contractor
and have the tank pumped, when necessary, generally every three to five years.
Don't:
• Pour cooking grease or oil down the
sink or toilet.
• Rinse coffee grounds into the sink.
• Pour household chemicals down the
sink or flush them.
Do:
• Eliminate or limit the use of a garbage
disposal.
• Properly dispose of coffee grounds a
food.
• Put grease in a container to harden
before discarding in the trash.
Don't:
• Flush non -degradable products or
chemicals, such as feminine hygiene
products, condoms, dental floss,
diapers, cigarette butts, cat litter, paper
towels, pharmaceuticals.
Do:
• Dispose of these items in the trash can!
Don't:
• Park or drive on your drainfield. The
weight can damage the drain lines.
• Plant trees or shrubs too close to your
drainfield, roots can grow into your
system and clog it.
Do:
• Consult a septic service professional to
advise you of the proper distance for
planting trees and shrubs, depending on
your septic tank location.
Don't
• Concentrate your water use by using
your dishwasher, shower, washing
machine, and toilet at the same time.
All that extra water can really strain
your septic system.
Do:
• Stagger the use of water -generating
appliances. This can be helpful
especially if your system has not been
pumped in a long time.
• Become more water efficient by fixing
plumbing leaks and consider installing
bathroom and kitchen faucet aerators
and water -efficient products.
