Pine Street Carbondale, CO 81623 OWTS Permit #: SEPT-6929 August 3, 2021 Steven, On July 22, 2021, we visited the residence at 41 S. Pine Street near Carbondale in Garfield County, Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT- 6929. At the time of our site visit, the contractor had installed the new 500-gallon pump tank with a high -head pump in the tank, the new effluent line, the control box within line of site of the tanks, and the two 13-feet wide by 25-feet long GeoMat beds. The pump tank was installed level with access brought to grade. An effluent filter had been added to the outlet tee of the primary septic tank. The GeoMat beds were level, with 4-feet between the beds. Orifices and orifice shields were present as provided in the design and plans that we reviewed. Additionally, inspection ports were present at each comer of the beds. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Please provide an As -Built drawing of the system, when available, for our files.
