HomeMy WebLinkAboutFinal Acceptance Letter from PH 08.02.2021Garfield County 195 W. 141h Street NIRMW Public Health 2014 Blake Avenue Rifle, CO 81650 Glenwood Springs, CO 81601 (970) 625-5200 (970) 945-6614 Jason and Jamie Olden 3143 8tr Street Boulder, CO 80304 Via email: }ason(@sidfactor.com Subject: Final Acceptance Letter Lot 5, Wooden Deer Subdivision Carbondale, CO 81623 OWTS Permit #: SEPT-6691 August 2, 2021 Jason and Jamie, On May 11, 2021, we visited the site of the proposed Olden Residence at Lot 5 of the Wooden Deer Subdivision near Carbondale in Garfield County, Colorado to observe the Onsite Wastewater Treatment System (OWTS) installation pursuant to OWTS Permit # SEPT-6691. At the time of our initial site visit, the contractor had installed a 1,250-gallon two compartment concrete septic tank, the effluent line, the 500-gallon concrete pump tank, the automatic distributing valve (ADV), and the three chamber trenches. The tank, ADV, and chambers were installed level, with access to the tanks and ADV brought to grade. The three chamber trenches consisted of 19 Quick4 chambers per trench, for 57 total chambers and a total STA of 684 ft2. At the time of our initial site visit, the sewer line from the residence and the cleanout near the residence were not yet installed. Also, at the time of our initial site visit, the ADV was not at the high point of the system, as required. We informed the contractor of these deficiencies. The contractor remedied these deficiencies, and we observed the completed installation on May 28, 2021. The remaining item to be addressed was to encase the effluent line in concrete where the effluent line crosses a water line. We observed this encasement on June 4, 2021. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Please provide an As -Built drawing of the system, when available, for our files.
